    Objective: To improve the treatment results of patients with traumatic injuries, and neoplastic lesions of the upper cervical spine on the basis of a differentiated algorithm of nutritional-metabolic therapy. Materials and methods: Based on clinical and instrumental methods of examination typical metabolic characteristics have been studied in 21 patients with compressive myelopathy at the level of the upper cervical spine. Results: Due to the received data differentiated algorithm of nutritional-metabolic therapy has been worked out. Conclusion: The use of nutritional-metabolic therapy in patients with compressive myelopathy at the level of the upper cervical spine may improve the treatment

    The aim of the research is to investigate activation of antitumor immune response efficiency using professional antigen-presenting autologous dendrite cells in the complex treatment of patients with cerebral hemisphere malignant gliomas. We have analysed 48 cases of patients with malignant gliomas (35 — glioblastoms, 13 — anaplastic astrocytoma) who received an complex treatment with specific immunotherapy use. All the patients were hospitalized at late stage of disease and tumor size exceeded 5 sm. After the operation all the patients achieved satisfactory quality of life (not less than 80 points according to Karnofsky). Specific antitumor immunotherapy was conducted in accordance with original methodic, including autologous dendrite cells with tumor antigens and tumor cells lysate. Treatment course included intradermal introduction of dendrite cells at the 10—12 day after the operation. The treatment included not less than 3 injections. The course of treatment was repeated every 2—3 months. Specific antitumor immunotherapy included into complex treatment of malignant gliomas allows to increase disease remission, life quality being satisfactory.В статье представлен метод специфической противоопухолевой иммунотерапии на основе аутологичных дендритных клеток в комплексном лечении пациентов со злокачественными полушарными глиомами супратенториальной локализации. Проанализированы результаты комплексного лечения 48 пациентов со злокачественными глиомами (35 глиобластом, 13 анапластических астроцитом). Все пациенты поступали в институт на поздней стадии заболевания и имели опухоль более 5 см в диаметре. В послеоперационном периоде качество жизни пациентов составляло не менее 80 баллов по шкале Карновского. Специфическая противоопухолевая иммунотерапия проводилась по оригинальной методике и включала аутологичные дендритные клетки, нагруженные опухолевыми антигенами, а также лизат клеток опухоли. Курс лечения заключается во внутрикожном введении специфической противоопухолевой вакцины на 10-12-й дни послеоперационного периода и составляет не более 3 инъекций. Курс лечения повторяется каждые 2-3 мес. Применение специфической противоопухолевой иммунотерапии в комплексном лечебном подходе повышает качество жизни и улучшает перспективы лечения больных со злокачественными полушарными глиомами

    A comprehensive evaluation of cervical spine surgery outcomes

    Aim: study of diagnostic value of different assessment scales to evaluate outcomes of surgeries in degenerative and post-injury deformities of cervical spine. Material and Methods. Between January and December 2015, 110 consecutive patients with degenerative and traumatic lesions of cervical spine were selected for surgery and results of treatment were followed. Preoperatively and 12 months postoperatively the patients filled in a questionnaire that included a VAS, Odom's scale, NDI, modified JOAand the Zung Depression Scale. To assess the results of the intervention, the concept of "effect size" was used. Distribution of different scores classified according to Odom's criteria as excellent/good and fair/poor. Results. All scores improved in all group of patients. For VAS, NDI and modified JOA scores the effect size was largest. NDI, JOAand ZDS seemed to correlate well with the classification of outcome by Odom. Conclusion. It is suggested many scales to assess the outcomes of the cervical spine surgery based on different criteria; nevertheless, there are no generally accepted and validated methods that can be used to analysis the outcome of cervical spine surgery.     Fine--Mesh Photodetectors for CMS Endcap Electromagnetic Calorimeter

    The behaviour of fine-mesh vacuum phototetrodes and phototriodes ( VPTs) in an axial magnetic field of ( 0 -- 4)T has been investigated. The measured VPT parameters are: fine-mesh cell dimensions, the photocathode sensitivity and its homogenity, the gain in zero and 4T magnetic field at tilt angles corresponding to the rapidity range of CMS ECAL Endcap 1.479 -- 3.0 and excess noise factor. Measurements have been performed on 21 and 30 mm diameter photodetectors with different fine-mesh structures: 30, 60 and 100 lines per mm under different types of photocathode illumination by green LED. Phototriodes with 30 or 60 lines per mm and an external diameter of 21 mm are found to be the best candidates for the CMS environment with the initial size of PWO crystals proposed to be used in the Endcap, by comparison with phototetrodes. They provide a gain of the order of 6 -- 8 in 4T magnetic field and an excess noise factor of 2 under full photocathode illumination

    Developed blood immunodeficiencies and activation of immunological processes in BAL-fluid were revaled in 28 patients with Histiocytosis-X during immunological examination. The activity of elastase and collagenase in BAL correlated with the intensity of the destructive process in the pulmonary tissue in patients with Histiocytosis-X. Corticosteroid treatment made it possible to control immunity disorders, but, however, there weren’t the satisfactory effect on the destructive process in the tissue in these patients.Развитые иммунодефициты в крови и активация иммунных процессов БАЛ-жидкости были выявлены у 28 пациентов с гистиоцитозом X при иммунологическом обследовании. Активность эластазы и коллагеназы в БАЛ соотносилась с интенсивностью деструктивного процесса в легочной ткани у пациентов с гистиоцитозом X. Стероидная терапия сделала возможным контролировать иммунные расстройства, но, однако, не было удовлетворительного воздействия на деструктивный процесс в ткани у этих пациентов

    The behaviour of fine-mesh vacuum phototriodes (VPTs) with the external diameters of 21 and 35 mm has been investigated in an axial magnetic field up to 4 T in view of their applications as readout devices for CMS Endcap Electromagnetic Calorimeter. The measured VPT parameters are: the photocathode's sensitivity and its homogeneity, the gain in zero and 4 T magnetic field at tilt angles corresponding to the pseudorapidity range of CMS ECAL Endcap 1.48-3.0 as a function of fine-mesh cell dimensions, excess noise factor and the stability of the photocathode response under the illumination by light emission diodes (LED) and the irradiation by 14 MeV neutrons. Phototriodes with 100 lines per mm fine-mesh and 25 mm external diameter are found to be the best candidates for coupling with rear PbWO/sub 4/ crystals by dimensions of 30*30 mm, proposed to be used in CMS ECAL Endcaps. VPTs provide a gain of the order (6-8) in a 4 T magnetic field and an excess noise factor of 2-2.5 under illumination of a full photocathode's area. (2 refs)
