    Management of the orbital angular momentum of vortex beams in a quadratic nonlinear interaction

    Light intensity control of the orbital angular momentum of the fundamental beam in a quadratic nonlinear process is theoretically and numerically presented. In particular we analyzed a seeded second harmonic generation process in presence of orbital angular momentum of the interacting beams due both to on axis and off axis optical vortices.     On the Noise Generated by Shear-Layer Instabilities in Turbulent Jets

    Solutions to the linearized equations of motion are used to study sound radiation by convected disturbances in the jet core. The spectrum of eigenmodes reveals the presence of modes that represent convected vortical and entropic motions in the potential core of the jet. We investigate the near-field acoustics produced by these core modes using the Parabolozied Stability Equations. At the conditions of commercial jet engines during take-off, the core modes radiate sound effectively along Mach lines due to the jet centerline velocity being supersonic relative to the fee-stream speed of sound. Summing three of these modes to create two distinct disturbances - one that is velocity dominated, and another that is entropy dominated - one observes that entropy variations radiate sound more effectively than vortical variations. The results yield a first insight into the impact of large-scale mixing inhomogeneities on the acoustic field. Such disturbances are created by devices in the jet engine itself, such as flow mixers, turbine exit vanes, and combustion-chamber (i.e. pattern factors)

    Health-related quality of life measurement in asthma and chronic obstructive pulmonary disease: review of the 2009-2014 literature

    Background: Asthma and chronic obstructive pulmonary disease (COPD) are frequent in the general population. These diseases can worsen the quality of life of people suffering from them, limiting their daily activities and disrupting their sleep at night. Some questionnaires to measure the impact of the diseases on the daily life of patients are available. The measurements of subjective outcomes have become a part of clinical practice, and are used very frequently in clinical trials. Our aim was to describe how data on HRQoL in asthma and COPD are reported in papers published in the medical literature. Methods: We identified papers on the recent respiratory drugs (chemical, not biological), that reported the HRQoL measurement and that were published from 2009 to April 2014. We planned to describe data about HRQoL, and we had no intention of comparing the degree of efficacy of drugs. Results: The most used questionnaires are the Asthma Quality of Life Questionnaire (AQLQ) and the Saint George's Respiratory Questionnaire (SGRQ). These tools, administered at the baseline and at the end of the study (and interim evaluations in the longer studies) allowed for the identification of improvements as perceived by the patient after the treatment, even if in some cases these improvements were limited and not clinically relevant. Subjective measurements have always been placed among the secondary endpoints and the number of patients (estimated for the main endpoint) has often statistically overestimated the result. In addition, it is clear that subjective data is normally reported, but rarely commented on. Conclusions: There are some methodology aspects that should be discussed in more depth, for example the necessity to express variations in the subjective perception, not as p-value but as effect-size

    Tamm Plasmon Resonance as Optical Fingerprint of Silver/Bacteria Interaction

    Incorporation of responsive elements into photonic crystals is an effective strategy for building up active optical components to be used as sensors, actuators and modulators. In these regards, Tamm Plasmon (TP) modes have arisen recently as powerful optical tools for the manipulation of light-matter interaction and for building sensors/actuators. These emerge at the interface between a dielectric mirror and a plasmonic layer and, interestingly, can be excited at normal incidence angle with relatively high quality factors. Although its field is located at the interface between the dielectric mirror and the metal, recent studies have demonstrated that corrugation at the nanoscale permits to access the TP mode from the outside, opening new exciting perspectives for many real-life applications. Here, we show that the TP resonance obtained by capping a distributed Bragg reflector with a nanostructured layer of silver is sensitive to the presence of bacteria. We observed that nanoscale corrugation is essential for accessing the TP field, while the well-known bio-responsivity of silver nanostructures renders such a localised mode sensible to the presence of Escherichia Coli. Electrodoping experiments confirm the pivotal role of nanostructuration, as well as strengthening our hypothesis that the modifications of the TP mode upon exposure to bacteria are related to the accumulation of negative charge due to the bacterial-driven removal of Ag+ ions from its lattice. Finally, we devised a case study in which we disentangled optically the presence of proliferative and non-proliferative bacteria using the TP resonance as a read-out, thus making these devices as promising simple all-optical probes for bacterial metabolic activity, including their response against drugs and antibiotics

    The Use of Psychotropic Medication in Pediatric Oncology for Acute Psychological and Psychiatric Problems: Balancing Risks and Benefits

    Severe acute behavioral and emotional problems represent one of the most serious treatment-related adverse effects for children and adolescents who have cancer. The critical and severe nature of these symptoms often makes necessary the use of psychotropic drugs. A working group composed of experts in multiple disciplines had the task of creating an agreement regarding a management plan for severe acute behavioral and emotional problems (SABEPs) in children and adolescents treated for cancer. To obtain global information on the use of psychotropic drugs in pediatric oncology, the working group first developed and mailed a 15-item questionnaire to many Italian pediatric oncology centers. Overall, an evident lack of knowledge and education regarding the use of psychotropic medications for the treatment of SABEPs was found. Thus, by referring to an adapted version of the Delphi method of consensus and standard methods for the elaboration of clinical questions (PICOs), the working group elaborated evidence-based recommendations for psychotropic drugs in the pediatric oncology setting. Furthermore, based on a thorough multivariate analysis of needs and difficulties, a comprehensive management flow was developed to optimize therapeutic interventions, which allows more accurate and efficient matching of the acute needs of patients while guiding treatment options

    Objectives: To define the Data Manager (DM) job description within the Clinical Trial Center (CTC) of the Alessandria Hospital (AO AL). To identify the number of authorized clinical studies after the implementation of three DMs in the CTC of the AO AL. Methods: The activities of the DM within the CTC of the AO AL take place in the activation, management and conclusion of clinical trials. The activities were monitored through specific indicators from June 01st, 2019 to May 31st, 2020. Results: During the reference period, an increased authorized studies were observed. Conclusion: The implementation of DMs in the CTC of AO AL has been demonstrated the importance of the figure itself, which, although it has not professionally recognized yet, is found to be fundamental in clinical research

    Rapid SARS-CoV-2 intra-host and within-household emergence of novel haplotypes

    In February 2020, the municipality of Vo’, a small town near Padua (Italy) was quarantined due to the first coronavirus disease 19 (COVID-19)-related death detected in Italy. To investigate the viral prevalence and clinical features, the entire population was swab tested in two sequential surveys. Here we report the analysis of 87 viral genomes, which revealed that the unique ancestor haplotype introduced in Vo’ belongs to lineage B, carrying the mutations G11083T and G26144T. The viral sequences allowed us to investigate the viral evolution while being transmitted within and across households and the effectiveness of the non-pharmaceutical interventions implemented in Vo’. We report, for the first time, evidence that novel viral haplotypes can naturally arise intra-host within an interval as short as two weeks, in approximately 30% of the infected individuals, regardless of symptom severity or immune system deficiencies. Moreover, both phylogenetic and minimum spanning network analyses converge on the hypothesis that the viral sequences evolved from a unique common ancestor haplotype that was carried by an index case. The lockdown extinguished both the viral spread and the emergence of new variant

    Beta-Blocker Use in Older Hospitalized Patients Affected by Heart Failure and Chronic Obstructive Pulmonary Disease: An Italian Survey From the REPOSI Register

    Beta (β)-blockers (BB) are useful in reducing morbidity and mortality in patients with heart failure (HF) and concomitant chronic obstructive pulmonary disease (COPD). Nevertheless, the use of BBs could induce bronchoconstriction due to β2-blockade. For this reason, both the ESC and GOLD guidelines strongly suggest the use of selective β1-BB in patients with HF and COPD. However, low adherence to guidelines was observed in multiple clinical settings. The aim of the study was to investigate the BBs use in older patients affected by HF and COPD, recorded in the REPOSI register. Of 942 patients affected by HF, 47.1% were treated with BBs. The use of BBs was significantly lower in patients with HF and COPD than in patients affected by HF alone, both at admission and at discharge (admission, 36.9% vs. 51.3%; discharge, 38.0% vs. 51.7%). In addition, no further BB users were found at discharge. The probability to being treated with a BB was significantly lower in patients with HF also affected by COPD (adj. OR, 95% CI: 0.50, 0.37-0.67), while the diagnosis of COPD was not associated with the choice of selective β1-BB (adj. OR, 95% CI: 1.33, 0.76-2.34). Despite clear recommendations by clinical guidelines, a significant underuse of BBs was also observed after hospital discharge. In COPD affected patients, physicians unreasonably reject BBs use, rather than choosing a β1-BB. The expected improvement of the BB prescriptions after hospitalization was not observed. A multidisciplinary approach among hospital physicians, general practitioners, and pharmacologists should be carried out for better drug management and adherence to guideline recommendations
