Status of the light ion source developments at CEA/Saclay
ACC NIMInternational audienceSILHI (High Intensity Light Ion Source) is an ECR ion source producing high intensity proton ordeuteron beams at 95 keV. It is now installed in the IPHI site building, on the CEA/Saclay center. IPHI is a frontend demonstrator of high power accelerator. The source regularly delivers more than 130 mA protons in CWmode and already produced more than 170 mA deuterons in pulsed mode at nominal energy. The last beamcharacterisations, including emittance measurements, space charge compensation analysis and diagnosticimprovements, will be reported. Taking into account the SILHI experience, new developments are in progress tobuild and test a 5 mA deuteron source working in CW mode. This new source will also operate at 2.45 GHz andpermanent magnets will provide the magnetic configuration. This source, of which the design will be discussed,will have to fit in with the SPIRAL 2 accelerator developed at GANIL to produce Radioactive Ion Beams. Clinical and biochemical prediction of early fatal outcome following hip fracture in the elderly
Hip fracture, a moderate musculoskeletal trauma, is associated with a high postoperative mortality. Most patients are elderly, with comorbid conditions and often with heart disease. The objective of this study was to find out if clinical parameters and analyses of specific muscle enzymes could predict three month postoperative mortality. A total of 302 patients above 75Â years of age with hip fracture were consecutively enrolled. Baseline information on age, sex and comorbidity assessed with the American Society of Anesthesiologists (ASA) score was obtained before surgery. Creatine kinase (CK), myocardium-specific creatine kinase (CK-MB) and troponin T (TnT) were analysed from venous blood, collected the day before surgery (â1) and postoperatively, within 24Â hours (0) and on days one (+1) and four (+4). The overall three month mortality was 19.5%. Multivariate analyses showed that age, male sex and comorbidity (ASA) correlated with mortality (pâ=â0.027, pâ=â0.002, pâ<â0.001, respectively). Surgery induced a two- to threefold increase of CK and CK-MB but without any correlation with mortality. However, high TnT levels >0.04Â ÎŒg/l correlated significantly with death (days â1, +1 and +4, pâ=â0.003, pâ=â0.005 and pâ=â0.003, respectively). Multivariate analyses, adjusted for age, sex and ASA category, confirmed this correlation (day +4, pâ=â0.008). Thus, in elderly patients with comorbidities undergoing hip fracture surgery information on sex, age, ASA category and postoperative laboratory analyses on TnT provide the clinicians with useful information on patients at risk of fatal outcome
Evaluating exposure of pedestrians to airborne contaminants associated with non-potable water use for pavement cleaning
International audienceClimate change and increasing demography press local authorities to look after affordable water resources and replacement of drinking water for city necessities like street and pavement cleaning by more available raw water. Though, the substitution of drinking by non-drinking resources demands the evaluation of sanitary hazards. This article aims therefore to evaluate the contribution of cleaning water to the overall exposure of city dwellers in case of wet pavement cleaning using crossed physical, chemical and biological approaches. The result of tracer experiments with fluorescein show that liquid water content of the cleaning aerosol produced is about 0.24 g m(-3), rending possible a fast estimation of exposure levels. In situ analysis of the aerosol particles indicates a significant increase in particle number concentration and particle diameter, though without change in particle composition. The conventional bacterial analysis using total coliforms as tracer suggests that an important part of the contamination is issued from the pavement. The qPCR results show a more than 20-fold increase of background genome concentration for Escherichia coli and 10-fold increase for Enterococcus but a negligible contribution of the cleaning water. The fluorescence analysis of the cleaning aerosol confirms the above findings identifying pavement surface as the major contributor to aerosol organic load. The physical, chemical and microbiological approaches used make it possible to describe accurately the cleaning bioaerosol and to identify the existence of significantly higher levels of all parameters studied during the wet pavement cleaning. Though, the low level of contamination and the very short time of passage of pedestrian in the zone do not suggest a significant risk for the city dwellers. As the cleaning workers remain much longer in the impacted area, more attention should be paid to their chronic exposure
