Successful management of a large traumatic iris cyst
Background: Iris cysts are rare lesions of the anterior segment of the eye, they can be congenital or acquired. Themanagement of acquired post-traumatic iris cysts remains difficult.
Case report: We report the case of a 24-year-old girl who presented with a progressive decrease in vision in the lefteye with pain and redness for the past few months. She had a history of penetrating injury in the same eye 14 yearsago. The slit lamp examination identified a corneal scar at 7’ o’clock and a large iris cyst deforming the pupillary area.
Ultrasound biomicroscopy (UBM) has confirmed the benign nature of the cyst. Anterior segment optical coherence tomography (AS-OCT) has shown the contact of the external layer of the cyst with the corneal endothelium. Surgicalmanagement consisted of soft viscodissection to separate the cyst from the corneal endothelium followed by complete excision of the external layer of the cyst.
Conclusion: The postoperative course was simple with good visual outcome
Ocular penetrating injuries in children
Background: Ocular penetrating injuries in children are common and potentially serious. They take on a special character because of the major risk of amblyopia that they generate in children. The objective of this work is to analyze the epidemiological and clinical aspects of these serious traumas and to study the functional prognosis in Moroccan children.
Material and methods: We conducted a retrospective study of 83 children between January 2016 and December 2019. The average age of children is 6.5 years, with 64 boys and 19 girls.
Results: The circumstances of penetrating eye injuries are accidental dominated by street games. Corneal wounds represented 67.5% associated with iris prolapse in 39 cases and hyphema in 34 cases. In 30.1% of cases, a post-traumatic cataract is associated, and a foreign body is detected in 6% of cases. Final visual acuity ≥ 5/10 is objectified in 30% of cases.
Results: This study highlights the importance of preventing these serious childhood traumas by implementing education and awareness-raising measures
Traumatic endophthalmitis with intraocular metallic foreign body
Background: Post-traumatic endophthalmitis with retained intraocular foreign body is a severe complication ofopen globe injuries. It is generally associated with poor visual outcome and requires prompt and adapted management.
Case report: A 39-year-old male patient presented with open globe injury of the right eye due to a metallic foreignbody projection. He was a victim of a neglected work accident 10 days ago when hammering a metallic bar. Visualacuity on admission was LP (+) in the right eye. The Slit-lamp examination revealed perikeratic injection, edematouscornea and severe inflammation of the anterior chamber with hypopyon. Ocular ultrasound showed a heterogeneousvitreous organization compatible with a vitreous abscess. Orbito-cerebral CT scan revealed a retained intraocularmetallic foreign body located in the posterior segment, within the vitreous humor. The patient received local andgeneral antibiotherapy associated with intravitreal injections of vancomycin and ceftazidime followed by a vitrectomyto remove the intraocular foreign body. The visual outcome was poor with an evolution towards phtysis bulbi. Theaim of this case report is to study the risk factors of post-traumatic endophthalmitis with retained intraocular foreignbody and to examine treatment principles and visual outcome of this severe complication
Management of orbital cellulitis: a retrospective study
Background: Orbital cellulitis is an infectious involvement of the tissues posterior to the orbital septum causingsevere local and general complications.
Material and methods: The aim of our work was to study the clinical, paraclinical and therapeutic characteristics of orbital cellulitis through a retrospective study of 89 cases collected between 2015 and 2019. The diagnosis of orbital cellulitis was based on clinical and imaging elements.
Results: The average age was 17.5 years. Exophthalmos was noted in 33% of cases, ophthalmoplegia in 18% of cases, diplopia in 4.5% of cases and ocular hypertonia in 11% of cases. Orbital CT scan allowed the diagnosis of subperiosteal abscess in 20% of cases and orbital abscess in 10% of cases. Sinusitis was the main infectious origin in children, while dacryocystitis predominated in adults. All patients received parenteral antibiotherapy combined with corticotherapy after 48 hours. Complications occurred in 10 cases, dominated by exposure keratitis, panophthalmitis and optic atrophy with permanent blindness.
Conclusions: We emphasize the importance of rapid diagnosis and urgent treatment of orbital cellulitis
Métastases choroïdiennes bilatérales d’origine inconnue: à propos d’un cas
Nous rapportons l'observation d'un patient de 52 ans sans antécédents pathologiques notables, qui présente depuis 10 mois un flou visuel de l'œil droit et des métamorphopsies au niveau de l'œil gauche, suivies de troubles visuels très gênants et baisse importante de l'acuité visuel du coté droit, motivant une consultation. Le bilan oculaire a mis en évidence des métastases choroïdiennes bilatérales ; le bilan d'extension révèle des métastases multifocales dont le site primitif est inconnu, notre patient a décédé durant les jours d'explorations
Abundance and structure of copepod communities along the Atlantic coast of southern Morocco
Zooplankton composition and the spatial and temporal abundance distribution of copepods were examined on the southern Atlantic coast of Morocco. In 1998, 31 stations during March and 30 stations during July were sampled on the continent shelf between Cape Blanc (21° O’N) and Cape Boujdor (26°30’N). Holoplankton dominated the zooplankton assemblages. Copepods represented 86% of the zooplankton in March and 73% in July. Herbivorous species were the main components of the 78 identified copepod species (60 species in March and 49 in July). Four species were recorded for the first time in this area: Calanus hyperboreus, Scaphocalanus brevicornis, S. medius, and Heterocope saliens. Both zooplankton abundance and diversity were related to changes in ecological parameters resulting from upwelling currents. The uplift of cold water enriched superficial water layers with nutrients and induced strong primary production upon which zooplankton depend. This study discusses the environmental features that influence plankton resources and the relationship of plankton production to the pelagic fisheries off northwest Africa
Le glaucome congénitale et la neurofibromatose type 1
Le glaucome congénital constitue une complication ophtalmologique dans la neurofibromatose type 1 ou maladie de von Recklinghausen. Choriorétinite extensive bilatérale révélant une infection par le virus de l’immunodéficience humaine (VIH)
Au cours de l'infection par le virus de l'immunodéficience humaine(VIH), Les atteintes oculaires sont polymorphes, pouvant compromettre le pronostic fonctionnel. Nous rapportons l'observation d'un patient présentant une choriorétinite infectieuse sévère révélant une infection par le VIH. Patient âgé de 35 ans avec antécédent de tuberculose pulmonaire en 2007, consulte pour BAV bilatérale progressive depuis 6 mois. Une acuité visuelle à compte les doigts au niveau de l'oeil droit et à mouvement des doigts au niveau de l'oeil gauche, avec présence de foyers choriorétiniens diffus visualisés au fond d'oeil et à l'angiographie. Les sérologies VIH, toxoplasmose et CMV sont positives. Le patient a été mis sous traitement anti-toxoplasmose (Sulfadiazine et pyriméthamine) et anti-CMV (Ganciclovir per os). L'évolution sous traitement a été marquée par une régression de la hyalite avec la persistance des foyers choriorétiniens évolutifs et une acuité visuelle réduite à perception lumineuse.Key words: Choriorétinite, virus de l´immunodéficience humaine(VIH), Sulfadiazine, pyriméthamine, Ganciclovi
Aspect Clinique Atypique De La Tuberculose Oculaire A Propos D'un Cas
Introduction: The ophthalmological manifestation of tuberculosis varies significantly, and almost all the elements of the globe and orbit can be reached. Methods and Results: The aim of this work was to report a clinical aspect of ocular tuberculosis in a young woman who has a notion of tuberculous contagion and who was presented at the fundus of a macular papillary membrane with a tubercle of Bouchut or papillary tubercle. The diagnosis was retained on a bundle of clinical, biological, and radiological arguments. The evolution under treatment was favorable with an improvement of its visual acuity. Conclusion: It is necessary to know how to evoke the tuberculous etiology in view of all ocular inflammation, even atypical. This is required considering the great polymorphism of its clinical characteristics
