    Prevention of urinary tract infections in nursing homes: lack of evidence-based prescription?

    <p>Abstract</p> <p>Background</p> <p>Urinary tract infections (UTIs, including upper and lower symptomatic) are the most common infections in nursing homes and prevention may reduce patient suffering, antibiotic use and resistance. The spectre of agents used in preventing UTIs in nursing homes is scarcely documented and the aim of this study was to explore which agents are prescribed for this purpose.</p> <p>Methods</p> <p>We conducted a one-day, point-prevalence study in 44 Norwegian nursing homes during April-May 2006. Nursing home residents prescribed any agent for UTI prophylaxis were included. Information recorded was type of agent and dose, patient age and gender, together with nursing home characteristics. Appropriateness of prophylactic prescribing was evaluated with references to evidence in the literature and current national guidelines.</p> <p>Results</p> <p>The study included 1473 residents. 18% (n = 269) of the residents had at least one agent recorded as prophylaxis of UTI, varying between 0-50% among the nursing homes. Methenamine was used by 48% of residents prescribed prophylaxis, vitamin C by 32%, and cranberry products by 10%. Estrogens were used by 30% but only one third was for vaginal administration. Trimethoprim and nitrofurantoin were used as prophylaxis by 5% and 4%, respectively.</p> <p>Conclusions</p> <p>The agents frequently prescribed to prevent UTIs in Norwegian nursing homes lack documented efficacy including methenamine and vitamin C. Recommended agents like trimethoprim, nitrofurantoin and vaginal estrogens are infrequently used. We conclude that prescribing of prophylactic agents for UTIs in nursing homes is not evidence-based.</p

    Global Impact of the COVID-19 Pandemic on Cerebral Venous Thrombosis and Mortality

    Background and purpose: Recent studies suggested an increased incidence of cerebral venous thrombosis (CVT) during the coronavirus disease 2019 (COVID-19) pandemic. We evaluated the volume of CVT hospitalization and in-hospital mortality during the 1st year of the COVID-19 pandemic compared to the preceding year. Methods: We conducted a cross-sectional retrospective study of 171 stroke centers from 49 countries. We recorded COVID-19 admission volumes, CVT hospitalization, and CVT in-hospital mortality from January 1, 2019, to May 31, 2021. CVT diagnoses were identified by International Classification of Disease-10 (ICD-10) codes or stroke databases. We additionally sought to compare the same metrics in the first 5 months of 2021 compared to the corresponding months in 2019 and 2020 (ClinicalTrials.gov Identifier: NCT04934020). Results: There were 2,313 CVT admissions across the 1-year pre-pandemic (2019) and pandemic year (2020); no differences in CVT volume or CVT mortality were observed. During the first 5 months of 2021, there was an increase in CVT volumes compared to 2019 (27.5%; 95% confidence interval [CI], 24.2 to 32.0; P&lt;0.0001) and 2020 (41.4%; 95% CI, 37.0 to 46.0; P&lt;0.0001). A COVID-19 diagnosis was present in 7.6% (132/1,738) of CVT hospitalizations. CVT was present in 0.04% (103/292,080) of COVID-19 hospitalizations. During the first pandemic year, CVT mortality was higher in patients who were COVID positive compared to COVID negative patients (8/53 [15.0%] vs. 41/910 [4.5%], P=0.004). There was an increase in CVT mortality during the first 5 months of pandemic years 2020 and 2021 compared to the first 5 months of the pre-pandemic year 2019 (2019 vs. 2020: 2.26% vs. 4.74%, P=0.05; 2019 vs. 2021: 2.26% vs. 4.99%, P=0.03). In the first 5 months of 2021, there were 26 cases of vaccine-induced immune thrombotic thrombocytopenia (VITT), resulting in six deaths. Conclusions: During the 1st year of the COVID-19 pandemic, CVT hospitalization volume and CVT in-hospital mortality did not change compared to the prior year. COVID-19 diagnosis was associated with higher CVT in-hospital mortality. During the first 5 months of 2021, there was an increase in CVT hospitalization volume and increase in CVT-related mortality, partially attributable to VITT

    Properties of Stereocomplex PLA for Melt Spinning

    Fibers made from biopolymers are one solution for conserving both resources and the environment. However, these fibers currently have limited strengths, which limit their use for textile applications. In this paper, a biopolymer stereocomplex poly(-lactide) (scPLA) formation on a technical scale of high-molecular-weight poly(D-lactide) (PDLA) and poly(L-lactide) (PLLA) is presented. This scPLA material is the basis for further research to develop scPLA yarns in melt spinning with technical strengths for technical application. scPLA is compared with standard and commercially available semi-crystalline PLA for the production of fibers in melt spinning (msPLA) with textile strengths. Differential scanning calorimetry (DSC) gives a degree of crystallization of 59.7% for scPLA and 47.0% for msPLA. X-ray diffraction (XRD) confirms the pure stereocomplex crystal structure for scPLA and semi-crystallinity for msPLA. scPLA and msPLA are also compared regarding their processing properties (rheology) in melt spinning. While complex viscosity of scPLA is much lower compared to msPLA, both materials show similar viscoelastic behavior. Thermal gravimetric analysis (TGA) shows the influence of the molecular weight on the thermal stability, whereas essentially the crystallinity influences the biodegradability of the PLA materials

    Enhanced Cardio Vascular Image Analysis by Combined Representation of Results from Dynamic MRI and Anatomic CTA

    The diagnosis support in the field of coronary artery disease (CAD) is very complex due to the numerous symptoms and performed studies leading to the final diagnosis. CTA and MRI are on their way to replace invasive catheter angiography. Thus, there is a need for sophisticated software tools that present the different analysis results, and correlate the anatomical and dynamic image information. We introduce a new software assistant for the combined result visualization of CTA and MR images, in which a dedicated concept for the structured presentation of original data, segmentation results, and individual findings is realized. Therefore, we define a comprehensive class hierarchy and assign suitable interaction functions. User guidance is coupled as closely as possible with available data, supporting a straightforward workflow design. The analysis results are extracted from two previously developed software assistants, providing coronary artery analysis and measurements, function analysis as well as late enhancement data investigation. As an extension we introduce a finding concept directly relating suspicious positions to the underlying data. An affine registration of CT and MR data in combination with the AHA 17-segment model enables the coupling of local findings to positions in all data sets. Furthermore, sophisticated visualization in 2D and 3D and interactive bull’s eye plots facilitate a correlation of coronary stenoses and physiology.     New Software Assistants for Cardiovascular Diagnosis

    Abstract: An important design criterion for current application development in cardiac image analysis is the applicability in clinical routine as well as in research environments. We present two novel applications dedicated to diagnosis support of the coronary heart disease. CT data of coronary arteries and cardiac function as well as MR data of myocardial perfusion are processed for quantitative anatomical and functional analysis. Semi-automatic segmentation methods are provided for a reliable representation of the coronary arteries and the left ventricular myocardium. Furthermore, quantitative parameters of the coronary vessels and the myocardium are computed. The novel approach is a standardization of the multi-parameter representation using advanced visualization techniques, that reveals possible correlations between vascular morphometry, ventricular function, and myocardial perfusion. Both prototypical applications are based on the visual programming and prototyping platform MeVisLab.

    Particulate HgS is generally thought to be the most abundant Hg species in stored petroleum hydrocarbons stocked on-shore. It is also assumed that due to its high stability constant, HgS is chemically inert. However, results from the current study would suggest otherwise. First, a comparison study of the analytical performance of ICP-MS and CV-AFS with respect to matrix effects showed no significant difference in the measured Hg concentrations in studied samples when CV-AFS is used in the alkali mode. Subsequently, the suitability of three quantification methods during Hg speciation was investigated. Both external calibration and standard addition methods proved troublesome with the former showing matrix dependence and the latter being hindered by the formation of an emulsion during the derivatization step. Results from species-specific isotope dilution (SS-ID-GC-ICP-MS), on the other hand, obtained at different equilibration times, showed a random variation in the calculated Hg<sup>2+</sup> concentration (RSD 32%), suggesting that factors independent of equilibration time cause the observed variation. Further sedimentation of Hg particulate by means of ultracentrifugation improved the precision of SS-ID-GC-ICP-MS by 10-fold. These results would suggest that Hg particles in petroleum products are reactive at low temperature during Grignard alkylation
