    A case report of desmoplastic ameloblastoma

    Ameloblastomas are the most common benign neoplasm of odontogenic nature. Desmoplastic ameloblastoma (DA) was included in the World Health Organization Classification of Head and Neck Tumors (WHO-2005) as a variant of ameloblastoma with specific clinical, image, and histological features. This case report focuses on a DA which occurred in the mandible of a 25 year-old man. The main signals and symptoms included painless swelling with buccal expansion and tooth displacement. Panoramic and periapical radiographs and cone-bean computed to- mography demonstrated an image mimicking a benign fibro-osseous lesion. The lower border of the mandible was not involved, but tooth displacement could be observed. An incisional biopsy was performed, and the DA was diag- nosed. A marginal resection, maintaining the lower border of the mandible, was performed on teeth 46 to 31. The post-operative period was uneventful. The patient is undergoing routine follow-up and is currently free of disease

    The concerning regarding to the treatment of facial fractures has been reported since the antiquity to now a days. The literature shows the development of the techniques and materials used for this intention. There was a gradual alteration of the empiricists treatments to the ones based on scientific evidences. The materials described in the beginning vary from bandages, extraoral leather straps, gutters manufactured through horns of animals to metal used for closed and open fractures treatment. Through the technological advances used in the production of the fixation systems it started to use materials that were well accepted for the organism, as metallic system of plates and screws.  More recently another system was introduced in the market, the resorbable materials of fixation. Since then, many researches come being developed with intention to formulate a protocol, to test the new materials and to stand out its main indications and possible disadvantages. This paper has the aim to make a brief historical report about the evolution of the materials used in oral maxillofacial surgeries, and to review important topics related to resorbable plates and screws.A preocupação quanto ao tratamento das fraturas faciais é registrada desde a antiguidade até os dias atuais. A literatura demonstra o crescente desenvolvimento de técnicas e materiais utilizados para esse fim, constatando-se uma gradativa alteração dos tratamentos empíricos, baseados em evidências científicas. Os materiais inicialmente descritos variavam desde o uso de bandagens, tiras de couro extra-orais, calhas fabricadas através de chifres de animais até a utilização do metal no tratamento fechado e aberto das fraturas. Através dos avanços tecnológicos utilizados na produção dos sistemas de fixação, passou-se a utilizar materiais que fossem bem aceitos pelo organismo, como o sistema de placas e parafusos metálicos. Mais recentemente, outro sistema foi introduzido no mercado, os materiais de fixação bioabsorvíveis. Desde então, trabalhos vêm sendo desenvolvidos com o intuito de formular um protocolo de utilização, testar os novos materiais e ressaltar suas principais indicações e possíveis desvantagens. Este trabalho tem como objetivo fazer um breve relato histórico sobre a evolução dos materiais utilizados nas cirurgias maxilofaciais, além de revisar tópicos relevantes em relação às placas e parafusos bioabsorvíveis

    Advanced osteosarcoma of the maxilla : a case report

    Osteosarcomas are primary malignant bone tumours in which mesenchymal cells produce osteoid. It is generally the most common malignant bone neoplasm, although lesions of the jaw are uncommon. Osteosarcoma of the jaw (JOS) presents a lower incidence of metastasis and a better prognosis than osteosarcoma of the long bones. However, patients with JOS can exhibit advanced tumours, mainly when early diagnosis is not performed. This article reports on a case of an advanced osteosarcoma of the maxilla. A 38-year-old woman was referred for evaluation and treatment of recurrent fibrous dysplasia of the facial bones. The patient related that she had been diagnosed with fibrous dysplasia four years earlier and, since the first diagnosis, she was submitted to four surgical interventions, all followed by recurrences. The main clinical findings were redness and swelling of the facial skin, upper lip ulceration, and hard palate swelling. Computed tomography showed a large hyperdense mass in right maxilla extending to right orbit and left maxillary sinus. An incisional biopsy was performed and microscopic examination showed areas of osteoid and chondroid formation surrounded by a cellular stroma. The diagnosis of osteosarcoma was established and the patient was recommended for oncologic treatment. Unfortunately, she died six months after the diagnosis due to uncontrollable local spread

    O objetivo neste trabalho foi de analisar a geometria nasal, a nasofaringe e a orofaringe, em crianças respiradoras orais com indicação de cirurgia desobstrutiva das vias aéreas superiores, e verificar a existência de uma possível influência direta da respiração na determinação do tipo facial. Foram avaliadas 657 crianças e, dentre elas, foram selecionadas 75, que se submeteram a avaliação otorrinolaringológica por meio de exames clínico, de radiografia cavum e/ou nasofibroscopia, e com as quais ficaram evidenciadas as obstruções das vias aéreas superiores com indicação cirúrgica. Ao final 41 crianças (21 do gênero masculino e 20 do feminino), entre 6,16 e 14,66 anos de idade, brasileiras, leucodermas, sem história de tratamento ortodôntico ou otorrinolaringológico cirúrgico (remoção de tonsilas faringeanas, palatinas, ou estruturas internas da cavidade nasal), aceitaram participar do estudo. Foram obtidas telerradiografias em norma lateral para a determinação do tipo facial e realizado o exame de rinometria acústica para a obtenção da área transversal mínima da cavidade nasal. A análise estatística dos dados (ANOVA, Razão de Verossimilhanças, análises de variâncias com medidas repetidas com dois fatores, comparações múltiplas de Bonferroni, teste Kruskal- Wallis; com nível de significância de 5%) e a interpretação dos resultados obtidos não mostraram diferença estatisticamente significativa na área total em MCA1 e MCA2 entre os tipos faciais (p > 0,05). Além disso, a obstrução das tonsilas faringeanas e palatinas avaliadas separadamente ou combinadas não variou estatisticamente segundo os tipos faciais (p = 0,582 para tonsila faringeana e p = 0,733 para tonsila palatina; e p = 0,925 quando combinadas). Conclui-se que, no presente estudo, não foi encontrada evidência de que a obstrução das vias aéreas superiores tenha influência determinante na definição do tipo facial.The objective of this paper was to analyze the nasal geometry, the nasopharynx and the oropharynx in oral breathing children with indication for surgery clearence upper airway, in order to verify the existence of a possible direct influence of breathing in the determination of the facial type. A group of 657 children was evaluated and, among them, 75 were selected for otorhinolaryngologic evaluation through clinical exams, cavum radiography and/or nasal endoscopy , when the obstruction of the upper airway with surgical indication has been confirmed. From this subgroup, 41 children agreed to participate in the study (21 males: 20 females), with ages between 6,16 and 14,66 years, brazilians, whites, with no history of orthodontic treatment nor otorhinolaryngologic surgery (removal of the pharyngeal or palatine tonsils or internal structures of the nasal cavity). Radiographs were taken in lateral norm for the determination of the facial type and the acoustic rhinometry exam was performed for the determination of the nasal minimal transversal area. The statistical analyses of the data were made with the variance analysis (ANOVA), likelihood ratio test, analyses of variance with measures repeated with two factors, Bonferroni multiple comparisons, and Kruskal-Wallis test. The significance level was chosen as 0.05. No statistically significant difference was detected in the total area in MCA1 and MCA2 between the facial types. Besides that, the obstruction of the pharyngeal or palatine tonsils, taken separately or in combination, did not vary statistically according to the facial pattern (p = 0.582 for pharyngeal tonsil; p = 0.733 for palatine tonsil; and p = 0.925 when combined). Therefore, in the present study, no evidence has been found that the obstruction of the superior airway has determinant influence in the determination of the facial type

    O presente estudo consiste no relato de um caso onde a distalização do segundo molar inferior impactado do lado direito foi obtida através da ancoragem esquelética com o uso de miniplaca. A verticalização dos segundos molares inferiores impactados, durante muito tempo, colocou-se como um grande desafio para ortodontistas e cirurgiões bucais devido à escassez de recursos de ancoragem destinados para esse fim. A utilização da ancoragem esquelética foi iniciada na clínica ortodôntica em meados dos anos 80 e, desde então, diversas modalidades têm sido desenvolvidas para esse princípio, como a utilização de mini-implantes, implantes dentários e, finalmente, miniplacas, que foram testadas e apresentaram resultados animadores. O assunto é relevante para ortodontistas e cirurgiões bucais, uma vez que o uso das miniplacas pode influir de forma significativa no tratamento de molares inferiores impactados

    Evaluation of children nasal geometry, employing accoustic rhinometry

    The area above the nasal cavity plays a role in respiratory physiology. Aim: To analyze, during a period of growth, a possible change in the minimum cross sectional area (MCA) and nasal volume of the anterior nasal cavity. Materials and Methods: We evaluated 29 children (14 boys and 15 girls) with a mean age of 7.81 years at first examination (M1) and 11.27 years in the second examination (M2), without symptoms of nasal obstruction. The interval between examinations was 36-48 months. Children were subjected to the examination of acoustic rhinometry in which we recorded the minimum cross-sectional areas, volumes and their correlations with gender. Study design: Cohort. Results: The mean cross-sectional area of the nasal cavity of MCA for girls was 0.30 +/- 0.09 cm2 (M1) and 0.30 +/- 0.14 cm2 (M2), while for boys was 0.24 +/- 0.12 cm2 (M1) and 0.32 +/- 0.10 cm2 (M2). The mean values of the total volumes found for the whole sample were 2.17 +/- 0.23 cm3 (MCA1-M1), 2.56 +/- 0.27 cm3 (MCA1-M2), 4.24 +/- 1.17 cm3 (MCA2-M2) and 4.63 +/- 1.10 cm3 (MCA2-M2). Conclusion: There was no significant change in the minimum cross sectional area of the anterior nasal cavity. There was no significant difference between genders for both MCA and for the volume. There was a significant increase in MCA1
