3 year resultes of complex treatment diabetic foot patients

This research presents the experience in treatment of two groups of diabetics (1696 and 246 cases ) with various types of diabetic foot syndrome in podiatric offices as well as specialized surgical settings respectively from 2005 till 2007. The course of diabetes mellitus, peripheral neuropathy along with macroangiopathy ( peripheral arterial disease ), and peculiarities in the course of wound infections in hospitalized diabetics were investigated and evaluated. The basis principle in conservative management as the authors considered was the employment of medicine undergone clinical trials and proven effective in treatment. The surgical procedures included debridements, wide incisions, minor amputations, skin grafting as well as arterializations via a short saphenous vein. The employment of multidisciplinary approach in treatment let save lower limb function in neuropathic and neuroischaemic diabetic foot in 92.4% and 85.6% of cases respectively. It is mandatory to accentuate specialization in the work of podiatric offices, choose patients rationally for timely hospitalization, to use widely revascularization techniques.8 работе представлен опыт лечения пациентов с различными типами синдрома диабетической стопы (СДС) в подиатрическом кабинете (1696 больных) и хирургическом стационаре (246 больных) специализированной службы за период 2005-2007 гг. У пациентов стационара изучена ситуация по течению сахарного диабета, оценены периферическая полинейропатия, макроангиопатия и особенности течения инфекционного процесса. Основным принципом консервативной терапии авторы считали применение препаратов с доказанным влиянием на патологический синдром. Хирургическая тактика включала этапные некрэктомии, широкое вскрытие флегмон, малые ампутации, кожную пластику, а также артериализацию через систему малой подкожной вены. Применение мультидисциплинарного подхода позволило сохранить опорную функцию конечности при нейропатическом типе СДС у 92,4% больных, при нейро-ишемическом — у 85,6%. Для улучшения результатов лечения требуется повышение профильности работы подиатрического кабинета, рациональный отбор пациентов для своевременного лечения в стационаре, широкое применение реваскуляризирующих вмешательств
