    Phylogenetic analysis and genotype distribution of Hepatitis B Virus (HBV) in Roraima, Brazil

    Hepatitis B virus (HBV) infection is a serious global health problem. HBV has a high viral genetic diversity, with 10 genotypes recognized. In Brazil, the Roraima State is the third in the Northern region regarding the number of hepatitis B cases. On the other hand, few data on HBV genotyping and phylogenetic analysis are available. The purpose of this study is to characterize the HBV genotypes circulating in Roraima State. Of the 113 chronic hepatitis B patients enrolled in this study, 40 were HBV-DNA positive. A fragment of 280 bp (S gene) was amplified by PCR and submitted to nucleotide sequencing. A dataset containing the viral sequences obtained in this study, plus 130 obtained from GenBank was used for genotyping by phylogenetic analysis. The HBV subgenotype distribution found was A1 (62.5%), A2 (7.5%), D2, D3, D4 (2.5%), F2a (12.5%), and F3 (10%). We characterized the genotypes and subgenotypes of HBV circulating among patients in the State of Roraima. In addition, our study shows for the first time the HBV/F3 genotype circulating in Brazil. In conclusion, our findings showed a high diversity of HBV genotypes in Roraima, which is also found in other Brazilian geographical regions

    Gold miners augment malaria transmission in indigenous territories of Roraima state, Brazil

    Abstract Background Endemic malaria is present in all 15 municipalities of Roraima state, Brazilian Amazonia. Knowledge of epidemiological data of specific populations can guide health policies to formulate effective strategies for integrated control of health-disease care. This study aims to ascertain when, where and who fell ill with malaria in Roraima state from 2010 to 2020. Methods This descriptive study was based on statistical secondary surveillance data through the analysis of relationships underlying numbers of cases, hospitalizations and deaths using the Malaria Epidemiological Surveillance Information System, Mortality Information System and Hospitalization Information System. Results From 2010 to 2020, there were 138,504 autochthonous cases, 26,158 Venezuelan imported cases, 3765 hospitalizations, and 77 deaths from malaria reported in Roraima. Annual parasitic incidence and the number of hospitalizations showed impressive changes over the period, but without significantly correlating with number of deaths. The proportion of Plasmodium falciparum infections had significant shifts throughout this study. Malaria prevalence in indigenous and mining areas has been increasing since 2014. Conclusion The presence of miners in indigenous areas is a reality that has been contributing to the increase of malaria cases in Roraima. The need to implement health policies that also meet this contingent is reinforced

    Endocardite bacteriana em paciente com diabetes mellitus tipo 2

    Introdução: A endocardite infecciosa é uma infecção microbiana da superfície do endocárdio. Acomete sobretudo valvas cardíacas, em geral, a mitral. Etiologias comuns são: uso de drogas injetáveis, infecção oral, hemodiálise a longo prazo e diabetes mellitus (DM). O diagnóstico é realizado por meio dos Critérios de Duke. Objetivos: Relatar caso de endocardite de paciente do Hospital Santa Lucinda (HSL). Metodologia: As informações foram obtidas pelo exame clínico, prontuário da paciente, relato da equipe médica e revisão de literatura. Relato de Caso: Paciente L.F.P.L., F., 54 anos, DM tipo 2, hipertensa, com insuficiência renal crônica não-dialítica, com úlceras de membros inferiores (MMII). Foi realizado debridamento e uso de antibiótico (ATB). Posteriormente, procurou hospital com queixa de dispneia. Diagnosticada com pneumonia (sic), usou ATB. A posteriori, apresentou anasarca e febre. Não houve leucocitose provavelmente devido ao uso de ATB. Constatou-se sopro mitral; no ecocardiograma: insuficiência mitral e tricúspide e vegetação em valva mitral, indicando endocardite infecciosa. Encaminhada ao HSL para cirurgia, apresentou na admissão: derrame pleural, hemo- e leucograma (LEU) sem alterações, ureia 142, glicose 256 (mg/dL) e albumina 2,6 (g/dL). Após 6 dias, foi inserida a prótese biomitral e cerclagem de valva tricúspide. Realizou-se transfusões sanguíneas pós-cirúrgicas. Manifestou parada cardiorrespiratória (PCR), realizada ressuscitação cardiopulmonar. Após 10 dias, apresentou arritmia, PCR, sepse e foi a óbito. Conclusões: O manejo adequado do DM pode evitar complicações, como úlceras de MMII, evitando, consequentemente, a disseminação via hematogênica e o quadro de endocardite. Deve-se considerar o uso prévio de ATB ao se analisar o LEU do paciente

    FUNDAMENTOS: Micoses superficiais estritas são infecções fúngicas que se localizam nas camadas superficiais da pele e seus anexos. As micoses superficiais cutâneas representadas pelas dermatofitoses e candidíases podem ultrapassar a camada córnea da pele. Na região amazônica possuem incidência elevada. OBJETIVOS: Estudar as micoses superficiais, estritas e cutâneas, diagnosticadas sob o ponto de vista epidemiológico e micológico. PACIENTES E MÉTODOS: Pacientes com suspeita clínica de micoses superficiais submetidos a exame micológico no período de março a novembro de 2003 no Laboratório de Micologia Médica/CPCS/INPA. RESULTADOS: Foram realizados 394 exames, tendo 256 apresentado diagnóstico positivo. As micoses mais incidentes foram onicomicoses (135) e pitiríase versicolor (98). Malassezia spp. (77) e Candida spp. (72) foram os agentes fúngicos mais isolados. Tinea capitis apresentou maior ocorrência nos pré-escolares (3), e onicomicoses em adultos (94). O sexo feminino foi o mais acometido (91). Todas as classes sociais foram infectadas, com predominância da C (37). CONCLUSÃO: Onicomicoses e pitiríase versicolor acometeram sobretudo adultos. A Tinea capitis ocorre principalmente, em crianças. As micoses superficiais apresentaram mais incidentes nas mulheres. Malassezia spp. e Candida spp. foram os agentes mais isolados.<br>BACKGROUND - Restricted superficial mycoses are fungal infections that appear on the skin superficial layers and their adnexa. However skin superficial mycoses represented by dermatophytoses and candidiasis can invade the corneal layer. This type of mycosis has a high incidence in the Amazon region. OBJECTIVES - To study the restricted superficial mycoses under the epidemiological and mycological point of view. PATIENTS AND METHODS - Patients presenting clinical suspicion of superficial mycoses submitted to mycological examination from March to November 2003 at the Clinical Mycology Laboratory/CPCS-INPA. RESULTS - Three hundred and ninety-four examinations were carried out throughout the period and 256 were positive. The mycoses with higher incidence were onychomycosis (135) and pityriasis versicolor (98). The most often isolated agents were Malassezia spp. (77) and Candida spp. (72). Tinea capitis was more frequent in pre-school children (3) and onychomycosis in adults (94). Mycoses were more prevalent in women (91). All socioeconomic classes were affected, with a predominance in class C (37). CONCLUSION - Onychomycosis and pityriasis versicolor affected mostly adults and Tinea capitis occured mainly in children. Superficial mycoses were more predominant in women. Malassezia spp. and Candida spp. were the most often isolated agents

    NEOTROPICAL ALIEN MAMMALS: a data set of occurrence and abundance of alien mammals in the Neotropics

    Biological invasion is one of the main threats to native biodiversity. For a species to become invasive, it must be voluntarily or involuntarily introduced by humans into a nonnative habitat. Mammals were among first taxa to be introduced worldwide for game, meat, and labor, yet the number of species introduced in the Neotropics remains unknown. In this data set, we make available occurrence and abundance data on mammal species that (1) transposed a geographical barrier and (2) were voluntarily or involuntarily introduced by humans into the Neotropics. Our data set is composed of 73,738 historical and current georeferenced records on alien mammal species of which around 96% correspond to occurrence data on 77 species belonging to eight orders and 26 families. Data cover 26 continental countries in the Neotropics, ranging from Mexico and its frontier regions (southern Florida and coastal-central Florida in the southeast United States) to Argentina, Paraguay, Chile, and Uruguay, and the 13 countries of Caribbean islands. Our data set also includes neotropical species (e.g., Callithrix sp., Myocastor coypus, Nasua nasua) considered alien in particular areas of Neotropics. The most numerous species in terms of records are from Bos sp. (n = 37,782), Sus scrofa (n = 6,730), and Canis familiaris (n = 10,084); 17 species were represented by only one record (e.g., Syncerus caffer, Cervus timorensis, Cervus unicolor, Canis latrans). Primates have the highest number of species in the data set (n = 20 species), partly because of uncertainties regarding taxonomic identification of the genera Callithrix, which includes the species Callithrix aurita, Callithrix flaviceps, Callithrix geoffroyi, Callithrix jacchus, Callithrix kuhlii, Callithrix penicillata, and their hybrids. This unique data set will be a valuable source of information on invasion risk assessments, biodiversity redistribution and conservation-related research. There are no copyright restrictions.
