    A randomised controlled trial of a lengthened and multi-disciplinary consultation model in a socially deprived community: a study protocol

    <p>Abstract</p> <p>Background</p> <p>There has been little development of the general practice consultation over the years, and many aspects of the present consultation do not serve communities with multiple health and social problems well. Many of the problems presenting to general practitioners in socio-economically disadvantaged areas are not amenable to a purely medical solution, and would particularly benefit from a multidisciplinary approach. Socio-economic deprivation is also associated with those very factors (more psychosocial problems, greater need for health promotion, more chronic diseases, more need for patient enablement) that longer consultations have been shown to address. This paper describes our study protocol, which aims to evaluate whether a lengthened multidisciplinary primary care team consultation with families in a socially deprived area can improve the psychological health of mothers in the families.</p> <p>Methods/Design</p> <p>In a randomised controlled trial, families with a history of social problems, substance misuse or depression are randomly allocated to an intervention or control group. The study is based in three general practices in a highly deprived area of North Dublin. Primary health care teams will be trained in conducting a multidisciplinary lengthened consultation. Families in the intervention group will participate in the new style multidisciplinary consultation. Outcomes of families receiving the intervention will be compared to the control group who will receive only usual general practitioner care. The primary outcome is the psychological health of mothers of the families and secondary outcomes include general health status, quality of life measures and health service usage.</p> <p>Discussion</p> <p>The main aim of this study is to evaluate the effectiveness of a lengthened multidisciplinary team consultation in primary care.     HADES experiment: di-lepton spectroscopy in p + p (2.2 GeV) and C+C (1 and 2 A GeV) collisions

    The HADES (High Acceptance Di-Electron Spectrometer) is a tool designed for lepton pair (e+e−) spectroscopy in pion, proton and heavy ion induced reactions in the 1–2AGeV energy range. One of the goals of the HADES experiment is to study in-medium modifications of hadron properties like effective masses, decay widths, electromagnetic form factors etc. Such effects can be probed with vector mesons ( ρ,ω,ɸ ) decaying into e+e− channel. The identification of vector mesons by means of a HADES spectrometer is based on invariant mass reconstruction of e+e− pairs. The combined information from all spectrometer sub-detectors is used to reconstruct the di-lepton signal.     Enhancing Electron Coherence via Quantum Phonon Confinement in Atomically Thin Nb3SiTe6

    The extraordinary properties of two dimensional (2D) materials, such as the extremely high carrier mobility in graphene and the large direct band gaps in transition metal dichalcogenides MX2 (M = Mo or W, X = S, Se) monolayers, highlight the crucial role quantum confinement can have in producing a wide spectrum of technologically important electronic properties. Currently one of the highest priorities in the field is to search for new 2D crystalline systems with structural and electronic properties that can be exploited for device development. In this letter, we report on the unusual quantum transport properties of the 2D ternary transition metal chalcogenide - Nb3SiTe6. We show that the micaceous nature of Nb3SiTe6 allows it to be thinned down to one-unit-cell thick 2D crystals using microexfoliation technique. When the thickness of Nb3SiTe6 crystal is reduced below a few unit-cells thickness, we observed an unexpected, enhanced weak-antilocalization signature in magnetotransport. This finding provides solid evidence for the long-predicted suppression of electron-phonon interaction caused by the crossover of phonon spectrum from 3D to 2D.Comment: Accepted by Nature Physic

    Somatic Mutations in UBA1 and Severe Adult-Onset Autoinflammatory Disease

    BACKGROUND Adult-onset inflammatory syndromes often manifest with overlapping clinical features. Variants in ubiquitin-related genes, previously implicated in autoinflammatory disease, may define new disorders. METHODS We analyzed peripheral-blood exome sequence data independent of clinical phenotype and inheritance pattern to identify deleterious mutations in ubiquitin-related genes. Sanger sequencing, immunoblotting, immunohistochemical testing, flow cytometry, and transcriptome and cytokine profiling were performed. CRISPR-Cas9–edited zebrafish were used as an in vivo model to assess gene function. RESULTS We identified 25 men with somatic mutations affecting methionine-41 (p.Met41) in UBA1, the major E1 enzyme that initiates ubiquitylation. (The gene UBA1 lies on the X chromosome.) In such patients, an often fatal, treatment-refractory inflammatory syndrome develops in late adulthood, with fevers, cytopenias, characteristic vacuoles in myeloid and erythroid precursor cells, dysplastic bone marrow, neutrophilic cutaneous and pulmonary inflammation, chondritis, and vasculitis. Most of these 25 patients met clinical criteria for an inflammatory syndrome (relapsing polychondritis, Sweet’s syndrome, polyarteritis nodosa, or giant-cell arteritis) or a hematologic condition (myelodysplastic syndrome or multiple myeloma) or both. Mutations were found in more than half the hematopoietic stem cells, including peripheral-blood myeloid cells but not lymphocytes or fibroblasts. Mutations affecting p.Met41 resulted in loss of the canonical cytoplasmic isoform of UBA1 and in expression of a novel, catalytically impaired isoform initiated at p.Met67. Mutant peripheral-blood cells showed decreased ubiquitylation and activated innate immune pathways. Knockout of the cytoplasmic UBA1 isoform homologue in zebrafish caused systemic inflammation. CONCLUSIONS Using a genotype-driven approach, we identified a disorder that connects seemingly unrelated adult-onset inflammatory syndromes. We named this disorder the VEXAS (vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic) syndrome.     Accurate Image Localization Based on Google Maps Street View

    Finding an image\u27s exact GPS location is a challenging computer vision problem that has many real-world applications. In this paper, we address the problem of finding the GPS location of images with an accuracy which is comparable to hand-held GPS devices.We leverage a structured data set of about 100,000 images build from Google Maps Street View as the reference images. We propose a localization method in which the SIFT descriptors of the detected SIFT interest points in the reference images are indexed using a tree. In order to localize a query image, the tree is queried using the detected SIFT descriptors in the query image. A novel GPS-tag-based pruning method removes the less reliable descriptors. Then, a smoothing step with an associated voting scheme is utilized; this allows each query descriptor to vote for the location its nearest neighbor belongs to, in order to accurately localize the query image. A parameter called Confidence of Localization which is based on the Kurtosis of the distribution of votes is defined to determine how reliable the localization of a particular image is. In addition, we propose a novel approach to localize groups of images accurately in a hierarchical manner. First, each image is localized individually; then, the rest of the images in the group are matched against images in the neighboring area of the found first match. The final location is determined based on the Confidence of Localization parameter. The proposed image group localization method can deal with very unclear queries which are not capable of being geolocated individually. © 2010 Springer-Verlag
