Grade Group accuracy is improved by extensive prostate biopsy sampling, but unrelated to prostatectomy specimen sampling or use of immunohistochemistry
Assessing the accurate Grade Group of a prostate needle biopsy specimen is essential for choosing the adequate therapeutic modality for prostate cancer patients. However, it is well-known that biopsy Grade Group tends to up- or downgrade significantly at radical prostatectomy. We aimed to investigate the correlation between accuracy and biopsy core number, performed immunohistochemical staining (IHC) or prostatectomy specimen sampling, with the latest also being correlated with higher detection rates of adverse pathological features, e.g., positive surgical margins, higher pathological stage or presence of perineural invasion (PnI status). The study cohort consisted of 315 consecutive patients diagnosed with prostate adenocarcinoma via transrectal ultrasound-guided needle biopsy who later underwent radical prostatectomy. We grouped and compared patients based on Grade Group accuracy, presence of IHC on biopsy, margin status, pathological stage, and PnI status. Inter-observer reproducibility was also calculated. Statistical analyzes included ANOVA, Tukey’s multiple comparisons post hoc test, Chi-squared test, and Fleiss kappa statistics. Undergraded cases harboured a significantly lower number of biopsy cores (p < 0.05), than accurately graded cases. Using IHC did not affect grading accuracy significantly, nor did the number of slides from prostatectomy specimens. The mean number of slides was virtually identical when margin status, pathological stage and PnI status of prostatectomy specimens were compared. Inter-observer reproducibility at our institute was calculated as fair (overall kappa = 0.29). Grade Group accuracy is significantly improved by obtaining more cores at biopsy but is unrelated to performed IHC. The extent of sampling prostatectomy specimens, however, did not affect accuracy and failed to significantly improve detection of adverse pathological features
Dongaláb-specifikus életminőség-kérdőív magyar adaptációja = Hungarian validation of the Clubfoot Disease-Specific Instrument
Absztrakt:
Bevezetés: A veleszületett strukturális dongaláb a leggyakoribb
veleszületett lábdeformitás. Eredményesnek mondható kezelése ellenére is az
érintett oldalon a lábszár atrophiájával, a láb hosszkülönbségével, funkcionális
elmaradásával és esetleges fájdalmával kell számolnunk. Számos nemzetközi
kutatás foglalkozik a dongalábbal élők életminőségének vizsgálatával, ennek
ellenére hazánkban még nem történt ilyen felmérés. A betegalapú értékelést
használják a kezelés eredményességének megállapításához, illetve a hagyományos
és a Ponseti-féle kezelési módszer összehasonlításához.
Célkitűzés: A szerzők célja a dongaláb esetén nemzetközileg
legismertebb speciális életminőség-kérdőív, a 10 tételből és két alskálából
(elégedettség, funkció) álló „Clubfoot Disease-Specific Instrument” (a
továbbiakban: „Dongaláb Specifikus Mérőeszköz”) használatának engedélyeztetése,
illetve szülői és gyermekváltozatának hazai adaptálása, majd a belső és külső
validitás vizsgálata. Módszer: A kérdőíveket 75 fő (44 fiú, 28
lány) töltötte ki három korcsoporti megoszlásban (12 év alattiak, 12–17,99 év
közöttiek és 18 év felettiek); átlagéletkor: 12,45 év (2–44 év). Mérőeszközök: a
„Dongaláb Specifikus Mérőeszköz” magyar nyelvű (korcsoportnak megfelelő)
változatai; az általános életminőség mérésére az EQ-5D-3L magyar nyelvű
verziója. Eredmények: A „Dongaláb Specifikus Mérőeszköz”
gyermek- és szülői változata megfelelő belső validitással rendelkezik (szülői:
0,872, gyermek: 0,923), illetve az egyes tételei és az EQ-5D-3L dimenziói között
közepes és erős korrelációs értékek jellemzik az összefüggéseket a meglévő
szignifikáns kapcsolatok mellett. Következtetés: A mérőeszköz
hazai adaptálása sikeresnek mondható, amely hozzájárult a dongalábbal élő
betegek életminőségének megismeréséhez, és lehetőséget nyújt a gyakorlatban a

Abstract:
Introduction: Congenital structural clubfoot is the most common
congenital deformity of the foot. Despite successful treatment, the affected
side may present signs of athrophy, differences in length, functional
maldevelopment and in some cases it may also result in pain. Although several
international studies have been investigating the quality of life of individuals
with clubfoot, no research has been done in Hungary. Patient-based assessment is
used for the evaluation of the effectiveness of treatment and for the comparison
of the traditional versus the Ponseti method.
Aim: To gain permission for the use of the internationally
most well-known Clubfoot Disease-Specific Instrument (hereafter: ‘Clubfoot
Specific Questionnaire’) consisting of 10 items and two subscales, and
furthermore, to adapt its parent and child-version for Hungary and to carry out
the internal and external validity testing of the measure.
Method: The Questionnaire was completed by 75 individuals
(44 boys, 28 girls) belonging to three age groups (under age 12, aged 12–17.99
years and those aged above 18); the mean age was 12.45 years (range: 2–44
years). The measures used: the Hungarian version of the ‘Clubfoot Specific
Questionnaire’ (adapted versions for the given age groups), and the Hungarian
version of EQ-5D-3L to measure the general quality of life.
Results: Both the parent- and the child-version of the
‘Clubfoot Specific Questionnaire’ showed adequate internal validity
(parent-version: 0.872, child-version: 0.923); the individual items showed
medium to strong correlations with the EQ-5D-3L dimensions besides the existing
significant connections. Conclusion: The Hungarian adaptation
of the measure proved to be successful. The validated questionnaire provides a
further tool for the assessment of the quality of life of individuals with
clubfoot and also for the evaluation of the success rate of therapies. Orv

New short cationic antibacterial peptides. Synthesis, biological activity and mechanism of action
We report a theoretical and experimental study on a new series of small-sized antibacterial peptides. Synthesis and bioassays for these peptides are reported here. In addition, we evaluated different physicochemical parameters that modulate antimicrobial activity (charge, secondary structure, amphipathicity, hydrophobicity and polarity). We also performed molecular dynamic simulations to assess the interaction between these peptides and their molecular target (the membrane). Biophysical characterization of the peptides was carried out with different techniques, such as circular dichroism (CD), linear dichroism (LD), infrared spectroscopy (IR), dynamic light scattering (DLS), fluorescence spectroscopy and TEM studies using model systems (liposomes) for mammalian and bacterial membranes. The results of this study allow us to draw important conclusions on three different aspects. Theoretical and experimental results indicate that small-sized peptides have a particular mechanism of action that is different to that of large peptides. These results provide additional support for a previously proposed four-step mechanism of action. The possible pharmacophoric requirement for these small-sized peptides is discussed. Furthermore, our results indicate that a net +4 charge is the adequate for 9 amino acid long peptides to produce antibacterial activity. Szövetfixálás a klinikai gyakorlatban – a 120 éves formalin | Tissue fixation in the clinical praxis – the 120-year-old formalin
A formalint, azaz a formaldehid telített vizes oldatát szinte valamennyi egészségügyi intézményben nagy mennyiségben használják hígított formában emberi szövetminták fixálása céljából. A fixálási eljárás lényegében változatlan formában maradt fenn a fixálószer 120 évvel ezelőtti felfedezése óta. Az alapvető hisztológiai technikák mellett folyamatosan nő a klinikai igény, hogy a szövetek szinte valamennyi biológiai alkotója molekuláris szinten is megvizsgálható legyen. A mára magától értetődő DNS-izolálás és DNS-mutáció-vizsgálatok sikere nagyrészt a minta megfelelő előkészítésén múlik. A modern patológiai diagnosztikai eljárások a fixálás folyamatának szigorú ellenőrzését és magának a formalinoldat aktuális összetételének ismeretét is megkívánják. Orv. Formalin, the saturated watery solution of formaldehyde is used in every health care institution worldwide as an effective and cheap tissue fixative. The beneficial effect of the formaldehyde solution was discovered and published exactly 120 years ago and, despite significant technological developments, the formula is still used in the same way. However, tissue based molecular techniques including multiple gene mutation testing from isolated DNA require a highly standardized tissue preservation procedure and the strict control of the composition of the fixative solution applied.
