High throughput measurement of antibody and complement mediated immunity to meningococcal disease
The accepted correlate of protection for meningococcal disease is serum bactericidal activity. The importance of bactericidal activity was established by Goldschneider et al. (1969) who demonstrated an inverse relationship between disease incidence and bactericidal titres of ≥1:4. The importance of antibody and complement mediated immune mechanisms is also emphasised by the increased susceptibility of complement deficient individuals to meningococcal disease.
However, the absence of bactericidal activity does not necessarily indicate susceptibility and protection has been demonstrated in the absence of bactericidal activity. This has indicated the importance of other protection mechanisms such as opsonophagocytosis.
In this study a high throughput antibody-mediated complement deposition assay has been developed to measure the deposition of both C3b/iC3b and C5b-9 onto Neisseria meningitidis. Antibody mediated C5b-9 deposition measured in this assay has been shown to correlate highly with bactericidal activity for several strains and C3b/iC3b deposition has been shown to correlate with opsonophagocytosis. This assay has also been shown to be reproducible. Measurement of antibody mediated C5b-9 and C3b/iC3b deposition will not be a replacement for opsonic killing assays or the accepted correlate SBA, but could be used as a tool to evaluate large panels of sera against multiple strains due to the very low serum volumes required.
A respiratory burst assay has also been developed to measure both the uptake of fluorescently-stained bacteria and the induction of a respiratory burst response. This assay was optimised with respect to the reagents used to measure the respiratory burst response, bacterial stain and the assay buffers.
Complement is an important reagent used in immunoassays to evaluate antibody mediated immunity to N. meningitidis. Due to the high levels of nasopharyngeal carriage in adults it is difficult to obtain a complement source without cross- reactive bactericidal activity, often resulting in the requirement for a source from a different individual for each strain. An IgG-depleted complement source has been developed for use in immunoassays. The method developed has been shown to be reproducible and effective at removing the IgG whilst retaining functional complement and will be a valuable tool for assessing immunity to N. meningitidis

Immune responses in pigs vaccinated with adjuvanted and non-adjuvanted A(H1N1)pdm/09 influenza vaccines used in human immunization programmes.
Following the emergence and global spread of a novel H1N1 influenza virus in 2009, two A(H1N1)pdm/09 influenza vaccines produced from the A/California/07/09 H1N1 strain were selected and used for the national immunisation programme in the United Kingdom: an adjuvanted split virion vaccine and a non-adjuvanted whole virion vaccine. In this study, we assessed the immune responses generated in inbred large white pigs (Babraham line) following vaccination with these vaccines and after challenge with A(H1N1)pdm/09 virus three months post-vaccination. Both vaccines elicited strong antibody responses, which included high levels of influenza-specific IgG1 and haemagglutination inhibition titres to H1 virus. Immunisation with the adjuvanted split vaccine induced significantly higher interferon gamma production, increased frequency of interferon gamma-producing cells and proliferation of CD4(-)CD8(+) (cytotoxic) and CD4(+)CD8(+) (helper) T cells, after in vitro re-stimulation. Despite significant differences in the magnitude and breadth of immune responses in the two vaccinated and mock treated groups, similar quantities of viral RNA were detected from the nasal cavity in all pigs after live virus challenge. The present study provides support for the use of the pig as a valid experimental model for influenza infections in humans, including the assessment of protective efficacy of therapeutic interventions
Magnetic resonance imaging-based scores of small vessel diseases: Associations with intracerebral haemorrhage location
Introduction: Total small vessel disease (SVD) score and cerebral amyloid angiopathy (CAA) score are magnetic
resonance imaging-based composite scores built to preferentially capture deep perforator arteriopathy-related
and CAA-related SVD burden, respectively. Non-lobar intracerebral haemorrhage (ICH) is related to deep
perforator arteriopathy, while lobar ICH can be associated with deep perforator arteriopathy or CAA; however,
the associations between ICH location and these scores are not established.
Methods: In this post-hoc analysis from a prospective cohort study, we included 153 spontaneous non-cerebellar
ICH patients. Wald test, univariable and multivariable logistic regression analysis were performed to investigate
the association between each score (and individual score components) and ICH location.
Results: Total SVD score was associated with non-lobar ICH location (Wald test: unadjusted, p = 0.017; adjusted,
p = 0.003); however, no individual component of total SVD score was significantly associated with non-lobar
ICH. CAA score was not significantly associated with lobar location (Wald test: unadjusted, p = 0.056;
adjusted, p = 0.126); cortical superficial siderosis (OR 8.85 [95%CI 1.23–63.65], p = 0.030) and ≥ 2 strictly
lobar microbleeds (OR 1.63 [95%CI 1.04–2.55], p = 0.035) were related with lobar ICH location, while white
matter hyperintensities showed an inverse relation (OR 0.53 [95%CI 0.26–1.08; p = 0.081]).
Conclusions: Total SVD score was associated with non-lobar ICH location. The lack of significant association
between CAA score and lobar ICH may in part be due to the mixed aetiology of lobar ICH, and to the inclusion of
white matter hyperintensities, a non-specific marker of SVD type, in the CAA score
"Me's me and you's you": Exploring patients' perspectives of single patient (n-of-1) trials in the UK
BACKGROUND: The n-of-1 trial offers a more methodologically sound approach to determining optimum treatment for an individual patient than "trials of therapy" routinely conducted in clinical practice. However, such methodology is rarely used in the UK. This pilot study explores the acceptability of n-of-1 trials to patients in the UK. METHODS: Patients with osteoarthritis of the knee were recruited to their own 12-week n-of-1 trial comparing either two knee supports or an NSAID with simple analgesic. Patients were interviewed at the start and completion of their trial to explore reasons for participation, understanding of the trial design and experiences of participation. Daily diaries were completed to inform future treatment. RESULTS: Nine patients participated (5 supports, 4 drugs). Patients were keen to participate, believing that the trial may lead to personal gains such as improved symptom control and quality of life. However, recruitment to the pharmacological comparison was more difficult since this could also entail risk. All patients were eager to complete the trial, even when difficulties were encountered. Completing the daily diary provided some patients with greater insight into their condition, which allowed them to improve their self-management. The n-of-1 trial design was viewed as a 'logical' design offering an efficient method of reaching a personalised treatment decision tailored to suit individual needs and preferences. CONCLUSION: This pilot study suggests that patients perceive the n-of-1 trial as an acceptable approach to the individualisation of treatment. In addition, further benefits over and above any gained from the interventions can be derived from involvement in such a study

A tool for functional brain imaging with lifespan compliance
The human brain undergoes significant functional and structural changes in the first decades of life, as the foundations for human cognition are laid down. However, non-invasive imaging techniques to investigate brain function throughout neurodevelopment are limited due to growth in head-size with age and substantial head movement in young participants. Experimental designs to probe brain function are also limited by the unnatural environment typical brain imaging systems impose. However, developments in quantum technology allowed fabrication of a new generation of wearable magnetoencephalography (MEG) technology with the potential to revolutionise electrophysiological measures of brain activity. Here we demonstrate a lifespan-compliant MEG system, showing recordings of high fidelity data in toddlers, young children, teenagers and adults. We show how this system can support new types of experimental paradigm involving naturalistic learning. This work reveals a new approach to functional imaging, providing a robust platform for investigation of neurodevelopment in health and disease
Research methods for subgrouping low back pain
<p>Abstract</p> <p>Background</p> <p>There is considerable clinician and researcher interest in whether the outcomes for patients with low back pain, and the efficiency of the health systems that treat them, can be improved by 'subgrouping research'. Subgrouping research seeks to identify subgroups of people who have clinically important distinctions in their treatment needs or prognoses. Due to a proliferation of research methods and variability in how subgrouping results are interpreted, it is timely to open discussion regarding a conceptual framework for the research designs and statistical methods available for subgrouping studies (a method framework). The aims of this debate article are: (1) to present a method framework to inform the design and evaluation of subgrouping research in low back pain, (2) to describe method options when investigating prognostic effects or subgroup treatment effects, and (3) to discuss the strengths and limitations of research methods suitable for the hypothesis-setting phase of subgroup studies.</p> <p>Discussion</p> <p>The proposed method framework proposes six phases for studies of subgroups: studies of assessment methods, hypothesis-setting studies, hypothesis-testing studies, narrow validation studies, broad validation studies, and impact analysis studies. This framework extends and relabels a classification system previously proposed by McGinn et al (2000) as suitable for studies of clinical prediction rules. This extended classification, and its descriptive terms, explicitly anchor research findings to the type of evidence each provides. The inclusive nature of the framework invites appropriate consideration of the results of diverse research designs. Method pathways are described for studies designed to test and quantify prognostic effects or subgroup treatment effects, and examples are discussed. The proposed method framework is presented as a roadmap for conversation amongst researchers and clinicians who plan, stage and perform subgrouping research.</p> <p>Summary</p> <p>This article proposes a research method framework for studies of subgroups in low back pain. Research designs and statistical methods appropriate for sequential phases in this research are discussed, with an emphasis on those suitable for hypothesis-setting studies of subgroups of people seeking care.</p
