Self-assembly of three-dimensional ensembles of magnetic particles with laterally shifted dipoles
We consider a model of colloidal spherical particles carrying a permanent
dipole moment which is laterally shifted out of the particles' geometrical
centres, i.e. the dipole vector is oriented perpendicular to the radius vector
of the particles. Varying the shift from the centre, we analyze ground
state structures for two, three and four hard spheres, using a simulated
annealing procedure. We also compare to earlier ground state results. We then
consider a bulk system at finite temperatures and different densities. Using
Molecular Dynamics simulations, we examine the equilibrium self-assembly
properties for several shifts. Our results show that the shift of the dipole
moment has a crucial impact on both, the ground state configurations as well as
the self-assembled structures at finite temperatures
Legislation and implementation of Azerbaijan's legal obligations towards its ethnic minorities
The article is devoted to analysis of the legislation and implementation of Azerbaijan's legal obligations towards its ethnic minorities. The author analysed the legislation of the Republic of Azerbaijan, which determines the state’s policy towards minorities, such as the Constitution, laws reflecting the government’s policy towards ethnic minorities and also the debates around the implementation of Azerbaijan’s legal obligations towards its ethnic minorities. The article explains the state’s policy towards protection of languages of national minorities. The relevant provisions, laws, and instructions provide for the right of the people who do not speak the Azerbaijani language, to use their native languages in places where they receive legal or social aid or hold cultural or religious events. In accordance with the Criminal Procedural Code of the Republic of Azerbaijan, persons participating in the criminal process, particularly witnesses, victims, suspects, and defendants shall have the right to testify in their mother tongue. In addition, the author looks at legal protection of human rights of minorities. There are different opinions about practical application of the laws and treaties concerning Azerbaijan's ethnic minorities. Thus, the government assumes that ethnic minorities experience no problems. However, according to some media reports, human rights activists and reports from non-governmental organizations, these minorities are subjected to discrimination and attempts of assimilation. Secondhand aerosol exposure during mechanical ventilation with and without expiratory filters: An in-vitro study
Background: Concerns have been expressed about risk of exposure to exhaled aerosols to ICU personnel.
AIM: To quantify amount of aerosol collected at the exhaust outlet of mechanical ventilators operated with and without filters in the expiratory limb.
Methods: Two categories of ventilators were tested: (1) Ventilators without Proprietary Filters: Servo-i (Maquet) and Galileo (Hamilton) and (2) Ventilator with proprietary filters: PB 840 (Covidien). Each ventilator was attached to a simple test lung and operated with VT 500 ml, RR 20 bpm, PIF 50 L/min, PEEP 5 cmH2O. Four separate doses of albuterol (2.5 mg/3mL) were administered via jet nebuliser (eValueMed, Tri-anim) placed at the “Y”. In Experiment A, a filter (Respirgard 303) was placed at the exhaust port. In Experiment B, two filters were attached to the ventilators without proprietary filters: (1) at the end of expiratory limb and (2) at the exhaust outlet. Drug was eluted from filters and measured using spectrophotometry.
Results: Drug deposited at the exhaust port without expiratory filtering was \u3e160 fold higher than with expiratory filtering. The collecting filter used in this study was less efficient than the proprietary filter designed for use with the ventilator. Regardless of type of filter used, placement of filter in the expiratory limb reduced secondhand aerosol exposure significantly.
Conclusion: Risk of secondhand exposure to exhaled aerosol can account for \u3e45% of nominal dose as well as droplet nuclei produced by patients. Using expiratory filters decreases risk of exposure to aerosol released to the atmosphere during mechanical ventilation
Effect of leucine on NF-?B pathway in liver regeneration after partial hepatectomy in rats as determined by miniarray analysis
PubMed: 29455543BACKGROUND: Although some amino acids are recognized to have favorable effects on the liver regeneration after partial hepatectomy (PH), molecular mechanisms underlying these effects are barely known. OBJECTIVE: Our study was aimed to investigate the effects of valine, glutamine, and leucine amino acids on PH-induced NF-?B signal pathway. The research team studied Leucine in a rat model in vivo. The study took place in the medical and surgical experimental research center at the Eskisehir Osmangazi University in Eskisehir, Turkey. The animals were Wistar albino male rats. RESULTS: Group I, the sham group, was administered phosphate buffered saline (PBS) after laparotomy. After 70 % PH procedure, group II, III, IV, and V received single intraperitoneal doses of PBS, valine, glutamine, and leucine amino acids, respectively. At hour 6 after PH, expressions of 88 genes involving in NF-?B signal pathway were examined by RT-PCR mini array method in the liver tissue specimen. Fold values below 0.5 and above 2 were regarded as significant. CONCLUSIONS: Our results suggested that valine, glutamine, and leucine amino acids may alter expressions of the genes of NF-?B signal pathway. Effect of heparin in the intraocular irrigating solution on postoperative inflammation in the pediatric cataract surgery

Setting: Kartal Training and Research Hospital, First Eye Clinic, Istanbul, Turkey.Design: Randomized prospective double-blind study.Methods: Fourteen consecutive eyes from 14 patients aged 8.9 ± 5.9 years, (range 3–18 years) (group 1) and 19 eyes from 19 patients aged 9.1 ± 5.2 (range 1.5–18 years) (group 2) underwent pediatric cataract surgery. Five patients in group 1 were between three and five years old. One patient was 1.5 years old and six patients in group 2 were between three and five years old. During the procedure, group 1 received anterior chamber irrigation with heparin sodium (5 IU/cc) and 1 ml of heparin sodium (concentration 10 IU/ml) added to the irrigating balanced salt solution (BSS Plus; Alcon Laboratories, Inc., Fort Worth, TX, USA) while group 2 received BSS without heparin sodium only. Cases aged under three years received anterior vitrectomy in addition to posterior capsulorrhexis. One eye received anterior vitrectomy in group 1 and two eyes received anterior vitrectomy in group 2. Cases with preoperative complications were not included in the study. Early and late postoperative inflammatory complications, including fibrin formation, anterior and posterior synechia, cyclitic and pupillary membrane formation were recorded and compared.Results: Mild anterior chamber reaction was observed in three patients in Group 1, while nine cases in group 2 experienced marked anterior chamber reaction. In four of nine patients from group 2, anterior chamber reaction was severe and resulted in pupillary membrane and synechia despite treatment in the postoperative 7th day, while in all three cases in group 1, reaction disappeared by the 7th day.Conclusion: Anterior chamber irrigation with heparin during pediatric cataract surgery may minimize early inflammatory reaction and decrease the number of postoperative inflammatory related complications.Keywords: pediatric, cataract, surgery, inflammatio
The reality of managing asthma in sub-Saharan Africa – Priorities and strategies for improving care
Asthma is the most common non-communicable disease in children and remains one of the most common throughout the life course. The great majority of the burden of this disease is seen in low-income and middle-income countries (LMICs), which have disproportionately high asthma-related mortality relative to asthma prevalence. This is particularly true for many countries in sub-Saharan Africa. Although inhaled asthma treatments (particularly those containing inhaled corticosteroids) markedly reduce asthma morbidity and mortality, a substantial proportion of the children, adolescents, and adults with asthma in LMICs do not get to benefit from these, due to poor availability and affordability. In this review, we consider the reality faced by clinicians managing asthma in the primary and secondary care in sub-Saharan Africa and suggest how we might go about making diagnosis and treatment decisions in a range of resource-constrained scenarios. We also provide recommendations for research and policy, to help bridge the gap between current practice in sub-Saharan Africa and Global Initiative for Asthma (GINA) recommended diagnostic processes and treatment for children, adolescents, and adults with asthma

Carbamazepine overdose after exposure to simethicone: a case report
<p>Abstract</p> <p>Introduction</p> <p>Carbamazepine is an anticonvulsant drug and is also used as a treatment for patients with manic-depressive illness, post-herpetic neuralgia or phantom limb pain. The drug itself has many drug interactions. Simethicone is an antifoaming agent and is reported to be an inert material with no known drug interaction with carbamazepine.</p> <p>Case presentation</p> <p>We present a case of a patient who was routinely using carbamazepine 400 mg three times per day and levetiracetam 500 mg twice daily, and experienced carbamazepine overdose after exposure to simethicone. After cessation of simethicone therapy normal drug levels of carbamazepine were obtained again with the standard dose of the drug. The mechanism of interaction is unknown but the risk of overdose should be considered when prescribing simethicone to a patient who is using carbamazepine.</p> <p>Conclusion</p> <p>Simethicone and carbamazepine, when taken together, may be a cause of carbamazepine toxicity. The risk of carbamazepine overdose should be considered when prescribing simethicone to a patient who is using carbamazepine.</p
Discovery and anticancer screening of novel oxindole-based derivative bearing pyridyl group as potent and selective dual FLT3/CDK2 kinase inhibitor
Protein kinases regulate cellular activities and make up over 60% of oncoproteins and proto-oncoproteins. Among these kinases, FLT3 is a member of class III receptor tyrosine kinase family which is abundantly expressed in individuals with acute leukemia. Our previous oxindole-based hit has a particular affinity toward FLT3 (IC50 = 2.49 μM) and has demonstrated selectivity towards FLT3 ITD-mutated MV4-11 AML cells, with an IC50 of 4.3 μM. By utilizing the scaffold of the previous hit, sixteen new compounds were synthesized and screened against NCI-60 human cancer cell lines. This leads to the discovery of a potent antiproliferative compound, namely 5l, with an average GI50 value against leukemia and colon cancer subpanels equalling 3.39 and 5.97 µM, respectively. Screening against a specific set of 10 kinases that are associated with carcinogenesis indicates that compound 5l has a potent FLT3 inhibition (IC50 = 36.21 ± 1.07 nM). Remarkably, compound 5l was three times more effective as a CDK2 inhibitor (IC50 = 8.17 ± 0.32 nM) compared to sunitinib (IC50 = 27.90 ± 1.80 nM). Compound 5l was further analyzed by means of docking and molecular dynamics simulation for CDK2 and FLT3 active sites which provided a rational for the observed strong inhibition of kinases. These results suggest a novel structural scaffold candidate that simultaneously inhibits CDK2 and FLT3 and gives encouragement for further development as a potential therapeutic for leukemia and colon cancer
Performance Comparisons of Jet and Mesh Nebulizers Using Different Interfaces in Simulated Spontaneously Breathing Adults and Children
Background: Different types of nebulizers and interfaces are used for the treatment of adults and children with pulmonary diseases. The purpose of this study was to determine the efficiency of a mesh nebulizer (MN) with a proprietary adapter and a jet nebulizer (JN) under different configurations in adult and pediatric models of spontaneous breathing. We hypothesize that delivery efficiency of JN and MN will differ depending on the interface used during aerosol therapy in simulated spontaneously breathing adult and pediatric models. While we expect that aerosol delivery with JN will be less efficient than MN, we also hypothesize that lung deposition obtained with the adult lung model will be more than that with the pediatric lung model in all conditions tested in this study. Methods: A lung model using a teaching manikin connected to a sinusoidal pump via a collecting filter at the level of the bronchi simulating a spontaneously breathing adult (Vt 500 mL, RR 15 bpm, I:E ratio 1:2) or pediatric patient (Vt 150 mL, RR 25 bpm, I:E ratio 1:2). Albuterol sulfate (2.5 mg/3 mL) was aerosolized with JN (Mistymax 10, Airlife) or MN (Aerogen Solo®, Aerogen) with the Adapter (Aerogen Solo® Adapter, Aerogen Ltd, Galway, Ireland) using mouthpiece, aerosol mask, and valved-mask in adults and the dragon mask, aerosol mask, and valved-mask in pediatrics (n=3). The Adapter, specifically designed for MN, was attached to all the interfaces used in this study with supplemental oxygen of 2 lpm, and in addition, the MP was tested with no additional flow in the adult model. The JN was driven with 10 lpm based on the manufacturer\u27s label. Drug was eluted from the filter and analyzed via spectrophotometry. Descriptive statistics, dependent t-test and one-way analysis of variance were used for data analysis. Significant level was set at 0.05. Results: In adults, delivery efficiency of JN with the valved mask was significantly greater than that with the aerosol mask (p=0.01). Aerosol delivery of JN with the mouthpiece was not statistically significant from the valved mask (p=0.123) and the aerosol mask (p=0.193). Drug delivery with MN with mouthpiece (15.42±1.4%) and valved-mask (15.15±1.1%) was greater than the open aerosol mask (7.54±0.39%; p=0.0001) in the adult lung model. With no flow mouthpiece delivery increased\u3e2 fold (34.9±3.1%; p=.0001) compared to use of 2 lpm of flow. Using the JN with the pediatric model deposition with valved-mask (5.3±0.8%), dragon mask (4.7±0.9%), and aerosol mask (4.1±0.3%) were similar (p\u3e0.05); while drug delivery with MN via valved-mask (11.1±0.7%) was greater than the dragon mask (6.44±0.3%; p=0.002) and aerosol mask (4.6±0.4%; p=0.002), and the dragon mask was more efficient than the open aerosol mask (p=0.009) Conclusion: The type of nebulizer and interface used for aerosol therapy affects delivery efficiency in these simulated spontaneously breathing adult and pediatric models. Drug delivery was greatest with the valved-mouthpiece and mask with JN and MN, while the standard aerosol mask was least efficient in these simulated spontaneously breathing adult and pediatric lung models. Delivery efficiency of JN was less than MN in all conditions tested in this study except in the aerosol mask. Lung deposition obtained with the adult lung model was more than that with the pediatric lung model
