    In situ evaluation of fluoride-, stannous- and polyphosphate-containing solutions against enamel erosion

    Objective To evaluate the anti-erosive effect of solutions containing sodium fluoride (F: 225 ppm of fluoride), sodium fluoride + stannous chloride (F + Sn: 225 ppm of fluoride + 800 ppm of stannous), sodium fluoride + stannous chloride + sodium linear polyphosphate (F + Sn + LPP: 225 ppm of fluoride + 800 ppm of stannous + 2% of sodium linear polyphosphate), and deionized water (C: control), using a four-phase, single-blind, crossover in situ clinical trial. Methods In each phase, 12 volunteers wore appliances containing 4 enamel specimens, which were submitted to a 5-day erosion-remineralization phase that consisted of 2 h of salivary pellicle formation with the appliance in situ, followed by 2 min extra-oral immersion in 1% citric acid (pH 2.4), 6x/day, with 90 min of exposure to saliva in situ between the challenges. Treatment with the test solutions was performed extra-orally for 2 min, 2x/day. At the end of the experiment, surface loss (SL, in μm) was evaluated by optical profilometry. Data were analyzed using ANOVA and Tukey tests (α = 0.05). The surface of additional specimens was evaluated by x-ray diffraction after treatments (n = 3). Results C (mean SL ± standard-deviation: 5.97 ± 1.70) and F (5.36 ± 1.59) showed the highest SL, with no significant difference between them (p > 0.05). F + Sn (2.68 ± 1.62) and F + Sn + LPP (2.10 ± 0.95) did not differ from each other (p > 0.05), but presented lower SL than the other groups (P < 0.05). Apatite and stannous deposits on specimen surfaces were identified in the x-ray analysis for F + Sn and F + Sn + LPP. Conclusions Sodium fluoride solution exhibited no significant anti-erosive effect. The combination between sodium fluoride and stannous chloride reduced enamel erosion, irrespective of the presence of linear sodium polyphosphate. Clinical significance Under highly erosive conditions, sodium fluoride rinse may not be a suitable alternative to prevent enamel erosion. A rinse containing sodium fluoride and stannous chloride was shown to be a better treatment option, which was not further improved by addition of the sodium linear polyphosphate

    Association of Nd:YAG laser and calcium-phosphate desensitizing pastes on dentin permeability and tubule occlusion

    Objective: To evaluate the efficacy of Nd:YAG laser associated with calcium-phosphate desensitizing pastes on dentin permeability and tubule occlusion after erosive/abrasive challenges.&nbsp;Methodology: Dentin specimens were exposed to 17% ethylene diamine tetra-acetic acid (EDTA) solution for 5 min and randomly allocated into five groups: G1, control (no treatment); G2, Nd:YAG laser (1 W, 10 Hz, 100 mJ, 85 J/cm2); G3, Laser + TeethmateTM Desensitizer; G4, Laser + Desensibilize Nano P; and G5, Laser+Nupro®. Specimens underwent a 5-day erosion-abrasion cycling. Hydraulic conductance was measured post-EDTA, post-treatment, and post-cycling. Post-treatment and post-cycling permeability (%Lp) was calculated based on post-EDTA measurements, considered 100%. Open dentin tubules (ODT) were calculated at the abovementioned experimental moments using scanning electron microscopy and ImageJ software (n=10). Data were analyzed using two-way repeated measures ANOVA and Tukey’s test (α=0.05).&nbsp;Results: G1 presented the highest %Lp post-treatment of all groups (p&lt;0.05), without significantly differences among them. At post-cycling, %Lp significantly decreased in G1, showed no significant differences from post-treatment in G3 and G4, and increased in G2 and G5, without significant differences from G1 (p&gt;0.05). We found no significant differences in ODT among groups (p&gt;0.05) post-EDTA. At post-treatment, treated groups did not differ from each other, but presented lower ODT than G1 (p&lt;0.001). As for post-cycling, we verified no differences among groups (p&gt;0.05), although ODT was significantly lower for all groups when compared to post-EDTA values (p&lt;0.001).     Temperature changes on the root surfaces of mandibular incisors after an 810-nm high-intensity intracanal diode laser irradiation

    Temperature changes caused by laser irradiation can promote damage to the surrounding dental tissues. In this study, we evaluated the temperature changes of recently extracted human mandibular incisors during intracanal irradiation with an 810-nm diode laser at different settings. Fifty mandibular incisors were enlarged up to an apical size of ISO No. 40 file. After the final rinse with 17% ethylenediaminetetraacetic acid, 0.2% lauryl sodium sulfate biologic detergent, and sterile water, samples were irradiated with circular movements from apex to crown through five different settings of output power (1.5, 2.0, 2.5, 3.0, and 3.5 W) in continuous mode. The temperature changes were measured on both sides of the apical and middle root thirds using two thermopar devices. A temperature increase of 7 degrees C was considered acceptable as a safe threshold when applying the diode laser. Results: The results showed that only 3.5-W output power increased the outer surface temperature above the critical value. Conclusion: The recommended output power can be stipulated as equal to or less than 3 W to avoid overheating during diode laser irradiation on thin dentin walls. Исследование проводилось на основе данных документов "Осмотр в стационаре при поступлении" из историй болезни пациентов, страдающих рожистыми воспалениями. Произведен выбор наилучшей модели классификатора, подбор оптимальных гиперпараметров, а также оценка качества классификации.In the present study selection of significant text features and the machine learning model for the classification of medical records were made. The study was conducted on the basis of "Inspection in the hospital at admission" documents from the records of patients suffering from erysipelas.     O objetivo deste estudo foi o de avaliar os efeitos da irradiação laser de Er,Cr:YSGG em superfícies dentinárias no tratamento da hipersensibilidade dentinária cervical, em busca de um protocolo para ser aplicado clinicamente. Para o teste de permeabilidade, foram preparados 144 pré-molares, as coroas seccionadas e as raízes impermeabilizadas. Duas áreas de irradiação antagônicas foram delimitadas e livres da impermeabilização (controle e experimental). A seguir, os dentes foram subdivididos em dois sub-grupos, diferindo o condicionamento da superfície irradiada (EDTA 24% e ácido fosfórico 35%) e em grupos diferindo o protocolo:G1)Er:YAG, 60mJ/2Hz; G2)0,25W; G3)0,5W; G4)0,75W; G5)1W, G6)1,25W; G7)1,50W; G8)1,75W G9)2W. Após as irradiações, os espécimes foram imersos em azul de metileno 2% por 4 horas, e incluídos em resina epóxica para a realização dos cortes longitudinais. Estes tiveram suas imagens trabalhadas em computador para mensuração do nível de infiltração. Os dados foram submetidos à análise estatística. O Sub-grupo 1, condicionado com EDTA, apresentou diferenças entre os grupos sendo que as amostras irradiadas com Er:YAG demonstraram menor infiltração, diferindo estatisticamente dos grupos 3, 6 e 9. Os resultados do sub-grupo 2 mostraram que as médias das amostras irradiadas com Er:YAG tenderam a zero entretanto, não houve diferença entre os grupos. Na Fase B, fragmentos de dentina foram obtidos da região cervical de dentes humanos, preparados para análise em microscopia eletrônica de varredura. Morfologicamente, observou-se oclusão parcial dos túbulos dentinários após irradiação com Er:YAG e Er,Cr:YSGG nas condições de 0,25W e 0,50W. A partir de 0.75W, foi observado abertura dos túbulos e à medida que as energias aumentaram, foram observadas carbonização e fendas. De posse dos resultados dos trabalhos in vitro, foram selecionados os parâmetros para a aplicação e o acompanhamento clínico de 1 mês: G1)controle (placebo); G2)Er:YAG; G3)Er,Cr:YSGG 0,25W e G4)Er,Cr:YSGG 0,75W. Após a triagem, foram selecionados voluntários de acordo com critérios de exclusão e inclusão. O nível de sensibilidade de cada voluntário foi avaliado através da escala visual analógica de dor (VAS) com auxílio do ar da seringa tríplice 1 semana antes (Pré-1), 5 minutos anterior ao tratamento (Pré-2), após 5 minutos (Pós-1), 1 semana (Pós-2) e 1 mês após o tratamento (Pós-3). Os dados foram coletados e submetidos à análise estatística separadamente para os dois estímulos realizados: spray de ar e sonda exploradora. Tanto para o estímulo ar quanto para o estímulo sonda, não foram observadas diferenças estatisticamente significantes nas escalas Pré-1 e Pré-2, concluindo que os pacientes estavam padronizados e não houve efeito da profilaxia nos níveis de dor. Ao estímulo do spray de ar, nota-se uma redução nos níveis de dor na escala Pós-1, porém há uma estabilidade dos valores das escalas seguintes. O grupo 2 (Er:YAG) apresentou o menor nível de dor quando comparado aos outros grupos. Ao estímulo mecânico foi observado que o grupo 4 (Er,Cr:YSGG 0,50W) demonstrou o decréscimo de dor mais acentuado imediatamente após o tratamento, porém ao final do estudo, apresentou os maiores níveis de dor. Os grupos 1, 2 e 3 apresentam redução de dor, porém estatisticamente diferentes do grupo 4. Com base nos resultados apresentados e dentro dos limites e parâmetros estudados, pode-se concluir que nenhuma condição foi capaz de eliminar completamente a microinfiltração e diminuir a permeabilidade dentinária; porém o laser de Er:YAG e o laser de Er,Cr:YSGG 0,25W apresentaram resultados satisfatórios nos três estudos realizados.The aim of the present study was to determine the correct parameters of the Er,Cr:YSGG laser in the treatment of cervical dentin hypersensitivity. This work was presented in 3 different stages. In the first one, it was evaluated the decrease in dentin permeability of dentinal tubules after Er,Cr:YSGG laser irradiation in radicular surfaces. Ninety premolars were prepared with its crows sectioned, and the roots completely impermeabilized. Two antagonist areas of irradiation were determined as control and experimental. Each surface was conditioned with EDTA 24% (sub-group1) and phosphoric acid 35% (sub-group2) and the selected parameters were: 1)Er:YAG, 60mJ, 2Hz, 4 irradiations of 20sec each, defocused mode; and groups 2 to 9 received irradiation with Er,Cr:YSGG laser, 20Hz, Z6 tip, 600µm diameter, defocus mode, 0% of air and water: 2)Er,Cr:YSGG 0,25W; 3) 0,5W; 4) 0,75W; 5) 1,0W; 6) 1,25W, 7) 1,50W, 8) 2W; 9) 2 W. After irradiation, samples were immersed in a solution of methylene blue for 4 hours, washed for 5 min and included in epoxy resin to allow longitudinal cuts. The images were digitalized and computed worked. Data was statistically analyzed. Although the samples irradiated with Er:YAG showed less microleakage, the sub-group 1 showed differences between the groups, statistically different from groups 3, 6 e 9. The results of the sub-group 2 showed that the mean values of the Er:YAG samples had a tendency to be negative, however, no differences were detected between the groups. In the second stage, cuts from the cervical area were obtained and prepared for scanning electron microscopy to evaluate the superficial morphology. It was observed the partial closure of dentinal tubules after the irradiation with Er:YAG and Er,Cr:YSGG laser in the 0.25 and 0.50W protocols. From the 0.75W until 2W, it was observed the dentinal tubules opened as the energy densities raised. With the results of the in vitro studies, the parameters were selected: G1)control; G2)Er:YAG; G3)Er,Cr:YSGG 0,25W e G4)Er,Cr:YSGG 0,75W. Thirty subjects, who met the entry criteria, were admitted into the study. The assessment method used to quantify sensitivity was the cold air syringe, recorded by the visual analogue scale (VAS), prior to treatment as baseline (Pre-1), immediately before (Pre-2) and immediately after (Post-1) the topical treatment, after 1 week (Post-2) and after 1 month (Post-3). Teeth were assigned to the 4 groups. After the follow-up, data was collected and submitted to statistical analysis for both kinds of stimulus: evaporative (air) and mechanical (probe). For both air and probe it was not observed differences among the Pre-1 and Pre-2 moments, for what it can be concluded that patients were standardized. Analyzing the evaporative stimulus, it can be observed the reduction in the pain level immediately after the treatment; however there was stability in the next values. Group 2 (Er:YAG) showed the least level of pain. Throughout the mechanical stimulus, it was observed that group 4 (Er,Cr:YSGG 0,50W) showed the most pronounced decrease of pain immediately after treatment, however, at the end of the study, the levels of pain increased. Groups 1, 2 e 3 showed reduction of pain, statistically different from the group 4. Based on the results and within the limits of this study, it can be concluded that none of the parameters and lasers studied were capable of eliminate the microleakage and decrease the dentin permeability, however, the Er:YAG and Er,Cr:YSGG 0,25W lasers showed satisfactory results in the three studies performed

    The aim of the present study was to determine clinical parameters for the use of Er,Cr:YSGG laser in the treatment of dentine hypersensitivity. Each surface was conditioned with 24% EDTA (sub-group 1) and 35% phosphoric acid (sub-group 2) and irradiated with the following settings: 1) Er:YAG, 60 mJ, 2 Hz, defocused; groups 2 to 9: irradiation with Er,Cr:YSGG laser, 20 Hz, Z6 tip, 0% of air and water: 2) Er,Cr:YSGG 0.25 W; 3) 0.5 W; 4) 0.75 W; 5) 1.0 W; 6) 1.25 W, 7) 1.50 W, 8) 2 W; 9) 2 W. After irradiation, samples were immersed in methylene blue solution and included in epoxy resin to obtain longitudinal cuts. The images were digitalized and analyzed by computer software. Although the samples irradiated with Er:YAG laser showed less microleakage, sub-group 1 showed differences between the groups, differing statistically from groups 3, 6, and 9. The results of sub-group 2 showed that the mean values of Er:YAG samples showed a negative trend, however, no differences were detected between the groups. For scanning electron microscopy analysis, dentine squares were obtained and prepared to evaluate the superficial morphology. Partial closure of dentinal tubules was observed after irradiation with Er:YAG and Er,Cr:YSGG laser in the 0.25 and 0.50 W protocols. As the energy densities rose, open dentinal tubules, carbonization and cracks were observed. It can be concluded that none of the parameters were capable of eliminating microleakage, however, clinical studies with Er:YAG and Er,Cr:YSGG lasers should be conducted with the lowest protocols in order to determine the most satisfactory setting for dentine hypersensitivity

    Объектом исследования являются реактор гидроочистки, кожухотрубчатый теплообменник. Цель работы – рассчитать и подобрать реактор гидроочистки и кожухотрубчатый теплообменник для производства гидроочищенного дизельного топлива мощностью 228310 кг/ч по сырью. В процессе исследования проводилось изучение и расчёт оборудования для производства дизельного топлива на установке гидроочистки. В результате производственной практики было выявлено, что установка гидроочистки не модернизировалась и работает с момента строительства завода. Актуальность данной работы заключается в обновлении установки с целью выпуска продуктов соответствующих международным стандартам.The object of the research are hydrotreating reactor, shell-and-tube heat exchanger. The purpose of the work is to calculate and select a hydrotreating reactor and a shell-and-tube heat exchanger for the production of hydrotreated diesel fuel with a capacity of 228310 kg / h for raw materials. In the course of the study, the study and calculation of equipment for the production of diesel fuel at the Hydrotreating plant were carried out. As a result of production practice, it was found that the Hydrotreating unit has not been modernized and has been operating since the construction of the plant. The relevance of this work is to update the installation in order to pro-duce products corresponding to international standards

    Increasing dental zirconia micro-retentive aspect through ultra-short pulsed laser microstructuring: study on flexural strength and crystal phase characterization

    OBJECTIVES: Although ultra-short pulsed laser (USPL) microstructuring has previously improved zirconia bond-strength, it is yet unclear how different laser-machined surface microstructures and patterns may influence the material’s mechanical properties. Therefore, the aim of this study was to assess the flexural strength of zirconia after different USPL settings creating three different geometrical patterns with structures in micrometer scale. METHODS: One hundred sixty zirconia bars (3Y-TZP, 21 × 4 × 2.1 mm) were prepared and randomly divided into five groups (n = 32): no surface treatment (negative control-NC); sandblasting with Al(2)O(3) (SB); and three laser groups irradiated with USPL (Nd:YVO(4)/1064 nm/2-34 J/cm(2)/12 ps): crossed-lines (LC), random-hatching (LR), and parallel-waves (LW). Bars were subjected to a four-point flexural test (1 mm/min) and crystal phase content changes were identified by X-ray diffraction. Surface roughness and topography were analyzed through 3D-laser-profilometry and SEM. Data were analyzed with parametric tests for roughness and Weibull for flexural strength (α = 5%). RESULTS: LR (Mean[95%CI]: 852.0 MPa, [809.2–894.7]) was the only group that did not show a significantly different flexural strength than NC (819.8 MPa, [796.6–842.9]), (p > 0.05). All laser groups exhibited higher Weibull moduli than NC and SB, indicating higher reliability and homogeneity of the strength data. An increase of monoclinic phase peak was only observed for SB. CONCLUSION: In conclusion, USPL created predictable, homogeneous, highly reproducible, and accurate surface microstructures on zirconia ceramic. The laser-settings of random-hatching (12 ps pulses) increased 3Y-TZP average surface roughness similarly to SB, while not causing deleterious crystal phase transformation or loss of flexural strength of the material. Furthermore, it has increased the Weibull modulus and consequently material’s reliability. CLINICAL SIGNIFICANCE: Picosecond laser microstructuring (LR conditions) of 3Y-TZP ceramic does not decrease its flexural strength, while increasing materials realiability and creating highly reproducible and accurate microstructures. These features may be of interest both for improving clinical survival of zirconia restorations as well as enhancing longevity of zirconia implants
