Utility of 'dual phase' cone beam computed tomography during radioembolisation in patients with hepatocellular carcinoma : what is really changing in flow dynamics before and after 90Y delivery?

Purpose: The aims of the study were: 1) to compare two phases of dual-phase cone beam computed tomography (DPCBCT) achieved before and after Yttrium-90 (90Y) administration and to evaluate additional benefits during radioembolisation (RE) procedures; and 2) to compare DP-CBCT with pre-procedure contrast enhanced cross-sectional images in terms of tumour detection. Material and methods: Twenty-three hepatocellular carcinoma patients undergoing RE treatment were scanned with DPCBCT consisting of early arterial (EA) and late arterial (LA) phases before and after 90Y administration. The CT-like datasets were compared according to embolisation effect, enhancement patterns, lesion detectability, image quality, and artifacts by two interventional radiologists blinded to each other. The compatibility of the two radiologists was evaluated with kappa statistical analysis, and the difference between EA and LA phases was evaluated with marginal homogeneity test. Also, DP-CBCT images were compared with preprocedural cross-sectional images (CT/MRI). Results: For 23 patients 92 data were acquired. Thirteen patients showed a decrease on post-embolisation images both visually and on Hounsfield unit (HU) measurements. No statistical difference was found for tumour detection between EA and LA phases (p = 1.0). Tumour enhancement was visually superior at LA phases whereas EA phases were better for arterial mapping for selective catheterisation. DP-CBCT images were not inferior to preprocedural cross-sectional imaging findings. Conclusions: DP-CBCT is a promising tool for predicting tumour response to therapy and is not inferior to preprocedural cross-sectional imaging in terms of tumour detection. It allows better assessment during RE procedures because early phases provide good mapping for superselective catheterisation whereas late phases are better for visualisation of tumour enhancement

The aim of the study is to determine what kind of an in-service teacher training program teachers want, in other words to determine how is the ideal in-service education according to the teachers. As it is aimed to expose the existing situation it is a descriptive modeled study. Quantitative research method has been used for the data collection, data analysis, and interpretation of the data. Analyzing the data conjoint analysis has been used and these analyses have been done with SPSS 11, 5 packet program. In addition in the study it has been researched whether the expectations of the teachers from an in-service teacher training program change according to gender and working experience of them. WOS: 000388115900002Objective: The aim of this research is to investigate the effectiveness of solution focused therapy which is based on interpersonal relationship theory in retrospective methodology. Methods: This research was planned as a pre/post test without control group. The research population included 36 students who are registered a school of health of a university and applied to psychological consultation services of their school for their psychological issues. Student's introduction and consulting form, Beck Anxiety Scale, Beck Depression Scale, Problem Solving Inventory were used as data collection forms. All ethic permission was provided. In this research, the five sessions solution focused therapy approach which is based on H. Peplau's interpersonal relationship theory was used for students. Inventories were applied both before and after the therapy sessions. Descriptive statistics, independent sample t test, Mann Witney U test, Kruskall Wallis test were used for data analysis. Conclusion: When the scores of anxiety, depression and problem solving skills were compared before and after sessions; anxiety, depression and problem solving inventory. They were found that the mean scores of inventories decreased as statistically significant. Differences of scores when participating joyful activities and having someone which can be shared for their individual problems in depression was found decreased as statistically significant. Also it can be seen the some direction when having actively a kind of hobby in anxiety and problem solving skill scores differences. Discussion: It can be said that the five sessions solution focused therapy which is based on Peplau's interpersonal relationship therapy was affected for students which are applied psychological consultation services for their problems

WOS: 000580607500002Objectives: the aim of this study is to determine the effect that guided imagery has on the functionality of individuals diagnosed with schizophrenia in a community mental health center. Methods: This is a pretest-posttest-controlled group study including 631 individuals registered in the Community Mental Health Center (CMHC) of Ege University Medical Faculty Hospital, of which 55 individuals were diagnosed with schizophrenia, as well as 300 individuals registered in the CMHC of Katip Celebi University Ataturk Training and Research Hospital, of which 30 individuals were diagnosed with schizophrenia, and who were receiving continuous daytime service. the study sample included 48 individuals that fit the inclusion criteria among the 85 individuals who benefitted from daytime CMHC. the study was completed with 24 individuals in the experimental group and 24 individuals in control group. the value of power analysis is 98.8%. As data collection tools, an Introductory Information Form, Functional Remission of General Schizophrenia Scale (FROGS), and Subjective Recovery Assessment Scale (SubRAS) were used. For the numerical measurements, Mann-Whitney U test, Wilcoxon t test, one-way ANOVA test, and Chi-squared test were used. Results: As a result of guided imagery applied to the study group 10 minutes daily for two weeks, a statistically significant difference was detected between the mean score of daily living skills subscale of FROGS (z=-2.69, p=0.01); mean score of treatment subscale (z=-2.37, p=0.01); overall mean score of FROGS (z=-2.41, p=0.01); mean score of SubRAS (z=-3.70, p=0.01). However, no significant difference could be found between the mean scores of social functioning subscale of FROGS (z=-1.80, p=0.07) and the mean scores of professional functioning in FROGS (z=-0.46, p=0.64). Conclusion: According to the results of the study, it can be said that the guided imagery application for two weeks has created a significant difference in functional remission of the cases diagnosed with schizophrenia

RIGHT TO DEVELOPMENT AND EMOTIONAL EXHAUSTION: THE CASE OF HEALTHCARE INSTITUTIONS IN TURKEY

Right to development covers economic, social, cultural, and political development. Encouraging its subjects to participate actively in economic, social, cultural, and political development, right to development has significant impact on each person. Although it is wide in scope, person, being the central subject of development, this study focuses on right to development of health care professionals limited to doctors and nurses. This paper assessed right to development of health care staff, considering their work conditions and other demographic characteristics. For the implementation of regulations regarding to right to development, a significant fieldwork covering 20 health care institutions in three cities of Turkey was successfully completed. In this fieldwork, Maslach Burnout Inventory (MBI) was used for data collection. This article assessed emotional exhaustion of 185 health workers via SPSS program. The analyse found that education status and type of health care institution have effect on emotional exhaustion while other demographic characteristics such as work experience, annual income or the city were found non-effective on emotional exhaustion of health care professionals. Considering results of this fieldwork, the correlation of emotional exhaustion with the right to development was discussed. The findings reveal that the fear of aggression, lack of sufficient trainings, defamation or mobbing by senior doctors are potential adverse effects causing emotional exhaustion of health workers. To decrease emotional exhaustion caused by work, institutions are suggested to provide ongoing training or a sustainable method for decrement of patient burden and workload. Last but not least, as a sustainable solution, a national wide precise legal monitoring mechanism covering both public and private, ordinary and university health care institutions is strictly offered to be created for prevention of infringement on right to development of medical staff
