    Serum markers of B-cell activation in pregnancy during late gestation, delivery, and the postpartum period

    B cells are vital for the normal evolution of pregnancy due to their humoral and possible regulatory activities. Our group and others have documented that circulating B-cell subsets undergo changes from normal late pregnancy to the postpartum period. However, the underlying mechanisms are poorly understood. Therefore, this study examined the degree of B-cell activation in normal pregnancy by analyzing the levels of serum markers in healthy pregnant women during the third trimester of pregnancy, the day of delivery, and the postpartum period. METHOD OF STUDY: A prospective study including pregnant and non-pregnant women attending routine care was undertaken at a hospital clinic. Sociodemographic and clinical data were collected, along with peripheral blood samples. The serum levels of soluble CD23 (sCD23), B-cell-activating factor (BAFF), kappa (κ) and lambda (λ) free light chains (FLC), IgA, IgG, and IgM were quantified. RESULTS: Our study included 43 third trimester pregnant and 35 non-pregnant women. In the pregnant women, the median levels of sCD23, BAFF, IgG, and κ FLC were significantly higher during the postpartum period than during the third trimester of pregnancy. Compared to the non-pregnant women, the third trimester pregnant women had higher median BAFF levels and lower sCD23, IgA, IgG, and FLC levels. CONCLUSION: Changes in serum markers of B-cell kinetics that occur during pregnancy often persist into the postpartum period and affect the secretion of immunoglobulins from different classes.     Tree Nuts Anaphylaxis in Preschool Age Children

    Summary: Introduction. The incidence of food-induced anaphylaxis (FIA) is increasing in young children. Although the commonest culprits are cow's milk and egg, FIA to tree nuts (TNs) have been increasing. Objective. Characterization of children referred to our allergy department due to TNs-induced anaphylaxis (TNs-FIA) during preschool age. Materials and methods. We have retrospectively included 25 children with clinical history of preschool TNs-FIA, proven by allergological work-up. TNs sensitization was assessed by skin prick tests and/or specific IgE. Results. The mean age of the first anaphylactic episode was 3.1±1.2 years. The majority (92%) had an allergic disease (52% asthma). The implicated TNs were cashew (11 children), walnut (8), pine nut (5), hazelnut (2) and almond (1). The reaction occurred after the first known ingestion in 68%. In 92%, symptoms appeared within 30 minutes after exposure. The most frequent clinical symptoms were mucocutaneous (96%), respiratory (80%) and gastrointestinal (52%). Twenty-one children were admitted to the emergency department, although only 48% were treated with epinephrine. An underneath IgE-mediated mechanism was proven in all cases. Immunologic cross-reactivity with other TNs was identified in 84%, and with peanut in 36%. Overall, in our center, TNs-FIA represents 18% of all causes of FIA. Conclusions. In preschool age children with TNs-FIA, cashew and walnut were the commonest implicated nuts. Most reactions occurred briefly after exposure to minimal amounts of TNs, demonstrating the high potency of these allergens. About one-third also had peanut sensitization. Potentially life-threatening TNs allergy can occur early in childhood and adequate management should be undertaken.publishersversionpublishe

    Conformable Nanowire-in-Nanofiber Hybrids for Low-Threshold Optical Gain in the Ultraviolet

    The miniaturization of diagnostic devices that exploit optical detection schemes requires the design of light sources combining small size, high performance for effective excitation of chromophores, and mechanical flexibility for easy coupling to components with complex and nonplanar shapes. Here, ZnO nanowire-in-fiber hybrids with internal architectural order are introduced, exhibiting a combination of polarized stimulated emission, low propagation losses of light modes, and structural flexibility. Ultrafast transient absorption experiments on the electrospun material show optical gain which gives rise to amplified spontaneous emission with a threshold lower than the value found in films. These systems are highly flexible and can conveniently conform to curved surfaces, which makes them appealing active elements for various device platforms, such as bendable lasers, optical networks, and sensors, as well as for application in bioimaging, photo-cross-linking, and optogenetics

    Rhinitis is a common problem in childhood and adolescence and impacts negatively on physical, social and psychological well-being. This position paper, prepared by the European Academy of Allergy and Clinical Immunology Taskforce on Rhinitis in Children, aims to provide evidence-based recommendations for the diagnosis and therapy of paediatric rhinitis. Rhinitis is characterized by at least two nasal symptoms: rhinorrhoea, blockage, sneezing or itching. It is classified as allergic rhinitis, infectious rhinitis and nonallergic, noninfectious rhinitis. Similar symptoms may occur with other conditions such as adenoidal hypertrophy, septal deviation and nasal polyps. Examination by anterior rhinoscopy and allergy tests may help to substantiate a diagnosis of allergic rhinitis. Avoidance of relevant allergens may be helpful for allergic rhinitis (AR). Oral and intranasal antihistamines and nasal corticosteroids are both appropriate for first-line AR treatment although the latter are more effective. Once-daily forms of corticosteroids are preferred given their improved safety profile. Potentially useful add-on therapies for AR include oral leukotriene receptor antagonists, short bursts of a nasal decongestant, saline douches and nasal anticholinergics. Allergen-specific immunotherapy is helpful in IgE-mediated AR and may prevent the progression of allergic disease. There are still a number of areas that need to be clarified in the management of rhinitis in children and adolescents.publishersversionpublishe

    Frequência da Anafilaxia Induzida pelo Exercício numa Consulta de Imunoalergologia

    Introdução: A anafilaxia induzida pelo exercício (AIE) é uma forma rara de alergia física que ocorre na sequência de esforços físicos. A verdadeira incidência e prevalência da AIE permanecem por esclarecer, não existindo até à data dados publicados a nível nacional. Objectivos: Estimar a frequência da AIE no ambulatório de um serviço de Imunoalergologia e incrementar o conhecimento em relação a esta patologia. Métodos: De 7699 doentes observados na consulta de Imunoalergologia durante o período de um ano, incluímos os correspondentes a quadros de anafilaxia notificados pelo corpo clínico (“pelo menos um episódio de reacção sistémica grave”). Resultados: A AIE foi reportada em 5 de 103 doentes com história de anafilaxia; correspondendo a uma frequência de 0,06% na população observada na consulta. A média etária destes doentes era de 20,2 ± 10,3 anos (entre 10 e 37 anos) e a distribuição por sexo masculino/feminino de 4:1. Todos tinham história pessoal de atopia e de rinite alérgica; dois doentes (40%) tinham asma. As actividades desencadeantes das crises foram a corrida, o futebol, a natação e a dança. Todos os doentes tinham sintomas com o exercício dependente da ingestão prévia de alimentos: cereais em três doentes (trigo – dois, cevada – um), leguminosas em dois (amendoim – um, feijão -frade e feijão -verde – um); com teste cutâneo por picada positivo para os referidos alimentos. Conclusões: A AIE representa 5% dos casos de anafi laxia reportados. Todos os casos identifi cados apresentavam AIE dependente de alimentos, encontrando-se os doentes controlados com a evicção dos alimentos referidos 6 horas antes da prática de exercício e sendo portadores de dispositivo para autoadministração de adrenalina

    Efectos de un programa de actividad física en la composición corporal de escolares de 3-5 años

    Objectives: This paper focuses on studying the effects of a physical activity program on body composition of a group of school children. Method: This investigation involved 36 students (20 boys and 16 girls), aged between 3 and 5 years. The variables considered were: fat mass, fat-free mass, BMI (Body Mass Index), WHR (Waist to Hip Ratio). The body composition analysis was performed using bioelectrical impedance through the body fat monitor Tanita BC418-MA Segmental. The procedure was as follows: pre-test, intervention and post-test. The intervention consisted of 3 days perweek of high-intensity physical activity, 15 minutes per day, during 12 weeks. Results and Conclusions: Significant improvements in fat mass (Sig <0.05). No significant improvements in fat-free mass, BMIand WHR.Objetivos: Este artículo se centra en estudiar los efectos de un programa de actividad física sobre la composición corporal de ungrupo de escolares. Método: Han participado 36 escolares (20 niños y 16 niñas), entre los 3 y los 5 años de edad. Las variables estudiadas han sido: masa grasa, masa libre de grasa, IMC (Índice de Masa Corporal), ICC (Índice Cintura/Cadera). El análisis de la composición corporal se ha realizado mediante bioimpedancia eléctrica, por medio del Monitor de grasa corporal Tanita BC 418-MA Segmental. El procedimiento ha sido: pre-test, intervención y pos-test. La intervención ha consistido en 3 días a la semana de actividad física de alta intensidad, 15 minutos al día, durante 12 semanas. Resultados y conclusiones: Mejoras significativas en la masa grasa (Sig. < 0.05). No se han encontrado mejoras significativas en la masa libre de grasa, el IMC y el ICC
