    BACKGROUND: Staphylococcal infection of the canine epidermis and hair follicle is amongst the commonest reasons for antimicrobial prescribing in small animal veterinary practice. Topical therapy with fusidic acid (FA) is an attractive alternative to systemic therapy based on low minimum inhibitory concentrations (MICs, commonly <0.03 mg/l) documented in canine pathogenic staphylococci, including strains of MRSA and MRSP (methicillin-resistant Staphylococcus aureus and S. pseudintermedius). However, permeation of canine skin by FA has not been evaluated in detail. This study aimed to define the degree and extent of FA permeation in canine skin in vitro from two sites with different hair follicle density following application of a licensed ophthalmic formulation that shares the same vehicle as an FA-betamethasone combination product approved for dermal application in dogs. Topical FA application was modelled using skin held in Franz-type diffusion cells. Concentrations of FA in surface swabs, receptor fluid, and transverse skin sections of defined anatomical depth were determined using high-performance liquid chromatography and ultraviolet (HPLC-UV) analysis. RESULTS: The majority of FA was recovered by surface swabs after 24 h, as expected (mean ± SEM: 76.0 ± 17.0%). FA was detected within 424/470 (90%) groups of serial sections of transversely cryotomed skin containing follicular infundibula, but never in 48/48 (100%) groups of sections containing only deeper follicular structures, nor in receptor fluid, suggesting that FA does not permeate beyond the infundibulum. The FA concentration (mean ± SEM) in the most superficial 240 Όm of skin was 2000 ± 815 Όg/g. CONCLUSIONS: Topically applied FA can greatly exceed MICs for canine pathogenic staphylococci at the most common sites of infection. Topical FA therapy should now be evaluated using available formulations in vivo as an alternative to systemic therapy for canine superficial bacterial folliculitis.Peer reviewedFinal Published versio

    Early rise in central venous pressure during a spontaneous breathing trial: A promising test to identify patients at high risk of weaning failure?

    Background The spontaneous breathing trial (SBT) assesses the risk of weaning failure by evaluating some physiological responses to the massive venous return increase imposed by discontinuing positive pressure ventilation. This trial can be very demanding for some critically ill patients, inducing excessive physical and cardiovascular stress, including muscle fatigue, heart ischemia and eventually cardiac dysfunction. Extubation failure with emergency reintubation is a serious adverse consequence of a failed weaning process. Some data suggest that as many as 50% of patients that fail weaning do so because of cardiac dysfunction. Unfortunately, monitoring cardiovascular function at the time of the SBT is complex. The aim of our study was to explore if central venous pressure (CVP) changes were related to weaning failure after starting an SBT. We hypothesized that an early rise on CVP could signal a cardiac failure when handling a massive increase on venous return following a discontinuation of positive pressure ventilation.     The Proton Improvement Plan-II (PIP-II, [1]) linac will include 35 Single Spoke Resonators type 2 (SSR2). A total of eight pre-production SSR2 jacketed cavities will be procured and five installed in the first pre-production cryomodule. The mechanical design of the jacketed cavity has been finalized and it will be presented in this paper along with fabrication and processing experience. The importance of interfaces, quality controls and procurement aspects in the design phase will be remarked as well as lessons learned during the fabrication process. Furthermore, development studies will be presented together with other design validation tests

The expiratory airflow obstruction in the ventilator circuit leads to excessive positive pressure during exhalation called the auto-PEEP effect (2). Therefore, the intrinsic PEEP likely greatly exceeded the extrinsic PEEP set on the ventilator. The mechanisms of the hemodynamic consequences of the auto-PEEP effect include reduced cardiac output from diminished venous return and markedly increased pulmonary vascular resistance due to compression of intraalveolar vessels. Acute cor pulmonale is a consequence of this last phenomenon. Fluid loading in the setting of diminished left ventricular compliance from acute cor pulmonale can lead to pulmonary edema (3). Occult auto-PEEP can have significant cardiopulmonary effects. It is an important cause of death among patients treated with mechanical positive pressure ventilation (4)
