    Post-basic nursing students' perceptions of the emigration of nurses from the Republic of South Africa: research

    The emigration of nurses from South Africa to work in overseas countries continues to increase despite efforts to curb this phenomenon. This exploratory descriptive survey attempted to identify nurses' perceptions of their colleagues who have emigrated as well as their own intentions to emigrate should the opportunity arise. Selfcompletion questionnaires were posted to all students registered for under- and postgraduate studies at a distance education institution. The analysis of the data obtained from 453 completed questionnaires indicated that almost all respondents knew nurses who had emigrated, and that approximately 60% of the respondents would emigrate if the opportunity should arise. The major deterrent to their emigration was family ties which kept them in South Africa. The major reasons why they would emigrate related to better remuneration and to the ability to save money quickly for specific reasons. Besides improved remuneration and fringe benefits, employers could help nurses to remain in the Republic of South Africa (RSA) by showing appreciation for their work, by improving facilities and resources and by appointing competent health managers. The South African government could help nurses to remain within the RSA by appointing people in the frozen posts, eliminating corruption within the government and by improving safety and security in the country. Almost all respondents indicated that the government had no right to prevent South African nurses from emigrating to other countries. Benewens verbeterde besoldiging en byvoordele kan werkgewers verpleegkundiges help om in RSA te bly deur waardering te toon vir hul werk, deur fasiliteite en bronne te verbeter en deur bevoegde gesondheidsorgbestuurders aan te stel. Die Suid-Afrikaanse regering kan verpleegkundiges help om in die RSA aan te bly deur mense in bevrore poste aan te stel, korrupsie binne die regering uit te roei en die veiligheid en sekuriteit in die land te verbeter. Bykans alle respondente het aangetoon dat die regering nie oor die reg beskik om Suid-Afrikaanse verpleegkundiges te verbied om na ander lande te emigreer nie.     Validation of equations to estimate glomerular filtration rate in South Africans of mixed ancestry

    Background. The Modification of Diet in Renal Disease (MDRD) and Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equations are two commonly used formulae to estimate glomerular filtration rate (GFR) in adults. The CKD-EPI equation is recommended in current international and local guidelines for the diagnosis and management of chronic kidney disease (CKD), unless an alternative equation has been shown to have superior accuracy. Validation and comparison of the equations in local populations are therefore required. Previous studies have reported on the accuracy of these prediction equations in black South Africans and those of Indian ancestry.Objectives. To evaluate the MDRD and CKD-EPI equations in South African (SA) adults of mixed ancestry.Methods. In all participants, GFR was measured (mGFR) from plasma clearance of 99mTc-diethylenetetraaminepenta-acetic acid (99mTc-DTPA), using a standardised technique. Serum creatinine assays were isotope dilution mass spectrometry traceable. GFR was estimated (eGFR) using the MDRD and CKD-EPI equations, with and without the black ethnicity factor. The agreement, bias, precision and accuracy of each equation was determined.Results. Eighty adults were included (30 male, median age 39 years, median GFR 59 mL/min/1.73 m2). Sixty-eight had a diagnosis of CKD, 10 were potential kidney donors, and 2 were healthy volunteers. Both equations, without the black ethnicity factor, had good agreement with measured GFR. The equations tended to overestimate GFR, with bias of 1.6 and 7.9 mL/min/1.73 m2 for the MDRD and CKD-EPI equations, respectively. The interquartile ranges of the differences were 15.9 and 20.2 mL/min/1.73 m2, and as a measure of accuracy, the percentages of estimates that fell within 30% of the mGFR (P30) were 80% and 72.5% (p=0.18). For identification of individuals with a GFR <60 mL/min/1.73 m2, the sensitivity of MDRD eGFR was 97.3% and that of CKD-EPI eGFR was 97.1%.Conclusions. The MDRD and CKD-EPI equations have shown satisfactory and comparable performance in this SA mixed-ancestry adult population, with the MDRD equation marginally less biased than the CKD-EPI.

    Comparative palatability of orally disintegrating tablets (ODTs) of praziquantel (L-PZQ and Rac-PZQ) versus current PZQ tablet in African children: a randomized, single-blind, crossover study

    BACKGROUND: Praziquantel (PZQ) is currently the only recommended drug for infection and disease caused by the schistosome species that infects humans; however, the current tablet formulation is not suitable for pre-school age children mainly due to its bitterness and the large tablet size. We assessed the palatability of two new orally disintegrating tablet (ODT) formulations of PZQ. METHODOLOGY: This randomized, single-blind, crossover, swill-and-spit palatability study (NCT02315352) was carried out at a single school in Tanzania in children aged 6-11 years old, with or without schistosomiasis infection as this was not part of the assessment. Children were stratified according to age group (6-8 years or 9-11 years) and gender, then randomized to receive each formulation in a pre-specified sequence. Over 2 days, the children assessed the palatability of Levo-Praziquantel (L-PZQ) ODT 150 mg and Racemate Praziquantel (Rac-PZQ) ODT 150 mg disintegrated in the mouth without water on the first day, and L-PZQ and Rac-PZQ dispersed in water and the currently available PZQ 600 mg formulation (PZQ-Cesol) crushed and dispersed in water on the second day. The palatability of each formulation was rated using a 100 mm visual analogue scale (VAS) incorporating a 5-point hedonic scale, immediately after spitting out the test product (VASt = 0 primary outcome) and after 2-5 minutes (VASt = 2-5). PRINCIPAL FINDINGS: In total, 48 children took part in the assessment. Overall, there was no reported difference in the VASt = 0 between the two ODT formulations (p = 0.106) without water. Higher VASt = 0 and VASt = 2-5 scores were reported for L-PZQ ODT compared with Rac-PZQ ODT in older children (p = 0.046 and p = 0.026, respectively). The VASt = 0 and VASt = 2-5 were higher for both ODT formulations compared with the standard formulation (p<0.001 for both time points). No serious adverse events were reported. CONCLUSIONS/SIGNIFICANCE: The new paediatric-friendly formulations dispersed in water were both found to be more palatable than the existing standard formulation of PZQ. There may be gender and age effects on the assessment of palatability. Further research is needed for assessing efficacy and tolerability of the newly ODTs Praziquantel drug in younger children.     Entanglement in the quantum Ising model

    We study the asymptotic scaling of the entanglement of a block of spins for the ground state of the one-dimensional quantum Ising model with transverse field. When the field is sufficiently strong, the entanglement grows at most logarithmically in the number of spins. The proof utilises a transformation to a model of classical probability called the continuum random-cluster model, and is based on a property of the latter model termed ratio weak-mixing.     A numerical method is devised for study of stochastic partial differential equations describing directed percolation, the contact process, and other models with a continuous transition to an absorbing state. Owing to the heightened sensitivity to fluctuationsattending multiplicative noise in the vicinity of an absorbing state, a useful method requires discretization of the field variable as well as of space and time.
