    A study on management of congenital clubfoot by Ponsetti technique in a rural medical college in India

    Background: Congenital clubfoot also known as CTEV (congenital talipus equinovarus) is a common and debilitating congenital musculoskeletal anamoly affecting the children across the world with a slight preponderence in third world countries. The Ponseti technique of casting has now become the mainstay of treatment of this condition. This method is especially effective in rural and underdeveloped areas with limited health care facilities. The purpose of this study was to evaluate the effectiveness of the Ponsetti technique in the correction of this deformity, thorough the assessment of modified Pirani score.Methods: This was a prospective observational study conducted on 75 patients of either sex with 100 feet, with an average age of 3.6 months presenting to the Orthopaedic OPD of SHKM Government Medical College Hospital, Nalhar, Nuh, Haryana between September 2015 and September 2017, with a diagnosis of congenital clubfoot. All the patients were treated with Ponsetti technique and the results were analysed through the assessment of modified Pirani score. The patients were followed up for a peroid of 1 year after attainment of correction.Results: Majority of the patients obtained full correction with this method. The mean value of the modified Pirani score improved from the pre-treatment value of 5.30 to 0.36 at the final follow up. The average number of casts required for full correction was 7.32.Conclusions: Thus results of our study demonstrate that the Ponsetti method is a safe and effective method of treatment for congenital clubfoot, especially in rural and underdeveloped regions.

    Musculoskeletal Tuberculosis: Two Year Experience at a Tertiary Care Teaching Hospital of Northern India and Review of Literature

    Background: The aim of this study was to evaluate the present trend of osteo-articular tuberculosis at a teaching hospital in North India.Methods: All the cases of skeletal tuberculosis that were diagnosed and managed either on outdoor or in-hospital stay basis over a period of two years were evaluated with respect to age, gender, anatomical location of lesion, laboratory parameters (ESR, Mantoux test, HIV screening), concomitant skeletaltuberculous lesions, associated pulmonary tuberculosis, and other associated clinical features.Results: A total of 84 skeletal tuberculous lesions in 80 patients were enrolled for the study. The age of the patients ranged from 2 to 70 years with a mean of 29.1±19.1 years. 55% patients were in second and third decade of life and 63.7% were males. ESR was elevated in 61.25% and 56.25% tested positive forMantoux skin test. 21.25% patients had associated active or healed focus of tuberculosis in the lungs. Spine was the most common site involved (58.75%) followed by hip (13.1%), knee (8.3%), foot (4.7%) and shoulder joint (3.5%). Lumbar spine was the most common site affected in the spine followed by lowerdorsal spine. 3.75% cases had multifocal skeletal tuberculous lesions.Conclusion: Prevalence of osteo-articular tuberculosis continues to be high in North India with the disease predominantly affecting young population. Spine is the most common site for skeletal tuberculosis followed by hip, knee, foot and shoulder. Lumbar region followed by lower dorsal spine is much more affected.Multifocal skeletal lesions incidence is lower than expected. And one-fifth of the patients have associated pulmonary tubercular lesion

    Quantification of sodium present in dry aggregates and anodes

    In aluminum industry, it is important to determine the concentration of contaminants present in anode raw materials as rapidly as possible in order to adjust the anode recipe. Sodium, which is an impurity coming largely from anode butts, significantly influences the anode reactivity, and an increase in its concentration increases the anode consumption. A simple and inexpensive method was developed to quantify the sodium content in dry aggregates and anodes without grinding the samples. The method is based on potentiometric principles using a sodium-ion specific electrode. A sample can be analyzed easily within thirty minutes. In order to prevent the rapid degradation of the specific electrode due to experimental conditions, sodium is extracted from samples by electrophoresis prior to the test. The comparison shows that the measured sodium concentrations obtained are similar to the results determined by other test methods

    816-818Two different types of β-diketones, 1-phenyl-3-methyl- 4-p-nitrobenzoyl-5-pyrazoloneI (NMPP) and 3-phenyl- 4-benzoyl-5-isoxazolone (HPBI) have been employed for the solvent extraction of erbiun(III) from aqueous buffer media into toluene. The value of log Kex, where Kex refers to the equilibrium, Er(III) + 3HL ErL3 + 3H+ is - 1.1 and + 0.9 for Er-NMPP and Er-HPBI respectively. The effect of various lewis bases, viz., tri-n-octyl-phosphine oxide (TOPO), tri-n-butyl phosphafe (TBP) and dibenzyl sulphoxide (DBSO) in combination with NMPP and HPBI respectively on the extraction of Er(III) has also been studied. A large synergistic enhancement has been observed in both the cases with all the lewis bases. Synergistic extraction constants, stability constants, and the stoichiometry of the respective adducts formed during the extraction have been determined

Although used as an adjuvant method of accelerating wound healing from ancient times, honey has been sporadically used in the treatment of burns. Honey acts mainly as a hyperosmolar medium and prevents bacterial growth. Because of its high viscosity, it forms a physical barrier, and the presence of enzyme catalase gives honey an antioxidant property. Its high-nutrient content improves substrate supply in local environment promoting epithelialization and angiogenesis. In pediatric burn patients no exclusive study has been conducted using honey as a burn dressing.
