    Spatio-Temporal Patterning in Primary Motor Cortex at Movement Onset

    Voluntary movement initiation involves the engagement of large populations of motor cortical neurons around movement onset. Despite knowledge of the temporal dynamics that lead to movement, the spatial structure of these dynamics across the cortical surface remains unknown. In data from 4 rhesus macaques, we show that the timing of attenuation of beta frequency local field potential oscillations, a correlate of locally activated cortex, forms a spatial gradient across primary motor cortex (MI). We show that these spatio-temporal dynamics are recapitulated in the engagement order of ensembles of MI neurons. We demonstrate that these patterns are unique to movement onset and suggest that movement initiation requires a precise spatio-temporal sequential activation of neurons in MI

    Comparing Offline Decoding Performance in Physiologically Defined Neuronal Classes

    Objective: Recently, several studies have documented the presence of a bimodal distribution of spike waveform widths in primary motor cortex. Although narrow and wide spiking neurons, corresponding to the two modes of the distribution, exhibit different response properties, it remains unknown if these differences give rise to differential decoding performance between these two classes of cells. Approach: We used a Gaussian mixture model to classify neurons into narrow and wide physiological classes. Using similar-size, random samples of neurons from these two physiological classes, we trained offline decoding models to predict a variety of movement features. We compared offline decoding performance between these two physiologically defined populations of cells. Main results: We found that narrow spiking neural ensembles decode motor parameters better than wide spiking neural ensembles including kinematics, kinetics, and muscle activity. Significance: These findings suggest that the utility of neural ensembles in brain machine interfaces may be predicted from their spike waveform widths

    Large-scale Spatiotemporal Spike Patterning Consistent with Wave Propagation in Motor Cortex

    Aggregate signals in cortex are known to be spatiotemporally organized as propagating waves across the cortical surface, but it remains unclear whether the same is true for spiking activity in individual neurons. Furthermore, the functional interactions between cortical neurons are well documented but their spatial arrangement on the cortical surface has been largely ignored. Here we use a functional network analysis to demonstrate that a subset of motor cortical neurons in non-human primates spatially coordinate their spiking activity in a manner that closely matches wave propagation measured in the beta oscillatory band of the local field potential. We also demonstrate that sequential spiking of pairs of neuron contains task-relevant information that peaks when the neurons are spatially oriented along the wave axis. We hypothesize that the spatial anisotropy of spike patterning may reflect the underlying organization of motor cortex and may be a general property shared by other cortical areas

    Restoring Lateral Incisors and Orthodontic Treatment: Perceptions among General Dentists and Othodontists

    The purpose of this study was to identify and compare preferences and perceptions of orthodontists and general dentists when restoring peg-shaped lateral incisors. The investigation sought to summarize these preferences with regard to treatment planning, tooth preparation and interdisciplinary communication. A pair of mailed and electronic surveys was distributed to 1,500 general dentists and orthodontists, respectively. The results indicated that general dentists perceived that general dentists held the primary decision-making responsibility, while orthodontists disagreed (P\u3c0.0001). Orthodontists prioritized the treatment goals of Class I canine relationship and overbite/overjet more significantly than general dentists, whom valued tooth proportions more highly (P\u3c0.0001). General dentists reported receiving significantly less input than orthodontists report seeking (P\u3c0.0001).The consensus of both groups showed that the tooth should be positioned centered mesiodistally and guided by the gingival margins incisogingivally. Both groups agree that orthodontists must improve communication to improve treatment results

    Assessing a Pilot Scheme of Intensive Support and Assertive Linkage in Levels of Engagement, Retention, and Recovery Capital for people in Recovery Housing using Quasi-Experimental Methods

    Introduction: There is strong and ever-growing evidence highlighting the effectiveness of recovery housing to support and sustain substance use disorder (SUD) recovery augmented by intensive support comprising assertive linkages to community services.Aim: To assess a pilot intensive recovery support (IRS) intervention for individuals (n=175) entering certified Level II and III recovery residences who met at least three out of five conditions (no health insurance; no driving license; substance use in the last 14 days; currently unemployed; having less than $75 capital) and its impact on engagement, retention and changes in recovery capital for individuals compared with a business-as-usual Standard Recovery Support (SRS) approach (n=1,758).Methods: We deployed quasi-experimental techniques to create weighted and balanced counterfactual groups derived from the Recovery Capital (REC-CAP) assessment tool to compare outcomes for people receiving the pilot IRS compared to intervention against SRS.Results: After reweighting for resident demographics, service needs, and barriers to recovery, those receiving IRS exhibited improved retention rates, reduced likelihood of disengagement, and growth in recovery capital after living in the residence for 6-9 months.Conclusion: Our findings from this pilot intervention suggest that intensive recovery support incorporating assertive community linkages and enhanced recovery coaching can improve engagement, length of stay and recovery capital growth compared to a balanced counterfactual group. We suggest that this model may be particularly beneficial to those entering Level II and Level III recovery housing with lower levels of recovery capital at admission entry.<br/

    Measuring recovery capital for people recovering from alcohol and drug addiction:A systematic review

    Background: Recovery capital (RC) theory provides a biopsychosocial framework for identifying and measuring strengths and barriers that can be targeted to support recovery from alcohol and drug addiction. This systematic review analyzed and synthesized all quantitative approaches that have been used to measured recovery capital RC in the recent literaturesince 2016.Method: Systematic database searches were conducted in three databases to identifyThe reviewed studies were published from 2016 to 2023, . Eligible studiesand explicitly stated they measured RC recovery capital in participants recovering from alcohol and/or drug addiction. Studies focusing on other forms of addiction were excluded.Results: Sixty-nine studies met the inclusion criteria. Forty-six studies (66.7%) used one of the ten identified RC recovery capital questionnaires, and twenty-five studies (36.2%) used a measurement approach other than one of the ten RC recovery capital questionnaires. The ten RC recovery capital questionnaires are primarily developed for adult populations across clinical and community recovery settings, and between them measuredwere identified to measure altogether 41 separate RC recovery capital constructs. They, and are generally considered valid and reliable measures of RCrecovery capital. Nevertheless, a strong evidence base on the psychometric properties across diverse populations and settings is still needs to be established for all RC these questionnaires. Conclusion: The development of RC recovery capital questionnaires has been a significant advance in the addiction recovery field, in alignment with the modern emerging recovery-oriented approach to addiction recovery care. Additionally, the non-RC recovery capital questionnaire-based approaches to RC recovery capital measurement have an important place in the field. They could be used alongside RC recovery capital questionnaires to test RC theory, and in contexts where the application of the RC questionnaires is not feasible, such as analyses of data from online recovery forums

    Latent Stochastic Differential Equations for Modeling Quasar Variability and Inferring Black Hole Properties

    Active galactic nuclei (AGN) are believed to be powered by the accretion of matter around supermassive black holes at the centers of galaxies. The variability of an AGN's brightness over time can reveal important information about the physical properties of the underlying black hole. The temporal variability is believed to follow a stochastic process, often represented as a damped random walk described by a stochastic differential equation (SDE). With upcoming wide-field surveys set to observe 100 million AGN in multiple bandpass filters, there is a need for efficient and automated modeling techniques that can handle the large volume of data. Latent SDEs are well-suited for modeling AGN time series data, as they can explicitly capture the underlying stochastic dynamics. In this work, we modify latent SDEs to jointly reconstruct the unobserved portions of multivariate AGN light curves and infer their physical properties such as the black hole mass. Our model is trained on a realistic physics-based simulation of ten-year AGN light curves, and we demonstrate its ability to fit AGN light curves even in the presence of long seasonal gaps and irregular sampling across different bands, outperforming a multi-output Gaussian process regression baseline.Comment: 10 pages, 5 figures, accepted at the ICLR 2023 Workshop on Physics for Machine Learnin

    Reducing unnecessary crossmatching for hip fracture patients by accounting for preoperative hemoglobin concentration

    BACKGROUND: Maximum surgical blood order schedules were designed to eliminate unnecessary preoperative crossmatching prior to surgery in order to conserve blood bank resources. Most protocols recommend type and cross of 2 red blood cell (RBC) units for patients undergoing surgery for treatment of hip fracture. Preoperative hemoglobin has been identified as the strongest predictor of inpatient transfusion, but current maximum surgical blood order schedules do not consider preoperative hemoglobin values to determine the number of RBC units to prepare prior to surgery. AIM: To determine the preoperative hemoglobin level resulting in the optimal 2:1 crossmatch-to-transfusion (C:T) ratio in hip fracture surgery patients. METHODS: In 2015 a patient blood management (PBM) program was implemented at our institution mandating a single unit-per-occurrence transfusion policy and a restrictive transfusion threshold of \u3c 7 g/dL hemoglobin in asymptomatic patients and \u3c 8 g/dL in those with refractory symptomatic anemia or history of coronary artery disease. We identified all hip fracture patients between 2013 and 2017 and compared the preoperative hemoglobin which would predict a 2:1 C:T ratio in the pre PBM and post PBM cohorts. Prediction profiling and sensitivity analysis were performed with statistical significance set at P \u3c 0.05. RESULTS: Four hundred and ninety-eight patients who underwent hip fracture surgery between 2013 and 2017 were identified, 291 in the post PBM cohort. Transfusion requirements in the post PBM cohort were lower (51% vs 33%, P \u3c 0.0001) than in the pre PBM cohort. The mean RBC units transfused per patient was 1.15 in the pre PBM cohort, compared to 0.66 in the post PBM cohort (P \u3c 0.001). The 2:1 C:T ratio (inpatient transfusion probability of 50%) was predicted by a preoperative hemoglobin of 12.3 g/dL [area under the curve (AUC) 0.78 (95% confidence interval (CI), 0.72-0.83), Sensitivity 0.66] in the pre PBM cohort and 10.7 g/dL [AUC 0.78 (95%CI, 0.73-0.83), Sensitivity 0.88] in the post PBM cohort. A 50% probability of requiring \u3e 1 RBC unit was predicted by 11.2g/dL [AUC 0.80 (95%CI, 0.74-0.85), Sensitivity 0.87] in the pre PBM cohort and 8.7g/dL [AUC 0.78 (95%CI, 0.73-0.83), Sensitivity 0.84] in the post-PBM cohort. CONCLUSION: The hip fracture maximum surgical blood order schedule should consider preoperative hemoglobin in determining the number of units to type and cross prior to surgery
