    Feasibility research into the controlled availability of opioids, Volume 2a - Background Papers

    Executive Summary: The results of a three month exploration of legal, ethical, political, medical and logistic issues lead us to the interim conclusion that it would be feasible to undertake a randomised controlled trial as a test of the policy of expanding the availability of heroin in a controlled fashion for the management of heroin dependent users in the ACT. There is evidence that the ACT community is willing to consider such a trial, but also that ACT police have significant concerns about its logistics and possible ill effects. The trial would compare oral methadone treatment with a program of expanded opioid availability, in which dependent individuals would be able to take intravenous, oral or smoked heroin and/or methadone under careful medical supervision. Volunteers would be subject to strict residential eligibility criteria and would need to agree to extensive medical tests and data collections. They would be randomly assigned either to methadone treatment or to the expanded availability program. The two groups would be carefully followed for at least one year in an effort to discover whether or not the expanded availability program provides benefits for dependent drug users, their families and to society at large which methadone programs cannot provide. The purpose of the study would be to discover whether or not a policy of controlled heroin availability could ameliorate the massive burden which illegal heroin use currently imposes on Australian and ACT societies. Our exploration of these matters leads us to recommend to the Select Committee on HIV, Illegal Drugs and Prostitution of the ACT Legislative Assembly that it cautiously proceeds to a second stage exploration of the feasibility of such a study without commitment to the trial until logistic issues are more fully described

    The meeting today grows out of a study conducted jointly by the National Centre for Epidemiology and Population Health and the Australian Institute of Criminology in the early part of this year. That study was prompted by an invitation from the Chairman of the ACT Legislative Assembly’s Select Committee on HIV, Illegal Drugs and Prostitution - Mr Michael Moore - who invited us to examine the feasibility of a trial of the controlled availability of opioids in the ACT. Dr Gabriele Bammer, who directed that investigation, will be setting the scene for us by describing its conclusions at the outset of the day’s discussions. We hope that from that baseline we can move forward in the course of the day to explore the implications of those conclusions and to discuss whether or not it is appropriate to extend the feasibility study to the next stage. So our objective today is to explore the medical, health, social and law enforcement implications of evaluating, in the ACT, new approaches to the treatment of heroin dependent individuals. Drug policy is a highly political issue, any action to change the way we manage drug dependent people in the ACT has political implications for the ACT and for other parts of Australia as well. So I am delighted that we have representatives from drug and law enforcement agencies from most states of Australia here today and that many of the people who will frame attitudes to the proposed ACT trial will have an opportunity to discuss these issues in an open and uninhibited way.The meeting has been assisted by a grant from the ACT Government

    Benzodiazepine use of community-based violent offenders : a preliminary investigation

    Objective: To explore the relationship between benzodiazepine use and violent crime in a sample of community-based offenders. Methods: Participants were recruited via drug diversion and treatment programs in Melbourne, Australia. Data regarding benzodiazepine and other substance use, mental health, personality characteristics, and crime involvement was collected through semi-structured interviews conducted in 2011. Participants (n = 82, 79.3% male) were 21-56 years old, predominantly Australian-born (89%), with 14.6% identifying as Aboriginal or Torres Strait Islanders. Eligibility criteria were having been charged with a criminal offence in the previous six months and at least monthly benzodiazepine use. Group differences between violent (n = 11) and non-violent offenders were assessed via independent samples t-tests (two-tailed) and non-parametric tests. Results: Individuals charged with violent index offences were significantly more likely to use higher average doses of alprazolam (p = .040), and exhibit benzodiazepine dependence (p = .037) as well as report high levels of sensation seeking, prior violence, and the diagnoses of depression and personality disorder, than individuals charged with non-violent index offences. Conclusions: The findings suggest the existence of a complex dynamic between mental health and violent offending that may be influenced by benzodiazepine use, in particular alprazolam. A core implication of these preliminary findings includes attending to the interpersonal skills and adaptive coping resources of violent offenders.     Age-related differences in patterns of criminal activity among a large sample of polydrug injectors in Australia

    Background: The relationship between age and criminal activity among drug-using populations is poorly understood. Methods: Data from 10 years of repeat cross-sectional surveys of sentinel samples of regular people who inject drugs (PWID) across Australia (n=5844) were used to explore the relationship between age and past-month drug dealing, property crime and violent crime, and past-year arrest. Descriptive statistics were used to explore the prevalence and frequency of each outcome. The relationship between age and each outcome was measured using multivariable Poisson regression with robust error variance. Results: After adjusting for confounding factors, each 5-year increase in age was associated with significant reductions in drug dealing (adjusted incidence rate ratio [AIRR]: 0.90, 95% confidence interval [CI]: 0.87–0.94), property crime (AIRR: 0.85, 95% CI: 0.82–0.89) and violent crime (AIRR: 0.77, 95% CI: 0.70–0.85). Older participants were also significantly less likely to report being arrested in the past 12 months (AIRR: 0.91, 95% CI: 0.88–0.93). Conclusions: Younger PWID are more heavily involved in criminal activity compared with their older counterparts. This study highlights the need for early intervention programmes to prevent offending behaviour becoming entrenched, as well as continued efforts to redirect young PWID away from the criminal justice system and into treatment and education programmes

    Rates of self-reported delinquency among Western Australian male and female high school students: the malefemale gender gap

    The Adapted Self-Report Delinquency Scale (ASDS) was administered to 328 adolescents (174 males and 154 females) from eight high schools in Perth, Western Australia. The ages of the sample ranged from 13 to 17 years. Males reported a greater percentage level of involvement than females in 36 of 40 individual delinquent behaviours comprising the ASDS. A between-subjects multivariate analysis of variance using a Bonferroni adjusted alpha revealed a significant multivariate main effect of gender, F(6, 318) = 3.98, p < 0.001, partial η2 = 0.08. No significant main effect of age was evident. Univariate F-tests revealed that males scored significantly higher than females on only one of seven delinquent factors (physical aggression). These data are discussed in light of established evidence showing male predominance in delinquency, recent reports suggesting a male-female gender gap, and theories that have attempted to explain this disparity in offending among males and females

    Goal setting and self-efficacy among delinquent, at-risk and not at-risk adolescents

    Setting clear achievable goals that enhance self-efficacy and reputational status directs the energies of adolescents into socially conforming or non-conforming activities. This present study investigates the characteristics and relationships between goal setting and self-efficacy among a matched sample of 88 delinquent (18 % female), 97 at-risk (20 % female), and 95 not at-risk adolescents (20 % female). Four hypotheses related to this were tested. Findings revealed that delinquent adolescents reported fewest goals, set fewer challenging goals, had a lower commitment to their goals, and reported lower levels of academic and self-regulatory efficacy than those in the at-risk and not at-risk groups. Discriminant function analysis indicated that adolescents who reported high delinquency goals and low educational and interpersonal goals were likely to belong to the delinquent group, while adolescents who reported high educational and interpersonal goals and low delinquency goals were likely to belong to the not at-risk group. The at-risk and not at-risk groups could not be differentiated. A multinomial logistic regression also revealed that adolescents were more likely to belong to the delinquent group if they reported lower self-regulatory efficacy and lower goal commitment. These findings have important implications for the development of prevention and intervention programs, particularly for those on a trajectory to delinquency. Specifically, programs should focus on assisting adolescents to develop clear self-set achievable goals and support them through the process of attaining them, particularly if the trajectory towards delinquency is to be addressed

    Delinquency and reputational orientations of adolescent at-risk and not-at-risk males and females

    This research investigated differences in delinquent activities and the reputational orientations of at-risk and not-at-risk male and female adolescents. Initially, we sought to establish that adolescent males and females differed in these respects. This was found to be the case: males (n = 722) scored significantly higher than females (n = 738) on seven self-reported delinquency variables and on eight reputation enhancement variables pertaining to social deviance, non-conforming reputation, and power/evaluation private identity. When a sample of 31 at-risk females was subsequently pair-wise age matched with 31 not-at-risk females, at-risk females scored significantly higher on all delinquency variables other than school misdemeanors. These at-risk females also scored significantly higher on four reputation enhancement variables relating to social deviance and non-conformity. Given that at-risk females did not differ from their not-at-risk counterparts in level of involvement in school misdemeanors, we sought to determine whether this was also the case for at-risk and not-at-risk males. An age-matched sample of 91 pairs revealed that at-risk males reported significantly higher involvement than not-at-risk males in all aspects of delinquency, including school misdemeanors. They also sought a more non-conforming reputation. To explore the relationships between delinquency and reputation enhancement, a canonical correlation analysis was performed.     The status of women police officers: an international review

    This paper reports on a survey of English-language police department websites, annual reports and other reports in order to identify key aspects of the status of women police internationally. Findings are reported for England and Wales, Scotland, Northern Ireland, Eire, the United States, Canada, Australia (eight departments), New Zealand, South Africa, Ghana, Nigeria, India, Pakistan, Hong Kong, Papua New Guinea, and Fiji. Data on the proportion of female officers were available from 18 of 23 locations, with a range between 5.1% and 28.8%. Recruit numbers were available for six locations, and ranged between 26.6% and 37.0%. Limited data on rank and deployment indicated overall improvements. Available longer-term trend data suggested that growth in female officers was slowing or levelling out. Overall, the study showed an urgent need to improve gender-based statistics in order to better inform strategies aimed at maximising the participation of women in policing

    Patterns in bushfire arson

    Bushfires arson, like structural arson, is a strongly patterned activity. These patterns seem to be mainly determined by the interplay between socioeconomic and environmental dynamics. Since the fundamentals of these two factors change slowly, bushfire arson tends to happen at the same time and in the same place every year. Communities with a high propensity for arson often have a high proportion of children under 15 and lower education levels, employment rates and household income than the national average. Unfortunately, communities with this demographic profile are common on the outskirts of cities where there are opportunities for bushfire arson near at hand and consequently, the bulk of bushfire arson ignitions occur in this context. Most of these fires burn five hectares or less. Nineteen percent of bushfire arsonists convicted in NSW courts between 2001 and 2006 were Indigenous. There are also a high number of bushfire arson incidents in locations such as Kempsey and western NSW towns which suggests that some Indigenous communities may have elevated arson potentials. It is important to note, however, that fire and concepts of arson can have a different cultural context in these communities. Deliberate fire frequencies during daylight hours typically peak between 3 pm and 6 pm and are anecdotally attributed to children, juveniles or young adults. Non-deliberate fires generally peak in the earlier 1 pm to 4 pm period. A higher proportion of deliberate fires occur between 6 pm and 6 am compared with non-deliberate fires. In many jurisdictions, night-time fires are principally a feature of Friday night – Saturday morning and Saturday night – Sunday morning, but the timing of fires is highly location dependent, apparently linked to different social and cultural patterns of human activity within individual areas. Nearly all areas of Australia experience a significant rise in deliberately lit fires on Saturday and Sunday. In terms of patterns throughout the year, most areas have a relatively constant underlying arson incident rate, punctuated by a rapid rise over a two or three week interval then a gradual decline over several months. The occurrence of this pattern at different times of the year varies widely across Australia and depends largely on seasonal vegetation growth and fuel curing rates

    Spectator violence part 1: professional sporting events

    This is the first of a two part series on spectator violence at sporting events. Part one covers spectator violence at professional sporting events. The second will focus on issues of violence at amateur sporting events, particularly parents as spectators and the behaviours they exhibit. Spectator violence refers to any violent activity that occurs among those attending a sporting event. It can be directed at players, officials or fellow spectators. Types of spectator violence include verbal abuse, the throwing of missiles (such as bottles), property destruction and physical assaults (Madensen &amp; Eck 2008). Throughout Europe and the United States, spectator violence is a well-publicised and researched issue (Russell 2008) and it is becoming an increasing concern in Australia for local communities, sports clubs and police (NSW Department of Sport and Recreation 2006). The factors contributing to spectator violence can vary greatly, but often stem from poorly designed physical spaces, high energy events and inexperienced staff (Madensen &amp; Eck 2008). A number of key measures have been developed that can be used to reduce the potential for violence at an event. These include: promoting events as \u27family friendly\u27—events solely promoted at, and attracting, large numbers of young males are prone to be a setting for spectator violence (Russell 2008). Such \u27high energy\u27 events often use aggressive \u27war and enemy\u27 imagery to promote a great battle between teams. To minimise the risk of violence marketing events as \u27family friendly\u27 increases the likelihood of attracting a calmer, more diverse crowd less likely to engage in violent or threatening behaviour (Madensen &amp; Eck 2008); setting a good example—professional sports people should be mindful of their conduct as role models for young fans. Shaking hands at the conclusion of a match and being conscious of their responses to contentious refereeing decisions are gestures of sportsmanship that are important in encouraging positive conduct in younger spectators (Russell 2008); providing adequate seating—ensuring spectators have access to sufficient seating can minimise the risk of crowding and consequent scuffles between spectators (Russell 2008); reducing alcohol availability—events where full-strength alcohol is easily accessible and inexpensive to purchase are more likely to experience incidents of spectator violence. Providing adequate signage of policies on liquor consumption and codes of conduct, serving low-strength alcohol and ensuring policies are enforced can reduce this risk (Madensen &amp; Eck 2008); behaviour and competency of officials and players—police and security staff need to ensure the safety of patrons without giving the impression of over-policing, which can frustrate some spectators and lead to acts of aggression (Russell 2008); and banning unruly spectators—professional clubs have taken measures to ban spectators from future matches and from sporting grounds due to violent behaviours (ABC News 2009). This approach reinforces to the community that violent behaviour is unacceptable and will not be tolerated. Developing codes of conduct and event management strategies in partnership with police, sporting bodies, club management and government will help minimise the risk of spectator violence and ensure event staff are well prepared to effectively deal with any isolated incidents (Madensen &amp; Eck 2008)
