    Growth and yield variability of groundnut (Arachis hypogaea L.) cultivars infected with cowpea aphid-borne mosaic virus disease

    Groundnut (Arachis hypogaea) is a major legume crop with diverse uses in Nigeria. Its productivity is however threatened by Cowpea aphid-borne mosaic virus (CABMV) in most groundnut producing areas, resulting in huge losses. This study was conducted to determine the resistance of some commercial groundnut cultivars under CABMV disease. The experiment was conducted in Minna, Southern Guinea Savanna agro-ecological zone of Nigeria. Twenty groundnut cultivars were evaluated separately as healthy and CABMV infected trials. The experiment was laid out in randomised complete block design with three replications. Seedlings were infected with the virus by mechanical inoculation at 10 days after sowing. The plants were observed for disease incidence, growth and yield attributes. The data collected were subjected to analysis of variance, principal component and cluster analyses at p≤0.05. One hundred percent infection was found regardless of the cultivar. The healthy plants exhibited significantly (p<0.05) higher morphological and yield parameters than infected plants. Principal component analysis revealed that 100-seed weight accounted for the greatest variability in healthy (eigen vector = 0.6239) and CABMV infected (eigen vector = 0.6005) plants. Cluster analysis showed that 18 (90 %) cultivars formed cluster 1, whereas one cultivar each was found in cluster 2 and 3. The top three cultivars for 100-seed weight: “SAMNUT 23” (56.0 g), “SAMNUT 25” (50.5 g) and “SAMNUT 26” (50.9 g) were the most tolerant to CABMV, whereas “ICG-92267” was identified as the best cultivar for dry matter production (24.0 g/plant).     Systemic Lupus Erythematosus Masquerading as Disseminated Tuberculosis: Case Report in a Nigerian Adolescent

    Systemic lupus erythematosus (SLE) is a connective tissue disorder whose manifestations may mimic other common chronic diseases in children. In developing countries, its diagnosis is often delayed or missed leading to delay in instituting appropriate treatment and invariably high mortality. We report the case of A. B who was a 13‑year‑old girl referred from a peripheral hospital with chronic cough, weight loss, and dyspnea. She had signs of heart failure and developed depression as well as oliguria. The patient also had pleural effusion, but aspirate result was negative for Mycobacterium tuberculosis and cytology. She commenced antituberculous drugs and dexamethasone with other supportive care but died after 19 days on  admission. Serum assay was positive for antinuclear antibody. SLE is a potential masquerader of chronic diseases such as tuberculosis. Delay in  diagnosis and treatment is associated with poor outcome; hence, there is a need for high index of suspicion for early diagnosis with prompt initiation of appropriate treatment.     The health care sector is characterised by constant reforms aimed at the efficient delivery of safe, effective, and high-quality care. Effective leadership is required to lead and drive changes at all levels of the health system to actualise the goals of the ongoing reforms in health care organisations. Leadership in the health care sector is spread across management and clinical workforces, creating peculiar challenges. The chapter examines the concept of leadership in the health care sector within the context of the recent drive by health care organisations to identify essential competencies and training required by health management and leadership workforces for effective performance in roles. It concludes that further research is needed to build the evidence on the relationships between targeted training and professional development interventions, individual competence of leaders from health management and clinical backgrounds and organisational performance

    Growth and yield components of some groundnut (Arachis hypogaea L.) cultivars infected with blackeye cowpea mosaic virus

    Blackeye cowpea mosaic virus (BlCMV) is a major virus, infecting legumes with attendant huge losses. Cultivation of resistant varieties is the most effective and sustainable control strategy. Therefore, some groundnut (Arachis hypogaea) cultivars were evaluated against BlCMV in Minna, Southern Guinea savanna zone of Nigeria. The experiment was conducted at the Teaching and Research Farm, Federal University of Technology, Minna. It was arranged as infected and uninfected using Randomised Complete Block Design (RCBD) with three replications. Groundnut seeds were sowed in the second week of August, 2015. Seedlings were inoculated by sap transmission at 10 days after sowing. Disease incidence, severity, growth and yield attributes were recorded. Data were subjected to analysis of variance (ANOVA) and means separated at p≤0.05 probability level. Disease incidence varied significantly p<0.05 from 28.3 to 60.3 % at one week after inoculation (WAI) and 44.7 to 100 % at 2 WAI. ICGV 91317 which expressed mild infection (symptom score = 2) at 9 WAI also exhibited the lowest leaf diameter reduction (3.6 %) at that growth stage. FDRF7-82 which had the lowest reduction in number of leaves per plant at 3 and 6WAI (10.7 and 9.6 %, respectively) also exhibited the lowest reduction in fresh haulm weight per plant (42.2 %). None of the cultivars exhibited consistent reactions, FDRF7-82 and ICGV 91317 had an appreciable combination of growth and yield attributes under BlCMV infection. However, these cultivars could be improved upon either through conventional or molecular breeding by coding with desirable genes. The cultivars which were adversely affected by BlCMV disease could so be improved genetically through appropriate gene introgression from the resistant cultivars.Keywords: Blackeye cowpea mosaic virus; disease incidence and severity; growth and yield; groundnu

    Implications of New Zealand’s Primary Health Care Policies for Management and Leadership

    Introduction: Reforms have been introduced since 2000 to make New Zealand’s health system primary care-led. A competent health management workforce is necessary to provide leadership for the goals of the reforms to be realised. Aim and objective : To review New Zealand’s key primary health care policies from 2000 to 2016 and consider their implications for management and leadership. Methods : A document analysis was undertaken using qualitative content analysis. Eligible documents were identified through the websites of relevant government and non-government agencies, World Health Organisation, and through Google Scholar. Findings :Two key policy trends relating to primary health care were identified. Firstly, a population health orientation to improve access to health care through community participation, and secondly, an integrated approach to promote collaboration within the health system, and between the health system and other sectors. The inferred management and leadership skillsets required to realise these policies included relationship management and collaboration, change management, and leadership. Conclusion: New Zealand’s primary health care sector underwent substantial reform between 2000 and 2016. Management and leadership capabilities need to be strengthened and developed for the benefits of the reforms to be realised

    A protocol for developing, disseminating, and implementing a core outcome set for stress urinary incontinence.

    INTRODUCTION: Randomized trials evaluating interventions for stress urinary incontinence (SUI) have been using variable outcome measures, reporting a variety of outcomes. Alongside this variation across studies, outcome-reporting flaws contribute to a limited use of research to inform clinical practice. The development and use of core outcome sets (COSs) in future trials would ensure that outcomes important to different stakeholders and primarily women with SUI are reported more consistently and comprehensively. METHODS: An international steering group including healthcare professionals, researchers, and women with urinary incontinence will guide the development of this COS. Potential outcomes will be identified through comprehensive literature reviews. These outcomes will be entered into an international, multiperspective online Delphi survey. All key stakeholders, including healthcare professionals, researchers, and women with urinary incontinence, will be invited to participate. The modified Delphi method encourages stakeholder group convergence toward collective agreement, also referred as consensus, core outcomes. DISCUSSION: Dissemination and implementation of the resulting COS within an international context will be promoted and reviewed. Embedding the COS for SUI within future clinical trials, systematic reviews and clinical practice guidelines could make a significant contribution to advancing the value of research in informing clinical practice, enhancing patient care and improving outcomes. The infrastructure created by developing a COS for SUI could be leveraged in other settings, for example, selecting research priorities and clinical practice guideline development

    Outcomes and Impact of Training and Development in Health Management and Leadership in Relation to Competence in Role: A Mixed-Methods Systematic Review Protocol

    Background: The need for competence training and development in health management and leadership workforces has been emphasised. However, evidence of the outcomes and impact of such training and development has not been systematically assessed. The aim of this review is to synthesise the available evidence of the outcomes and impact of training and development in relation to the competence of health management and leadership workforces. This is with a view to enhancing the development of evidence-informed programmes to improve competence. Methods and Analysis: A systematic review will be undertaken using a mixed-methods research synthesis to identify, assess and synthesise relevant empirical studies. We will search relevant electronic databases and other sources for eligible studies. The eligibility of studies for inclusion will be assessed independently by two review authors. Similarly, the methodological quality of the included studies will be assessed independently by two review authors using appropriate validated instruments. Data from qualitative studies will be synthesised using thematic analysis. For quantitative studies, appropriate effect size estimate will be calculated for each of the interventions. Where studies are sufficiently similar, their findings will be combined in meta-analyses or meta-syntheses. Findings from quantitative syntheses will be converted into textual descriptions (qualitative themes) using Bayesian method. Textual descriptions and results of the initial qualitative syntheses that are mutually compatible will be combined in mixed-methods syntheses. Discussion: The outcome of data collection and analysis will lead, first, to a descriptive account of training and development programmes used to improve the competence of health management and leadership workforces and the acceptability of such programmes to participants. Secondly, the outcomes and impact of such programmes in relation to participants’ competence as well as individual and organisational performance will be identified. If possible, the relationship between health contexts and the interventions required to improve management and leadership competence will be examined
