    Study on the vibration characteristics of the telescope T80 in the Javalambre astrophysical observatory (JAO) aimed at detecting invalid images

    The location of large telescopes, generally far from the data processing centers, represents a logistical problem for the supervision of the capture of images. In this work, we carried out a preliminary study of the vibration signature of the T80 telescope at the Javalambre Astrophysical Observatory (JAO). The study analyzed the process of calculating the displacement that occurs because of the vibration in each of the frequencies in the range of interest. We analyzed the problems associated with very low frequencies by means of simulation, finding the most critical vibrations below 20 Hz, since they are the ones that generate greater displacements. The work also relates previous studies based on simulation with the real measurements of the vibration of the telescope taken remotely when it is subjected to different positioning movements (right ascension and/or declination) or when it performs movement actions such as those related to filter trays or mirror cover. The obtained results allow us to design a remote alarm system to detect invalid images (taken with excess vibration)

    Modified filtered-x hierarchical lms algorithm with sequential partial updates for active noise control

    In the field of active noise control (ANC), a popular method is the modified filtered-x LMS algorithm. However, it has two drawbacks: Its computational complexity higher than that of the conventional FxLMS, and its convergence rate that could still be improved. Therefore, we propose an adaptive strategy which aims at speeding up the convergence rate of an ANC system dealing with periodic disturbances. This algorithm consists in combining the organization of the filter weights in a hierarchy of subfilters of shorter length and their sequential partial updates (PU). Our contribution is threefold: (1) we provide the theoretical basis of the existence of a frequency-depend-ent parameter, called gain in step-size. (2) The theoretical upper bound of the step-size is compared with the limit obtained from simulations. (3) Additional experiments show that this strategy results in a fast algorithm with a computational complexity close to that of the conventional FxLMS

    El tratamiento de las heridas complejas articulares se muestra siempre complicado, el abordaje razonable conjunto por traumatólogos y cirujanos plásticos mejora el resultado clínico, funcional y estético. Presentamos el caso de un paciente politraumatizado con pérdida de sustancia a nivel de la rodilla secundaria a artritis séptica, necrosis de patela y exposición de cóndilos femorales que fue tratado mediante colgajo muscular de vastus lateralis de pedículo distal. El vastus lateralis permite la cobertura eficiente de l articulación expuesta sin afectar la extensión de la rodilla y dejando mínima secuela estética.The treatment of the joint complex wounds is difficult. A good clinic, functional and aesthetic outcome is possible when multidisciplinary approach by orthopaedics and plastic surgeons is carried out. We report a case of a patient with multiple fractures who presents a severe loss of soft tissue at the knee. This lesion was due to a septic arthritis with patellar necrosis and condillar exposition. The treatment was made by a distally pedicled vastus lateralis muscular flap.     La osteomielitis crónica (OC) en extremidades inferiores afecta al hueso y tejidos blandos circundantes, produciéndose úlceras crónicas que exponen el hueso necrosado. El tratamiento de la OC combina: antibióticos, desbridamiento-estabilización ósea, y una adecuada cobertura cutánea. La reconstrucción de las pérdidas de sustancia localizadas en el tercio medio de la pierna puede realizarse con colgajos fasciocutáneos, colgajos libres, colgajos en piernas cruzadas y colgajos locales musculares. La edad avanzada del paciente es un factor importante a considerar para la indicación de la técnica reparadora. El colgajo sóleo tiene múltiples ventajas: rica vascularización, corto tiempo quirúrgico, postoperatorio cómodo para el paciente y mínima limitación funcional. Presentamos 2 casos de pacientes ancianos con defectos osteocutáneos tibiales secundarios a OC, en ambos casos la cobertura del defecto se realizó con el colgajo muscular sóleo, la evolución clínica fue favorable y la recuperación funcional total.Cronic osteomyelitis (CO) in lower legs involves tibial-fibular bones and soft tissues around, with cronic ulcers by necrosed bone exposure. CO treatment combines: antibiotics, removes necrotic bone, bony stabilization and an adequate soft tissue coverage. Soft tissue loss reconstruction in the medium third of the legs can be made with fasciocutaneous flaps, free flaps, cross leg flaps and local muscle flaps. Elderly age is an important condition for the reconstruction procedure indication. Soleus muscle flap has multiple advantages: great blood supply, short surgical time, comfortable postoperative and minimum functional sequels. Two cases: Both elderly patients with osteocutaneous defects in tibia, by CO. Treatment coverage was performed with soleus muscle flap. Clinic course increased positively and the patients got completely functional restitution

    Las pérdidas de sustancia traumática localizadas en la muñeca afectan frecuentemente a los tendones, vasos y nervios. Estas lesiones requieren un tratamiento reparador de las estructuras lesionadas y una cobertura adecuada que permita el deslizamiento de los tendones, y proporcione protección neurovascular segura. El colgajo fasciocutáneo basado en perforantes de la arteria cubital aporta piel, tejido subcutáneo y fascia sobre las estructuras expuestas en la muñeca tanto en la cara palmar como dorsal. Presentamos cuatro casos de traumatismos de partes blandas en la muñeca, en ellos tras la reparación neurotendinosa requerida, se optó por la cobertura final mediante el colgajo fasciocutáneo cubito-dorsal. Destacamos la versatilidad de estos colgajos, su fácil ejecución, mínima secuela estética y la posibilidad de iniciar precozmente el tratamiento rehabilitador, consiguiendo así una mejor recuperación funcional del paciente.Traumatic soft tissue loss in wrist usually involves tendons, vessels and nerves. These injuries need the adequate reconstructive treatment and the safe coverage which lets to slide the tendons and it gives a safe protection to neurovascular structures. The fasciocutaneous flap based in perforator branches of the ulnar artery brings skin, fat and fascia enough over the exposed tissues in the dorsal or volar surface of the wrist. We report four cases of traumatic lesions in soft tissues of the wrist, in all of them the fasciocutaneous dorsal cubital flap was the option for the end coverage. We emphasize the versatility of these flaps, its easy ejecution, the minimal aesthetic sequel and the early rehabilitation beginning, so we obtain the best functional restitution for the patien

    Las úlceras por presión (UPP) suponen una complicación importante que puede afectar a pacientes cuya movilidad está limitada. El tratamiento quirúrgico de las fracturas en pacientes ancianos y el encamamiento prolongado pueden ser predisponentes de lesiones por decúbito. En este trabajo de revisión buscamos las medidas de prevención de UPP basadas en la evidencia médica. La Medicina Basada en la Evidencia (MBE) propone utilizar la mejor evidencia científica y clínica sobre las UPP, según la cual, la prevención es el objetivo fundamental en estos pacientes. Se destaca en la literatura relativa a las úlceras por presión que las medidas de prevención con evidencia médica probada incluyen: valoración integral del paciente, identificación del riesgo, reconocer el estadio de la UPP, aminorar el efecto de la presión, evaluar el estado nutricional, eliminar el exceso de humedad (incontinencia), evitar encamamientos prolongados y preservar la integridad de la piel.Pressure sores are an important complication, which can affects to patients with restricted motion. Surgical treatment of elderly people with fractures in addition prolonged time lying in same position are able to develop bedsores. This review paper searches the prevention features of pressure sores in evidence based medicine. Evidence Based Medicine proposes the best clinical and scientifical evidence available about to pressure sores, being prevention the main goal in these patients. The results founded in the literature relative to pressure sores affirms these are the facts in evidence based medicine: the general approach of the patient, recognising risk, staging pressure sores, reducing pressure effect, assessing nutritional status, avoiding excess moisture (incontinence) reducing bed rest and preserving the integrity of the skin

    INTEGRAL observations of the Be/X-ray binary EXO 2030+375 during outburst

    We present a type-I outburst of the high-mass X-ray binary EXO 2030+375, detected during INTEGRAL's Performance and Verification Phase in December 2002 (on-source time about 10e+06 seconds). In addition, six more outbursts have been observed during INTEGRAL's Galactic Plane Scans. X-ray pulsations have been detected with a pulse period of 41.691798+-0.000016 s. The X-ray luminosity in the 5-300 keV energy range was 9.7*10e+36 erg/s, for a distance of 7.1 kpc. Two unusual features were found in the light curve, with an initial peak before the main outburst and another possible spike after the maximum. RXTE observations confirm only the existence of the initial spike. Although the initial peak appears to be a recurrent feature, the physical mechanisms producing it and the possible second spike are unknown. Moreover, a four-day delay between periastron passage and the peak of the outburst is observed. We present for the first time a 5-300 keV broad-band spectrum of this source. It can be modelled by the sum of a disk black body (kT_bb~8 keV) with either a power law model with Gamma=2.04+-0.11 keV or a Comptonized component (spherical geometry, kT_e=30 keV, tau=2.64, kT_W=1.5 keV).Comment: 10 pages, 11 figures, to be published in A&

    The IFMIF-EVEDA accelerator beam dump design

    The IFMIF-EVEDA accelerator will be a 9 MeV, 125 mA cw deuteron accelerator prototype for verifying the validity of the 40 MeV accelerator design for IFMIF. A beam dump designed for maximum power of 1.12 MW will be used to stop the beam at the accelerator exit. The conceptual design for the IFMIF-EVEDA accelerator beam dump is based on a conical beam stop made of OFE copper. The cooling system uses an axial high velocity flow of water pressurized up to 3.4 × 105 Pa to avoid boiling. The design has been shown to be compliant with ASME mechanical design rules under nominal full power conditions. A sensitivity analysis has been performed to take into account the possible margin on the beam properties at the beam dump entrance. This analysis together with the study of the maintenance issues and the mounting and dismounting operations has led to the complete design definition

    Clinical features and health-related quality of life in adult patients with mucopolysaccharidosis IVA: the Spanish experience

    Background: Mucopolysaccharidosis (MPS) IVA or Morquio A syndrome is a progressive and disabling disease characterized by a deficiency of the enzyme N-acetylgalactosamine-6-sulphate sulphatase. Its clinical presentation is very heterogeneous and poorly understood in adults. The aim of this study was to describe the clinical manifestations of MPS IVA in adult patients in Spain and to assess their health-related quality of life (HRQoL). Results: Thirty-three patients from nine reference centres participated in the study. The median age was 32 (interquartile range [IQR]: 20.5–40.5) years. The phenotype was classical in 54.5% of patients, intermediate in 33.3% of patients, and non-classical in 12.1% of patients. The most common clinical manifestation was bone dysplasia, with a median height of 118 (IQR: 106–136) cm. Other frequent clinical manifestations were hearing loss (75.7%), ligamentous laxity (72.7%), odontoid dysplasia (69.7%), limb deformities that required orthopaedic aids (mainly hip dysplasia and genu valgus) (63.6%), and corneal clouding (60.6%). In addition, 36.0% of patients had obstructive sleep apnoea/hypopnoea syndrome and 33.3% needed non-invasive ventilation. Cervical surgery and varisation osteotomy were the most common surgical interventions (36.4% each). Almost 80% of patients had mobility problems and 36.4% used a wheelchair at all times. Furthermore, 87.9% needed help with self-care, 33.3% were fully dependent, and 78.8% had some degree of pain. HRQoL according to the health assessment questionnaire was 1.43 (IQR: 1.03–2.00) in patients with the non-classical phenotype, but 2.5 (IQR: 1.68–3.00) in those with the classical phenotype. Seven patients were initiated on enzyme replacement therapy (ERT), but two of them were lost to follow-up. Lung function improved in four patients and slightly worsened in one patient. The distance achieved in the six-minute walk test increased in the four patients who could perform it. HRQoL was better in patients treated with elosulfase alfa, with a median (IQR) of 1.75 (1.25–2.34) versus 2.25 (1.62–3.00) in patients not treated with ERT. Conclusions: The study provides real-world data on patients with MPS IVA. Limited mobility, difficulties with self-care, dependence, and pain were common, together with poor HRQoL. The severity and heterogeneity of clinical manifestations require the combined efforts of multidisciplinary teams
