Characterizing the diurnal patterns of errors in the prediction of evapotranspiration by several land‐surface models: An NACP analysis
Land‐surface models use different formulations of stomatal conductance and plant hydraulics, and it is unclear which type of model best matches the observed surface‐atmosphere water flux. We use the North American Carbon Program data set of latent heat flux (LE) measurements from 25 sites and predictions from 9 models to evaluate models' ability to resolve subdaily dynamics of transpiration. Despite overall good forecast at the seasonal scale, the models have difficulty resolving the dynamics of intradaily hysteresis. The majority of models tend to underestimate LE in the prenoon hours and overestimate in the evening. We hypothesize that this is a result of unresolved afternoon stomatal closure due to hydrodynamic stresses. Although no model or stomata parameterization was consistently best or worst in terms of ability to predict LE, errors in model‐simulated LE were consistently largest and most variable when soil moisture was moderate and vapor pressure deficit was moderate to limiting. Nearly all models demonstrate a tendency to underestimate the degree of maximum hysteresis which, across all sites studied, is most pronounced during moisture‐limited conditions. These diurnal error patterns are consistent with models' diminished ability to accurately simulate the natural hysteresis of transpiration. We propose that the lack of representation of plant hydrodynamics is, in part, responsible for these error patterns. Discovery of mating in the major African livestock pathogen Trypanosoma congolense
The protozoan parasite, Trypanosoma congolense, is one of the most economically important pathogens of livestock in Africa and, through its impact on cattle health and productivity, has a significant effect on human health and well being. Despite the importance of this parasite our knowledge of some of the fundamental biological processes is limited. For example, it is unknown whether mating takes place. In this paper we have taken a population genetics based approach to address this question. The availability of genome sequence of the parasite allowed us to identify polymorphic microsatellite markers, which were used to genotype T. congolense isolates from livestock in a discrete geographical area of The Gambia. The data showed a high level of diversity with a large number of distinct genotypes, but a deficit in heterozygotes. Further analysis identified cryptic genetic subdivision into four sub-populations. In one of these, parasite genotypic diversity could only be explained by the occurrence of frequent mating in T. congolense. These data are completely inconsistent with previous suggestions that the parasite expands asexually in the absence of mating. The discovery of mating in this species of trypanosome has significant consequences for the spread of critical traits, such as drug resistance, as well as for fundamental aspects of the biology and epidemiology of this neglected but economically important pathogen
Influence of Spring and Autumn Phenological Transitions on Forest Ecosystem Productivity
We use eddy covariance measurements of net ecosystem productivity (NEP) from 21 FLUXNET sites (153 site-years of data) to investigate relationships between phenology and productivity (in terms of both NEP and gross ecosystem photosynthesis, GEP) in temperate and boreal forests. Results are used to evaluate the plausibility of four different conceptual models. Phenological indicators were derived from the eddy covariance time series, and from remote sensing and models. We examine spatial patterns (across sites) and temporal patterns (across years); an important conclusion is that it is likely that neither of these accurately represents how productivity will respond to future phenological shifts resulting from ongoing climate change. In spring and autumn, increased GEP resulting from an ¿extra¿ day tends to be offset by concurrent, but smaller, increases in ecosystem respiration, and thus the effect on NEP is still positive. Spring productivity anomalies appear to have carry-over effects that translate to productivity anomalies in the following autumn, but it is not clear that these result directly from phenological anomalies. Finally, the productivity of evergreen needleleaf forests is less sensitive to phenology than is productivity of deciduous broadleaf forests. Integration of MODIS land and atmosphere products with a coupled-process model to estimate gross primary productivity and evapotranspiration from 1 km to global scales
We propose the Breathing Earth System Simulator (BESS), an upscaling approach to quantify global gross primary productivity and evapotranspiration using MODIS with a spatial resolution of 1-5 km and a temporal resolution of 8 days. This effort is novel because it is the first system that harmonizes and utilizes MODIS Atmosphere and Land products on the same projection and spatial resolution over the global land. This enabled us to use the MODIS Atmosphere products to calculate atmospheric radiative transfer for visual and near infrared radiation wave bands. Then we coupled atmospheric and canopy radiative transfer processes, with models that computed leaf photosynthesis, stomatal conductance and transpiration on the sunlit and shaded portions of the vegetation and soil. At the annual time step, the mass and energy fluxes derived from BESS showed strong linear relations with measurements of solar irradiance (r(2) = 0.95, relative bias: 8%), gross primary productivity (r(2) = 0.86, relative bias: 5%) and evapotranspiration (r(2) = 0.86, relative bias: 15%) in data from 33 flux towers that cover seven plant functional types across arctic to tropical climatic zones. A sensitivity analysis revealed that the gross primary productivity and evapotranspiration computed in BESS were most sensitive to leaf area index and solar irradiance, respectively. We quantified the mean global terrestrial estimates of gross primary productivity and evapotranpiration between 2001 and 2003 as 118 +/- 26 PgC yr(-1) and 500 +/- 104 mm yr(-1) (equivalent to 63,000 +/- 13,100 km(3) yr(-1)), respectively. BESS-derived gross primary productivity and evapotranspiration estimates were consistent with the estimates from independent machine-learning, data-driven products, but the process-oriented structure has the advantage of diagnosing sensitivity of mechanisms. The process-based BESS is able to offer gridded biophysical variables everywhere from local to the total global land scales with an 8-day interval over multiple years.Keywords: Energy fluxes,

Infant Feeding Information, Attitudes and Practices: a Longitudinal Survey in Central Nepal
Background: Infant feeding is governed by environmental as well as cultural factors. Breastfeeding knowledge and attitudes are known to be associated with breastfeeding duration. This study investigated breastfeeding information, attitudes and supplementary feeding in the central hills district of Nepal.Methods: A community-based prospective cohort study of 701 pregnant women was conducted. Information on breastfeeding attitudes, feeding practices and supplementary feeding was sought from the cohort at 4 weeks, 12 weeks and 22 weeks postpartum through repeated interviews using validated questionnaires.Results: Average duration of intended breastfeeding was 28 months (SD 7.9) and average target time to introduce solid foods was 6.1 months (SD 1.2). About 80% of women reported their husband, mother/mother-in-law preferred breastfeeding. Eleven percent of the cohort said that breastfeeding was not enjoyable. At 12 weeks and 22 weeks after birth, about a quarter (24.8%) and half (52.8%) of the infants were introduced cow/buffalo milk, respectively, while only 6.3% and 13.4% of them were given infant formula. Overall, any breastfeeding rate remained high at over 98% throughout the follow up period.Conclusions: Breastfeeding attitudes were encouraging in this population. Breastfeeding was almost universal. Use of infant formula was quite low, whereas cow or buffalo milk appeared to be popular supplementary foods

Metabolomic Analysis in Severe Childhood Pneumonia in The Gambia, West Africa: Findings from a Pilot Study
Pneumonia remains the leading cause of death in young children globally and improved diagnostics are needed to better identify cases and reduce case fatality. Metabolomics, a rapidly evolving field aimed at characterizing metabolites in biofluids, has the potential to improve diagnostics in a range of diseases. The objective of this pilot study is to apply metabolomic analysis to childhood pneumonia to explore its potential to improve pneumonia diagnosis in a high-burden setting. and Random Forests (RF). ‘Unsupervised’ (blinded) data were analyzed by Principal Component Analysis (PCA), while ‘supervised’ (unblinded) analysis was by Partial Least Squares-Discriminant Analysis (PLS-DA) and Orthogonal Projection to Latent Structures (OPLS). Potential markers were extracted from S-plots constructed following analysis with OPLS, and markers were chosen based on their contribution to the variation and correlation within the data set. The dataset was additionally analyzed with the machine-learning algorithm RF in order to address issues of model overfitting and markers were selected based on their variable importance ranking. Unsupervised PCA analysis revealed good separation of pneumonia and control groups, with even clearer separation of the groups with PLS-DA and OPLS analysis. Statistically significant differences (p<0.05) between groups were seen with the following metabolites: uric acid, hypoxanthine and glutamic acid were higher in plasma from cases, while L-tryptophan and adenosine-5′-diphosphate (ADP) were lower; uric acid and L-histidine were lower in urine from cases. The key limitation of this study is its small size.Metabolomic analysis clearly distinguished severe pneumonia patients from community controls. The metabolites identified are important for the host response to infection through antioxidant, inflammatory and antimicrobial pathways, and energy metabolism. Larger studies are needed to determine whether these findings are pneumonia-specific and to distinguish organism-specific responses. Metabolomics has considerable potential to improve diagnostics for childhood pneumonia

Adrenaline to improve survival in out-of-hospital cardiac arrest : the PARAMEDIC2 RCT
Background
Adrenaline has been used as a treatment for cardiac arrest for many years, despite uncertainty about its effects on long-term outcomes and concerns that it may cause worse neurological outcomes.
Objectives
The objectives were to evaluate the effects of adrenaline on survival and neurological outcomes, and to assess the cost-effectiveness of adrenaline use.
Design
This was a pragmatic, randomised, allocation-concealed, placebo-controlled, parallel-group superiority trial and economic evaluation. Costs are expressed in Great British pounds and reported in 2016/17 prices.
Setting
This trial was set in five NHS ambulance services in England and Wales.
Participants
Adults treated for an out-of-hospital cardiac arrest were included. Patients were ineligible if they were pregnant, if they were aged < 16 years, if the cardiac arrest had been caused by anaphylaxis or life-threatening asthma, or if adrenaline had already been given.
Interventions
Participants were randomised to either adrenaline (1 mg) or placebo in a 1 : 1 allocation ratio by the opening of allocation-concealed treatment packs.
Main outcome measures
The primary outcome was survival to 30 days. The secondary outcomes were survival to hospital admission, survival to hospital discharge, survival at 3, 6 and 12 months, neurological outcomes and health-related quality of life through to 6 months. The economic evaluation assessed the incremental cost per quality-adjusted life-year gained from the perspective of the NHS and Personal Social Services. Participants, clinical teams and those assessing patient outcomes were masked to the treatment allocation.
Results
From December 2014 to October 2017, 8014 participants were assigned to the adrenaline (n = 4015) or to the placebo (n = 3999) arm. At 30 days, 130 out of 4012 participants (3.2%) in the adrenaline arm and 94 out of 3995 (2.4%) in the placebo arm were alive (adjusted odds ratio for survival 1.47, 95% confidence interval 1.09 to 1.97). For secondary outcomes, survival to hospital admission was higher for those receiving adrenaline than for those receiving placebo (23.6% vs. 8.0%; adjusted odds ratio 3.83, 95% confidence interval 3.30 to 4.43). The rate of favourable neurological outcome at hospital discharge was not significantly different between the arms (2.2% vs. 1.9%; adjusted odds ratio 1.19, 95% confidence interval 0.85 to 1.68). The pattern of improved survival but no significant improvement in neurological outcomes continued through to 6 months. By 12 months, survival in the adrenaline arm was 2.7%, compared with 2.0% in the placebo arm (adjusted odds ratio 1.38, 95% confidence interval 1.00 to 1.92). An adjusted subgroup analysis did not identify significant interactions. The incremental cost-effectiveness ratio for adrenaline was estimated at £1,693,003 per quality-adjusted life-year gained over the first 6 months after the cardiac arrest event and £81,070 per quality-adjusted life-year gained over the lifetime of survivors. Additional economic analyses estimated incremental cost-effectiveness ratios for adrenaline at £982,880 per percentage point increase in overall survival and £377,232 per percentage point increase in neurological outcomes over the first 6 months after the cardiac arrest.
Limitations
The estimate for survival with a favourable neurological outcome is imprecise because of the small numbers of patients surviving with a good outcome.
Conclusions
Adrenaline improved long-term survival, but there was no evidence that it significantly improved neurological outcomes. The incremental cost-effectiveness ratio per quality-adjusted life-year exceeds the threshold of £20,000–30,000 per quality-adjusted life-year usually supported by the NHS.
Future work
Further research is required to better understand patients’ preferences in relation to survival and neurological outcomes after out-of-hospital cardiac arrest and to aid interpretation of the trial findings from a patient and public perspective.
