    Primary Percutaneous Coronary Intervention for Acute Myocardial Infarction in a Young Female with Idiopathic Thrombocytopenic Purpura: A Case Report and Review

    A 23-year-old female with the diagnosis of idiopathic thrombocytopenic purpura (ITP) was admitted to our hospital with severe chest pain. The electrocardiogram (ECG) revealed acute anterior myocardial infarction. She underwent an immediate cardiac catheterization. An occluded left anterior descending (LAD) was detected by coronary angiography. Reperfusion was performed successfully by angioplasty and stenting with optimal distal flow without any complications.     The methodology for developing a prospective meta-analysis in the family planning community

    <p>Abstract</p> <p>Background</p> <p>Prospective meta-analysis (PMA) is a collaborative research design in which individual sites perform randomized controlled trials (RCTs) and pool the data for meta-analysis. Members of the PMA collaboration agree upon specific research interventions and outcome measures, ideally before initiation but at least prior to any individual trial publishing results. This allows for uniform reporting of primary and secondary outcomes. With this approach, heterogeneity among trials contributing data for the final meta-analysis is minimized while each site maintains the freedom to design a specific trial. This paper describes the process of creating a PMA collaboration to evaluate the impact of misoprostol on ease of intrauterine device (IUD) insertion in nulliparous women.</p> <p>Methods</p> <p>After the principal investigator developed a preliminary PMA protocol, he identified potential collaborating investigators at other sites. One site already had a trial underway and another site was in the planning stages of a trial meeting PMA requirements. Investigators at six sites joined the PMA collaborative. Each site committed to enroll subjects to meet a pre-determined total sample size. A final common research plan and site responsibilities were developed and agreed upon through email and face-to-face meetings. Each site committed to contribute individual patient data to the PMA collaboration, and these data will be analyzed and prepared as a multi-site publication. Individual sites retain the ability to analyze and publish their site's independent findings.</p> <p>Results</p> <p>All six sites have obtained Institutional Review Board approval and each has obtained individual funding to meet the needs of that site's study. Sites have shared resources including study protocols and consents to decrease costs and improve study flow. This PMA protocol is registered with the Cochrane Collaboration and data will be analyzed according to Cochrane standards for meta-analysis.</p> <p>Conclusions</p> <p>PMA is a novel research method that improves meta-analysis by including several study sites, establishing uniform reporting of specific outcomes, and yet allowing some independence on the part of individual sites with respect to the conduct of research. The inclusion of several sites increases statistical power to address important clinical questions.     Influence of socioeconomic factors on pregnancy outcome in women with structural heart disease

    OBJECTIVE: Cardiac disease is the leading cause of indirect maternal mortality. The aim of this study was to analyse to what extent socioeconomic factors influence the outcome of pregnancy in women with heart disease.  METHODS: The Registry of Pregnancy and Cardiac disease is a global prospective registry. For this analysis, countries that enrolled ≥10 patients were included. A combined cardiac endpoint included maternal cardiac death, arrhythmia requiring treatment, heart failure, thromboembolic event, aortic dissection, endocarditis, acute coronary syndrome, hospitalisation for cardiac reason or intervention. Associations between patient characteristics, country characteristics (income inequality expressed as Gini coefficient, health expenditure, schooling, gross domestic product, birth rate and hospital beds) and cardiac endpoints were checked in a three-level model (patient-centre-country).  RESULTS: A total of 30 countries enrolled 2924 patients from 89 centres. At least one endpoint occurred in 645 women (22.1%). Maternal age, New York Heart Association classification and modified WHO risk classification were associated with the combined endpoint and explained 37% of variance in outcome. Gini coefficient and country-specific birth rate explained an additional 4%. There were large differences between the individual countries, but the need for multilevel modelling to account for these differences disappeared after adjustment for patient characteristics, Gini and country-specific birth rate.  CONCLUSION: While there are definite interregional differences in pregnancy outcome in women with cardiac disease, these differences seem to be mainly driven by individual patient characteristics. Adjustment for country characteristics refined the results to a limited extent, but maternal condition seems to be the main determinant of outcome

    ÖZETProf. Güngör Güner, Çağdaş Türk Seramik Sanatı’nın en önemli sanatçılarından ve eğitimcilerinden biridir. Bu özelliklerinin dışında araştırmacı yönüyle de dikkate değerdir. 1972-78 tarihleri arasında yaptığı Anadolu’da Yaşamakta Olan İlkel Çömlekçilik başlıklı araştırması kitap olarak yayınlanmıştır. Bu çalışması Türkiye’de bu konuda yapılan bu kadar kapsamlı ilk ve tek araştırmadır. Devlet Tatbiki Güzel Sanatlar Yüksek Okulu (bugünkü Marmara Üniversitesi Güzel Sanatlar Fakültesi) Seramik Bölümü’nden mezun olan sanatçı, 37 yıl boyunca bu kurumda eğitimci olarak hizmet vermiştir. Toplam 18 yıl Seramik Bölüm Başkanlığı yapmış ve 28 Nisan 2008’de emekli olmuştur. Bölüm başkanlığı sırasında endüstri kuruluşlarıyla ilişki kurup bölümün eksikliklerinin giderilmesini sağlamış ve bugüne kadar çok sayıda öğrenci yetiştirmiştir. Prof. Güngör Güner, seramiğin bütün alanlarıyla ilgilenmiş, ancak yapıtlarının biçimlendirilmesinde geleneksel çömlekçi çarkı yani tornanın ayrıcalıklı bir yeri olmuştur. Serbest form ve panolarının yanı sıra yapıtlarının büyük bir bölümünü, çömlekçi çarkında biçimlendirilmiş, yorum ağırlıklı üç boyutlu nesneler ve işlevsel, özgün kap kaçaklar oluşturur. 1990’larda geleneği yadsımaksızın daha soyut kaplar ve soyut nesneler yapmaya devam etmiştir. Ayrıca seramik sanatının sınırlarını aşan kavramsal nitelikli çalışmalar da yapmıştır. Öte yandan endüstriyel seramik alanında da ürünler vermiş, bazı formları seri üretilmek üzere Alman firmaları Hutschenreuther ve Goebel firmaları tarafından satın alınmıştır. Tüm sanat çalışmalarının en belirgin özelliği yalın bir üsluba sahip olmasıdır. Almanya’da seramik mühendisliği eğitimi de gören Güngör Güner, seramik teknolojisine en hakim sanatçılardan biridir. Seramik sanatı, teknikleri ve eğitimi üzerine çok sayıda makalesi bulunmaktadır. Sanat eleştirmenleri, meslektaşları, hocaları, öğrencileri ve endüstri kuruluşlarının temsilcileri Güngör Güner’in Türk seramiğinde ayrıcalıklı bir yeri olduğunu doğrulamaktadırlar. SUMMARYProf. Güngör Güner is one of the most important artists and the academics of the Contemporary Ceramics Art in Turkey. Beyond to this characteristic, she has an importance in the level investigator. Her research, The Living Primitive Pottery in Anatolia, written between years 1972-78, has been published in book shape. This her work is the first and only contained search in this argument. The artist, graduated in the Ceramic Department of the Faculty of Fine Arts of the University of Marmara, has engaged as the academic in the same institute during 37 years. She has been worked as head director in the Ceramic Department 18 years and in the 28 April 2008 has gone in pension. During her management period established the relationship with the industrial agencies, removed the deficiencies of the department and has instructed many students until today. Prof Güngör Güner is engaged all branches of the ceramics, but she had privileges the lathe of traditional pottery in the formation of its works. Beyond to the wall panel and free shapes, the majority of her works is formed of the three-dimensional objects is based on the comment and functional, original potteries, formed on the lathe. In years 90 without to deny the tradition she has continued to form more abstract objects and potteries. Moreover she has composed the conceptual works that are exceed the limits of the ceramics art. From the other part she has given the life to the works in the industrial field of the ceramics, some her shapes have been bought by the German companies Hutschenreuther and Goebel for the production in series. The most evident characteristic of her all works is having a pure style. Güngör Güner that has taken also the formation of engineering of ceramics in Germany, is one of the artists dominant on the ceramics technology.She has many articles on the ceramics art, of the techniques and the educazine.. The art critics, the colleagues, the professors, the students and the representatives of the industrial agencies confirm that Güngör Güner has privileges in the Turkish ceramics art.SOMMARIOProf. Güngör Güner é una dei piu` importanti artisti e degli accademici del Arte Ceramica Contemporanea in Turchia. Oltre a questa caratteristica ha una importanza nel livello ricercatore.La sua ricerca nominata Il Vassoio Primitivo Che Sta Vivendo In Anatolia, scritta tra gli anni 1972-78, é pubblicato in forma di libro. Questo suo lavoro é la prima ed l’unica ricerca contenuta in questo argomento.L’artista, laureata nel dipartimento di Ceramica della Facolta delle Belle Arti dell’ Universita di Marmara, si é impegnata come l’accademica nello stesso istituto durante 37 anni. Dall’altra parte ha dato la vita alle opere nel settore industriale della ceramica, qualche sue forme si sono comprate delle ditte tedesche Hutschenreuther e Goebel per la produzione in serie. La caratteristica piu` impegnativa di tutte le sue opere é di avere un stile puro.Güngör Güner che ha preso anche la formazione delle ingegneria di ceramica in Germania, é una degli artisti dominante sulla tecnologia ceramica. Ha tanti articoli sull’arte ceramica, delle tecniche e dell’educazione.
