Investigations on entropy layer along hypersonic hyperboloids using a defect boundary layer
A defect approach coupled with matched asymptotic expansions is used to derive a new set of boundary layer equations. This method ensures a smooth matching of the boundary layer with the inviscid solution. These equations are solved to calculate boundary layers over hypersonic blunt bodies involving the entropy gradient effect. Systematic comparisons are made for both axisymmetric and plane flows in several cases with different Mach and Reynolds numbers. After a brief survey of the entropy layer characteristics, the defect boundary layer results are compared with standard boundary layer and full Navier-Stokes solutions. The entropy gradient effects are found to be more important in the axisymmetric case than in the plane one. The wall temperature has a great influence on the results through the displacement effect. Good predictions can be obtained with the defect approach over a cold wall in the nose region, with a first order solution. However, the defect approach gives less accurate results far from the nose on axisymmetric bodies because of the thinning of the entropy layer

Time Variability in the X-ray Nebula Powered by Pulsar B1509-58
We use new and archival Chandra and ROSAT data to study the time variability
of the X-ray emission from the pulsar wind nebula (PWN) powered by PSR B1509-58
on timescales of one week to twelve years. There is variability in the size,
number, and brightness of compact knots appearing within 20" of the pulsar,
with at least one knot showing a possible outflow velocity of ~0.6c (assuming a
distance to the source of 5.2 kpc). The transient nature of these knots may
indicate that they are produced by turbulence in the flows surrounding the
pulsar. A previously identified prominent jet extending 12 pc to the southeast
of the pulsar increased in brightness by 30% over 9 years; apparent outflow of
material along this jet is observed with a velocity of ~0.5c. However, outflow
alone cannot account for the changes in the jet on such short timescales.
Magnetohydrodynamic sausage or kink instabilities are feasible explanations for
the jet variability with timescale of ~1.3-2 years. An arc structure, located
30"-45" north of the pulsar, shows transverse structural variations and appears
to have moved inward with a velocity of ~0.03c over three years. The overall
structure and brightness of the diffuse PWN exterior to this arc and excluding
the jet has remained the same over the twelve year span. The photon indices of
the diffuse PWN and possibly the jet steepen with increasing radius, likely
indicating synchrotron cooling at X-ray energies.Comment: accepted to ApJ, 14 pages, 8 figure
Non-detection of a pulsar-powered nebula in Puppis A, and implications for the nature of the radio-quiet neutron star RX J0822-4300
We report on a deep radio search for a pulsar wind nebula associated with the
radio-quiet neutron star RX J0822-4300 in the supernova remnant Puppis A. The
well-determined properties of Puppis A allow us to constrain the size of any
nebula to less than 30 arcsec; however we find no evidence for such a source on
any spatial scale up to 30 arcmin. These non-detections result in an upper
limit on the radio luminosity of any pulsar-powered nebula which is three
orders of magnitude below what would be expected if RX J0822-4300 was an
energetic young radio pulsar beaming away from us, and cast doubt on a recent
claim of X-ray pulsations from this source. The lack of a radio nebula leads us
to conclude that RX J0822-4300 has properties very different from most young
radio pulsars, and that it represents a distinct population which may be as
numerous, or even more so, than radio pulsars.Comment: 5 pages, including 2 embedded EPS figures, uses emulateapj.sty.

Developing a health state classification system from NEWQOL for epilepsy using classical psychometric techniques and Rasch analysis: a technical report
Aims: Resource allocation amongst competing health care interventions is informed by evidence of both clinical- and cost-effectiveness. Cost-utility analysis is increasingly used to assess cost effectiveness through the use of Quality Adjusted Life Years (QALYs). This requires health state values. Generic measures of health related quality of life (HRQL) are usually used to produce these values, but there are concerns about their relevance and sensitivity in epilepsy. This study develops a health state classification system for epilepsy from the NEWQOL battery, a validated questionnaire measuring QoL in epilepsy. The classification system will be amenable to valuation for calculating QALYs. Methods: Factor and other psychometric analyses were undertaken to investigate the factor structure of the battery, and assess the validity and responsiveness of the items. These analyses were used alongside Rasch analysis to select the dimensions included in the classification system, and the items used to represent each domain. Analysis was carried out on a trial dataset of patients with epilepsy (n=1611). Rasch and factor analysis were performed on one half of the sample and validated on the remaining half. Dimensions and items were selected that performed well across all analyses. Results: The battery was found to demonstrate reliability and validity but responsiveness across time periods for many of the items was low. A six dimension classification system was developed: worry about seizures, depression, memory, cognition, stigmatism and control, each with four response levels. Conclusions: It is feasible to develop a health state classification system from a battery of instruments using a combination of classical psychometric, factor and Rasch analysis. This is the first condition-specific health state classification developed for epilepsy and the next stage will produce preference weights to enable the measure to be used in cost-utility analysis.quality adjusted life years; health related quality of life; Rasch analysis; preference-based measures of health; health states; epilepsy
Cost effectiveness of using cognitive screening tests for detecting dementia and mild cognitive impairment in primary care.
INTRODUCTION: We estimated the cost effectiveness of different cognitive screening tests for use by General Practitioners (GPs) to detect cognitive impairment in England. METHODS: A patient-level cost-effectiveness model was developed using a simulated cohort that represents the elderly population in England (65 years and older). Each patient was followed over a lifetime period. Data from published sources were used to populate the model. The costs include government funded health and social care, private social care and informal care. Patient health benefit was measured and valued in Quality Adjusted Life Years (QALYs). RESULTS: Base-case analyses found that adopting any of the three cognitive tests (Mini-Mental State Examination, 6-Item Cognitive Impairment Test or GPCOG (General Practitioner Assessment of Cognition)) delivered more QALYs for patients over their lifetime and made savings across sectors including healthcare, social care and informal care compared with GP unassisted judgement. The benefits were due to early access to medications. Among the three cognitive tests, adopting the GPCOG was considered the most cost-effective option with the highest Incremental Net Benefit (INB) at the threshold of £30 000 per QALY from both the National Health Service and Personal Social Service (NHS PSS) perspective (£195 034 per 1000 patients) and the broader perspective that includes private social care and informal care (£196 251 per 1000 patients). Uncertainty was assessed in both deterministic and probabilistic sensitivity analyses. CONCLUSIONS: Our analyses indicate that the use of any of the three cognitive tests could be considered a cost-effective strategy compared with GP unassisted judgement.
