    Illustration of the diversity and heterogeneity in health information sources on Internet

    International audienceIn France, there are thousands of information sites dedicated to health. Ranging from the simple popularisation of medical facts to institutional sites, including sites for associations of patients and professionals, sites freely available or requiring a free or paid-for access code, medical Net is growing fast. Ranking these information sources is difficult because most of these sites present several of the above characteristics. For example, many of the sites originally intended for healthcare professionals are easily viewed by the public, where scientific articles can be readily found but which cannot be exploited by the layperson. In order to reduce the problems linked to the quality of information found on the internet, the Health ON the Net (HON) Foundation established a quality charter, the Hon Code, for health sites. Moreover, in the medical field the terminology used by authors is complex and each database has its own thesaurus and its own terminology

    Comparative analysis between impact factor and h-index for psychiatry journals

    International audienceObjective: Journal Impact Factor (IF) is well known for being the document measure of scientific journal impact, despite several recognised limitations. Our study tried to propose a new rating system (journal h-index) applied to a sample of psychiatry journals and compared to IF. Method: In order to strictly compare their IF and h-index using the same data, we wanted to work on all the citations obtained in 2006 by the articles published in 2004-2005 in the 50 journals of our Web of Science sample of psychiatry. We studied the statistical correlation between the IF 2006 and h-index 2006. Results: The rankings of the 50 psychiatry journals were different when we took into account respectively the descending order of IF 2006 and h-index 2006 which revealed 16 steps and therefore 16 groups of journals. We noted that two journals were up 21 places (record) in h-index 2006 ranking and one journal lost 17 places. Nevertheless we obtained a high correlation coefficient well illustrated by the group of the seven first journals whose the two rankings were very close. We noted that our sample had only two journals really specialised in the publication of reviews. Conclusions: The rating of journals starting from the h-index may represent an interesting and complementary alternative to the well-known rating based on the IF. The h-index rating proposes a categorization of journals making it possible to create classes of journals with the same h-index. This type of ranking by classes is often appreciated and used by experts and scientific committees of evaluation

    International audienceUsing strictly the same parameters (identical two publication years (2004-2005) and identical one-year citation window (2006)), IF 2006 was compared with h-index 2006 for two samples of "Pharmacology and Pharmacy" and "Psychiatry" journals computed from the ISI Web of Science. For the two samples, the IF and the h-index rankings of the journals are very different. The correlation coefficient between the IF and the h-index is high for Psychiatry but lower for Pharmacology. The linearity test performed between the h-index and showed the great sensitivity of the model compared with . The IF and h-index can be completely complementary when evaluating journals of the same scientific discipline

    Measuring the Efficiency of Commonality Implementation: Application to Commercial Aircraft Cockpits

    Increasing commonality across their models allows commercial aircraft manufacturers to reduce the product development cycle time and tailor their models to their customers’ needs. The cockpit is an area where commonality is thought to be particularly desirable, since it also simplifies the pilots training process for the airline companies. However, no set of metrics is currently able to measure the efficiency of commonality application at this level from a total lifecycle standpoint. In this paper, we propose a set of metrics adapted to large commercial aircraft and taking into account the main parts of the product lifecycle. The concept of lead time is central in our measurement of commonality, as it strongly influences the value of the aircraft for all primary stakeholders. We examine the extent to which these metrics are independent from aircraft particularities (such as physical dimensions). We indicate their limitations and what additional research should be performed to obtain a set of metrics able to measure the efficiency of commonality implementation for the whole aircraft. We use the example of two competing medium-haul aircraft families to test our set of metrics. We evaluate the efficiency of commonality according to our metrics and we compare with field results obtained from the manufacturers’ studies and from the results of interviews of airlines pilots.     Relationships between Consumption, Publication and Impact in French Universities in a value perspective: A Bibliometric Analysis

    International audienceThe study aims to investigate the relationships between consumption of e-journals distributed by Elsevier ScienceDirect platform, publication (articles) and impact (citations) in a sample of 13 French universities, from 2003 to 2009. It adopts a value perspective as it questions whether or not publication activity and impact are some kind of return led by consumption. A bibliometric approach was used to explore the relations between these three variables. The analysis developed indicators inspired by the mathematical h-Index technique. Results show that the relation between consumption, publication and citations depends on the discipline’s profile, the intensity of research and the size of each institution. Moreover, although relations have been observed between the three variables, it is not possible to determine which variable comes first to explain the phenomena. The study concludes by showing strong correlations, which nevertheless do not lead to clear causal relations. The article provide practical implication for academic library managers who want to show the added value of their electronic e-journals collections can replicate the study approach. Also for policy makers who want to take into account e-journals usage as an informative tool to predict the importance of publication activity. Originality: The study is the first French contribution to e-journal value studies.     A rare case of a male infant with down-turner syndrome and review of cases

    Individuals with double aneuploidy of Down-Turner syndrome are very rare and to date, fewer than 50 cases have been reported, worlwide. We report a case of a male infant who presented with dysmorphic features of upslanting eyes, flat nasal bridge, wide spaced nipples and macroglossia. Based on the clinical features, he was diagnosed with Down syndrome. His peripheral blood sample was taken and sent for cytogenetic analysis for confirmation. Chromosome analysis of his lymphocyte cell culture revealed a mosaic pattern of double aneuploidy with monosomy X identified in 31 metaphases and trisomy 21 in 14 metaphases: (45,X[31]/47,XY,+21[14]). Further analysis with fluorescence in situ hybridization (FISH) using Vysis LSI SRY Spectrum Orange/CEP X Spectrum Green Probe and Vysis CEP Y Spectrum Aqua Probe and Vysis LSI 21 Spectrum Orange Probe performed on the cells (nuclei and metaphases) has confirmed the presence of the abnormal two cell lines (81% monosomy X and 19% trisomy 21) in the patient. Ultrasound investigations of his pelvic region showed normal testes and no evidence of uterus, ovary or vagina. To the best of our knowledge, this is the first Down-Turner syndrome reported in Malaysia. In conclusion, this case demonstrates the importance of Giemsa-banded karyotype and FISH analyses as diagnostic tools in identifying the chromosomal abnormality and determining the ratio of the normal:abnormal cells present in the patient. An annotated bibliography of earlier reported cases of Down-Turner with documented karyotyping is also included in this report

    Résistance aux antibiotiques par mécanisme d'efflux chez Achromobacter xylosoxidans

    Achromobacter xylosoxidans est un bacille à Gram négatif non fermentaire pathogène opportuniste. Il est de plus en plus fréquemment isolé chez les patients atteints de mucoviscidose, colonisant leur arbre bronchique et pouvant être responsable d une dégradation de la fonction respiratoire. Il s agit d une espèce bactérienne naturellement résistante à de nombreux antibiotiques : aux céphalosporines (hors ceftazidime), à l aztréonam et aux aminosides. Les résistances acquises sont fréquentes, en particulier dans les souches isolées d expectorations de patients atteints de mucoviscidose. Ces résistances acquises concernent des molécules antibiotiques très utilisées pour le traitement des exacerbations respiratoires de la maladie, ce qui conduit parfois à de véritables impasses thérapeutiques. Au début de notre travail, seuls quelques mécanismes de résistance acquise aux b-lactamines avaient été décrits, mais aucun mécanisme impliqué dans la multi-résistance naturelle d A. xylosoxidans.La résistance aux antibiotiques par efflux actif de type Resistance-Nodulation-cell Division (RND) est très répandue chez les bacilles à Gram négatif non fermentaires. Nous avons identifié dans le génome d A. xylosoxidans trois opérons pouvant coder pour des systèmes d efflux RND. Par une technique d inactivation génique nous avons montré que les trois systèmes d efflux (AxyABM, AxyXY-OprZ et AxyCDJ) pouvaient exporter des antibiotiques. Deux d entre eux participent à l antibio-résistance naturelle d A. xylosoxidans : AxyABM (résistance à l aztréonam et à plusieurs céphalosporines) et AxyXY-OprZ (résistance aux aminosides)Achromobacter xylosoxidans is a nonfermentative Gram-negative bacillus considered to be an opportunistic agent. It is an emerging pathogen in cystic fibrosis (CF), increasingly recovered from the respiratory tract of CF patients. It can cause inflammation and therefore might be involved in the decline of the lung function.This species is innately resistant to many antibiotics, including cephalosporins (except ceftazidime), aztreonam, and aminoglycosides. Moreover the isolates recovered from CF patient sputum are often resistant to major antimicrobial components usually prescribed to treat pulmonary infections. There was very little known about acquired resistance and nothing about innate resistance mechanisms when we started this work.Antibiotic resistance mediated by Resistance-Nodulation-cell Division (RND)-type efflux pumps is widespread among nonfermentative Gram-negative bacilli. We have characterized three putative RND operons in A. xylosoxidans genome. By using a gene inactivation technique we have demonstrated that these operons encode efflux systems (AxyABM, AxyXY-OprZ and AxyCDJ) able to export antibiotics.     Role of Magnetic Rsonance Imaging (MRI) in Grading Supratentorial Gliomas Comparable with Pathology

    Objective: Gliomas are one of the most common brain tumors in adults  with poor prognosis in most patients. Imaging plays a critical role in the diagnosis ,management and follow up of gliomas. The aim of this study was to determine sensitivity and specificity of MRI as a diagnostic tool for supratentorial gliomas in comparison with histopathological findings Methods: Our study included 39 patients with MRI supratentorial  gliomas whose age between 40-75 years in one year 2018-2019  in Neurosurgery department in Tishreen University Hospital in Lattakia. Tumors were sent for histopathological typing and grading according to the WHO 2016. Results: All of supratentorial gliomas were hyperintense in T2,while (95%) of them were hypointense in T1, in FLAIR all tumors were hyperintense , heterogenous enhancement in high grade gliomas, other radiologic findings such as: necrosis, hemorrhage and edema were common in high grade gliomas. Agreement between histological findings and MRI was 72% with sensitivity 100% and specificity 95% Conclusion: MRI plays an essential clinical role in  initial diagnosis and grading  of supratentorial gliomas with high sensitivity and specificity comparable with histological findings. هدف الدراسة: تعد الأورام الدبقية من أورام الدماغ الأولية الأكثر شيوعاً عند البالغين و يكون الانذار سيء لدى معظم المرضى ،تلعب تقنيات التصوير الطبي بMRI  دوراً هاماً في تشخيص الأفة وتوجيه الاجراءات العلاجية، وتهدف هذه الدراسة لتحديد حساسية و نوعية ال ـMRI  كاختبار تشخيصي للأورام الدبقية فوق الخيمة مقارنة مع الموجودات النسيجية. طرق البحث: تم إجراء دراسة تحليلية مقطعية عرضانية لـ 39 حالة من الأورام الدبقية فوق الخيمة ممن تتراوح أعمارهم من 40-75 سنة من مراجعي قسم الجراحة العصبية في مشفى تشرين الجامعي في اللاذقية بين عامي 2018-2019، تمت دراسة التوافق بين نتائج الرنين والتشريح المرضي مع الاعتماد على تصنيف (WHO2016)  لتصنيف الأورام نسيجياً. النتائج: أعطت جميع الأورام الدبقية اشارة عالية في T2، وبينما أعطى 95% منها اشارة ناقصة في T1 لوحظ في FLAIR أن جميع الأورام كانت عالية الإشارة الأورام عالية الدرجة أعطت تعزيز غير متجانس ومظاهر التصوير الأخرى بالرنين مثل النزف، النخر، الوذمة شائعة مع الدرجات العالية للورم. بلغت درجة التوافق بين نتائج التشريح المرضي والرنين 72% ووصلت حساسية التصوير بالرنين المغناطيسي 100% بدقة 95%. الاستنتاجات: يساعد الرنين في وضع تشخيص مبدئي وتحديد درجة الأورام الدبقية بحساسية ونوعية عالية مقارنة مع الدراسة النسيجية ويمكن أن يستخدم بشكل موثوق في الممارسة السريرية وفي الحالات التي يكون فيها إجراء الخزعة غير ممكن
