    GLUT1 expression patterns in different Hodgkin lymphoma subtypes and progressively transformed germinal centers

    Background: Increased glycolytic activity is a hallmark of cancer, allowing staging and restaging with 18F-fluorodeoxyglucose-positron-emission-tomography (PET). Since interim-PET is an important prognostic tool in Hodgkin lymphoma (HL), the aim of this study was to investigate the expression of proteins involved in the regulation of glucose metabolism in the different HL subtypes and their impact on clinical outcome. Methods: Lymph node biopsies from 54 HL cases and reactive lymphoid tissue were stained for glucose transporter 1 (GLUT1), lactate dehydrogenase A (LDHA) and lactate exporter proteins MCT1 and MCT4. In a second series, samples from additional 153 HL cases with available clinical data were stained for GLUT1 and LDHA. Results: Membrane bound GLUT1 expression was frequently observed in the tumor cells of HL (49% of all cases) but showed a broad variety between the different Hodgkin lymphoma subtypes: Nodular sclerosing HL subtype displayed a membrane bound GLUT1 expression in the Hodgkin-and Reed-Sternberg cells in 56% of the cases. However, membrane bound GLUT1 expression was more rarely observed in tumor cells of lymphocyte rich classical HL subtype (30%) or nodular lymphocyte predominant HL subtype (15%). Interestingly, in both of these lymphocyte rich HL subtypes as well as in progressively transformed germinal centers, reactive B cells displayed strong expression of GLUT1. LDHA, acting downstream of glycolysis, was also expressed in 44% of all cases. We evaluated the prognostic value of different GLUT1 and LDHA expression patterns; however, no significant differences in progression free or overall survival were found between patients exhibiting different GLUT1 or LDHA expression patterns. There was no correlation between GLUT1 expression in HRS cells and PET standard uptake values. Conclusions: In a large number of cases, HRS cells in classical HL express high levels of GLUT1 and LDHA indicating glycolytic activity in the tumor cells. Although interim-PET is an important prognostic tool, a predictive value of GLUT1 or LDHA staining of the primary diagnostic biopsy could not be demonstrated. However, we observed GLUT1 expression in progressively transformed germinal centers and hyperplastic follicles, explaining false positive results in PET. Therefore, PET findings suggestive of HL relapse should always be confirmed by histology

    Effect of treatment with a full-occlusion biofeedback splint on sleep bruxism and TMD pain: a randomized controlled clinical trial

    OBJECTIVES The purpose of the present study was to analyze treatment outcome with a full-occlusion biofeedback (BFB) splint on sleep bruxism (SB) and TMD pain compared with treatment with an adjusted occlusal splint (AOS). MATERIALS AND METHODS Forty-one patients were randomly allocated to a test (BFB) or a control (AOS) group and monitored over a 3-month period. Output variables were frequency and duration of bruxing events (bursts) and various pain symptoms. RESULTS The BFB group showed a statistically significant reduction in the frequency and duration of bursts and a statistically significant improvement in the patients' global well-being and the facial muscle pain parameter. After the treatment was stopped, the BFB group showed a statistically significant reduction in the average and maximum duration but no statistically significant change in the frequency of bursts. CONCLUSIONS The tested BFB splint is highly effective in reducing SB at the subconscious level, i.e., without waking the patient, and in achieving improvements in global pain perception. The results suggest that the BFB splint also provides a better treatment option for bruxism-related pain than an AOS. However, further research is needed, and specifically studies with a larger patient population displaying higher levels of pain at baseline. CLINICAL RELEVANCE By reducing burst duration and therefore the pathological load on the masticatory apparatus, the BFB splint reduces TMD and bruxism-related symptoms and improves patients' physical well-being.     Influence of intraoral scanning on the quality of preparations for all-ceramic single crowns

    OBJECTIVES To evaluate the influence of intraoral scanning on the quality of preparations for all-ceramic single crowns. MATERIAL AND METHODS A total of 690 randomly selected and anonymized in vivo single crown preparations were examined. Three hundred twenty-three preparations were directly recorded with an intraoral scanner (group IS). Data from plaster casts digitized by a laboratory scanner (group ID; N = 367) served as control. Comparisons included convergence angle, marginal design, marginal substance reduction, homogeneity of the finish line, and undercuts. Evaluation was performed using fully automated specialized software. Data were analyzed applying Kolmogorov-Smirnov, Mann-Whitney U test, and Fisher's exact test. Level of significance was set at p < 0.05. RESULTS Convergence angle was above optimum in both groups, but significantly larger for group IS (p < 0.001). Marginal design was more ideal in group IS concerning the absence of featheredge design (p < 0.001) and reverse bevel (p = 0.211). Marginal substance reduction was closer to prerequisites for all-ceramic restorations in group IS (p < 0.001). Finish lines were more homogeneous in group IS regarding the uniformity of their course (p < 0.001). Undercuts were more frequently found in group ID than in group IS (p < 0.001). CONCLUSIONS Intraoral scanning of prepared teeth has positive impact on the quality of preparations for all-ceramic single crowns regarding marginal substance reduction, marginal design, homogeneity of the finish line, and undercuts. CLINICAL RELEVANCE Accurate preparation design represents a fundamental condition for success of ceramic crowns. Since there is potential for optimization, intraoral scanning might enhance preparation quality providing instant visual feedback

    Impact of tropospheric ozone on terrestrial biodiversity: A literature analysis to identify ozone sensitive taxa

    Tropospheric ozone has long been known as highly phytotoxic. However, currently hardly anything is known whether this air pollutant can also pose a threat to the overall biodiversity in terrestrial ecosystems. Identifying the relative ozone sensitivities of relevant taxa or species can be a first step in an assessment if biodiversity is at risk from ozone. A literature survey was conducted describing experimental and observational results of exposure of organisms and particularly plant species to ozone at environmentally relevant concentrations. For plants ozone effects considered were vegetative growth (e.g. biomass of shoots, foliage, single leaves, stems, and roots), reproduction (number and biomass of seeds and flowers), species development, and symptoms of visible foliar injury. A total of 474 literature references were evaluated which described such effects. For crop plants 54 species with 350 varieties could be considered, while (semi)natural vegetation was represented by 465 vascular plant species comprising 298 herbaceous and 165 woody plant species. Overall, these ozone studies cover only a small fraction of the entire global flora. About two third of woody and about one half of native herbaceous plant species investigated so far have been described as ozone sensitive in at least one study. Ozone sensitivity is slightly higher with respect to visible leaf injury as compared to growth effects, and herbs and deciduous tree species are more responsive than grasses and coniferous trees. Observational results from field surveys conducted along ozone gradients to assess ecosystem effects of ozone in North America and Europe revealed visible macroscopic leaf injuries for 258 herbaceous species. However, these findings often have not been verified under experimental ozone exposure. Albeit the numbers of ozone studies related to a particular plant family varied considerably, high proportions of ozone sensitive species were found e.g. for the families Myrtaceae, Salicaceae and Onograceae, while low proportions of ozone sensitive species were found e.g. for the families Brassicaceae, Boraginaceae and Plantaginaceae. Intra-specific variations of ozone sensitivity of vascular plants were primarily detected in crop species (e.g. wheat, soybean, snap bean, clover, rice), most often derived from screening studies of cultivars for their relative ozone sensitivity/tolerance to ozone. In some cases intra-specific variation of ozone sensitivity is also true for different populations of woody and herbaceous plant species, which often resulted from temporal or spatial differentiation of the relative ozone susceptibility. Therefore, there is some evidence that ozone pollution in the past has already affected plant selection and modified the genetic pool of ozone sensitive genotypes. Information on direct ozone effects on species other than vascular plants (e.g. ferns, mosses, fungi, algae, vertebrates) is very poor or irrelevant, i.e. ozone sensitivities for these taxa could not be described. This is also true for organisms like microbes, arthropods or insects which have not been tested so far for their responses to direct ambient ozone exposure. However, these organisms may be indirectly impaired by ozone via loss of vitality of the plant system to which they are associated

    Effect of Transcranial Brain Stimulation for the Treatment of Alzheimer Disease: A Review

    Available pharmacological treatments for Alzheimer disease (AD) have limited effectiveness, are expensive, and sometimes induce side effects. Therefore, alternative or complementary adjuvant therapeutic strategies have gained increasing attention. The development of novel noninvasive methods of brain stimulation has increased the interest in neuromodulatory techniques as potential therapeutic tool for cognitive rehabilitation in AD. In particular, repetitive transcranial magnetic stimulation (rTMS) and transcranial direct current stimulation (tDCS) are noninvasive approaches that induce prolonged functional changes in the cerebral cortex. Several studies have begun to therapeutically use rTMS or tDCS to improve cognitive performances in patients with AD. However, most of them induced short-duration beneficial effects and were not adequately powered to establish evidence for therapeutic efficacy. Therefore, TMS and tDCS approaches, seeking to enhance cognitive function, have to be considered still very preliminary. In future studies, multiple rTMS or tDCS sessions might also interact, and metaplasticity effects could affect the outcome
