2020 - A TIME OF CRISIS AND SOCIAL DISTANCING, PANDEMIC COVID-19 - TIME TO STRENGTHEN SCIENCE AND HOPE TO FACE LIFE - EDITORIAL
Two thousand twenty was an awkward and challenging year. Snatched up by a virus, All of a sudden, we saw ourselves working at home. We were physically away from some of the ones we love and from a daily life we were already used to. We have been living a different and introspective year, with social distance under isolation and solitude. At this time, we had the opportunity to withdraw and rethink our purposes, attitudes, values, and our place in the world. We have the chance to think about the meaning of life, look around and see the relationships we have established with one another, with my brotherhood, and with all living and non-living beings that inhabit this planet, our “Common Home”.It is difficult to think of something more challenging and able to test our resilience! This is not only from a personal point of view, but also from a social, economic and environmental point of view. That is why it is important to talk about resilience, discuss it, test it, prove it and approve it. Such resilience has made us face this Pandemic - COVID-19 and fight for our existence, fight for health, look for respect, welcome and solidarity.Many were the examples of people and companies that helped with donations, delivered masks, hospital equipment, food and so on across all places and cities. But the challenges went further. Many hospital beds and ICUs had to be installed. Health professionals, doctors, nurses and so many others were had to and still face this struggle. They are anonymous generals in their barracks who fought this War of Death for Life. Unfortunately, not everyone could be saved from COVID. The number of those who were treated and recovered in Brazil was 6,501.341 people and 44,169.851 patients worldwide, considering this year, 2020. But 2020 will be remembered as a catastrophe and calamity, because despite efforts many fell down. The causalities’ numbers was in Brazil 188,259 and 1,722.307 in the world. An invaluable loss of human life.We also had other losses such as economic, environmental and social, especially in health. Many will be left with afterpains and a weakened wellbeing as a result of this virus infection; thus, it will not be possible to evaluate the extent of this problem. Many people have gotten worse or developed psycho-emotional pathologies such as depression, panic, among them, and their quality of life has been permanently affected.Many challenges since we have been living under a restricted mobility around the world, which had never been witnessed. There were empty streets, closed shops, bus stations and airports without people for several months.It should be told that political crises, the science negationism, discriminatory acts, polarization regarding data and fake news have exacerbated this crisis. A lot of misinformation, attacks on the environment and a sever backtracking of environmental protection laws and regulations, dismantling of environmental policies, fire was set in Amazon and in Midwestern Brazil that have reached the highest levels in centuries. A relatively large group of Brazilian citizens and in other countries who said they did not believe on how risky Corona virus could be. They neither believe in press information nor insisted on crowding in bars, streets and beaches, as soon as these spaces were opened. We consider unthinkable and even irrational attitudes with scientific knowledge nowadays, since they put their lives and the population’s at risk. Future is the judge of all actions, therefore, the results will be known only tomorrow.The researchers who make up the International Research Network on Climate Resilient Development - RIPEDRC have not faded! They proposed themselves to overcome this challenge of seeking resilience and sustainability! So, with Faith in our life ideals and in a superior motivation that guides our steps and makes us overcome challenges, we have launched to public notices, investigated, questioned and disseminated knowledge supported by several projects, live lectures, events and workshops. They created programs, joint projects and online platforms were developed. In Brazil, Paraguay and Argentina Trinational Border region, the Western Paraná State University - UNIOESTE, was in charge or partner of many efforts, among which we highlight two experiences that RIPEDRC Network participants took part.
O Que Falta Saber Quando Falamos em Educação Ambiental Climática? Uma discussão sobre “Os Sete Saberes Necessários para a Educação sobre Mudanças Climáticas”
This paper presents a discussion of the book "The Seven Necessary Knowledge for Education About Climate Change", written by Alfredo Pena-Vega, published by Cortez Editora in 2023. The text brings reflections from the authors on their interactions in the interview given by Dr. Alfredo Pena-Vega to the authors, published on the YouTube Channel NAPI CLIMATE EMERGENCE - RIPERC Research Network. The book is thought-provoking and challenging in proposing that understanding climate change and working with it in an educational way is an ethical commitment from older people to younger people. This ethical commitment involves both building knowledge about climate change equally between generations and people and creating mechanisms so that the voice of the youngest and poorest is heard. This requires that knowledge about what is happening to the climate be studied outside of the watertight boxes in which it exists today and understood as something that permeates all activity and all human life. The entire interview and also the book are enriched with examples, discussions and reflections on the activity of the Global Youth Climate Pact, an activity developed by Pena-Vega at a global level with young people of school age.

O livro é instigante e desafiador ao propor que compreender as mudanças climáticas e trabalhar com elas de maneira educativa é um compromisso ético dos mais velhos para com os mais novos. Outro aspecto relevante enfatizado no livro é o compromisso ético envolve tanto construir o conhecimento sobre as mudanças climáticas de maneira igualitária entre gerações e povos quanto criar mecanismos para que a voz dos mais novos e dos mais pobres seja ouvida. Isto requer que o conhecimento sobre o que está acontecendo com o clima seja considerado em sua complexidade estanques em que está hoje e compreendido como algo que perpassa toda a atividade e toda a vida human. Positron emission tomography response and minimal residual disease impact on progression-free survival in patients with follicular lymphoma. A subset analysis from the FOLL05 trial of the Fondazione Italiana Linfomi
The aim of the present study was to analyze the prognostic role of combined PET and BCL2/IGH analysis, performed at the EOT, in a subset study of the phase III trial FOLL05 (NCT00774826), in which patients with FL were randomized to R-CVP (rituximab plus cyclophosphamide, vincristine and prednisone), R-CHOP (rituximab plus cyclophosphamide, doxorubicin, vincristine and prednisone) or R-FM (rituximab plus fludarabine and mitoxantrone).6 This study was conducted in compliance with the Declaration of Helsinki, was approved by the appropriate research ethics committee, and required each patient to provide written informed consent
Advanced Hodgkin's lymphoma: results in 216 patients treated with ABVD in Brazil
The outcome of Hodgkin's lymphoma (HL) has markedly improved over the last few decades, placing HL among the human cancers with highest cure rates. However, data about treatment outcomes in developing countries are scarce. From 1996 to 2005, 370 consecutive patients with HL treated in three public institutions in Rio de Janeiro were identified. A total of 216 patients who presented with advanced stage (IIB-IV) HL were selected for the present analysis. Patients with advanced disease were treated with ABVD, complemented or not by radiation therapy. The median follow-up time of survivors was 6.3 years (1-11.8). Fifteen patients died during first-line treatment. The complete remission rate was 80%. The 5-year progression-free survival (PFS) and the 5-year overall survival (OS) probabilities were 69% and 83%, respectively. The 5-year PFS in low-risk and high-risk patients were 81% and 62% (p=0.003), respectively. The 5-year OS in low-risk and high-risk International Prognostic Score patients were 89% and 78% (p=0.02), respectively. The present study provides a representative estimate of current treatment results for advanced HL in public institutions in an urban area in Brazil. It is clear that full treatment can be given to most patients, although those with very low socio-economic status might require special attention and support. Since Brazil is a large country, with substantial interregional heterogeneity, a nationwide registry of HL patients is currently being implemented
CD137 Is Expressed in Follicular Dendritic Cell Tumors and in Classical Hodgkin and T-Cell Lymphomas Diagnostic and Therapeutic Implications
CD137 (also known as 4-1BB and TNFRSF9) is a member of the tumor necrosis factor receptor superfamily. Originally identified as a costimulatory molecule expressed by activated T cells and NK cells, CD137 is also expressed by follicular dendritic cells, monocytes, mast cells, granulocytes, and endothelial cells. Anti-CD137 immunotherapy has recently shown promise as a treatment for solid tumors and lymphoid malignancies in preclinical models. We defined the expression of CD137 protein in both normal and neoplastic hematolymphoid tissue. CD137 protein is expressed by follicular dendritic cells in the germinal center and scattered paracortical T cells, but not by normal germinal-center B cells, bone marrow progenitor cells, or maturing thymocytes. CD137 protein is expressed by a select group of hematolymphoid tumors, including classical Hodgkin lymphoma, T-cell and NK/T-cell lymphomas, and follicular dendritic cells neoplasms. CD137 is a novel diagnostic marker of these tumors and suggests a possible target for tumor-directed antibody therapy
The outcome of peripheral t-cell lymphoma patients failing first-line therapy: A report from the prospective, international t-cell project
This analysis explored factors influencing survival of patients with primary refractory and relapsed peripheral T-cell lymphomas enrolled in the prospective International T-cell Project. We analyzed data from 1020 patients with newly diagnosed disease, enrolled between September 2006 and December 2015. Out of 937 patients who received first-line treatment, 436 (47%) were identified as refractory and 197 (21%) as relapsed. Median time from the end of treatment to relapse was 8 months (range 2-73). Overall, 75 patients (8%) were consolidated with bone marrow transplantation, including 12 refractory and 22 relapsed patients. After a median follow up of 38 months (range 1-96 months) from documentation of refractory/relapsed disease, 440 patients had died. The median overall survival (OS) was 5.8 months; 3-year overall survival rates were 21% and 28% for refractory and relapsed patients, respectively (P12 months, HR 0.57, P=0.001) and salvage therapy with transplantation (HR=0.36, P<0.001) were associated with a better OS. No difference was found in OS with respect to histology. This study accurately reflects outcomes for patients treated according to standards of care worldwide. Results confirm that peripheral T-cell lymphomas patients had dismal outcome after relapse or progression. Patients with chemotherapy sensitive disease who relapsed after more than 12 months might benefit from consolidation bone marrow transplantation. Health Policy Brief: i pilastri dell'Engagement in Emofilia
About 5000 people in Italy suffer from hemophilia, the most common coagulation disorder. As for other chronic diseases, even in the case of hemophilia, the engagement of the patient is essential: the patients, in fact, must be empowered and helped to become strong partners of the care team and sensitized with respect to their rights and duties for the successful achievement of the goals set by their healthcare path. Hence the initiative to start a new research-intervention project in the field of hemophilia. The study had different phases of research: a first moment inspired by the principles of narrative medicine, aimed at collecting stories and narratives of patients with hemophilia related to the experience of the disease and therapy and expectations of active involvement in the relationship with the clinician. At the same time, among hematologists and patients has been surveyed the experience of therapeutical relationship and communication, to capture the aspects in which they feel effective and the areas of improvement and unmet needs. Subsequently, a workshop dedicated to patients and hematologists was organized to foster mutual awareness between these two targets and the formation of a better communication and relational skills of clinicians.
The results of the project formed the basis for a policy brief document, aimed at disseminating recommendations to support better relationship and empathic communication between clinicians and patients
