Aim: This study, conducted in Turkey, aims to increase the practicability of health counselling and protective services offered to young people in the long term. In the short term, it is aimed to determine health counselling needs of students at dormitories and it might enlighten researchers working in this field. Methods: A pilot questionnaire, developed in order to determine health needs of students staying at dormitories, was conducted in two dormitories in Ankara. Focus group interviews were done, also. After that, in nine dormitories from nine provinces from all over the Turkey, questionnaire and health screening tests were applied to 5,852 volunteers. Results: The common topics for both sexes which students would like to receive counselling are nutrition and anxiety about exams. Conclusion: As a result of the study, service has been planned for the identified needs of the students.   &nbsp

Does bile reflux reduce Helicobacter pylori gastritis?

Background. Chronic abdominal pain is a frequent childhood complaint. This study aims to determine the relationship between bile reflux, which is increasing with the growth in packaged food consumption resulting from the changing food industry, and Helicobacter pylori gastritis. Methods. In this retrospective study, 804 cases where there was an endoscopic examination for abdominal pain were included. We recorded the patients' age, sex, and macroscopic and microscopic endoscopic findings. Patients with chronic diseases were excluded. Results. Our study included 804 cases. Of patients, 61.8% were female and 38.2% were male. The mean age was 11.56 +/- 4.14 years. The Helicobacter pylori gastritis rate was found to be 22.3% among all patients. Bile reflux was seen in 192 (23.9%) patients. Only 27 (14.1%) of the 192 patients had Helicobacter pylori positivity (p=0.002). Conclusions. Helicobacter pylori gastritis is less common among patients with bile reflux. In another study conducted in our outpatient clinic before the 2000s, the frequency of Helicobacter pylori gastritis was found to be 40%, but after 2000 this rate decreased to 22.3% due to bile reflux caused by the changing food industry. This result may be explained by the bactericidal effects of bile acids

An autoimmune disease refractory to immunosuppressive regimens: Celiac disease diagnosed long after liver transplantation

WOS: 000325369400002PubMed ID: 23962034CD is defined as T-lymphocyte-mediated gluten sensitivity. Although CD is known to affect the small intestine, it is nonetheless a multisystem disorder. Liver involvement in CD may vary from isolated hypertransaminasemia to cirrhosis. Because CD is an inappropriate immune response to gluten proteins, strict gluten-free diet is the principal therapy, along with management of liver dysfunction. In patients who fail to respond to a gluten-free diet, immunosuppressive drugs may improve intestinal inflammatory activity in untreated CD. The present case report is of a 25-yr-old woman with diarrhea lasting several weeks. The patient had received a liver transplant 13yr earlier, and presented with cryptogenic cirrhosis diagnosed as CD. This appears to be the first case of its kind in which a pediatric long-term liver transplant patient presents with diarrhea eventually diagnosed as CD whose diet included gluten, and who was treated by an immunosuppressive drug regimen. Because of the normalization of CD-related antibodies in the post-transplantation period without gluten restriction, CD should be part of a list of diagnostic possibilities in liver transplant patients presenting with diarrhea of unknown etiology

Efficacy and Maternal Comfort of Sequential versus Simultaneous Breast Expression by Mothers of Critically III Newborns

WOS: 000418709600009Aim: Expressed breast milk is beneficial for infants in neonatal intensive care unit (NICU) when direct breastfeeding is not possible. Breast expression with manual or electric breast pumps is promoted for the initiation and maintenance of lactation in this critical period. In this study, we aimed to compare the efficacy and maternal comfort of sequential versus simultaneous breast expression in newly delivered mothers whose infants were admitted to NICU. Materials and Methods: Thirty five mothers were followed prospectively for milk expression either sequentially (n=21, group 1) or simultaneously (n=14, group 2) with breast pumps for 10 days. The total amount of milk expressed per day and per each period was recorded together with the duration of breast expression. The mothers' impressions about the easiness and comfort of the procedure, and their satisfaction with the amount of milk were evaluated with a questionnaire at the end of the study. Results: The amounts of expressed milk per day and per each expression period were similar in both groups. However, time spent for each expression period was significantly lower in the simultaneous breast expression group. Mothers in this group gave higher scores with regard to ease of use when compared to mothers in the sequential expression group (4.36 +/- 0.50 vs. 4.00 +/- 0.44, p=0.046). The mothers graded both methods with similar scores in all other parameters. Conclusion: Simultaneous breast expression is time saving while both simultaneous and sequential breast expression are similarly efficient for milk production, and both methods are helpful and tolerable in promoting breastfeeding for NICU mothers

Protective effect of Pycnogenol on cisplatin-induced ototoxicity in rats

Context: Pycnogenol((R)), which is French maritime pine bark extract, is a potent antioxidant. It is used in medical conditions caused by oxidative stress. Cisplatin (cis-diamminedichloroplatinum II) is an antineoplastic agent. However, its serious side effects such as ototoxicity limit its usage. Objective: Antioxidants can be used to prevent ototoxicity. We investigated the effect of Pycnogenol((R)) on cisplatin-induced ototoxicity.Materials and methods: Rats were randomly assigned to four groups of five. Distortion product-evoked otoacoustic emissions (DPOAE) test was performed for each rat. The experimental groups were as follows: Control Group, Pycnogenol((R)) Group: 10mg/kg Pycnogenol((R)) intraperitoneally for 7 days, Cisplatin Group: intraperitoneally 15mg/kg single injection of cisplatin on the fifth day, Cisplatin+Pycnogenol((R)) Group: intraperitoneally 10mg/kg Pycnogenol((R)) treatment for 7 days, additionally on the fifth day, 15mg/kg single injection of cisplatin was given. On the eighth day, DPOAE was re-performed and rats were sacrificed. Apoptosis was evaluated histopathologically. Results: Mean percentage of apoptotic cells was 1.5, 3, 30 and 11% in organ of Corti and 2, 2, 40, 15% in spiral ganglion neurons in Control Group, Pycnogenol((R)) Group, Cisplatin Group and Cisplatin+Pycnogenol((R)) Group, respectively. Cisplatin Group and Cisplatin+Pycnogenol((R)) Group were significantly different when compared to Control Group histopathologically both in organ of Corti and spiral ganglion neuron (p<0.001, p=0.019, p=0.001, p=0.015). DPOAE results showed that Cisplatin+Pycnogenol((R)) Group was significantly different when compared to Cisplatin Group at 3, 6 and 8kHz (p<0.05).Conclusion: Pycnogenol protected against cisplatin ototoxicity. Also, pycnogenol is not ototoxic
