    Urgent need to clarify the definition of chronic critical limb ischemia - a position paper from the European Society for Vascular Medicine

    Chronic critical lower limb ischemia (CLI) has been defined as ischemia that endangers the leg. An attempt was made to give a precise definition of CLI, based on clinical and hemodynamic data (Second European Consensus). CLI may be easily defined from a clinical point of view as rest pain of the distal foot or gangrene or ulceration. It is probably useful to add leg ulcers of other origin which do not heal because of severe ischemia, and to consider the impact of frailty on adverse outcome. From a hemodynamic viewpoint there is no consensus and most of the existing classifications are not based upon evidence. We should thus propose a definition and then validate it in a prospective cohort in order to define the patients at major risk of amputation, and also to define the categories of patients whose prognosis is improved by revascularisation. From today\u27s available data, it seems clear that the patients with a systolic toe pressure (STP) below 30 mmHg must be revascularised whenever possible. However other patients with clinically suspected CLI and STP above 30 mmHg must be evaluated and treated in specialised vascular units and revascularisation has to be discussed on a case by case basis, taking into account other data such as the WiFi classification for ulcers.In conclusion, many useful but at times contradictory definitions of CLI have been suggested. Only a few have taken into account evidence, and none have been validated prospectively. This paper aims to address this and to give notice that a CLI registry within Europe will be set up to prospectively validate, or not, the previous and suggested definitions of CLI

    Bulgarian sport policy 1945-1989: A strategic relation perspective

    The 2008 Beijing Olympic Games have stimulated discussions about the success of different sport systems and the Chinese model in particular. Revisiting explanations of sport in the former communist countries of Eastern Europe during the Cold War seems timely, as the current Chinese model of sport was largely designed after the Soviet example established in this period. This paper examines Bulgarian sport policy between 1945 and 1989. It employs a Strategic Relation approach (Jessop, 1990) to analyse sport policy making as a strategic relation closely linked to the dominant state project of building a new stateness. It goes beyond ideological interpretations and argues that the state represents a strategic terrain where these relations have to be established in struggles, the outcomes of which are always uncertain. Furthermore, past and present struggles and their outcomes create various socio-political environments that presuppose the forms of state selectivity and intervention in sport. The process of constructing sport policy was influenced by two main categories of strategic relations: intra-state, including political, organisational and personal relations between the Party, state apparatus and various sport and non-sport organisations and their managers, and transnational, concerning ideological, political, economic and organisational relations with both communist and western countries and international sport organisations

    Armadillo 1.1: An Original Workflow Platform for Designing and Conducting Phylogenetic Analysis and Simulations

    In this paper we introduce Armadillo v1.1, a novel workflow platform dedicated to designing and conducting phylogenetic studies, including comprehensive simulations. A number of important phylogenetic and general bioinformatics tools have been included in the first software release. As Armadillo is an open-source project, it allows scientists to develop their own modules as well as to integrate existing computer applications. Using our workflow platform, different complex phylogenetic tasks can be modeled and presented in a single workflow without any prior knowledge of programming techniques. The first version of Armadillo was successfully used by professors of bioinformatics at Université du Quebec à Montreal during graduate computational biology courses taught in 2010–11.     The Vein Patterning 1 (VEP1) Gene Family Laterally Spread through an Ecological Network

    Lateral gene transfer (LGT) is a major evolutionary mechanism in prokaryotes. Knowledge about LGT— particularly, multicellular— eukaryotes has only recently started to accumulate. A widespread assumption sees the gene as the unit of LGT, largely because little is yet known about how LGT chances are affected by structural/functional features at the subgenic level. Here we trace the evolutionary trajectory of VEin Patterning 1, a novel gene family known to be essential for plant development and defense. At the subgenic level VEP1 encodes a dinucleotide-binding Rossmann-fold domain, in common with members of the short-chain dehydrogenase/reductase (SDR) protein family. We found: i) VEP1 likely originated in an aerobic, mesophilic and chemoorganotrophic α-proteobacterium, and was laterally propagated through nets of ecological interactions, including multiple LGTs between phylogenetically distant green plant/fungi-associated bacteria, and five independent LGTs to eukaryotes. Of these latest five transfers, three are ancient LGTs, implicating an ancestral fungus, the last common ancestor of land plants and an ancestral trebouxiophyte green alga, and two are recent LGTs to modern embryophytes. ii) VEP1's rampant LGT behavior was enabled by the robustness and broad utility of the dinucleotide-binding Rossmann-fold, which provided a platform for the evolution of two unprecedented departures from the canonical SDR catalytic triad. iii) The fate of VEP1 in eukaryotes has been different in different lineages, being ubiquitous and highly conserved in land plants, whereas fungi underwent multiple losses. And iv) VEP1-harboring bacteria include non-phytopathogenic and phytopathogenic symbionts which are non-randomly distributed with respect to the type of harbored VEP1 gene. Our findings suggest that VEP1 may have been instrumental for the evolutionary transition of green plants to land, and point to a LGT-mediated ‘Trojan Horse’ mechanism for the evolution of bacterial pathogenesis against plants. VEP1 may serve as tool for revealing microbial interactions in plant/fungi-associated environments

    &lt;b&gt;&lt;/b&gt; Endovascular interventions and diagnostic examinations using iodinated contrast media (ICM) are standard of care in current vascular medicine. Although ICM use is generally considered safe, it may be associated with adverse reactions, vary from minor disturbances to rare, but severe life-threatening complications. This position paper of European Society of Vascular Medicine integrates current knowledge and summarizes the key information related to the use of intravascular ICM, serving as recommendation on prevention and management of acute, late, and very late adverse reactions. It should help the health professionals in all fields of vascular medicine to make decisions in daily practice for safe use of contrast media

    Results from the first engineering run of BOLOCAM and plans for the future

    We present results from the engineering run of the BOLOCAM receiver at the Caltech Submillimeter Observatory in May, 2000 and simulations of the sensitivity of the completed BOLOCAM receiver to astrophysical sources. BOLOCAM is a 144 element array of bolometers designed to operate in the millimeter wave atmospheric windows at 150, 220, and 280 GHz. These bands are well suited to studies of the Sunyaev-Zel'dovich effect in distant clusters of galaxies as well as measurements of the emission of interstellar dust
