Forefoot pathology in rheumatoid arthritis identified with ultrasound may not localise to areas of highest pressure: cohort observations at baseline and twelve months
BackgroundPlantar pressures are commonly used as clinical measures, especially to determine optimum foot orthotic design. In rheumatoid arthritis (RA) high plantar foot pressures have been linked to metatarsophalangeal (MTP) joint radiological erosion scores. However, the sensitivity of foot pressure measurement to soft tissue pathology within the foot is unknown. The aim of this study was to observe plantar foot pressures and forefoot soft tissue pathology in patients who have RA.Methods A total of 114 patients with established RA (1987 ACR criteria) and 50 healthy volunteers were assessed at baseline. All RA participants returned for reassessment at twelve months. Interface foot-shoe plantar pressures were recorded using an F-ScanÂź system. The presence of forefoot soft tissue pathology was assessed using a DIASUS musculoskeletal ultrasound (US) system. Chi-square analyses and independent t-tests were used to determine statistical differences between baseline and twelve months. Pearsonâs correlation coefficient was used to determine interrelationships between soft tissue pathology and foot pressures.ResultsAt baseline, RA patients had a significantly higher peak foot pressures compared to healthy participants and peak pressures were located in the medial aspect of the forefoot in both groups. In contrast, RA participants had US detectable soft tissue pathology in the lateral aspect of the forefoot. Analysis of person specific data suggests that there are considerable variations over time with more than half the RA cohort having unstable presence of US detectable forefoot soft tissue pathology. Findings also indicated that, over time, changes in US detectable soft tissue pathology are out of phase with changes in foot-shoe interface pressures both temporally and spatially.Conclusions We found that US detectable forefoot soft tissue pathology may be unrelated to peak forefoot pressures and suggest that patients with RA may biomechanically adapt to soft tissue forefoot pathology. In addition, we have observed that, in patients with RA, interface foot-shoe pressures and the presence of US detectable forefoot pathology may vary substantially over time. This has implications for clinical strategies that aim to offload peak plantar pressures
Transformation of spin information into large electrical signals via carbon nanotubes
Spin electronics (spintronics) exploits the magnetic nature of the electron,
and is commercially exploited in the spin valves of disc-drive read heads.
There is currently widespread interest in using industrially relevant
semiconductors in new types of spintronic devices based on the manipulation of
spins injected into a semiconducting channel between a spin-polarized source
and drain. However, the transformation of spin information into large
electrical signals is limited by spin relaxation such that the magnetoresistive
signals are below 1%. We overcome this long standing problem in spintronics by
demonstrating large magnetoresistance effects of 61% at 5 K in devices where
the non-magnetic channel is a multiwall carbon nanotube that spans a 1.5 micron
gap between epitaxial electrodes of the highly spin polarized manganite
La0.7Sr0.3MnO3. This improvement arises because the spin lifetime in nanotubes
is long due the small spin-orbit coupling of carbon, because the high nanotube
Fermi velocity permits the carrier dwell time to not significantly exceed this
spin lifetime, because the manganite remains highly spin polarized up to the
manganite-nanotube interface, and because the interfacial barrier is of an
appropriate height. We support these latter statements regarding the interface
using density functional theory calculations. The success of our experiments

Development of hepatic pathology in GBVâBâinfected redâbellied tamarins (Saguinus labiatus)
GB virus B (GBVâB) is a new world monkeyâassociated flavivirus used to model acute hepatitis C virus (HCV) infection. Critical for evaluation of antiviral or vaccine approaches is an understanding of the effect of HCV on the liver at different stages of infection. In the absence of longitudinal human tissue samples at defined time points, we have characterized changes in tamarins. As early as 2 weeks postâinfection histological changes were noticeable, and these were established in all animals by 6 weeks. Despite high levels of liverâassociated viral RNA, there was reversal of hepatic damage on clearance of peripheral virus though fibrosis was demonstrated in four tamarins. Notably, viral RNA burden in the liver dropped to near undetectable or background levels in all animals which underwent a second viral challenge, highlighting the efficacy of the immune response in removing foci of replication in the liver. These data add to the knowledge of GBVâB infection in New World primates which can offer attractive systems for the testing of prophylactic and therapeutic treatments and the evaluation of their utility in preventing or reversing liver pathology
Exact ground states for the four-electron problem in a Hubbard ladder
The exact ground state of four electrons in an arbitrary large two leg
Hubbard ladder is deduced from nine analytic and explicit linear equations. The
used procedure is described, and the properties of the ground state are
analyzed. The method is based on the construction in r-space of the different
type of orthogonal basis wave vectors which span the subspace of the Hilbert
space containing the ground state. In order to do this, we start from the
possible microconfigurations of the four particles within the system. These
microconfigurations are then rotated, translated and spin-reversed in order to
build up the basis vectors of the problem. A closed system of nine analytic
linear equations is obtained whose secular equation, by its minimum energy
solution, provides the ground state energy and the ground state wave function

Hereditary angioedema: beyond international consensus - circa December 2010 - The Canadian Society of Allergy and Clinical Immunology Dr. David McCourtie Lecture
<p>Abstract</p> <p>Background</p> <p>The 2010 International Consensus Algorithm for the Diagnosis, Therapy and Management of Hereditary Angioedema was published earlier this year in this Journal (Bowen et al. <it>Allergy, Asthma & Clinical Immunology </it>2010, 6:24 - <url>http://www.aacijournal.com/content/6/1/24</url>). Since that publication, there have been multiple phase III clinical trials published on either prophylaxis or therapy of hereditary angioedema and some of these products have changed approval status in various countries. This manuscript was prepared to review and update the management of hereditary angioedema.</p> <p>Objective</p> <p>To review approaches for the diagnosis and management of hereditary angioedema (HAE) circa December 2010 and present thoughts on moving from HAE management from international evidence-based consensus to facilitate more local health unit considerations balancing costs, efficacies of treatments, and risk benefits. Thoughts will reflect Canadian and international experiences.</p> <p>Methods</p> <p>PubMed searches including hereditary angioedema and diagnosis, therapy, management and consensus were reviewed as well as press releases from various pharmaceutical companies to early December 2010.</p> <p>Results</p> <p>The 2010 International Consensus Algorithms for the Diagnosis, Therapy and Management of Hereditary Angioedema is reviewed in light of the newly published phase III Clinical trials for prevention and therapy of HAE. Management approaches and models are discussed.</p> <p>Conclusions</p> <p>Consensus approach and double-blind placebo controlled trials are only interim guides to a complex disorder such as HAE and should be replaced as soon as possible with large phase IV clinical trials, meta analyses, data base registry validation of approaches including quality of life and cost benefit analyses, safety, and head-to-head clinical trials investigating superiority or non-inferiority comparisons of available approaches. Since not all therapeutic products are available in all jurisdictions and since health care delivery approaches and philosophy vary between countries, each health care delivery sector will likely devise their own algorithms based on local practicalities for implementing evidence-based guidelines and standards for HAE disease management. Quality-of-life and cost affordability benefit conclusions will likely vary between countries and health care units. Data base registries for rare disorders like HAE should be used to detect early adverse events for new therapies and to facilitate phase IV clinical trials and encourage superiority and non-inferiority comparisons of HAE management approaches.</p
Charge asymmetries of top quarks at hadron colliders revisited
A sizeable difference in the differential production cross section of top-
compared to antitop-quark production, denoted charge asymmetry, has been
observed at the Tevatron. The experimental results seem to exceed the theory
predictions based on the Standard Model by a significant amount and have
triggered a large number of suggestions for "new physics". In the present paper
the Standard Model predictions for Tevatron and LHC experiments are revisited.
This includes a reanalysis of electromagnetic as well as weak corrections,
leading to a shift of the asymmetry by roughly a factor 1.1 when compared to
the results of the first papers on this subject. The impact of cuts on the
transverse momentum of the top-antitop system is studied. Restricting the ttbar
system to a transverse momentum less than 20 GeV leads to an enhancement of the
asymmetries by factors between 1.3 and 1.5, indicating the importance of an
improved understanding of the -momentum distribution. Predictions for
similar measurements at the LHC are presented, demonstrating the sensitivity of
the large rapidity region both to the Standard Model contribution and effects
