A parametric analysis of DIAL sensitivity with heterodyne detection is presented and comparisons with direct detection are discussed. Examples are given for monitoring vertical distributions of O3, H2O, and NH3 using a ground-, aircraft-, or shuttle-based pulsed tunable CO2 laser DIAL system. Results indicate that maximum sensitivity at minimum laser energy per measurement requires multiple pulse operation with the energy per pulse selected so that the measured photon rate is approximately equal to the detector IF bandwidth. Measurement sensitivities can be maximized and interference effects minimized by fine adjustment of measurement frequencies using the tunability of high pressure lasers. Atmospheric trace constituent measurements with higher vertical resolution than attainable with passive radiometers are discussed. Infrared differential absorption lidar (DIAL), which depends on Mie scattering from aerosols, has special advantages for tropospheric and lower stratospheric applications and has great potential importance for measurements from shuttle and aircraft. Differential absorption lidar data reduction involves comparing large amplitude signals which have small differences. The accuracy of the trace constituent concentration inferred from DIAL measurements depends strongly on the errors in determining the amplitude of the signals. Thus, the commonly used SNR expression (signal divided by noise in the absence of signal) is not adequate to describe DIAL measurement accuracy and must be replaced by an expression which includes the random coherent (speckle) noise within the signal. A comprehensive DIAL computer algorithm is modified to include heterodyne detection and speckle noise. Examples for monitoring vertical distributions of O3, H2O, and NH3 using a ground-, aircraft-, or shuttle-based pulsed tunable CO2 laser DIAL system are given

Drug resistance in Plasmodium falciparum from the Chittagong Hill Tracts, Bangladesh.
OBJECTIVE: To assess the efficacy of antimalarial treatment and molecular markers of Plasmodium falciparum resistance in the Chittagong Hill Tracts of Bangladesh. METHODS: A total of 203 patients infected with P. falciparum were treated with quinine 3 days plus sulphadoxine/pyrimethamine (SP) combination therapy, and followed up during a 4-week period. Blood samples collected before treatment were genotyped for parasite mutations related to chloroquine (pfcrt and pfmdr1 genes) or SP resistance (dhfr and dhps). RESULTS: Of 186 patients who completed follow-up, 32 patients (17.2%) failed to clear parasitaemia or became positive again within 28 days after treatment. Recurring parasitaemia was related to age (chi(2) = 4.8, P < 0.05) and parasite rates on admission (t = 3.1, P < 0.01). PCR analysis showed that some of these cases were novel infections. The adjusted recrudescence rate was 12.9% (95% CI 8.1-17.7) overall, and 16.6% (95% CI 3.5-29.7), 15.5% (95% CI 8.3-22.7) and 6.9% (95% CI 0.4-13.4) in three age groups (<5 years, 5-14, > or =15). The majority of infections carried mutations associated with chloroquine resistance: 94% at pfcrt and 70% at pfmdr. Sp-resistant genotypes were also frequent: 99% and 73% of parasites carried two or more mutations at dhfr and dhps, respectively. The frequency of alleles at dhfr, dhps and pfmdr was similar in cases that were successfully treated and those that recrudesced. CONCLUSIONS: The clinical trial showed that quinine 3-days combined to SP is still relatively effective in the Chittagong Hill Tracts. However, if this regimen is continued to be widely used, further development of SP resistance and reduced quinine sensitivity are to be expected. The genotyping results suggest that neither chloroquine nor SP can be considered a reliable treatment for P. falciparum malaria any longer in this area of Bangladesh
Surgical valvulotomy for tricuspid valve stenosis in a dog
A 2 year 4 month old female neutered Labrador retriever was presented for evaluation right sided congestive heart failure. Echocardiographic examination revealed tricuspid valve dysplasia with only two small orifices in the valve resulting in severe tricuspid stenosis. The dog underwent a right fifth lateral intercostal thoracotomy and surgical tricuspid valvulotomy, under cardiopulmonary bypass. The stenosis was relieved by dividing the valve leaflets between the two orifices with continuation to the commissures, creating a ‘bi-leaflet’ valve. The dog made a good recovery initially with echocardiography at 48 hours after surgery showing a reduction in tricuspid valve E and A wave velocities and pressure half time (from 230 ms to to 65 ms). She was discharged five days after surgery with spironolactone, benazepril, pimobendan and clopidogrel. The dog was re-presented two days later having collapsed, with pyrexia, facial swelling and pitting edema on the ventral neck and intermandibular region. Investigations did not reveal an underlying cause and the clinical signs resolved with supportive therapy. Two years after surgery the dog was free of clinical signs with normal exercise tolerance and only mild tricuspid regurgitation on echocardiography, with discontinuation of all medications
HLA-Associated viral mutations are common in human immunodeficiency virus type 1 elite controllers
Elite controllers (EC) of human immunodeficiency virus type 1 (HTV-1) maintain viremia below the limit of detection without antiretroviral treatment. Virus-specific cytotoxic CD8+ T lymphocytes are believed to play a crucial role in viral containment, but the degree of immune imprinting and compensatory mutations in EC is unclear. We obtained plasma gag, pol, and nef sequences from HLA-diverse subjects and found that 30 to 40% of the predefined HLA-associated polymorphic sites show evidence of immune selection pressure in EC., compared to approximately 50% of the sites in chronic progressors. These data indicate ongoing viral replication and escape from cytotoxic T lymphocytes are present even in strictly controlled HTV-1 infection
System, Device, and Method for Determination of Intraocular Pressure
A system for determination of intraocular pressure includes: an intraocular pressure sensor; a light source illuminating the sensor with one or more wavelengths of light; and a detector that measures emitted light from the sensor. The sensor includes a substrate member, a spacer member, and a flexible membrane, which define a sealed cavity. The flexible membrane moves in response to intraocular pressure changes. A device for measuring intraocular pressure includes: the sensor; an anchoring member attached to the sensor for immobilizing the sensor in an eye; and a protective member attached to the anchoring member and covering the sensor to prevent contact between the flexible membrane and the eye. A method for determination of intraocular pressure includes: placing the sensor in an eye; illuminating, with a light source, the sensor with one or more wavelengths of light; and detecting, with a detector, a resultant light that contains information about intraocular pressure
Collective neutrino flavor transitions in supernovae and the role of trajectory averaging
Non-linear effects on supernova neutrino oscillations, associated with
neutrino self-interactions, are known to induce collective flavor transitions
near the supernova core for theta_13 \neq 0. In scenarios with very shallow
electron density profiles, these transformations have been shown to couple with
ordinary matter effects, jointly producing spectral distortions both in normal
and inverted hierarchy. In this work we consider a complementary scenario,
characterized by higher electron density, as indicated by post-bounce
shock-wave simulations. In this case, early collective flavor transitions are
decoupled from later, ordinary matter effects. Moreover, such transitions
become more amenable to both numerical computations and analytical
interpretations in inverted hierarchy, while they basically vanish in normal
hierarchy. We numerically evolve the neutrino density matrix in the region
relevant for self-interaction effects. In the approximation of averaged
intersection angle between neutrino trajectories, our simulations neatly show
the collective phenomena of synchronization, bipolar oscillations, and spectral
split, recently discussed in the literature. In the more realistic (but
computationally demanding) case of non-averaged neutrino trajectories, our
simulations do not show new significant features, apart from the smearing of
``fine structures'' such as bipolar nutations. Our results seem to suggest
that, at least for non-shallow matter density profiles, averaging over neutrino
trajectories plays a minor role in the final outcome. In this case, the swap of
nu_e and nu_{\mu,\tau} spectra above a critical energy may represent an
unmistakable signature of the inverted hierarchy, especially for theta_{13}
