    Antibody Responses after Influenza Vaccination in Elderly People: Useful Information from a 27-Year Study (from 1988– 1989 to 2014–2015)

    Elderly people are more likely than younger people to get flu complications and respond suboptimally to influenza vaccination because of the presence of comorbidities and immunosenescence. In order to collect information about this issue, we evaluated data obtained in 27 winters of study, from 1988–1989 to 2014–2015, in frail elderly institutionalized people (≥60 years) vaccinated with commercially available seasonal trivalent inactivated influenza vaccines. The antibody response was examined comparing hemagglutination inhibition antibody titers in sera collected from 4461 volunteers before and 30 days after vaccination. Examining the results as crude mean responses, we evidenced the ability of influenza vaccines to induce significant increases in antibody titers against all the three vaccine antigens satisfying at least one of the three criteria of the Committee for Medical Products for Human Use (CHMP). Higher responses were found against A/H3N2 vaccine components and, examining different subgroups, in volunteers receiving 45 μg vaccine as compared with 30 μg and in female as compared with male subjects. Very elderly people (>75 years) gave better responses than younger elderly (≤75 years) at least against A/H1N1 strain and the last licensed potentiated vaccines (MF59-adjuvanted and intradermal) were more immunogenic than traditional vaccines (whole, subunit, and split)

    Case report: Coxiella burnetii endocarditis in the absence of evident exposure

    Q fever is a worldwide zoonotic disease caused by Coxiella burnetii. In humans, it can manifest clinically as an acute or chronic disease and endocarditis, the most frequent complication of chronic Q fever is associated with the greatest morbidity and mortality. We report a severe case of endocarditis in a 55-year-old man with a history of aortic valve replacement affected by monoclonal gammopathy of undetermined significance (MGUS), and living in a non-endemic area for C. burnetii. After two episodes of fever of unknown origin (FUO), occurring 2 years apart and characterized by negative blood cultures, a serological diagnosis of Q fever endocarditis was performed even though the patient did not refer to possible past exposure to C. burnetii. Since people with preexisting valvular heart disease, when infected with C. burnetii, have reported a 40% risk of Q fever endocarditis, clinicians should maintain a high index of suspicion for infective endocarditis in all patients with FUO even when the exposure to C. burnetii appears to be unlikely

    Induction of Cross-Reactive Antibodies to 2009 Pandemic H1N1 Influenza Virus (pH1N1) After Seasonal Vaccination (Winters 2003/04 and 2007/08)

    Abstract We measured haemagglutination inhibiting (HI) serum antibody titers to vaccine matched A/H1N1 influenza virus strain and to the new pandemic 2009 A/H1N1 (pH1N1) virus in two groups of volunteers prior and after 2003/2004 or 2007/2008 influenza seasonal vaccine administration. The responses were examined considering the overall volunteers studied in the two winters (144 and 79, respectively) and grouping those subjects in birth cohort classes (1903–1919; 1920–1957; 1958–1977). Before vaccination, HI antibody titers were found in all the groups examined and, on comparing the different age-groups, titers were higher in the younger groups as compared with the oldest against the A/H1N1 seasonal strains but titers were higher in the oldest as compared with the younger ones against the pH1N1 strain. Vaccination induced significant increases in HI titers against the matched A/H1N1 vaccine strains in all the groups examined. The responses satisfied the EMEA criteria and were higher in the youngest volunteers as compared with older groups. Increases were also found in the level of cross-reactive HI antibodies to the new pandemic 2009 A/H1N1 virus although in most instances the requirements of the EMEA were not met

    Comparative Study of Immunogenicity of Split, Intradermal and MF59-adjuvanted Influenza Vaccines in Elderly Institutionalized Subjects

    Abstract The reduced immunogenicity and effectiveness of influenza vaccines in subjects presenting high risk of influenza-related complications, hospitalization and death, led the innovative drive to search for new strategies to implement the immune response elicited by influenza vaccines including addition of adjuvants, and use of alternative routes of antigen delivery.In this study we evaluated and compared the immune antibody response induced in 252 elderly volunteers living in nursing homes after immunization with three different 2012-2013 seasonal trivalent inactivated influenza vaccines: a conventional split vaccine (n=26), and two potentiated vaccines (a subunit vaccine adjuvanted with MF59 (n=137) or a split vaccine administered intradermally (n=89)), specially licensed for elderly people. Haemagglutination inhibiting (HI) antibody titers were assessed in blood samples collected before and one month after vaccination.The results were evaluated as increase in HI titers found comparing pre- and post-vaccination sera and according to the Committee for Medicinal Products for Human Use (CHMP) criteria for approval of influenza vaccines in the elderly. Significant antibody increases and fulfillment of all the three CHMP requirements were observed against A/H3N2 and B antigens following immunization with the two potentiated vaccines. After immunization with conventional vaccine responses were lower against A/H3N2 and equivalent against the B antigen. The two potentiated vaccines induced significant antibody increases against A/H1N1 antigen, however, only one of the CHMP criteria was reached. The HI antibody increases after conventional vaccine were significant only for the geometric mean titer and none of the CHMP criteria was fulfilled. The antibody responses induced by the two potentiated vaccines against the three vaccine antigens wereequivalent although post-vaccination titers against the B antigen tended to be higher in subjects vaccinated with intradermal vaccine than in individuals receiving MF59-adjuvanted vaccine. In conclusion the use of MF59 adjuvant and intradermal vaccination appear to be appropriate strategies to address the challenge of declining immune response in the elderly after influenza vaccination

    Design and validation of a self-administered questionnaire to assess knowledge, attitudes and behaviours about Zika virus infection among general population in Italy

      Background Zika (ZIKV), a flavivirus firstly identified in rhesus monkeys in Zika Forest of Uganda, in 1947, is an emerging virus  transmitted mainly by mosquitoes bites. Due to ZIKV adaptation to humans, that can maintain a mosquito-human-mosquito transmission cycle, it is essential to know the attitudes, knowledge and behaviours of general population regarding ZIKV prevention. Our main study aims were to develop and validate a questionnaire administered to the general population, in order to assess attitudes, knowledge and behaviours around prevention and control of Zika infection. Methods A questionnaire was developed based on a comprehensive review of the extant literature, pre-existing questionnaires and experts focus groups. Results The final, validation version of the questionnaire comprised 8 items, with good psychometric properties (Cronbach’s alpha of 0.81). Overall test/re-test concordance was 0.86, ranging from 0.76 to 0.94 according to each item. Conclusion In conclusion, the questionnaire seems to be an appropriate and useful tool to detect cognitive gaps concerning behaviours responsible for possible transmissions of the disease, even in a non-endemic country such as Italy. Future analysis will explore the factorial structure of the questionnaire as well as knowledge, beliefs and attitudes concerning ZIKV among Italian general population

    Drugs for Influenza Treatment: Is There Significant News?

    Vaccines remain the best measure to reduce total influenza burden. However, presently available influenza vaccines have some limitations that cause a reduced efficacy compared to immunization practices with other respiratory pathogens. This paper shows the clinical roles of antiviral drugs against influenza that have been licensed in at least one country and the potential roles of compounds that are in development. Several attempts have been made to develop new agents against influenza viruses to overcome the supposed or demonstrated limitations of neuraminidase inhibitors (NAIs). Antibodies against the highly conserved stem region of the haemagglutinin molecule of influenza A viruses and drugs that target different stages of the influenza virus life cycle than NAIs in human cells have been developed and tested. Among these preparations, baloxavir marboxil (BAM), and favipiravir (FP) (i.e., polymerase inhibitors) are the only drugs that have reached the market (the first in Japan and the USA, and the second only in Japan). Other antiviral compounds and monoclonal antibodies are in advanced stage of development, but none of these new drugs and monoclonal antibodies in development have adequate characteristics to substitute for NAIs at present. However, although NAIs remain the drug of choice for influenza treatment, their overuse has to be avoided. Accurate selection of patients for whom treatment is truly needed is required

    Substantial Histone Reduction Modulates Genomewide Nucleosomal Occupancy and Global Transcriptional Output

    The basic unit of genome packaging is the nucleosome, and nucleosomes have long been proposed to restrict DNA accessibility both to damage and to transcription. Nucleosome number in cells was considered fixed, but recently aging yeast and mammalian cells were shown to contain fewer nucleosomes. We show here that mammalian cells lacking High Mobility Group Box 1 protein (HMGB1) contain a reduced amount of core, linker, and variant histones, and a correspondingly reduced number of nucleosomes, possibly because HMGB1 facilitates nucleosome assembly. Yeast nhp6 mutants lacking Nhp6a and -b proteins, which are related to HMGB1, also have a reduced amount of histones and fewer nucleosomes. Nucleosome limitation in both mammalian and yeast cells increases the sensitivity of DNA to damage, increases transcription globally, and affects the relative expression of about 10% of genes. In yeast nhp6 cells the loss of more than one nucleosome in four does not affect the location of nucleosomes and their spacing, but nucleosomal occupancy. The decrease in nucleosomal occupancy is non-uniform and can be modelled assuming that different nucleosomal sites compete for available histones. Sites with a high propensity to occupation are almost always packaged into nucleosomes both in wild type and nucleosome-depleted cells; nucleosomes on sites with low propensity to occupation are disproportionately lost in nucleosome-depleted cells. We suggest that variation in nucleosome number, by affecting nucleosomal occupancy both genomewide and gene-specifically, constitutes a novel layer of epigenetic regulation

    Monitoring of indoor bioaerosol for the detection of SARS-CoV-2 in different hospital settings

    BackgroundSpore Trap is an environmental detection technology, already used in the field of allergology to monitor the presence and composition of potentially inspirable airborne micronic bioparticulate. This device is potentially suitable for environmental monitoring of Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) in hospital, as well as in other high-risk closed environments. The aim of the present study is to investigate the accuracy of the Spore Trap system in detecting SARS-CoV-2 in indoor bioaerosol of hospital rooms.MethodsThe Spore Trap was placed in hospital rooms hosting patients with documented SARS-CoV-2 infection (n = 36) or, as a negative control, in rooms where patients with documented negativity to a Real-Time Polymerase Chain Reaction molecular test for SARS-CoV-2 were admitted (n = 10). The monitoring of the bioaerosol was carried on for 24 h. Collected samples were analyzed by real-time polymerase chain reaction.ResultsThe estimated sensitivity of the Spore Trap device for detecting SARS-CoV-2 in an indoor environment is 69.4% (95% C.I. 54.3-84.4%), with a specificity of 100%.ConclusionThe Spore Trap technology is effective in detecting airborne SARS-CoV-2 virus with excellent specificity and high sensitivity, when compared to previous reports. The SARS-CoV-2 pandemic scenario has suggested that indoor air quality control will be a priority in future public health management and will certainly need to include an environmental bio-investigation protocol

    Influenza vaccination is considered the best mean for preventing the higher rates of mortality associated with influenza virus infection in the elderly as compared with younger people. Since the number of very elderly subjects, aged >85 years, is rapidly increasing, and some authors reported increments in influenza-associated mortality with age, the aim of this study was to increase the limited information available on the immunogenicity of the influenza vaccines in this age group. This was a retrospective study which analyzed the antibody response induced by commercially available trivalent inactivated influenza vaccines in 1491 elderly subjects (60–85 years old) and 1139 very elderly subjects (>85 years old) during 21 winter seasons included between 1993–1994 and 2014–2015. The antibody response of the two age groups was, in most instances, acceptable according to the Committee for Medical Products for Human Use and comparable. In accordance with previous data obtained in the elderly, the use of MF59-adjuvanted or intradermal administered vaccines (enhanced vaccines) was found to be preferable as compared with conventional formulations (split or subunit vaccines). Vaccines containing new strains induced higher antibody response as compared with vaccines with the same antigenic composition of the previous years. These results suggest that the current recommendation for use of enhanced influenza vaccines for the elderly is appropriate, but that efforts to improve the effectiveness of the present prophylactic measures against influenza are needed, especially in the years with vaccines with the same antigenic composition of the previous winter season
