    Assertiveness bias in gender ethics research : why women deserve the benefit of the doubt

    Gender is one of the most researched and contentious topics in consumer ethics research. It is common for researchers of gender studies to presume that women are more ethical than men because of their reputation for having a selfless, sensitive nature. Nevertheless, we found evidence that women behaved less ethically than men in two field experiments testing a passive form of unethical behavior. Women benefited to a larger extent from a cashier miscalculating the bill in their favor than men. However, in three follow-up studies, we found that women did not necessarily intend to benefit at the expense of someone else. Women are less prone to speak up to a cashier than men are, even when the mistake is made in their disfavor. These results reveal that gender differences in assertiveness affect differences in unethical behavior

    Vacuum lamination of a stretchable sensor system in polypropylene

    The explosive growth of the so-called Internet-of-Things, where more and more everyday objects are becoming `smart' and connected, demands for reliable integration technologies for electronics in all kinds of materials. If we want these electronics to be as least intrusive as possible, they preferably conform to the shape of the contemplated object. In this article we want to present a technique that allows the integration of a smart sensor system in a thermoplastic material (polypropylene, PP) by vacuum lamination. This laminated stack can then be thermoformed from a flat sheet into the desired 3D shape. The sensor system in question is a sensor bus incorporating three inertial movement sensors. Each sensor is placed on a separate small, thin (200ÎĽm) FR4 PCB together with some necessary peripheral components. These smart sensor nodes are then placed on a flexible, stretchable circuit, which is then laminated between two 2mm-thick PP sheets. These sheets can subsequently be heated and thermoformed. In this particular case, the PP was used to create a smart ankle-foot orthosis (AFO). Although long-term reliability needs to be improved, we can show that the discussed technology allows for a successful lamination and thermoforming. This paper will elaborate on the sensor system, stretchable bus system and lamination technique, together with the encountered problems and implemented mitigations. While the current application is the aforementioned AFO, the sensor system can easily be expanded to other types of sensors and the thermoforming process allows for a wide range of possible applications

    The development of QUADAS : a tool for the quality assessment of studies of diagnostic accuracy included in systematic reviews

    BACKGROUND: In the era of evidence based medicine, with systematic reviews as its cornerstone, adequate quality assessment tools should be available. There is currently a lack of a systematically developed and evaluated tool for the assessment of diagnostic accuracy studies. The aim of this project was to combine empirical evidence and expert opinion in a formal consensus method to develop a tool to be used in systematic reviews to assess the quality of primary studies of diagnostic accuracy. METHODS: We conducted a Delphi procedure to develop the quality assessment tool by refining an initial list of items. Members of the Delphi panel were experts in the area of diagnostic research. The results of three previously conducted reviews of the diagnostic literature were used to generate a list of potential items for inclusion in the tool and to provide an evidence base upon which to develop the tool. RESULTS: A total of nine experts in the field of diagnostics took part in the Delphi procedure. The Delphi procedure consisted of four rounds, after which agreement was reached on the items to be included in the tool which we have called QUADAS. The initial list of 28 items was reduced to fourteen items in the final tool. Items included covered patient spectrum, reference standard, disease progression bias, verification bias, review bias, clinical review bias, incorporation bias, test execution, study withdrawals, and indeterminate results. The QUADAS tool is presented together with guidelines for scoring each of the items included in the tool. CONCLUSIONS: This project has produced an evidence based quality assessment tool to be used in systematic reviews of diagnostic accuracy studies.     Where is the supporting evidence for treating mild to moderate chronic obstructive pulmonary disease exacerbations with antibiotics? A systematic review

    BACKGROUND: Randomised trials comparing different drugs head-to-head are extremely valuable for clinical decision-making. However, it is scientifically and ethically sensible to demand strong evidence that a drug is effective by showing superiority over a placebo before embarking on head-to-head comparisons of potentially ineffective drugs. Our aim was to study the evolvement of evidence from placebo-controlled and head-to-head trials on the effects of antibiotics for the treatment of mild to moderate exacerbations of chronic obstructive pulmonary disease. METHODS: We conducted a historical systematic review. Through electronic databases and hand-searches, we identified placebo-controlled and head-to-head antibiotic trials for the treatment of mild to moderate chronic obstructive pulmonary disease exacerbations. We compared the numbers of patients recruited in placebo-controlled and head-to-head trials between 1957 and 2005. Using cumulative meta-analysis of placebo-controlled trials, we determined when, if ever, placebo-controlled trials had shown convincing evidence that antibiotics are effective in preventing treatment failure in patients with mild to moderate chronic obstructive pulmonary disease exacerbations. RESULTS: The first head-to-head trial was published in 1963. It was followed by another 100 trials comparing different antibiotics in a total of 34,029 patients with mild to moderate chronic obstructive pulmonary disease exacerbations. Over time, the cumulative odds ratio in placebo-controlled trials remained inconclusive throughout with odds ratios ranging from 0.39 (95% confidence intervals 0.04-4.22) to the most recent estimate (1995) of 0.81 (95% confidence intervals 0.52-1.28, P = 0.37). CONCLUSION: Placebo-controlled trials do not support the use of antibiotics in chronic obstructive pulmonary disease patients with mild to moderate exacerbations. Conducting head-to-head trials is, therefore, scientifically and ethically questionable. This underscores the requirement to perform or study systematic reviews of placebo-controlled trials before conducting head-to-head trials

    Can radiographers be trained to triage CT colonography for extracolonic findings?

    OBJECTIVES: Radiographers have been shown to be capable CT colonography observers. We evaluated whether radiographers can be trained to triage screening CT colonography for extracolonic findings. METHODS: Eight radiographers participated in a structured training program. They subsequently evaluated extracolonic findings in 280 low-dose CT colonograms (cases). This dataset contained 66 cases with possibly important findings (E3) and 27 cases with probably important findings (E4) [classification based on the highest classified finding (C-RADS)]. The first 40 and last 40 CT colonograms were identical test cases. Immediate feedback was given after each reading, except for test cases. Radiographers triaged cases based on C-RADS classification and indicated the need for a radiologist read. We constructed learning curves for correct case triaging by calculating moving averages. RESULTS: In the final test series, 84/120 (70 %) cases with E3 or E4 findings and 139/200 (70 %) without E3 or E4 findings were correctly triaged. Correct identification of cases with E3 findings improved with training from 46/88 (52 %) to 62/88 (70 %) (P < 0.0001) but not for E4 findings [both 22/32 (69 %) P = 1.00]. CONCLUSIONS: Radiographers improve after training in correctly triaging extracolonic findings at CT colonography but do not reach a high enough accuracy to consider their structural involvement in screening. KEY POINTS: • Radiographers were trained to triage CT colonography for extracolonic findings. • After training, radiographers improved sensitivity for likely unimportant findings. • After training, radiographers did not improve sensitivity for possibly important findings. • Radiographers should probably not be expected to identify all extracolonic findings

    Incidence and follow-up of Braunwald subgroups in unstable angina pectoris

    Objectives. This study was performed to establish the prognosis of patients with unstable angina within the subgroups of the Braunwald classification. Background. Among many classifications of unstable angina, the Braunwald classification is frequently used. However, the incidence and risk for each subgroup in clinical practice have not been established. Methods. Prospective data for 417 consecutive patients admitted for suspected unstable angina were analyzed. Patients were classified according to Braunwald criteria and followed up for 6 months. Survival, infarct-free survival and infarct-free survival without intervention are reported for each class. Results. After in-hospital observation the final diagnosis was acute myocardial infarction in 26 patients (6%), noncoronary chest pain in 109 (26%) and definite unstable angina in 282 (68%). Recurrence of chest pain was significantly different for the different severity classes (28%, 45% and 64% for classes I [accelerated angina], II [subacute angina at rest] and III [acute angina at rest], respectively) but not for clinical circumstances (49% and 53% for classes B [primary unstable angina]and C [postinfarction unstable angina], respectively). Six-month and infarct-free survival (96% and 88%, respectively) were not significantly different between severity classes but were significantly different (p = 0.01) between classes B (97% and 89%) and C (89% and 80%). Infarct-free survival without intervention was best for class II (72%), intermediate for class I (53%) and worst for class III (35%). In multivariate analysis, elderly age, male gender, hypertension, class C and maximal (intravenous) therapy were independent predictors for death; elderly age and class C for infarct-free survival; and male gender, class III, class C, electrocardiographic changes and maximal therapy were associated with infarct-free survival without intervention. Conclusions. Braunwald classification is an appropriate instrument to predict outcome. Risk stratification by these criteria provides a tool for patient selection in clinical trials and for evaluation of treatment strategies
