    The chaperone balance hypothesis: The importance of the extracellular to intracellular HSP70 ratio to inflammation-driven type 2 diabetes, the effect of exercise, and the implications for clinical management

    Recent evidence shows divergence between the concentrations of extracellular 70 kDa heat shock protein [eHSP70] and its intracellular concentrations [iHSP70] in people with type 2 diabetes (T2DM). A vital aspect regarding HSP70 physiology is its versatility to induce antagonistic actions, depending on the location of the protein. For example, iHSP70 exerts a powerful anti-inflammatory effect, while eHSP70 activates proinflammatory pathways. Increased eHSP70 is associated with inflammatory and oxidative stress conditions, whereas decreased iHSP70 levels are related to insulin resistance in skeletal muscle. Serum eHSP70 concentrations are positively correlated with markers of inflammation, such as C-reactive protein, monocyte count, and TNF-a, while strategies to enhance iHSP70 (e.g., heat treatment, chemical HSP70 inducers or coinducers, and physical exercise) are capable of reducing the inflammatory profile and the insulin resistance state. Here, we present recent findings suggesting that imbalances in the HSP70 status, described by the [eHSP70]/[iHSP70] ratio, may be determinant to trigger a chronic proinflammatory state that leads to insulin resistance and T2DM development. This led us to hypothesize that changes in this ratio value could be used as a biomarker for the management of the inflammatory response in insulin resistance and diabetes

    Evaluation of an educational programme for socially deprived asthma patients

    The aim of this study was to evaluate the effectiveness of an asthma education programme in moderate and severe asthma patients in a longitudinal, prospective and randomized study with a control group. Fifty-three asthmatic patients were studied, 26 of whom were assigned to the educational group and 27 to the control group.The educational group attended the programme regularly for a period of 6 months. the programme included information about asthma, instruction on the appropriate use of medication and training in the metered dose inhaler (MDI) technique, and information about the identification and control of asthma attacks and the recognition of early signs of exacerbation. the control group was submitted to the routine tare provided at the Asthma Clinic, with no formal instruction regarding asthma control. the groups were identical with regard to severity parameters, skills, lung function and quality of Life at the beginning of the trial.At the end of the study, the education group showed significant differences when compared with the control group education/control (mean values) with respect to: visits to the asthma emergency room over the previous 6 months, 0.7/2 (p=0.03); nocturnal symptoms, 0.3/0.7 (p=0.04); score of symptoms, 1.3/2 (p=0.04). Improvements were also observed in skills and quality of life, knowledge of how to deal with attacks and how to control the environmental triggering factors, 73/35 (<0.05); correct use of the MDI, 8/4 (0.001); understanding of the difference between relief and antiinflammatory medication, 86/20 (<0.05); and in the global limitation quality of life score, 28/50 (0.02).It is concluded that the educational programme led to a significant improvement in asthma morbidity and that the implantation of educational programmes is possible for special populations when these programmes are adapted to the socioeconomic profile of the patients, with a significant gain in terms of the reduction of symptoms and improved pulmonary function and quality of life of asthmatics.Universidade Federal de São Paulo, Dept Med, Lund Div, São Paulo, BrazilUniversidade Federal de São Paulo, Dept Med, Lund Div, São Paulo, BrazilWeb of Scienc

    Economic evaluation of dried banana production of the organic and conventional systems

    This work reports an economic evaluation of the dried banana production from an agroindustry located in Guaraqueçaba - PR State, Brazil. The conventional and the organic banana processings were evaluated by comparing the economic viability pointers. The dried organic banana is exported to the Europe and the dried conventional banana is commercialized in the region of Curitiba - PR. Both processings presented positive economic viability, however the dried organic banana presented better indices (TIR 94%, VPL R486,009.39andbenefitcostrelationof2.11)thantheconventionaldriedbanana(TIR14 486,009.39 and benefit cost relation of 2.11) than the conventional dried banana (TIR 14%, VPL R 34,668.00 and benefit cost relation of 1.17). The dried organic banana presented a cost of production of R3.64,being50.1 3.64, being 50.1% relative to the expense with insumos and 27% with labour. The dried conventional banana presented a cost of R 3.21, being 45.3% for insumos and 31.2% for labour.Neste trabalho, foi realizada a avaliação econômica da produção de banana-passa de uma agroindústria localizada no município de Guaraqueçaba - PR. Foi avaliado o processamento da banana-passa convencional e da banana orgânica produzida na região, comparando-se os indicadores de viabilidade econômica. A banana-passa orgânica é exportada para a Europa e a banana-passa convencional é comercializada na região de Curitiba - PR. Ambos os processamentos apresentaram viabilidade econômica positiva, apresentando a banana-passa orgânica os melhores índices (TIR 94%, VPL R486.009,39erelac\ca~obenefıˊcio−custode2,11)doqueabanana−passaconvencional(TIR14 486.009,39 e relação benefício-custo de 2,11) do que a banana-passa convencional (TIR 14%, VPL R 34.668,00 e relação benefício-custo de 1,17).     Hemispheric asymmetries in the ionospheric response observed in the American sector during an intense geomagnetic storm

    The main purpose of this investigation is to study the ionospheric F region response induced by the intense geomagnetic storm that occurred on 7-8 September 2002. The geomagnetic index Dst reached a minimum of -181 nT at 0100 UT on 8 September. In this study, we used observations from a chain of 12 GPS stations and another chain of 6 digital ionosonde stations. It should be mentioned that, soon after the sudden commencement (SC) at 1637 UT on 7 September, the TEC variations at midlatitude stations in both hemispheres showed an F region positive storm phase. However, during the recovery phase, a strong hemispheric asymmetry was observed in the ionospheric response. While a TID type soliton was observed to propagate in the Southern American sector, no TID activity was seen in the Northern American sector. Also, in the Southern Hemisphere, the TEC variations were less affected by the geomagnetic storm. The Northern Hemisphere observations showed a strong and long-lasting negative F region storm phase starting at about 1000 UT on 8 September (lasting for about 24 h). A perusal of TEC phase fluctuations and equatorial spread-F (ESF) ionospheric sounding data indicates that, on the disturbed night of 7-8 September, some stations showed the occurrence of ESF starting at about 0000 UT (2000 LT) on 8 September, whereas other stations showed that the ESF occurrence started much later, at about 0800 UT (0500 LT). This hemispheric asymmetric response of the ionospheric F region possibly indicates the presence of different mechanisms for the generation of ESF along the various latitudinal regions during the disturbed period.Facultad de Ciencias Astronómicas y Geofísica
