Non-radiologist-performed abdominal point-of-care ultrasonography in paediatrics - a scoping review
Background
Historically, US in the paediatric setting has mostly been the domain of radiologists. However, in the last decade, there has been an uptake of non-radiologist point-of-care US.
Objective
To gain an overview of abdominal non-radiologist point-of-care US in paediatrics.
Materials and methods
We conducted a scoping review regarding the uses of abdominal non-radiologist point-of-care US, quality of examinations and training, patient perspective, financial costs and legal consequences following the use of non-radiologist point-of-care US. We conducted an advanced search of the following databases: Medline, Embase and Web of Science Conference Proceedings. We included published original research studies describing abdominal non-radiologist point-of-care US in children. We limited studies to English-language articles from Western countries.
Results
We found a total of 5,092 publications and selected 106 publications for inclusion: 39 studies and 51 case reports or case series on the state-of-art of abdominal non-radiologist point-of-care US, 14 on training of non-radiologists, and 1 each on possible harms following non-radiologist point-of-care US and patient satisfaction. According to included studies, non-radiologist point-of-care US is increasingly used, but no standardised training guidelines exist. We found no studies regarding the financial consequences of non-radiologist point-of-care US.
Conclusion
This scoping review supports the further development of non-radiologist point-of-care US and underlines the need for consensus on who can do which examination after which level of training among US performers. More research is needed on training non-radiologists and on the costs-to-benefits of non-radiologist point-of-care US
Self-assembly of Microcapsules via Colloidal Bond Hybridization and Anisotropy
Particles with directional interactions are promising building blocks for new
functional materials and may serve as models for biological structures.
Mutually attractive nanoparticles that are deformable due to flexible surface
groups, for example, may spontaneously order themselves into strings, sheets
and large vesicles. Furthermore, anisotropic colloids with attractive patches
can self-assemble into open lattices and colloidal equivalents of molecules and
micelles. However, model systems that combine mutual attraction, anisotropy,
and deformability have---to the best of our knowledge---not been realized.
Here, we synthesize colloidal particles that combine these three
characteristics and obtain self-assembled microcapsules. We propose that mutual
attraction and deformability induce directional interactions via colloidal bond
hybridization. Our particles contain both mutually attractive and repulsive
surface groups that are flexible. Analogous to the simplest chemical bond,
where two isotropic orbitals hybridize into the molecular orbital of H2, these
flexible groups redistribute upon binding. Via colloidal bond hybridization,
isotropic spheres self-assemble into planar monolayers, while anisotropic
snowman-like particles self-assemble into hollow monolayer microcapsules. A
modest change of the building blocks thus results in a significant leap in the
complexity of the self-assembled structures. In other words, these relatively
simple building blocks self-assemble into dramatically more complex structures
than similar particles that are isotropic or non-deformable
A comparative analysis of the Libyan national essential medicines list and the WHO model list of essential medicines
Aim and Objectives: To examine the concordance of the Libyan Pharmaceutical List of Essential Medicines (LPLEM) with the World Health Organization Model List of Essential Medicines 2009 (WMLEM 2009). Methods: The concordance between generic medicines listed in the WMLEM 2009 (standard reference list) and the LPLEM 2006 (comparator list) was evaluated. Results: The total number of Basic Essential Medicines (BEMs) listed on the WMLEM 2009 was 347. The total number of generic medicines listed on the LPLEM was 584. Although the LPLEM has more listed medicines, only 270 (77.6%) of BEMs from the WMLEM were listed as available. However, 25 of the 77 missing medicines were deemed to have appropriate alternatives. A total of 52 medicines from the WMLEM 2009 were therefore missing from the LPLEM. Discrepancies compared to the WMLEM 2009 were identified in 15 out of 29 therapeutic sections. The highest discrepancy rate from the WMLEM 2009 was in the anti-infective section (35 missing medicines). Missing BEMs were noted in many subclassifications of the anti-infective medicines section, but omissions were particularly prevalent in the antibacterial medicines subsection (11 missing medicines). Antituberculosis medications had the highest discrepancy rate for antibacterial BEMs with one-third of the single medicines recommended by the WHO in the WMLEM 2009 not listed on the LPLEM. Of the 314 additional medicines on the LPLEM, 18 were deemed to be irrational non-essential medicines. Conclusion: The LPLEM does not include several essential medicines recommended by the WHO in the WMLEM 2009. These discrepancies may have serious public health implications for management of some infectious diseases, particularly, tuberculosis and HIV
Ergot resistance in sorghum in relation to flowering, inoculation technique and disease development
Ergot is an important disease of sorghum (Sorghum bicolor) in parts of Africa and Asia. Studies were conducted to determine the relationship between flowering biology and ergot infection, and to develop an artificial field-screening technique to identify ergot resistance in sorghum. Spikelets resisted infection after anthesis, but each day's delay in anthesis after inoculation supported 8-3% more ergot. The screening technique consisted of three components: trimming of panicles to remove pollinated spikelets before inoculation, a single inoculation of trimmed panicles, and panicle bagging for 7-10 days. Inoculated panicles were evaluated by a qualitative visual rating method (on a 1-5 scale) and a quantitative spikelet counting method. Selected accessions from the world collection of sorghum germplasm were screened at Karama Research Station, Rwanda, for two seasons and 12 ergot-resistant lines were identified. These were also resistant at ICRISAT Centre, India
‘Albania: €1’ or the story of ‘big policies, small outcomes’: how Albania constructs and engages its diaspora
Since the fall of the communist regime in the early 1990s, Albania has experienced one of the most significant emigrations in the world as a share of its population. By 2010 almost half of its resident population was estimated to be living abroad – primarily in neighbouring Greece and Italy, but also in the UK and North America. This chapter discusses the emergence and establishment of the Albanian diaspora, its temporal and geographical diversity, and not least its involvement with Albania itself. Albania’s policymaking and key institutions are considered, with a focus on matters of citizenship; voting rights; the debate on migration and development; and not least the complex ways in which kin-state minority policies – related to ethnic Albanians living in Kosovo, Montenegro, southern Serbia, Macedonia and Greece – are interwoven with Albania’s emigration policies
Energy consumption in chemical fuel-driven self-assembly
Nature extensively exploits high-energy transient self-assembly structures that are able to perform work through a dissipative process. Often, self-assembly relies on the use of molecules as fuel that is consumed to drive thermodynamically unfavourable reactions away from equilibrium. Implementing this kind of non-equilibrium self-assembly process in synthetic systems is bound to profoundly impact the fields of chemistry, materials science and synthetic biology, leading to innovative dissipative structures able to convert and store chemical energy. Yet, despite increasing efforts, the basic principles underlying chemical fuel-driven dissipative self-assembly are often overlooked, generating confusion around the meaning and definition of scientific terms, which does not favour progress in the field. The scope of this Perspective is to bring closer together current experimental approaches and conceptual frameworks. From our analysis it also emerges that chemically fuelled dissipative processes may have played a crucial role in evolutionary processes
