    Trauma-induced coagulopathy management

    In the last years significant progress has been made in the understanding and treatment of hemorrhage caused by trauma which (along with the advances made in early resuscitation, and critical care) has led to a reduction of "late" deaths, meaning those due to trauma-induced organ failure/sepsis. Depending on the case, trauma is usually characterized by a variable equilibrium between hypo and hypercoagulation, with a majority of hypercoagulation cases. Trauma-induced coagulopathy (TIC), instead, is not only characterized by a coagulation disorder but also a state of inflammation which increases the need for transfusions, the risk of multiorgan failure and thromboembolic complications. In this clinical picture, hemostatic resuscitation, damage control resuscitation and homeostasis maintenance have been shown to reduce mortality. According to guidelines, the heart of TIC treatment is an early individualised goal-directed treatment relying on coagulation support and thromboprophylactic strategies (administration of tranexamic acid, fibrinogen concentrate and packed red blood cells), platelet function monitoring and viscoelastic Point-of-care testing. The implementation of the protocol suggested by the guidelines has allowed a more effective support of coagulopathy and has led to a reduction in hemoderivatives usage, hospitalization time and the death rate in severely injured trauma patients. The aforementioned improvements in TIC management were explicitly confirmed by a multicentric study in which the year the guidelines were implemented was compared with a prior year when no specific protocol was executed

    Trend of Correlations between Psychological Symptoms and Socioeconomic Inequalities among Italian Adolescents: Cross-Sectional Study from 2006 to 2018 in Tuscany Region

    Abstract: Adolescence is a critical moment in life; people become individuals, create new relationships, develop social skills and learn behaviours that they will use for the rest of their lives. During this phase, adolescents establish patterns of behaviour that can protect their health. This study aims to 1. assess the presence of psychological disorders in adolescents of both genders, 2. determine their relation to socio-economic differences based on the Family Affluence Scale (FAS), and 3. assess trends from 2006 to 2018. Data were collected from the Italian Health Behaviour in School-aged Children survey given to a representative sample of Tuscan adolescents aged 11–15 years. Participants (n. 12,550) filled out questionnaires to assess whether psychological symptoms such as feeling low, irritability, nervousness, or sleeping difficulties manifested weekly or more often over the past six months. For the study we utilized a cross-sectional survey method and linear regression to examine the association between psychological symptoms (dependent variable measured on an interval scale (0–16)), gender and FAS. We conducted similar analyses using logistic regressions for each of the four symptoms. An increase in psychological symptoms in both genders was revealed between 2006 and 2018, with a statistically significant presence in females: 0.29 (95% Confidence Interval [CI], 0.17 to 0.41), 1.43 (95% CI, 1.37 to 1.48) and 1.43 (95% CI, 1.34 to 1.52) in low-, medium- and high-affluence families, respectively; whereas males presented 0.14 (95% CI, 0.01 to 0.27), 0.71 (95% CI, 0.65 to 0.77) and 0.31 (95% CI, 0.22 to 0.39), respectively. The probability of a predictive episode of psychological symptoms (feeling low, irritability, nervousness, sleeping difficulties) occurring weekly, or more, often was greatly increased in females of all socioeconomic classes. These findings suggest that the increase in psychological disorders in adolescents should be considered a public health problem and further investigated through longitudinal studies and continuous monitoring of health trends

    Defective chromatic and achromatic visual pathways in developmental dyslexia: Cues for an integrated intervention programme.

    Abstract PURPOSE: As well as obtaining confirmation of the magnocellular system involvement in Developmental dyslexia (DD); the aim was primarily to search for a possible involvement of the parvocellular system; and, furthermore, to complete the assessment of the visual chromatic axis by also analysing the koniocellular system. METHODS: Visual evoked potentials (VEPs) in response to achromatic stimuli with low luminance contrast and low spatial frequency, and isoluminant red/green and blue/yellow stimuli with high spatial frequency were recorded in 10 dyslexic children and 10 age- and sex-matched, healthy subjects. RESULTS: Dyslexic children showed delayed VEPs to both achromatic stimuli (magnocellular-dorsal stream) and isoluminant red/green and blue/yellow stimuli (parvocellular-ventral and koniocellular streams). To our knowledge, this is the first time that a dysfunction of colour vision has been brought to light in an objective way (i.e., by means of electrophysiological methods) in children with DD. CONCLUSION: These results give rise to speculation concerning the need for a putative approach for promoting both learning how to read and/or improving existing reading skills of children with or at risk of DD. The working hypothesis would be to combine two integrated interventions in a single programme aimed at fostering the function of both the magnocellular and the parvocellular streams

    Disabilità e sessualità. La voce degli insegnanti sull’assistenza sessuale

    In the present work the authors analyze and discuss the results of a survey aimed at detectingthe perceptions and opinions of a group of 231 teachers, regarding the theme of the union ofsexuality-disability and the introduction of the figure of the Sexual Assistant. The authors makea comparison (which does not want to have the character of an experimental statistical comparisonin the strict sense) between the issues that emerged and the results of a previous trainingresearchexperience. This latter involved a group of 200 students of the course of PrimaryEducation Sciences at “Roma Tre University”. The comparison highlights interesting aspects (suchas, for example, the right of everyone to have a sexual life, the support that the Sexual Assistantcould provide to families and the school itself or the still strong presence of prejudices andmedicalizing practices) around a theme (that of the sexuality of people with disabilities) whichstill must be problematized in the research of educational sciences. In this sense, the authors(convinced that the issue, even in its periphery, burst into the mainstream flow of socialdiscourses and requires listening, questioning and requesting answers) say they want to furtherexpand their investigation by involving people with disabilities, their families and other referencefigures (educators and social workers)

    On the dimensions of secant varieties of Segre-Veronese varieties

    This paper explores the dimensions of higher secant varieties to Segre-Veronese varieties. The main goal of this paper is to introduce two different inductive techniques. These techniques enable one to reduce the computation of the dimension of the secant variety in a high dimensional case to the computation of the dimensions of secant varieties in low dimensional cases. As an application of these inductive approaches, we will prove non-defectivity of secant varieties of certain two-factor Segre-Veronese varieties. We also use these methods to give a complete classification of defective s-th Segre-Veronese varieties for small s.
