Multiple sclerosis: psychological problems (emotional tension, stress, depressive disorders) and accompanying factors
Emotional tension, stress and depressive disorders (as a whole referred to as psychological disorders) accompany multiple sclerosis (MS). The aim of this paper has been to determine the frequency of their occurrence and their relation to other disorders common for MS. A group of 67 patients of a neurological clinic in Jarosław and neurological ward in Przemyśl (situated in south-east Poland) were provided with a tailor-made questionnaire. The patients who declared having psychological problems belonged to a group of older adults, who suffered from MS longer and who demonstrated more mobility impairment (higher EDSS score). It was found that the patients experienced also a higher frequency of occurrence of the following: pain (48.8% vs 4.0%), communication difficulties (34.1% vs 0.0%), fatigue syndrome (90.5% vs 13%), sexual dysfunctions (92.7% vs 20.0%). Emotional tension, stress or depressive disorders were found the most common among patients diagnosed with secondary progressive MS (100.0%), and the least common in patients with progressive-relapsing MS (46.9%). Upon combining the respondents’ age, duration of the disease as well as EDSS scores (0.57-0.73), the most significant factor influencing the occurrence of psychological disorders, the authors suggest, was the intensification of mobility impairment
Assessment of mobility impairment in patients with multiple sclerosis
Multiple sclerosis (Lat. sclerosis multiplex, SM) is a chronic inflammatory neurodegenerative disease that affects the central nervous system (CNS). Common symptoms of the SM include mobility impairment, sensory loss (paraesthesia), cerebellar dysfunctions (balance problems), visual disturbances, autonomic dysfunctions and pain syndromes as well as psychiatric disorders (cognitive and emotional disturbances).
Aim of work was to assess mobility impairment symptoms among patients with SM in relation to progression of the disease, length of time a patient suffered from it, and influence of the symptoms on functional capacity and self-care management.
The study was conducted on a group of 67 patients (74.6% women, 22-74 age range) of Neurology Ward of Medical Care Centre in Jarosław (south-eastern Poland) and its Neurology Clinic who were diagnosed with SM. The study was based on a self-constructed diagnostic questionnaire. The most prevalent (32; 47.8%) type of SM was progressive-relapsing.
Balance and coordination impairment (from 1 to 7, average 4.38) were indicated by 74.6% respondents. The incidence of spasticity and problems caused by it were indicated by 31.3%. Therapeutic possibilities of botulinum toxin in neurological disorders – treatment of limb spasticity in the course of brain damage
Introduction. Botulinum toxin is produced by the anaerobic bacterium Clostridium botulinum. The sporulation form of the C.
botulinum is widely found in the environment (in soil) and may develop in inappropriately stored food. The symptoms of poisoning occur 18-36 hours after consumption of contaminated food.
Aim. The aim of this study is to present the benefits of using botulinum toxin in the treatment of spasticity of the upper and
lower limbs in both adults and children.
Material and methods. A literature review of the following databases was carried out: PubMed, UpToDate.
Results. Botulinum toxin interferes with neural transmission by blocking the release of acetylcholine and causes muscle paralysis. The typical symptoms are diplopia, xerostomia, enteroparesis, speaking and swallowing disorders, as well as paralysis of
respiratory muscles which leads to death. However, botulinum toxin is also a very potent medication. The biggest application
is found in the field of neurology, inter alia, in the treatment of spasticity.
Conclusion. The study provides current evidence regarding the safety and efficacy of botulinum toxin injection for spasticity
of the upper and lower limbs. Botulinum toxin injections are applicable in the treatment of many neurological disorders and
the list of indications will certainly become wider

Skuteczność kinezyterapii w stwardnieniu rozsianym w opinii pacjentów
Multiple sclerosis (MS) is a demyelinating disease of the central nervous system. In
the course of the disease there are neurological deficits leading to patient’s disability.
In addition to essential medical treatment, rehabilitation, including kinesitherapy
(movement therapy), is significantly improving the clinical condition of MS patients’.
The aim of this study is to know the opinion of MS patients on the effectiveness of
kinesitherapy to which they were subjected. The study involves 60 people with MS.
The method of study was a questionnaire survey. Balance exercises were frequently
performed for MS patients, and general exercises to improve and strengthen the
lower limbs. Most of MS patients observed the decrease in the severity of the
symptoms after kinesitherapy. The greatest improvement concerned the imbalances
and dysfunctions of the lower limbs and kinesitherapy found impact on the patients
whose disease has lasted for a relatively short time. The improvement of physical
function and self-activity was observed after kinesitherapy usually in the type of
secondary progressive disease. In conclusion, the best effects of kinesitherapy were
observed in patients with secondary progressive type of MS.Stwardnienie rozsiane (SM) jest chorobą demielinizacyjną ośrodkowego układu nerwowego.
W przebiegu choroby występują ubytkowe objawy neurologiczne prowadzące do
inwalidztwa chorego. Oprócz leczenia farmakologicznego istotne znaczenie w poprawie
stanu klinicznego chorych na SM przypisuje się rehabilitacji, w tym kinezyterapii (leczeniu
ruchem). Celem pracy było poznanie opinii chorych na SM na temat skuteczności
przeprowadzonej u nich kinezyterapii. W badaniu uczestniczyło 60 osób chorych na SM.
Metodą badania był kwestionariusz ankiety. Najczęściej wykonywanymi ćwiczeniami
przez chorych na SM były ćwiczenia równoważne oraz ogólnousprawniające i wzmacniające
kończyny dolne. Większość chorych na SM obserwowała zmniejszenie nasilenia
wszystkich objawów choroby po kinezyterapii. Największa poprawa dotyczyła zaburzeń
równowagi i zaburzeń funkcji kończyn dolnych. Najlepiej oceniły wpływ kinezyterapii
na sprawność fizyczną osoby, u których choroba trwała stosunkowo krótko. Poprawę
sprawności fizycznej oraz czynności samoobsługowych po kinezyterapii najczęściej
obserwowano w typie wtórnie postępującym choroby. Podsumowując, największą skuteczność
kinezyterapii obserwowali u siebie chorzy z wtórnie postępującym typem SM
Czynniki wpływające na występowanie nietrzymania moczu wśród pacjentów oddziału psychogeriatrycznego
Introduction: Urinary incontinence (NTM) is a subjective symptom involving
uncontrolled leakage of urine. It is considered a social disease, often concealed,
causing many adverse changes in the functioning of the elderly.
Goal: Assessment of the problem of urinary incontinence among patients admitted
to a psychogeriatric ward.
Materials and methods: Studies have been conducted in the psychogeriatric ward
from October 2009 to February 2011. The study involved 176 patients. ADL scale
was used and the analysis of the Charter of Patient’s Nursing of psychogeriatric
ward.
Results: The subjects ages ranged from 60 to 93 years. Patients in whom there
is a problem of urinary incontinence during the fi rst week of stay in the ward
were 248, while the research group was ultimately 176 patients (46.93%) - in 72
patients (19.20%) resolved the problem of urinary incontinence. Patients with
limited mobility were 37 people (21.02%), and another 6 were not able to move by
themself (3.1%). Regarding the pathology of muscle tone, has been found in 37.5%.
Amblyopia was found in another 52 patients (29.55%), and blindness in 1 (0.57%).
Only 38 people applied the glasses. Impaired balance were 90 respondents, and
dizziness 41 (23.30%). Balance disorders occur together with dizziness in 11
patients, 6.25%). Another factor which urinary incontinence - disturbed sleep
patterns - occurred in 73 patients (41.48%). Also determined based on the ADL
scale efficiency and partially-disabled subjects were 38 patients (21.59%) and
completely disabled in terms of basic vital signs were 44 respondents (25.00%).
Conclusions: Urinary incontinence was present in 46.93% of hospitalized patients.
Many patients and their caregivers hide or do not admit to the occurrence of this
ailment. Patients admitted to psychogeriatic ward were much burdened in terms of
factors affecting the occurrence of the problem of urinary incontinence.Wprowadzenie: Nietrzymanie moczu (NTM) to objaw podmiotowy polegający
na niekontrolowanym wycieku moczu. Jest uznawane za chorobę o charakterze
społecznym, często ukrywaną, powodująca wiele niekorzystnych zmian
w funkcjonowaniu osób starszych.
Cel: Ocena występowania problemu nietrzymania moczu wśród pacjentów
przyjętych do oddziału psychogeriatrii.
Materiał i metody: Badania były prowadzone w Oddziale Psychogeriatrycznym
w okresie od października 2009 do lutego 2011 roku. Badaniu poddano 176
pacjentów. Wykorzystano skalę ADL oraz analizę Karty Pielęgnowania Pacjenta
Oddziału Psychogeriatrycznego.
Wyniki: Wiek badanych mieścił się w przedziale od 60 do 93 lat. Pacjentów
u których wystąpił problem nietrzymania moczu w ciągu pierwszego tygodnia
pobytu w oddziale było 248, natomiast grupę badawczą stanowiło ostatecznie 176
pacjentów (46,93%) – u 72 osób (19,20%) problem nietrzymania moczu ustąpił.
W ograniczony sposób poruszało się 37 osób (21,02%), a kolejnych 6 nie poruszało
się samodzielnie (3,1%). Odnośnie patologii w zakresie napięcia mięśniowego,
występował ona u 37,5%. Niedowidzenie stwierdzono u kolejnych 52 pacjentów
(29,55%), a ślepotę u 1 (0,57%). Tylko 38 osób stosowało okulary. Zaburzoną
równowagę miało 90 badanych, a zawroty głowy 41 (23,30%). Zaburzenia
równowagi występowały razem z zawrotami głowy u 11 pacjentów 6,25%).
Kolejny czynnik nasilający nietrzymanie moczu – zaburzony rytm snu – wystąpił
u 73 osób (41,48%). Określono też na podstawie skali ADL sprawność badanych
i częściowo sprawnych było 38 badanych (21,59%) a całkowicie niesprawnych
w zakresie podstawowych czynności życiowych było 44 badanych (25,00%).
Wnioski: Nietrzymanie moczu występowało u 46,93% pacjentów
hospitalizowanych.
Wielu pacjentów, ich opiekunów ukrywa lub nie przyznaje się do występowania
tej dolegliwości. Pacjenci przyjmowani do oddziały psychogeriatrycznego byli
znacznie obciążeni pod względem czynników mających wpływ na występowanie
problemu nietrzymania moczu
Ocena sprawności funkcjonalnej pacjentów z otępieniem
Introduction. The physical performance is affected by many factors, health
(systemic diseases, and organ) and socio-demographic characteristics (age, sex,
education, place of residence).
The aim of this study was to assess changes in functional capacity of patients with
dementia-ranking, Psychogeriatrycznym hospitalized.
The study used a method of diagnostic survey and measurement. The research
was conducted in the Department of Psychiatry Psychogeriatrycznym Specialist
Healthcare Team in Yaroslavl, in the period from October 2009 to February 2011.
The study group consisted of 176 patients. Among the respondents were 112 women
(63.64%) and 64 men (36.36%). Age of the patients ranged from 60 to 93 years.
Results. Length of hospital stay of patients ranged from 49 to 174 days. There was
a decrease in functional capacity hospitalized. Particularly noticeable is the drop
in performance after a week of hospitalization (2,375). However, in subsequent
measurements, and gradually increases at the end of achieving the result is similar
to the origina.
Conclusions. Functional effi ciency decreases with increasing severity of dementia.
The reduction in functional results in the need to use third-party assistance in
patient care conditions requires additional medical staff responsibilities.Wstęp. Na sprawność funkcjonalną ma wpływ wiele czynników
zdrowotnych(choroby układowe i narządowe) i społeczno-demografi cznych
(wiek, płeć, wykształcenie, miejsce zamieszkania).
Celem pracy była ocena zmian sprawności funkcjonalnej pacjentów z postawionym
rozpoznaniem otępienia, hospitalizowanych na oddziale Psychogeriatrycznym.
W badaniu wykorzystano metodę sondażu diagnostycznego i pomiar. Badania były
prowadzone w Oddziale Psychogeriatrycznym Specjalistycznego Psychiatrycznego
Zespołu Opieki Zdrowotnej w Jarosławiu, w okresie od października 2009 do lutego
2011 roku. Grupę badawczą stanowiło 176 pacjentów. Wśród osób badanych było 112
kobiet (63,64%) i 64 mężczyzn (36,36%). Wiek badanych mieścił się w przedziale od 60 do 93 lat.
Wyniki. Długość pobytu pacjentów w szpitalu wahał się od 49 do 174 dni.
Zaobserwowano spadek sprawności funkcjonalnej u hospitalizowanych.
Szczególnie widoczny jest spadek sprawności po tygodniu hospitalizacji (2,375).
Natomiast w kolejnych pomiarach stopniowo wzrasta i na koniec osiągania wynik
zbliżony do początkowego.
Wnioski. Sprawność funkcjonalna spada wraz ze wzrostem zaawansowania
otępienia. Obniżenie sprawności funkcjonalnej skutkuje koniecznością korzystania
z pomocy osób trzecich a w warunkach opieki stacjonarnej nakłada na personel
medyczny dodatkowe obowiązki
