    Comparación de dos métodos para identificar los factores asociados al inicio del consumo de cannabis en un estudio de cohortes

    ResumenObjetivoComparar la utilidad de 2 métodos analíticos multivariados, el análisis como casos y controles (CC) y como casos y controles anidados (CCa) en una cohorte, para identificar los factores asociados al inicio del consumo de cannabis.MétodosEstudio longitudinal con una muestra de 1.056 alumnos de primer curso de Educación Secundaria Obligatoria (ESO), seguidos anualmente hasta cuarto de ESO. En el análisis como CC se consideraron casos los que declararon, en el cuarto año, haber consumido cannabis y controles los que no habían consumido nunca, estimando modelos de regresión logística (RL). En el análisis como CCa, se compararon los casos de cada año de seguimiento con una muestra aleatoria de controles de riesgo en ese mismo año, estimando modelos de RL condicional.ResultadosEn el análisis como CC, se identificaron 6 variables en los chicos y 9 en las chicas en los modelos bivariados, y 3 en los chicos y 4 en las chicas en los multivariados. En el análisis como CCa se obtuvieron 17 variables en los modelos bivariados y 4 en los multivariados, tanto en los chicos como en las chicas. Los estimadores del análisis como CC tenían 1,2 veces más variabilidad.ConclusionesEl análisis como CCa permitió identificar un mayor número de factores asociados al consumo de cannabis y los estimadores fueron más precisos. El CCa puede ser una alternativa más eficiente frente al análisis como CC.AbstractObjectiveTo compare the utility of two multivariate analytic methods, case-control (CC) analysis and nested case-control (NCC) analysis in a cohort, to identify the factors associated with the onset of cannabis use.MethodsA longitudinal cohort study of a sample of secondary school students (n = 1,056) in the first year of secondary school was carried out. Participating students were followed-up annually until the fourth year of secondary school. In the CC analysis, students in the fourth year who reported having consumed cannabis at some time were considered cases and those who had never consumed cannabis were considered controls. Logistic regression (LR) models were estimated. In the NCC analysis, cases in each year of follow-up were compared with a random sample of controls at risk in the same year and conditional LR models were estimated. IResultsIn the CC analysis, 6 variables in boys and 9 variables in girls in bivariate models and 3 variables in boys and 4 variables in girls in multivariate models were identified. In the NCC analysis, 17 variables in univariate models and 4 in multivariate models were obtained in both boys and girls. The estimators of the CC analysis showed an average of 1.2-fold more variability.ConclusionsA higher number of factors associated with cannabis use were identified in the NCC analysis and the estimators were more precise. NCC could be a more efficient option than CC analysis

    Evaluación de la efectividad en salud pública: Fundamentos conceptuales y metodológicos

    ResumenEn los últimos años ha aumentado de forma notable el interés por la evaluación de las intervenciones en salud, especialmente en relación a su utilidad social y su eficiencia económica. Sin embargo, todavía estamos lejos de tener un grado suficiente de consenso en los aspectos básicos de la evaluación, como son la terminología, la finalidad y la metodología de trabajo. En este marco se revisan las principales definiciones y clasificaciones de la evaluación aplicada a los programas y políticas en salud pública. En relación a la evaluación de resultados, se presentan los principales diseños evaluativos y sus componentes, y se revisan las amenazas a la validez interna de los resultados de los diseños evaluativos débiles. Se analizan y discuten las características de las intervenciones de salud pública que limitan las opciones de evaluación con diseños tradicionales. Entre estas limitaciones destacan la complejidad de las intervenciones, que habitualmente tienen múltiples componentes, y la dificultad de establecer un grupo de comparación sin intervención, en especial mediante asignación aleatoria. Para finalizar, se describe una propuesta de evaluación a partir de diseños evaluativos débiles, consistente en la valoración de la adecuación y la plausibilidad. La adecuación estaría determinada por la existencia de un cambio observable en los indicadores de resultados, y podría ser suficiente para tomar decisiones bajo determinadas condiciones; otras veces sería necesario analizar la plausibilidad, o atribución de los resultados observados al programa.AbstractIn the last few years, interest has markedly increased in evaluating health programs, especially their social utility and economic efficiency. However, consensus on key issues in evaluation, such as terminology, goals and methods is still a long way off. In this context, we review the main definitions and classifications of evaluation applied to public health programs and policies. We describe the main evaluation designs and their components, focusing on outcome evaluation. Threats to the internal validity of the results of weak evaluation designs are also discussed. The characteristics of public health interventions that limit evaluation with traditional designs are also analyzed. These limitations include the complexity of interventions, usually with multiple components, and the difficulty of forming an equivalent control group with no intervention, especially through random assignment. Finally, a two-step approach to evaluation through weak designs, which takes into account adequacy and plausibility, is described. Adequacy consists of the observation of a change in the selected indicators after the intervention, and would be sufficient to take decisions under certain conditions; at other times, plausibility would need to be analyzed, defined as attribution of the results to the program or intervention

    Avaliação do cumprimento da lei portuguesa de prevenção do tabagismo no sector da restauração

    O objectivo deste estudo é avaliar o cumprimento da nova legislação no sector da restauração e similares. Realizou-se na cidade de Braga – Portugal, em 2008. Foram objecto de observação: restaurantes com mais de 100 m2 (14); restaurantes com menos de 100 m2 (16); cafés, pastelarias e similares (29); e bares, pubs e discotecas (10). 76,8% dos locais da restauração e similares optaram por se declarar espaços sem fumo. A maior taxa de adesão encontra-se nos restaurantes com mais de cem metros (85,7%) e a menor nas discotecas, bares e pubs (70,5%).The objective of this study is to evaluate the fulfilment of the new legislation of tobacco control at the sector of the hospitality. The study was carried out in the city of Braga – Portugal, in 2008. Which evaluated: 14 restaurants with more than 100 m2; 16 restaurants with less than 100 m2; 29 coffees, cake shops and similar; and 10 bars, pubs and discotheques. 76,8% of the places observed had been declared smoke free environments. The largest percentage of adhesion meets in the restaurants with more than 100 m2 (85,7%) and the least in the discotheques, bars and pubs (70,5%)

    ResumenObjetivoAnalizar la información sobre consumo de tabaco en adolescentes a partir de diversas encuestas escolares.DiseñoSe extraen de diversos estudios los datos relativos a prevalencia de fumadores diarios al final de la escuela secundaria obligatoria por sexo, analizando tendencias.EmplazamientoSe revisan los 5 estudios representativos de adolescentes en España: Encuesta Estatal sobre Uso de Drogas en Estudiantes de Secundaria (ESTUDES); Estudio de Comportamientos de los Escolares Relacionados con la Salud (ECERS-HBSC); Sistema de Vigilancia de Factores de Riesgo asociados a Enfermedades No Transmisibles dirigido a población Juvenil (SIVFRENT-J); estudio de Factores de Riesgo en Estudiantes de Secundaria (FRESC); Estudio de Monitorización de las Conductas de Salud de los Adolescentes (EMCSAT).ResultadosLa prevalencia de fumadores diarios varía entre estudios, en varones entre 8,5 y 13,3% y en chicas entre 12,7 y 16,4%. Aunque se aprecian oscilaciones en algunos estudios, la tendencia entre 1993 y 2008 es de descenso. Con los datos más recientes se puede estimar un declive anual ponderado de la prevalencia de tabaquismo en la adolescencia del 6,47% anual para los varones y 6,96% para las chicas.ConclusionesHay un patrón de descenso de la prevalencia de fumadores diarios adolescentes en España a partir de los diversos estudios existentes, que ofrecen datos consistentes, aunque hay que mantener la vigilancia debido a la existencia de oscilaciones. Esto concuerda con la información derivada de las ventas y de encuestas en población adulta. En cualquier caso, sería deseable que el ritmo de cambio fuera más acentuado y constante.AbstractObjectiveTo analyse information on adolescent use of tobacco in Spain from different school surveys.DesignData on daily smoking prevalence by sex at the end of compulsory education is extracted and figures are compared, analysing trends.SettingThe five representative studies on adolescents in Spain are reviewed: The National Survey on Drug Use in Secondary School Children (Encuesta estatal sobre uso de drogas en estudiantes de secundaria (ESTUDES); Survey of Health Behaviour in School-aged Children (HBSC-ECERS); Surveillance System of Risk Factors Associated With Non-Transmittable diseases in the Young Population (Sistema de Vigilancia de Factores de Riesgo Asociados a Enfermedades No Transmisibles dirigido a población Juvenil)(SIVFRENT-J); Study of Risk Factors in Secondary School Children (Estudio de factores de riesgo en estudiantes de secundaria) (FRESC); Surveillance Study of Health Behaviour in Adolescents (Estudio de Monitorización de las Conductas de Salud de los Adolescentes) (EMCSAT).ResultsThe prevalence of daily smokers varies among studies, in boys from 8.5 to 13.3% and in girls from 12.7 to 16.4%. Although some series show variations, the trend from 1993 to 2008 is downwards. With data from recent years, weighted annual declines in smoking prevalence in adolescence can be estimated to be 6.47% for boys and 6.96% for girls.ConclusionsThere is a decreasing pattern in adolescent daily smoking prevalence in Spain from the different existing studies, which provide consistent data, although surveillance must be kept due to fluctuations. This is in agreement with tobacco sales statistics and health surveys in the adult population. However, the pace of change should be more rapid and constant

    Three-year follow-up of the poiba intervention on childhood obesity : A quasi-experimental study

This quasi-exper-imental study aimed to assess the effectiveness of a multicomponent and multilevel school-based intervention (POIBA) at 3 years of follow-up. The nutrition intervention focused on food groups, food pyramid, nutrients, portions, and balanced menus. In total, 3624 children participated in the study. Anthropometric measurements and information on food frequency and behavior, physical activity, and daily screen use were collected in the intervention (IG) and comparison group (CG). Positive unadjusted changes toward adherence to recommendations were found for water, meat, sweets, and fried potato consumption, proper breakfast, not having dinner in front of the TV, out-of-school physical activity, and daily screen use. Three scores were used to calculate the proportion of children making more than one change to improve healthy habits regarding physical activity (global Activity score), nutrition (global Nutrition score), and both (global score). Students exposed to the intervention had a significantly better global Activity score (16.2% IG vs. 11.9% CG; p = 0.012) and Global score (63.9% IG vs. 58.5% CG; p = 0.025). Intervention effects on obesity incidence at 3-year follow-up lost significance but maintained the positive trend. In conclusion, school-based interventions including a family component could be useful to address the childhood obesity problem

Study population: third grade primary school students in the city of Barcelona in year 2011 (n = 3,624). Body mass index was studied as the dependent variable, and sex, age, social class, parental status, origin, ownership school, neighborhood socioeconomic status of the school and district were studied as independent. Logistic regression models were analyzed, obtaining odds ratio (OR) with 95% confidence intervals (95% CI). The prevalence of overweight and obesity in childhood were 24.5% and 13.8%, respectively. The variables associated with obesity were: boy (OR = 1.26; 95% CI = 1.02-1.57), low socioeconomic status (OR = 1.60; 95% CI = 1.16-2.20), single-parent family (OR = 1.35; 95% CI = 1.03-1.77), immigrant (OR = 1.68; 95% CI = 1.33-2.13) and living in one of the less favored districts of Barcelona. it is evident that social class, origin, family status and the district of residence play an important role in childhood obesity

    Adaptation and Evaluation of the Nutrition Environment Measures Survey in Stores to Assess Mediterranean Food Environments (NEMS-S-MED)

    The Nutrition Environment Measures Surveys are valid and reliable measures of community and consumer food environments. This article describes the adaptation and evaluation of the Nutrition Environment Measures Survey in Stores (NEMS-S) for Mediterranean urban contexts (NEMS-S-MED). Trained raters used the adapted NEMS-S-MED tool to observe and rate food outlets in 21 census tracts and 43 food stores across the city of Madrid, Spain. We evaluated inter-rater and intra-rater reliabilities, construct validity, and the tool’s ability to discriminate between store types and between stores by area-level Socio-Economic Status (SES). Overall, the mean NEMS-S-MED score was 20.7 (SD = 9.8), which ranged from 7 to 43. Most food items displayed substantial or almost perfect inter-rater and intra-rater agreements; the percentage agreement across availability items was almost perfect and kappa statistics were also very high (median κ = 1.00 for inter-rater; κ = 0.92 for intra-rater). Furthermore, the NEMS-S-MED tool was able to discriminate between store types and census tracts of different SES. The adapted NEMS-S-MED instrument is a reliable and valid audit tool to assess the consumer food environment in Mediterranean urban contexts.
