    Severe bone marrow suppression following single-dose methotrexate treatment for ectopic pregnancy

    Methotrexate is associated with multisystem side effects which depend on the dosage, route of administration and length of treatment. Bone marrow suppression is a very rare but one of the most mortal complications. A case of severe bone marrow suppression following single dose methotrexate for ectopic pregnancy was presented in this study. A 33-year-old woman, diagnosed as ectopic pregnancy, was given single-dose intramuscular methotrexate of 50 mg/m². On 10th day patient was admitted with lethargy, irritability, vomiting, hematemesis, diarrhea, oral mucositis and skin rash. Laboratory findings revealed severe bone marrow suppression. Supportive treatment together with antibacterial/ antifungal drugs and granulocyte colony stimulating factors were administered. On the 3rd day of treatment, mucositis and oral intake began to improve. After 1 week complete blood count was normal. Methotrexate may have mortal clinical consequences even with very low doses. Therefore, a careful patient history for patient selection and closer clinical follow-up are essential

Duygu analizi insanlara kullanıcı taraflı oluşturulan içeriklerde gezinmeye yardımcı, gelişmekte olan bir teknolojidir. Dilsel özetleme ise var olan sayısal verilerden anlaşılabilir özet cümlelerin çıkarılmasıdır. Bu tez çalışması ürün varlıklarını öğrenmeye ve sınıflandırmaya, düşünce ifadelerini otomatik olarak ürün yorumlarından çıkarmaya ve özetlemeye yöneliktir. Tez kapsamında iki ayrı veri seti kullanılarak iki uygulama gerçekleştirilmiştir. Birinci uygulamada bir otel hakkında yapılan ziyaretçi yorumlarının duygu analizine ve bulanık dilsel özetlenmesine odaklanılmıştır. İkinci uygulamada ise sayısal değerler içeren restoran değerlendirme veri seti üzerinde duygu analizi ve bulanık dilsel özetleme yöntemleri kullanılmıştır. Her iki uygulamada da otele ve restorana ait pozitif, negatif veya nötr duygular belirlenmiş ve uzun yorum cümlelerinden anlaşılabilir dilsel özetler çıkartılmıştır.As internet applications, such as blogging, review sites, social applications, are becoming more and more popular the amount of information is increasing. Also, the number of evaluations and reviews are increasing rapidly. So, it is now difficult to distinguish nonuseful reviews from useful ones. Opinion mining is an emerging technology. It helps people to search user-side generated content. Linguistic summarization is similar to opinion mining. It summarizes numerical data in an understandable short format. In this thesis, two applications are developed. In the first application, opinion expressions and linguistic summarizations of hotel reviews are obtained automatically. In the second application, linguistic summarization of numerical restaurants data are obtained automatically, the system gives understandable summaries of the reviews with fuzzy set theory. The system gives the binary sentiment classification of reviews such as positive, negative and neutral. Also, understandable summaries of the reviews are carried out.NUL

    Laparoscopic versus vaginal cuff closure in laparoscopic hysterectomy: Does it affect female sexuality?

    Objectives: To investigate the effect of vaginal cuff closure technique in laparoscopic hysterectomy on vaginal length and female sexual functions. Material and methods: This study was conducted at a tertiary research hospital. Women who underwent laparoscopic hysterectomy were included and classified according to vaginal cuff closure technique as laparoscopic (n = 75) and vaginal route (n = 25). Vaginal lengths were measured preoperatively and at 6th month postoperatively. Golombok-Rust Inventory of Sexual Satisfaction (GRISS) was used to evaluate female sexual functions. SPSS was used for statistical analysis and the level of significance was p = 0.05. Results: Preoperative GRISS scores and vaginal lengths were similar in two groups. The shortening of vaginal length and the worsening of GRISS scores were more prominent in vaginal cuff closure group (p = 0.002, p < 0.001). The alteration in vaginal length was positively correlated with the alteration in GRISS score in vaginal and laparoscopic route groups (r = 0.800, p < 0.001; r = 0.680, p < 0.001). The risk of female sexual dysfunction increases 69.88 fold for each 1 cm shortening of vaginal length (p = 0.039). Discriminative value of postoperative vaginal length for female sexual dysfunction in patients who underwent laparoscopic hysterectomy was investigated and a cut-off value of 7.4 cm (p < 0.001) was found. Conclusions: Laparoscopic route instead of vaginal route in laparoscopic hysterectomy is preferable to preserve a better vaginal length, which may be an important factor for female sexual functions

    Maternal serum glycodelin levels in preeclampsia and its relationship with the severity of the disease

Glycodelin is a regulator of immunosuppression, fertilization, implantation, and placentation. Because of its inhibitory effects on trophoblastic activity, trophoblast invasion is disturbed when its levels alter. We aimed to analyze serum glycodelin levels in preeclampsia and evaluate whether it correlates with the severity of disease.Methods: This is a prospective case-control study conducted in a research and training hospital between March and September 2016. In this study, a total of 55 preeclamptic and 65 healthy pregnants were included. Preeclamptic patients were divided into two subgroups: 25 severe and 30 mild. Maternal serum glycodelin levels were measured using enzyme-linked immunosorbent assay.Results: Glycodelin levels were higher in preeclamptic group as compared with controls (71.3822.78 versus 42.32 +/- 12.28ng/ml, p53.64ng/ml. Moreover, area under the curve for glycodelin to diagnose severe preeclampsia is 0.788 with p83.97ng/ml.Conclusion: Glycodelin may be a promising marker in predicting the presence and severity of preeclampsia

    Platelet indices in preterm premature rupture of membranes and their relation with adverse neonatal outcomes

    AimPreterm premature rupture of membranes (PPROM) is not only the most common distinguishable cause of preterm delivery, but is also associated with adverse neonatal outcomes. We determined the platelet indices in PPROM cases and evaluated their relationship to adverse neonatal outcomes. MethodsFifty patients with PPROM and 50 patients who experienced spontaneous preterm labor at 0.22, and the probability of RDS increased 5.86 times when plateletcrit values exceeded 0.22 (odds ratio 5.86, 95% confidence interval1.01-32.01; P=0.04). A one-unit increase in platelet distribution width resulted in a 1.33-fold increase in the risk of RDS (odds ratio 1.33, 95% confidence interval1.01-1.77; P =0.04). ConclusionMean platelet volumes and plateletcrit significantly increased and plateletcrit had a predictive value for RDS in PPROM cases. Monitoring plateletcrit may be promising for predicting the development of RDS, one of the most common and serious complications of PPROM rupture
