    Papillary Renal Cell Carcinoma Arising in a Lymph Node Metastasis of a Testicular Teratoma: A Very Rare Occurrence

    We present a case of a teratoma with somatic type malignancy (TSM) in the form of papillary renal cell carcinoma (pRCC) within supraclavicular and retroperitoneal lymph node metastases of a testicular pure teratoma. Resection of both masses revealed a teratoma without any other germ cell tumor component. A papillary carcinoma component was also detected intermingled with the teratomatous elements. The carcinoma cells displayed eosinophilic cytoplasm and prominent nucleoli. Groups of foamy histiocytes in the fibrovascular cores was a striking finding that brought pRCC to mind. Immunoreactivity for CK7, PAX8, AMACR, CD10, napsin, and vimentin along with morphologic findings confirmed renal cell differentiation. No radiological evidence of a primary renal cell carcinoma was found in the kidney. Consequently, pRCC arising in a teratoma was diagnosed. TSM is described as teratoma with a malignant component that is typically encountered in other organs and tissues. TSM in the form of pRCC is an extremely rare entity. Our case is the second example of a testicular germ cell tumor metastasis with a somatic malignancy in the form of pRCC. In conclusion, carcinomas of renal cell differentiation should be kept in mind as a rare form of TSM, especially in metastatic germ cell tumors

    Role of Agricultural Diversification in Improving Resilience to Climate Change: An Empirical Analysis with Gaussian Paradigm

    Agricultural diversification efforts towards sustainable agriculture generates environmental and economic benefits. Climate change and agricultural production are characterized by a complex cause-effect relationship. In the present study, the primary dataset is collected through an interview-based survey from 410 farmers in 3 districts located in different agro-ecological zones of Punjab, Pakistan. Detailed analysis is conducted by employing the Gaussian treatment effects approach. Results of the study show that the farmers who adopted agricultural diversification to mitigate the impact of climate change were less and insignificantly benefited e.g., on an average of RS 95,260 (US 635)perannumwhereasnonadoptedfarmerslosttheirfarmincomeonanaverageofRS115,750(US635) per annum whereas non-adopted farmers lost their farm income on an average of RS 115,750 (US 772) per annum if they had practiced the agricultural diversification. Moreover, determinants of agricultural diversification such as demographic and institutional indicators were significant and larger effects to adopt as compared to social indicators. This study suggests that policies should be designed in the regional context particularly related to the improvement in demographic characteristics and institutional factors such as providing subsidies, training, and awareness to the farmers, particularly to those who practice agricultural diversification. These measures will help to raise the farmers’ adaptive capacity for the adoption of agricultural diversification, and it will enable them to generate tangible benefits by increasing income through adopting sustainable agricultural livelihood

    Induced locomotor hyperactivity on ethanol withdrawal syndrome in rats is inhibited by quetiapine.

    Our aim is to investigate the effects of two atypical antipsychotics; quetiapine and olanzapine on locomotor activity that is a sign of ethanol withdrawal syndrome in rats. Adult male Wistar rats were subjects. Ethanol (7.2%, v/v) was given to rats by a liquid diet for 30 days. Control rats were pair fed an isocaloric liquid diet containing sucrose as a caloric substitute to ethanol. Quetiapine (10 mg/kg), olanzapine (5 mg/kg) and saline were injected to the rats intraperitoneally 7 days after ethanol withdrawal syndrome and the last one 30 min before ethanol withdrawal testing. After 2nd hour of ethanol withdrawal, rats were observed for 5 min and withdrawal signs that included locomotor hyperactivity were recorded. We have found increased vertical and horizontal locomotor activity in ethanol withdrawal group to control and reduced vertical and horizontal locomotor activity in quetiapine-injected rats. In olanzapine injected rats were seen no reduced locomotor activity. Significant inhibitory effects were produced by quetiapine on the signs of ethanol withdrawal. Our results suggest that acute quetiapine treatment has some beneficial efects on ethanol withdrawal in rats. Thus, this drug may be useful for treatment of ethanol withdrawal syndrome

    Genotype-Phenotype correlations of SCARB2 associated clinical presentation: a case report and in-depth literature review

    Background Biallelic pathogenic variants in the SCARB2 gene have been associated with action myoclonus-renal failure (AMRF) syndrome. Even though SCARB2 associated phenotype has been reported to include typical neurological characteristics, depending on the localization and the feature of the pathogenic variants, clinical course and the presentations have been shown to differ. Case presentation Whole exome sequencing (WES) analysis revealed a homozygous truncating variant (p.N45MfsX88) in SCARB2 gene in the index case, and subsequent sanger sequencing analysis validated the variant in all affected family members from a Turkish family with the clinical characteristics associated with AMRF and related disorders. Intrafamilial clinical heterogeneity with common features including dysarthria, tremor and proteinuria, and distinct features such as peripheral neuropathy (PNP), myoclonus and seizures between the affected cases, was observed in the family. In-depth literature review enabled the detailed investigation of the reported variants associated with AMRF and suggested that while the type of the variant did not have a major impact on the course of the clinical characteristics, only the C terminal localization of the pathogenic variant significantly affected the clinical presentation, particularly the age at onset (AO) of the disease. Conclusions In this study we showed that biallelic SCARB2 pathogenic variants might cause a spectrum of common and distinct features associated with AMRF. Of those features while the common features include myoclonus (100%), ataxia (96%), tonic clonic seizures (82%), dysarthria (68%), tremor (65%), and renal impairment (62%), the uncommon features involve PNP (17%), hearing loss (6.8%), and cognitive impairment (13.7%). AO has been found to be significantly higher in the carriers of the p.G462DfsX34 pathogenic variant. SCARB2 pathogenic variants have not been only implicated in AMRF but also in the pathogenesis of Parkinson's disease (PD) and Gaucher disease (GD), suggesting the importance of genetic and functional studies in the clinical and the diagnostic settings.     Expanding the clinical and immunological phenotypes and natural history of MALT1 deficiency

    Purpose MALT1 deficiency is a combined immune deficiency characterized by recurrent infections, eczema, chronic diarrhea, and failure to thrive. Clinical and immunological characterizations of the disease have not been previously reported in large cohorts. We sought to determine the clinical, immunological, genetic features, and the natural history of MALT-1 deficiency. Methods The clinical findings and treatment outcomes were evaluated in nine new MALT1-deficient patients. Peripheral lymphocyte subset analyses, cytokine secretion, and proliferation assays were performed. We also analyzed ten previously reported patients to comprehensively evaluate genotype/phenotype correlation. Results The mean age of patients and disease onset were 33 +/- 17 and 1.6 +/- 0.7 months, respectively. The main clinical findings of the disease were recurrent infections (100%), skin involvement (100%), failure to thrive (100%), oral lesions (67%), chronic diarrhea (56%), and autoimmunity (44%). Eosinophilia and high IgE were observed in six (67%) and two (22%) patients, respectively. The majority of patients had normal T and NK cells, while eight (89%) exhibited reduced B cells. Immunoglobulin replacement and antibiotics prophylaxis were mostly ineffective in reducing the frequency of infections and other complications. One patient received hematopoietic stem cell transplantation (HSCT) and five patients died as a complication of life-threatening infections. Analyzing this cohort with reported patients revealed overall survival in 58% (11/19), which was higher in patients who underwent HSCT (P = 0.03). Conclusion This cohort provides the largest analysis for clinical and immunological features of MALT1 deficiency. HSCT should be offered as a curative therapeutic option for all patients at the early stage of life
