Advanced ovarian pregnancy: an elusive diagnosis

Primary ovarian ectopic is a rare variant of ectopic pregnancies. It is commonly confused with tubal pregnancy aborted over ovary, hemorrhagic ovarian cyst, ruptured corpus luteal cyst. The women with ovarian ectopic generally presents early because of early onset hemorrhage in ovary. This is a rare case where woman with ectopic pregnancy presents at 13 weeks. The clinical picture is also highly unusual with just spotting and fainting attacks at the end of first trimester to make a diagnosis of ectopic pregnancy. The laparotomy done showed an unruptured ovarian ectopic pregnancy and with 350 cc hemoperitoneum. Salpingoopherectomy was done and the ectopic mass was removed as hardly any ovarian tissue was left to conserve the ovary. Hence, clinicians should be cautious enough to keep a differential diagnosis of ectopic even at advanced gestation

Cesarean scar pregnancy: an upcoming challenge

Background: Cesarean scar pregnancy (CSP) is a type of ectopic pregnancy implanted in the myometrium at the site of previous cesarean scar. Incidence of cesarean deliveries are increasing globally, leading to rise in incidence of cesarean scar pregnancy. Caesarean scar pregnancies are associated with some life-threatening complications such as scar rupture, haemorrhage, disseminated intravascular coagulation, requiring lifesaving hysterectomy. We present our experience with 11 patients with cesarean scar pregnancy, diagnosed using transvaginal colour doppler ultrasound during 3-year period and treated conservatively to preserve the uterus with successful outcome in all patients.Methods: This was a retrospective study, conducted in the Department of Obstetrics and Gynecology of DMC&H, Ludhiana from January 2015 to December 2017. Out of total deliveries (4278), 3.9% (171/4278) were diagnosed as ectopic pregnancy. 6.43% (11/171) of them were diagnosed as cesarean scar ectopic pregnancy. After counseling, all patients underwent conservative management. Injection methotrexate 50mg was administered intramuscularly and beta- HCG was monitored after 4 days and then weekly till it was <1.Results: 2 patients had an increase in beta HCG levels on day 7 and required second dose of methotraxate. 8 patients required blood transfusion due to excessive bleeding on admission. In 9 patients beta HCG levels reduced to <1 in 7 weeks post first methotraxate dose administration, and in remaining 3 it returned to <1 after 8 weeks . No patient required any surgical intervention.Conclusions: Cesarean scar pregnancy, a type of ectopic pregnancy can be safely managed conservatively if diagnosed early

A PLACEBO CONTROLLED DOUBLE BLIND COMPARATIVE CLINICAL STUDY TO EVALUATE THE EFFICACY OF KATUKI AND SITA CHURNA IN URDHVAGA AMLAPITTA

The disease Urdhvagha Amlapitta is a common functional disease of Annavaha srotas due to irregular, improper food habits and busy stressful lifestyle. This disease is a constant challenge to treat because of its recurrence nature due to faulty dietary habits. The management of this disease is somewhat difficult and patients are looking with a hope towards Ayurveda to overcome this challenge. So, this study was designed as a placebo controlled double blind trial to evaluate the efficacy of Katuki and Sita Churna capsules in Urdhvagha Amlapitta. 30 patients of Urdhvaga Amalpitta were selected and randomly allocated in two groups. Group A received Katuki and Sita capsules and group B received placebo capsules, both in the dose of two capsules twice daily before food for 30 days. It is observed that Urdhvagha Amlapitta is seen in middle age and most common in females. It is usually seen in those whose lifestyle is not fixed and having irregular food habits. All 30 patients had completed the treatment and no adverse effects were reported during the treatment. After statistical analysis the inter group difference were insignificant in all parameters after the end of treatment period except Hrit/Kanta daha where in difference was slightly significant with more improvement in group A than the group B. By the overall result we can conclude that Katuki and Sita capsules are having significant effect in the Urdhvaga Amlapitta

Activity Tracker Wrist Band for Children Monitoring using IOT

Today in present world of digital technology and global computing every person is connected with each other in number of ways. In current global computing world, the children and women harassment, chain snatchings, kidnapping, sexual harassments, eve teasing, etc. are increased day-by day, becoming more unsafe and helpless. When these perilous situations occur there must be a trending technology to be comfortable to handle. So we are proposing a system that works on the controversy of children using IOT. In this project we proposed a device which is integrated with multiple devices, comprising of wearable “Activity Tracker Wrist Band” which is programmed with all the required data which includes the behavior of the human reactions like anger, anxiety, nervousness and fear. When these situations are faced by the victim, the various sensors generate the emergency signals which are to be transmitted to the smart phone. The system effectively monitors the children presence within the expected zone. When the person crosses the monitoring zone, then based on IOT Monitoring system, GSM sends help request by sending messages to the nearest police station, parents and the people in the near radius

Study of serum calcium and serum magnesium levels in newly diagnosed hypertensives

Background: In normal vascular tissue physiology, calcium plays a crucial role. Calcium and magnesium metabolism have been implicated in the pathogenesis of primary hypertension. The present study aimed to assess serum calcium and serum magnesium levels and to study the association of the serum magnesium and serum calcium levels with hypertension in newly diagnosed hypertensive patients of Bikaner, Rajasthan. Methods: This cross- sectional study was carried out on 60 subjects aged between 18 years to 60 years in the department of physiology and in association with department of medicine and department of cardiology, S. P. Medical College and associated group of hospitals, Bikaner. Data was compiled and entered into Microsoft software statistical package for the social sciences (SPSS) version 22.0. Data was summarised as frequencies, mean and standard deviations. For continuous data the means were compared between cases and controls using unpaired or independent t test; and for nominal data, chi-square or Fisher’s exact tests were applied to compare frequency distribution between cases and controls to see the statistical significance. Results: The present study shows statistically insignificant change in serum calcium and serum magnesium levels in newly detected hypertensive patients when compared to normotensive controls. Conclusions: In view of statistically insignificant change in serum calcium and magnesium levels in newly detected hypertensive patients in our study, further studies with a large sample size and more uniform population are required to find if any significant association exists between these cations and hypertension

A CLINICAL TRIAL OF AROGYA VARDHINI VATI AND LEKHANIYA MAHAKASHAYA IN THE MANAGEMENT OF OBESITY

Obesity is a condition in which there is an excessive accumulation of fat in the body which is a risk to health. The WHO now considers obesity to be a global epidemic and public health problem. Globally an estimated 300 million adults are now obese and many are overweight. A person with a BMI of 30 or more is generally considered obese. The problem is due to calorie imbalance resulting from an excessive food intake coupled with inadequate exercise. It is associated with increased mortality by predisposing to the development of important diseases like diabetes, hypertension, atherosclerosis, heart diseases, arthritis, infertility etc and diminishes the efficacy and happiness of affected. As per Ayurveda, Acharya charaka has counted Sthoulya under the eight varieties of impediments which are designated as Ninditapurusha. As Chikitsa sutra of Sthoulya, Lekhan karma is an important therapeutic measure said by Acharya charaka. Moreover, drugs of Arogya vardhini vati and Lekhaniya mahakashaya are most effective in the management of Sthoulya. Female frontline community healthcare workforce in India during Covid-19

Since the advent of the COVID-19 pandemic, 1 million Accredited Social Health Activists (ASHAs), 2.6 million Anganwadi workers (AWWs) and an entire cadre of Auxiliary Nurse Midwives (ANMs) have been at the forefront of the emergency health response system in India. During the pandemic, these three cadres of healthcare workers managed tasks like monitoring and conducting syndromic surveys, disease surveillance, data reporting, public health messaging, delivering essential food and ration services, at the community level. They routinely clocked over 12 hours work days and were on call the rest of the time. Despite being the backbone of India’s public health response, female frontline workers from across the country have reported being underpaid, undervalued, and alienated without any state support or protection. This report explores the role played by ASHAs, AWWs and ANMs during the pandemic, by analysing the information gathered from interviewing 201 frontline health workers across the 3 cadres from ten states. It offers crucial insight into their increased burden of work, economic vulnerabilities brought by reduced income and delayed payments, absence of medical insurance and adequate state support, as well as their consequently deteriorating physical and mental health
