    Streptococcus Intermedius Brain and Diverticular Abscesses After Dental Manipulation: A Case Report.

    A brain abscess is defined as a focal intracerebral infection consisting of an encapsulated collection of pus, which can be a life-threatening complication of infections, trauma, or surgery. While immunocompromised patients can have a wide array of causative organisms, bacterial species represent the most common etiology in immunocompetent individuals. The incidence of brain abscesses ranges from 0.4 to 0.9 per 100,000, with a high predisposition among immunocompromised patients and in those with disruption of the blood-brain barrier. The most common causative organisms found were Streptococcus species, particularly S. viridians and S. pneumonia, Enterococcus, and Staphylococcus species, mainly S. aurieus and S. epidermidis. Microorganism can invade the brain through different mechanisms, either directly by contiguous spread and odontogenic infections, which usually cause a single brain abscess, or indirectly through hematogenous spread which can cause multiple brain abscesses. Both surgical and conservative dental procedures contribute to hematogenous spreading of oral microorganisms. Although most of those organisms are eliminated shortly after they gain access to the bloodstream, some can persist and contribute to the pathogenesis of abscesses in the appropriate environment. Odontogenic origins are rarely implicated in the formation of brain abscesses, and oral foci comprise approximately 5% of identified cases. We report a case of brain and diverticular abscesses due to S. intermidius occurring two months after dental extraction. This case highlights the fact that even usual dental workup can result in the development of bacteremia and disseminated abscesses including but not restricted to the brain. Consequently, in addition to identifying the possible source of bacteremia with an extensive history and physical exam, the diagnosis of Streptococcus milleri organisms should prompt the physicians to screen for sites of possible metastatic infection spread

    L-branes

    The superembedding approach to pp-branes is used to study a class of pp-branes which have linear multiplets on the worldvolume. We refer to these branes as L-branes. Although linear multiplets are related to scalar multiplets (with 4 or 8 supersymmetries) by dualising one of the scalars of the latter to a pp-form field strength, in many geometrical situations it is the linear multiplet version which arises naturally. Furthermore, in the case of 8 supersymmetries, the linear multiplet is off-shell in contrast to the scalar multiplet. The dynamics of the L-branes are obtained by using a systematic procedure for constructing the Green-Schwarz action from the superembedding formalism. This action has a Dirac-Born-Infeld type structure for the pp-form.     High Spin Gauge Fields and Two-Time Physics

    All possible interactions of a point particle with background electromagnetic, gravitational and higher-spin fields is considered in the two-time physics worldline formalism in (d,2) dimensions. This system has a counterpart in a recent formulation of two-time physics in non-commutative field theory with local Sp(2) symmetry. In either the worldline or field theory formulation, a general Sp(2) algebraic constraint governs the interactions, and determines equations that the background fields of any spin must obey. The constraints are solved in the classical worldline formalism (h-bar=0 limit) as well as in the field theory formalism (all powers of h-bar). The solution in both cases coincide for a certain 2T to 1T holographic image which describes a relativistic particle interacting with background fields of any spin in (d-1,1) dimensions. Two disconnected branches of solutions exist, which seem to have a correspondence as massless states in string theory, one containing low spins in the zero Regge slope limit, and the other containing high spins in the infinite Regge slope limit.Comment: LaTeX 22 pages.     Subgroup analyses from a phase 3, open-label, randomized study of eribulin mesylate versus capecitabine in pretreated patients with advanced or metastatic breast cancer

    Purpose and methods: Our secondary analyses compared survival with eribulin versus capecitabine in various patient subgroups from a phase 3, open-label, randomized study. Eligible women aged ≥18 years with advanced/metastatic breast cancer and ≤3 prior chemotherapies (≤2 for advanced/metastatic disease), including an anthracycline and taxane, were randomized 1:1 to intravenous eribulin mesylate 1.4 mg/m² on days 1 and 8 or twice-daily oral capecitabine 1250 mg/m² on days 1–14 (21-day cycles). Results: In the intent-to-treat population (eribulin 554 and capecitabine 548), overall survival appeared longer with eribulin than capecitabine in various subgroups, including patients with human epidermal growth factor receptor 2-negative (15.9 versus 13.5 months, respectively), estrogen receptor-negative (14.4 versus 10.5 months, respectively), and triple-negative (14.4 versus 9.4 months, respectively) disease. Progression-free survival was similar between the treatment arms. Conclusions: Patients with advanced/metastatic breast cancer and human epidermal growth factor receptor 2-, estrogen receptor-, or triple-negative disease may gain particular benefit from eribulin as first-, second-, and third-line chemotherapies

    Woody species encroachment is occurring within the semi-arid grasslands of the Nebraska Sandhills U.S., primarily driven by native Juniperus virginiana and Pinus ponderosa, altering ecosystems and the services they provide. Effective, low cost, and cross-scale monitoring of woody species growth and performance is necessary for integrated grassland and forest management in the face of climate variability and change. In this study, we sought to establish a relationship between remote sensing-derived vegetation indices (VIs), tree dendrochronological (raw and standardized tree ring width) measurements, and the abiotic environment [(precipitation, temperature, Palmer Drought Severity Index (PDSI), and soil water content (0–300 cm depth)], over a 30-year period (1984–2013), to assess the performance of encroaching woody J. virginiana and P. ponderosa within the Nebraska Sandhills. We also investigated whether VIs can be used as an effective alternative tool to replace or complement ground measurements. Our results indicate that precipitation, temperature, and PDSI were significant (p \u3c 0.05) predictors of J. virginiana and P. ponderosa growth based on dendrochronological measurements and VIs, while soil water content from 40 to 300 cm depth was a significant predictor of J. virginiana performance. Out of the six VIs that were investigated, four were significant predictors of tree ring growth. R2 values between grassland VIs and growing season climate were greater than those of J. virginiana or P. ponderosa, while grassland performance was decoupled from soil water content. Additionally, climatic conditions in the previous year were significant determinants of current year growth of tree species but did not affect current year grassland performance. This study provides evidence for the efficacy of remote sensing-based VIs in monitoring interannual variation in the growth of woody species, while determining abiotic factors impacting the growth of grassland vegetation, J. virginiana, and P. ponderosa in the Nebraska Sandhills

    M-Theory on (K3 X S^1)/Z_2

    We analyze MM-theory compactified on (K3Ă—S1)/Z2(K3\times S^1)/Z_2 where the Z2Z_2 changes the sign of the three form gauge field, acts on S1S^1 as a parity transformation and on K3 as an involution with eight fixed points preserving SU(2) holonomy. At a generic point in the moduli space the resulting theory has as its low energy limit N=1 supergravity theory in six dimensions with eight vector, nine tensor and twenty hypermultiplets. The gauge symmetry can be enhanced (e.g. to E8E_8) at special points in the moduli space.
