    "For a number of reasons, progress in improving child feeding practices in the developing world has been remarkably slow. First, complementary feeding practices encompass a number of interrelated behaviors that need to be addressed simultaneously. Child feeding practices are also age-specific within narrow age ranges, which add to the complexity of developing recommendations and measuring responses. Finally, the lack of clear international recommendations for some aspects of complementary feeding has prevented the development of universal indicators to define optimal feeding. Without appropriate measurement tools, the design and evaluation of programs to improve complementary feeding practices cannot move forward. The present paper is the first systematic attempt at filling this gap. It puts forth a framework for the development of indicators of complementary feeding practices and proposes a series of possible indicators to measure some of the most critical aspects of infant and young child feeding. The emphasis is on simple indicators for use in large surveys or in program contexts. Indicators for the following aspects of complementary feeding of 6-23-month-old children are discussed: (1) breastfeeding; (2) energy from complementary foods; (3) nutrient density of complementary foods; and (4) safe preparation and storage of complementary foods.     Quantitative sensory testing in children with sickle cell disease: additional insights and future possibilities.

    Quantitative sensory testing (QST) is used in a variety of pain disorders to characterize pain and predict prognosis and response to specific therapies. In this study, we aimed to confirm results in the literature documenting altered QST thresholds in sickle cell disease (SCD) and assess the test-retest reliability of results over time. Fifty-seven SCD and 60 control subjects aged 8-20 years underwent heat and cold detection and pain threshold testing using a Medoc TSAII. Participants were tested at baseline and 3 months; SCD subjects were additionally tested at 6 months. An important facet of our study was the development and use of a novel QST modelling approach, allowing us to model all data together across modalities. We have not demonstrated significant differences in thermal thresholds between subjects with SCD and controls. Thermal thresholds were consistent over a 3- to 6-month period. Subjects on whom hydroxycarbamide (HC) was initiated shortly before or after baseline testing (new HC users) exhibited progressive decreases in thermal sensitivity from baseline to 6 months, suggesting that thermal testing may be sensitive to effective therapy to prevent vasoocclusive pain. These findings inform the use of QST as an endpoint in the evaluation of preventative pain therapies

    Circulation, retention, and mixing of waters within the Weddell-Scotia Confluence, Southern Ocean:The role of stratified Taylor columns

    The waters of the Weddell-Scotia Confluence (WSC) lie above the rugged topography of the South Scotia Ridge in the Southern Ocean. Meridional exchanges across the WSC transfer water and tracers between the Antarctic Circumpolar Current (ACC) to the north and the subpolar Weddell Gyre to the south. Here, we examine the role of topographic interactions in mediating these exchanges, and in modifying the waters transferred. A case study is presented using data from a free-drifting, intermediate-depth float, which circulated anticyclonically over Discovery Bank on the South Scotia Ridge for close to 4 years. Dimensional analysis indicates that the local conditions are conducive to the formation of Taylor columns. Contemporaneous ship-derived transient tracer data enable estimation of the rate of isopycnal mixing associated with this column, with values of O(1000 m2/s) obtained. Although necessarily coarse, this is of the same order as the rate of isopycnal mixing induced by transient mesoscale eddies within the ACC. A picture emerges of the Taylor column acting as a slow, steady blender, retaining the waters in the vicinity of the WSC for lengthy periods during which they can be subject to significant modification. A full regional float data set, bathymetric data, and a Southern Ocean state estimate are used to identify other potential sites for Taylor column formation. We find that they are likely to be sufficiently widespread to exert a significant influence on water mass modification and meridional fluxes across the southern edge of the ACC in this sector of the Southern Ocean
