Integration of computer technology into the medical curriculum: The King's experience
King's College London has developed a new curriculum which prepares students to deliver a high‐quality clinical service. A variety of active‐learning techniques are utilized to produce a deep approach to learning with an emphasis on vocational needs in the domains of knowledge, attitudes and skills. Integration of academic and clinical studies, as recommended by the General Medical Council (GMC), poses a number of difficulties, particularly in terms of communication between disparate geographical sites. The new curriculum aims to take advantage of computer technology to supplement and enhance traditional methods of learning and to ensure that students are competent in a defined range of skills. To aid integration, all students joining the new course are offered notebook computers and enhanced access to desktop computers, both with facilities to link to the main network This allows students to use multimedia material incorporated into the new curriculum and to access online services from remote locations. Practice-based Qualitative Research: Participant Experiences of Walk-in Counselling and Traditional Counselling
Walk-in single session counselling is becoming a more widely used model for delivering mental health services across Ontario. This paper reports findings from the qualitative phase of a mixed method study, exploring the experiences of those attending walk-in counselling (WIC) model compared to the traditional service delivery model employing a wait list. We used a comparative case study design for the qualitative phase. Findings reveal that participant outcomes of the walk-in counselling model is influenced by accessibility, how a participant makes sense of the service, and the degree to which a participant is motivated and able to engage in counselling. WIC supports the mental health system by reducing wait lists associated with traditional service delivery models, and meeting the needs many people identify for immediate consultation. Other participants still perceive themselves as requiring ongoing counselling over time and involving in-depth exploration. This research supports health systems providing access to both models
How an online questionnaire can explore leadership teaching in an undergraduate curriculum.
OBJECTIVES: To design a tool to explore current leadership teaching in an undergraduate curriculum, using the medical leadership competency framework (MLCF)
DESIGN: An online questionnaire was designed based on the MLCF competences and sent to all course leads at Imperial College, London in Autumn 2011
SETTING: Imperial College, London
PARTICIPANTS: Sixty-nine course leads were invited to participate in the questionnaire study
MAIN OUTCOME MEASURES: Course leads were asked whether they teach each MLCF competence, which teaching methods they use, and how long they spend teaching each competency
RESULTS: Overall there was a 78% questionnaire response rate (54/69). From the questionnaires received it was possible to extrapolate results across the remaining courses to achieve a 100% response rate. We were then able to produce a map of current leadership teaching showing that all MLCF competences are taught to varying degrees across the curriculum. The tool does not however provide information on the quality of teaching provided, or what students learn
CONCLUSIONS: There is a strong emphasis on the development of teaching leadership skills to undergraduates in Tomorrow's Doctors 2009 (TD09). It is difficult to know what teaching occurs across the curriculum of a large medical school. The design of a simple, electronic questionnaire will enable medical schools to map their current leadership teaching to the TD09 outcomes. This will help to inform further curriculum development and integration as well as signposting of learning opportunities
The Walk-in Counselling Model of Service Delivery: Who Benefits Most?
The walk-in counselling (WIC) model of service delivery has been found to reduce psychological distress more quickly than a traditional model of service delivery involving a wait list. A question remains, however, as to the relative benefit of the WIC model for different client groups. The present study uses graphical inspection and multilevel modeling to conduct moderator analyses comparing two agencies, one with a WIC clinic and the other with a traditional wait list approach, and their relative impact on psychological distress. Key findings regarding the differential benefits for different types of presenting problems as well as clients at different stages of change are discussed.

Stress-Mediated Allee Effects Can Cause the Sudden Collapse of Honey Bee Colonies.
The recent rapid decline in global honey bee populations could have significant implications for ecological systems, economics and food security. No single cause of honey bee collapse has yet to be identified, although pesticides, mites and other pathogens have all been shown to have a sublethal effect. We present a model of a functioning bee hive and introduce external stress to investigate the impact on the regulatory processes of recruitment to the forager class, social inhibition and the laying rate of the queen. The model predicts that constant density-dependent stress acting through an Allee effect on the hive can result in sudden catastrophic switches in dynamical behaviour and the eventual collapse of the hive. The model proposes that around a critical point the hive undergoes a saddle-node bifurcation, and that a small increase in model parameters can have irreversible consequences for the entire hive. We predict that increased stress levels can be counteracted by a higher laying rate of the queen, lower levels of forager recruitment or lower levels of natural mortality of foragers, and that increasing social inhibition can not maintain the colony under high levels of stress. We lay the theoretical foundation for sudden honey bee collapse in order to facilitate further experimental and theoretical consideration
PD-L1 testing for lung cancer in the UK: recognizing the challenges for implementation.
A new approach to the management of non-small-cell lung cancer (NSCLC) has recently emerged that works by manipulating the immune checkpoint controlled by programmed death receptor 1 (PD-1) and its ligand programmed death ligand 1 (PD-L1). Several drugs targeting PD-1 (pembrolizumab and nivolumab) or PD-L1 (atezolizumab, durvalumab, and avelumab) have been approved or are in the late stages of development. Inevitably, the introduction of these drugs will put pressure on healthcare systems, and there is a need to stratify patients to identify those who are most likely to benefit from such treatment. There is evidence that responsiveness to PD-1 inhibitors may be predicted by expression of PD-L1 on neoplastic cells. Hence, there is considerable interest in using PD-L1 immunohistochemical staining to guide the use of PD-1-targeted treatments in patients with NSCLC. This article reviews the current knowledge about PD-L1 testing, and identifies current research requirements. Key factors to consider include the source and timing of sample collection, pre-analytical steps (sample tracking, fixation, tissue processing, sectioning, and tissue prioritization), analytical decisions (choice of biomarker assay/kit and automated staining platform, with verification of standardized assays or validation of laboratory-devised techniques, internal and external quality assurance, and audit), and reporting and interpretation of the results. This review addresses the need for integration of PD-L1 immunohistochemistry with other tests as part of locally agreed pathways and protocols. There remain areas of uncertainty, and guidance should be updated regularly as new information becomes available
Acanthamoeba genotype T4 from the UK and Iran and isolation of the T2 genotype from clinical isolates
The majority of the keratitis-causing Acanthamoeba isolates are genotype T4. In an attempt to determine whether predominance of T4 isolates in Acanthamoeba keratitis is due to greater virulence or greater prevalence of this genotype, Acanthamoeba genotypes were determined for 13 keratitis isolates and 12 environmental isolates from Iran. Among 13 clinical isolates, eight (61.5 %) belonged to T4, two (15.3 %) belonged to T3 and three (23 %) belonged to the T2 genotype. In contrast, the majority of 12 environmental isolates tested in the present study belonged to T2 (7/12, 58.3 %), followed by 4/12 T4 isolates (33.3 %). In addition, the genotypes of six new Acanthamoeba isolates from UK keratitis cases were determined. Of these, five (83.3 %) belonged to T4 and one was T3 (16.6 %), supporting the expected high frequency of T4 in Acanthamoeba keratitis. In total, the genotypes of 24 Acanthamoeba keratitis isolates from the UK and Iran were determined. Of these, 17 belonged to T4 (70.8 %), three belonged to T2 (12.5 %), three belonged to T3 (12.5 %) and one belonged to T11 (4.1 %), confirming that T4 is the predominant genotype (S2 = 4.167; P = 0.0412) in Acanthamoeba keratitis
Systematic studies of positron accumulation and transfer
The Swansea positron beam line is a system comprised of a two-pressure-stage nitrogen buffer gas accumulator followed by a third stage trap. This study investigates the ballistic transfer of positrons from the two-stage accumulator to the third stage trap, with the interest to improve the understanding and efficiency of so-called “stacking” technique. The re-trapping efficiency for a single transferhas been categorised by systematically investigating parameters describing the ejected cloud, such as its parallel and transverse energy within the re-trapping region, and its subsequent behaviour within the trap, such as expansion rate and lifetime. A greater than ninety percent transfer efficiency is possible for positrons ejected from the accumulator with a 2.7 eV parallel energy spread, and positron lifetimes in the third stage trap are seen to be greater than 43 s with trap aligned on axis injection of particles and decreases with further off axis transfer injections. A variety of particle manipulation techniques (such as the magnetron manipulation) have been used to systematically study the transfer efficiencies of multiple clouds and the dynamics of these stacked clouds within the trap
