    Narrowing the Gap? Describing women's outcomes in Midwifery Group Practice in remote Australia

    © 2016 Australian College of Midwives Background In Australia, Aboriginal women and babies experience higher maternal and perinatal morbidity and mortality rates than their non-Aboriginal counterparts. Whilst midwifery led continuity of care has been shown to be safe for women and their babies, with benefits including reducing the preterm birth rate, access to this model of care in remote areas remains limited. A Midwifery Group Practice was established in 2009 in a remote city of the Northern Territory, Australia, with the aim of improving outcomes and access to midwifery continuity of care. Aim The aim of this paper is to describe the maternal and newborn outcomes for women accessing midwifery continuity of care in a remote context in Australia. Methods A retrospective descriptive design using data from two existing electronic databases was undertaken and analysed descriptively. Findings In total, 763 women (40% of whom were Aboriginal) gave birth to 769 babies over a four year period. There were no maternal deaths and the rate of perinatal mortality was lower than that across the Northern Territory. Lower rates of preterm birth (6%) and low birth weight babies (5%) were found in comparison to population based data. Conclusion Continuity of Midwifery Care can be effectively provided to remote dwelling Aboriginal women and appears to improve outcomes for women and their infants

    Contribution of the after-school period to children’s daily participation in physical activity and sedentary behaviours

    Objectives: Children’s after-school physical activity (PA) and sedentary behaviours (SB) are not well understood, despite the potential this period holds for intervention. This study aimed to describe children’s after-school physical activity and sedentary behaviours; establish the contribution this makes to daily participation and to achieving physical activity and sedentary behaviours guidelines; and to determine the association between after-school moderate- to vigorous-intensity physical activity (MVPA), screen-based sedentary behaviours and achieving the physical activity and sedentary behaviour guidelines.Methods: Children (n=406, mean age 8.1 years, 58% girls) wore an ActiGraph GT3X accelerometer. The percentage of time and minutes spent sedentary (SED), in light- physical activity (LPA) and MVPA between the end-of-school and 6pm (weekdays) was calculated. Parents (n=318, 40 years, 89% female) proxy-reported their child’s after-school participation in screen-based sedentary behaviours. The contribution that after-school SED, LPA, MVPA, and screen-based sedentary behaviours made to daily levels, and that after-school MVPA and screen-based sedentary behaviours made to achieving the physical activity/sedentary behaviour guidelines was calculated. Regression analysis determined the association between after-school MVPA and screen-based sedentary behaviours and achieving the physical activity/sedentary behaviours guidelines.Results: Children spent 54% of the after-school period SED and this accounted for 21% of children’s daily SED levels. Boys spent a greater percentage of time in MVPA than girls (14.9% vs. 13.6%; p<0.05) but this made a smaller contribution to their daily levels (27.6% vs 29.8%; p<0.05). After school, boys and girls respectively performed 18.8 minutes and 16.7 minutes of MVPA which is 31.4% and 27.8% of the MVPA (p<0.05) required to achieve the physical activity guidelines. Children spent 96 minutes in screen-based sedentary behaviours, contributing to 84% of their daily screen-based sedentary behaviours and 80% of the sedentary behaviour guidelines. After-school MVPA was positively associated with achieving the physical activity guidelines (OR: 1.31, 95%CI 1.18, 1.44, p<0.05) and after-school screen-based sedentary behaviours was negatively associated with achieving the sedentary behaviours guidelines (OR: 0.97, 95%CI: 0.96, 0.97, p<0.05).Conclusions: The after-school period plays a critical role in the accumulation of children’s physical activity and sedentary behaviours. Small changes to after school behaviours can have large impacts on children’s daily behaviours levels and likelihood of meeting the recommended levels of physical activity and sedentary behaviour. Therefore interventions should target reducing after-school sedentary behaviours and increasing physical activity

    Fluctuating asymmetry, parasitism and reproductive fitness in two species of gammarid crustacean

    Fluctuating asymmetry (FA), defined as random deviations from perfect bilateral symmetry, is assumed to reflect developmental instability. FA is predicted to increase in response to environmental stress, including parasite infection. In addition, based on theory we predict a higher FA in sexually selected traits, due to their greater sensitivity to stress. We investigated the relationships between FA, parasitism and reproductive fitness in 2 species of gammarid crustacean, incorporating both sexual and non-sexual traits. We tested the hypothesis that gammarids infected by vertically transmitted Microsporidia will display higher levels of FA than those infected by horizontally transmitted trematodes, because vertically transmitted Microsporidia can be present at the earliest stages of host development. We found little evidence for a relationship between FA and fecundity in Gammarus spp.; however, egg diameter for infected female Gammarus duebeni was significantly smaller than uninfected female G. duebeni. FA was not correlated with brood size in females or with sperm number in males. In contrast to our prediction, we report a lower relative FA in response to sexual traits than non-sexual traits. However, FA in sexual traits was found to be higher in males than females, supporting the theory that sexual selection leads to increased FA. Additionally, we report a negative correlation between FA and both trematode (Podocotyle atomon) and PCR-positive microsporidian (Nosema granulosis and Dictyocoela duebenum) infections and interpret these results in the context of the parasites’ transmission strategies. FA in G. duebeni and G. zaddachi appears to associate with trematode and microsporidian presence, although reproductive fitness is less altered by infection

However, this period makes a large contribution to daily physical activity levels and becomes more important as children transition into secondary school.     “Five hours to sort out your life”: A qualitative study of university students experience of mental health support

    Background: Previous qualitative research suggests that university students feel that current service provision does not meet their needs. Exploring the reasons for this may help to promote service change, encourage the uptake of care, improve outcomes and increase satisfaction within university services. Aims: This study aimed to improve the understanding of how students experience the process of accessing and using mental health support, barriers and facilitators to treatment, and how students would adapt provision to improve experiences. Method: Semi-structured interviews were conducted with 16 full-time students who had used mental health services at university. Data were analysed using thematic analysis. Results: Five higher-order themes were identified: personalisation and informed choice, simplifying the process, feeling abandoned ignored or invisible, stigma, and superiority of private and external services. Sixteen subthemes were identified within these themes. Conclusions: Findings indicate that access to mental health support should be simplified, with collaboration across university and external health and care services, to prevent students feeling lost or abandoned when seeking care. An inclusive approach to support access and provision of services for all presentations of mental health problems should be developed

    Locovents is the name of our android app-based project that will display to our users an interactive list detailing the local events going on in their area. Our goal for this project was to create a custom web scraper to display the any cool events that might be occurring in their respective area. This project was build using java and python along with android studios. With in the past 3 months we have worked rigorously to create a fully functional android app that scrapes the web and appropriately provides the user with relative information. This way our users will not have to look up any concerts or events that might be happening, all they would have to do is plug in their relative location

    The effectiveness of adapted psychological interventions for people from ethnic minority groups: A systematic review and conceptual typology.

    This review assessed the efficacy of adapted psychological interventions for Black and minority ethnic (BME) groups. A conceptual typology was developed based on adaptations reported in the literature, drawing on the common factors model, competence frameworks and distinctions between types of cultural adaptations. These distinctions were used to explore the efficacy of different adaptations in improving symptoms of a range of mental health problems for minority groups. Bibliographic searches of MEDLINE, Embase, PsycINFO, HMIC, ASSIA, CENTRAL, CDSR and CINAHL spanned the period from 1965 to December 2020. Adaptations to interventions were categorised: i) treatment specific: therapist-related, ii) treatment-specific: content-related and iii) organisation-specific. Meta-analyses of RCTs found a significant effect on symptom reduction when adapted interventions were compared to non-adapted active treatments (K = 30, Hedge's g = -0.43 [95% CI: -0.61, -0.25], p < .001). Studies often incorporated multiple adaptations, limiting the exploration of the comparative effectiveness of different adaptation types, although inclusion of organisation-specific adaptations may be associated with greater benefits. Future research, practitioner training and treatment and service development pertaining to adapted care for minority groups may benefit from adopting the conceptual typology described
