    Flaring X-ray Emission from HST-1, a Knot in the M87 Jet

    We present Chandra X-ray monitoring of the M87 jet in 2002, which shows that the intensity of HST-1, an optical knot 0.8" from the core, increased by a factor of two in 116 days and a factor of four in 2 yrs. There was also a significant flux decrease over two months, with suggestive evidence for a softening of the spectrum. From this variability behavior, we argue that the bulk of the X-ray emission of HST-1 comes from synchrotron emission. None of the other conceivable emission processes can match the range of observed characteristics. By estimating synchrotron model parameters for various bulk relativistic velocities, we demonstrate that a model with a Doppler factor, delta, in the range 2 to 5 fits our preliminary estimates of light travel time and synchrotron loss timescales.Comment: 4 pages with 3 embedded figures; 1 of which is color but prints ok in b/w.     Dual-frequency VLBI study of Centaurus A on sub-parsec scales

    Centaurus A is the closest active galactic nucleus. High resolution imaging using Very Long Baseline Interferometry (VLBI) enables us to study the spectral and kinematic behavior of the radio jet-counterjet system on sub-parsec scales, providing essential information for jet emission and formation models. Our aim is to study the structure and spectral shape of the emission from the central-parsec region of Cen A. As a target of the Southern Hemisphere VLBI monitoring program TANAMI (Tracking Active Galactic Nuclei with Milliarcsecond Interferometry), VLBI observations of Cen A are made regularly at 8.4 and 22.3 GHz with the Australian Long Baseline Array (LBA) and associated telescopes in Antarctica, Chile, and South Africa. The first dual-frequency images of this source are presented along with the resulting spectral index map. An angular resolution of 0.4 mas x 0.7 mas is achieved at 8.4 GHz, corresponding to a linear scale of less than 0.013 pc. Hence, we obtain the highest resolution VLBI image of Cen A, comparable to previous space-VLBI observations. By combining with the 22.3 GHz image, which has been taken without contributing transoceanic baselines at somewhat lower resolution, we present the corresponding dual-frequency spectral index distribution along the sub-parsec scale jet revealing the putative emission regions for recently detected gamma-rays from the core region by Fermi/LAT. We resolve the innermost structure of the milliarcsecond scale jet and counterjet system of Cen A into discrete components.     Opacity in compact extragalactic radio sources and its effect on astrophysical and astrometric studies

    The apparent position of the "core" in a parsec-scale radio jet (a compact, bright emitting region at the narrow end of the jet) depends on the observing frequency, owing to synchrotron self-absorption and external absorption. While providing a tool probing physical conditions in the vicinity of the core, this dependency poses problems for astrometric studies using compact radio sources. We investigated the frequency-dependent shift in the positions of the cores (core shift) observed with very long baseline interferometry (VLBI) in parsec-scale jets. We discuss related physics, as well as its effect on radio astrometry and the connection between radio and optical positions of astrometric reference objects. We searched for the core shift in a sample of 277 radio sources imaged at 2.3 GHz (13 cm) and 8.6 GHz (4 cm) frequency bands using VLBI observations made in 2002 and 2003. The core shift was measured by referencing the core position to optically thin jet features whose positions are not expected to change with frequency. We present here results for 29 selected active galactic nuclei (AGN) with bright distinct VLBI jet features that can be used in differential measurements and that allow robust measurements of the shift to be made. In these AGN, the magnitude of the measured core shift between 2.3 and 8.6 GHz reaches 1.4 mas, with a median value for the sample of 0.44 mas. Nuclear flares result in temporal variability of the shift. An average shift between the radio (4 cm) and optical (6000 Angstrom) bands is estimated to be approximately 0.1 mas, and it should be taken into account in order to provide the required accuracy of the radio-optical reference frame connection.     Development of a lung slice preparation for recording ion channel activity in alveolar epithelial type I cells

    BACKGROUND: Lung fluid balance in the healthy lung is dependent upon finely regulated vectorial transport of ions across the alveolar epithelium. Classically, the cellular locus of the major ion transport processes has been widely accepted to be the alveolar type II cell. Although evidence is now emerging to suggest that the alveolar type I cell might significantly contribute to the overall ion and fluid homeostasis of the lung, direct assessment of functional ion channels in type I cells has remained elusive. METHODS: Here we describe a development of a lung slice preparation that has allowed positive identification of alveolar type I cells within an intact and viable alveolar epithelium using living cell immunohistochemistry. RESULTS: This technique has allowed, for the first time, single ion channels of identified alveolar type I cells to be recorded using the cell-attached configuration of the patch-clamp technique. CONCLUSION: This exciting new development should facilitate the ascription of function to alveolar type I cells and allow us to integrate this cell type into the general model of alveolar ion and fluid balance in health and disease

    In pursuit of excellence for patients with cancer: the Scottish Cancer Therapy Network model

    The Scottish Cancer Therapy Network (SCTN) was created against a background of rising concerns about perceived variation in the quality of care available to patients with cancer. SCTN has established itself as a major organization with the necessary recognition and infrastructure to provide leadership, support and impetus in the field of clinical guidelines, clinical audit and clinical trials of cancer therapy in Scotland. Since being formed in 1993, SCTN has been instrumental in the development of three evidence-based, clinical guidelines and in the completion of detailed, national, retrospective audits of the treatment of five major tumour sites. The infrastructure has been used successfully to support and encourage trial participation. Challenges for the future are a re-orientation towards prospective audit, widening the constituency and sense of ownership of SCTN as a resource for practising clinicians, and further increasing recruitment into clinical trials. © 1999 Cancer Research Campaig

    Anaplastic thyroid carcinoma: three protocols combining doxorubicin, hyperfractionated radiotherapy and surgery

    Patients with anaplastic thyroid carcinoma can rarely be cured, but every effort should be made to prevent death due to suffocation. Between 1984 and 1999, 55 consecutive patients with anaplastic thyroid carcinoma were prospectively treated according to a combined regimen consisting of hyperfractionated radiotherapy, doxorubicin, and when feasible surgery. Radiotherapy was carried out for 5 days a week. The daily fraction until 1988 was 1.0 Gy×2 (A) and 1989–92 1.3 Gy×2 (B) . Thereafter 1.6 Gy×2 (C) was administered. Radiotherapy was administered to a total target dose of 46 Gy; of which 30 Gy was administered preoperatively in the first two protocols (A and B), while the whole dose was given preoperatively in the third protocol (C). The therapy was otherwise identical. Twenty mg doxorubicin was administered intravenously weekly. Surgery was possible in 40 patients. No patient failed to complete the protocol due to toxicity. In only 13 cases (24%) was death attributed to local failure. Five patients (9%) ‘had a survival’ exceeding 2 years. No signs of local recurrence were seen in 33 patients (60%); 5 out of 16 patients in Protocol A, 11 out of 17 patients in Protocol B, 17 out of 22 patients in Protocol C (P=0.017). In the 40 patients undergoing additional surgery, no signs of local recurrence were seen in 5 out of 9 patients, 11 out of 14 patients and 17 out of 17 patients, respectively (P=0.005)

    Theoretical overview on high-energy emission in microquasars

    Microquasar (MQ) jets are sites of particle acceleration and synchrotron emission. Such synchrotron radiation has been detected coming from jet regions of different spatial scales, which for the instruments at work nowadays appear as compact radio cores, slightly resolved radio jets, or (very) extended structures. Because of the presence of relativistic particles and dense photon, magnetic and matter fields, these outflows are also the best candidates to generate the very high-energy (VHE) gamma-rays detected coming from two of these objects, LS 5039 and LS I +61 303, and may be contributing significantly to the X-rays emitted from the MQ core. In addition, beside electromagnetic radiation, jets at different scales are producing some amount of leptonic and hadronic cosmic rays (CR), and evidences of neutrino production in these objects may be eventually found. In this work, we review on the different physical processes that may be at work in or related to MQ jets. The jet regions capable to produce significant amounts of emission at different wavelengths have been reduced to the jet base, the jet at scales of the order of the size of the system orbital semi-major axis, the jet middle scales (the resolved radio jets), and the jet termination point. The surroundings of the jet could be sites of multiwavelegnth emission as well, deserving also an insight. We focus on those scenarios, either hadronic or leptonic, in which it seems more plausible to generate both photons from radio to VHE and high-energy neutrinos.     Patient and Public Involvement and Engagement for PhD Students

    This paper aims to provide useful advice regarding the development of skills for patient and public involvement and engagement (PPIE) in research. The authors of this paper comprise experienced PhD supervisors and trainers, researchers leading PPIE activities including in the National Institute for Health and Social Care Research (NIHR) Nottingham Biomedical Research Centre (BRC), and experts in the conduct of PPIE. The paper arose from discussions in preparation for a programme of PPIE training for PhD students in the University of Nottingham, UK. We offer this advice on the basis that it is likely to be of use to others undertaking research training such as undergraduates or research associates moving into health-related research areas. Although the PPIE team of the NIHR Nottingham Biomedical Research Centre prepared this paper, we hope that it is of value across the whole spectrum of health research, not solely to experimental medicine and is of relevance to research more widely than that supported by the UK’s NIHR.     A feasibility study incorporating a pilot randomised controlled trial of oral feeding plus pre-treatment gastrostomy tube versus oral feeding plus as-needed nasogastric tube feeding in patients undergoing chemoradiation for head and neck cancer (TUBE trial): study protocol

    Background There are 7000 new cases of head and neck squamous cell cancers (HNSCC) treated by the NHS each year. Stage III and IV HNSCC can be treated non-surgically by radio therapy (RT) or chemoradiation therapy (CRT). CRT can affect eating and drinking through a range of side effects with 90 % of patients undergoing this treatment requiring nutritional support via gastrostomy (G) or nasogastric (NG) tube feeding. Long-term dysphagia following CRT is a primary concern for patients. The effect of enteral feeding routes on swallowing function is not well understood, and the two feeding methods have, to date, not been compared to assess which leads to a better patient outcome. The purpose of this study is to explore the feasibility of conducting a randomised controlled trial (RCT) comparing these two options with particular emphasis on patient willingness to be randomised and clinician willingness to approach eligible patients. Methods/design This is a mixed methods multicentre study to establish the feasibility of a randomised controlled trial comparing oral feeding plus pre-treatment gastrostomy versus oral feeding plus as required nasogastric tube feeding in patients with HNSCC. A total of 60 participants will be randomised to the two arms of the study (1:1 ratio). The primary outcome of feasibility is a composite of recruitment (willingness to randomise and be randomised) and retention. A qualitative process evaluation investigating patient, family and friends and staff experiences of trial participation will also be conducted alongside an economic modelling exercise to synthesise available evidence and provide estimates of cost-effectiveness and value of information. Participants will be assessed at baseline (pre-randomisation), during CRT weekly, 3 months and 6 months. Discussion Clinicians are in equipoise over the enteral feeding options for patients being treated with CRT. Swallowing outcomes have been identified as a top priority for patients following treatment and this trial would inform a future larger scale RCT in this area to inform best practice
