Ajudar a desenvolver hábitos alimentares saudáveis na infância
As doenças relacionadas com a alimentação são hoje consideradas um grave problema de saúde pública, cujas causas e consequências têm sido amplamente estudadas. A aprendizagem do comportamento alimentar inicia-se na infância, tendo a família e todo o contexto sociocultural onde as refeições ocorrem, um papel determinante no processo de formação dos hábitos alimentares da criança, tendo estes grande probabilidade de prevalecerem na vida adulta.
Com este artigo pretendeu-se, através duma revisão da literatura, reflectir sobre a alimentação no período infantil, seus determinantes e processo de aprendizagem, com base nas várias evidências dos autores, bem como definir algumas linhas orientadoras para uma boa prática de promoção da saúde alimentar no ambiente familiar
Preparação dos Pais para o Cuidar do Recém‐Nascido Após a Alta: Avaliação dos Registos de Enfermagem
Resumo
A excelência dos cuidados ao recém-nascido
depende, em grande parte, da capacitação dos pais para a
autonomia nos cuidados ao filho, exigindo aprendizagem da
informação veiculada pelo enfermeiro. O registo dos ensinos
permite avaliar o processo de (in) formação e a definição do
tipo de ensinos abordados, o que contribui para a eficiência e
qualidade dos cuidados de enfermagem.
Com este estudo pretendeu-se descrever quais os
ensinos efetivamente abordados e preconizados na preparação
dos pais para o cuidar do recém-nascido (RN) após a alta e
identificar os locais de anotação/registo mais utilizados para o
registo desses mesmos ensinos no aplicativo SAPE (Sistema
de Apoio à Prática de Enfermagem).
Dos resultados obtidos emergiu não existirem
critérios na efetivação dos registos de enfermagem e que, dos
ensinos preconizados, nem todos são alvo de abordagem,
tornando evidente a necessidade da definição de boas práticas
na preparação dos pais para a alta do RN.Abstract
The excellence of care to the newborn depends
largely of parent capacity for autonomy of the child care,
demanding learning conveyed by the nurse. The record of
teachings allows evaluating the process of (in) formation and
the definition of the type of teachings addressed, which
contributes to the efficiency and quality of nursing care.
With the study carried out it was intended to
describe which advocated teaching in preparing parents to care
for the newborn after discharge are effectively addressed and
to identify the location of annotation more used in SAPE
(Support System for Nursing Practice) to record these same
teachings.
Emerged from this study that there are no criteria in
the effectiveness of nursing records and that, from the
teachings advocated, not all are target of approach, making
clear the need of developing good practices in preparing
parents for discharge of the newborn
Boas Práticas nos Cuidados ao Coto Umbilical: Um Estudo de Revisão
Resumo
Os cuidados ao coto umbilical foram, ao longo dos
anos, considerados fulcrais e foram sofrendo mudanças
consideráveis, muito ao sabor dos tempos ou experiências do
prestador de cuidados. Na prática, o que se verificou foi uma
diversidade de atuações nos cuidados ao coto umbilical.
Perante esta problemática, com este artigo pretendeu-se
identificar recomendações efetivas, baseadas na evidência
científica, de boas práticas nos cuidados de promoção da
queda e na prevenção da infeção do coto umbilical do
recém-nascido (RN).Abstrat
Care of the umbilical stump were considered, over
the years, pivotal, but were suffering considerable changes,
much to the flavor of the times or experiences of the care
provider. In practice, what we found was a variety of
performances in the care of the umbilical stump. Faced with
this problem, in this article we intended to identify effective
recommendations, based on scientific evidence of good
practice in promoting care of the fall and prevent infection of
the umbilical stump of the newborn
Olhar dos Pais sobre o Estado Nutricional das Crianças Pré‐escolares
Portugal é um dos países europeus com maior
prevalência de obesidade infantil, o que se traduz num
problema de saúde crescente não só na geração actual, mas
que se repercutirá na próxima, dada a grande probabilidade da
doença transitar para a idade adulta. Estudos em diversos
países têm revelado que os pais se mostram despreocupados e
pouco conscientes do estado nutricional das crianças, dada a
tendência para subestimarem o peso excessivo dos seus filhos.
Ajudar os pais a reconhecerem que a obesidade nas crianças é
um problema de saúde e a identificarem o verdadeiro estado
nutricional dos seus filhos, pode ser o primeiro passo na
promoção de um estilo de vida mais saudável.

A Motivação Sexual dos Adolescentes: Influência dos Fatores Sociodemográficos
Resumo
Enquadramento: A sexualidade, presente ao longo
da vida, na adolescência adquire novos contornos. O início da
atividade sexual pode ser considerado um dos momentos com
maior impacto na vida do adolescente, tornando-se, assim,
importante conhecer as motivações que estão na base da
decisão de iniciar ou não a atividade sexual.
Objetivo: Foi objetivo deste trabalho identificar as
variáveis sociodemográficas que influenciam a motivação dos
adolescentes para fazer ou não fazer sexo.
Métodos: Trata-se de um estudo de análise
quantitativa, não experimental, com características de estudo
descritivo e correlacional. A amostra é não probabilística por
conveniência, constituída por 545 adolescentes (262 rapazes e
283 raparigas), com uma média de idade de 13,95 anos
(Dp=1,25), a frequentar o 3º ciclo do ensino básico de quatro
agrupamentos de escolas, três do concelho do Fundão e uma
do concelho de Tabuaço (53,1% residentes em aldeias). O
protocolo de avaliação incluiu um questionário, que
possibilitou, numa primeira parte, fazer a caracterização
sociodemográfica e da experiência sexual da amostra e, numa segunda parte, identificar as motivações dos adolescentes para
ter sexo, utilizando para o efeito a “Escala de motivação para
fazer ou não fazer sexo” (Leal & Maroco, 2010).
Resultados: O estudo revelou haver diferenças
estatísticas entre o sexo e a motivação dos adolescentes para
fazer ou não fazer sexo (p=0,000). Os rapazes são os que se
apresentam mais motivados para fazer sexo por motivos de
hedonismo e saúde, e a não fazer sexo por medo,
conservadorismo/desinteresse e porque é imoral. De referir
que a motivação das raparigas está relacionada com a
interdependência relacional. Também a escolaridade se
associou de forma significativa com a motivação sexual dos
adolescentes (p=0,001).
Conclusão: Os resultados evidenciam que quer o
sexo quer a escolaridade influenciam a motivação dos
adolescentes para fazer ou não fazer sexo. Identificar as causas
que determinam as opções dos adolescentes neste âmbito
permitirá aos pais e profissionais desenvolver estratégias de
intervenção dirigidas às reais necessidades dos adolescentes.Abstract
Background: Sexuality, present through life,
acquires new dimensions in adolescence. The onset of sexual
activity can be considered one of the moments with the
greatest impact on teenage life, becoming thus important to
know the motivations that underlie the decision to initiate
sexual activity or not.
Objectives: The aim of this study was to identify
sociodemographic variables that influence adolescents'
motivation to do or not to do sex.
Methods: This is a study of quantitative analysis,
not experimental, with characteristics of a descriptive and
correlational study. The sample is non-probability by
convenience, consisting of 545 adolescents (262 boys and 283
girls) with a mean age of 13.95 years old (SD =1.25), who
attend the 3rd cycle of basic education of four groups of
schools, three in the county of Fundão and one in the county
of the Tabuaço (53.1% living in villages). The evaluation
protocol includes a questionnaire which in first part allowed
making the sociodemographic and sexual characterization of the sample and, in a second part, to know the motivations of
teenagers to have sex, using for this purpose the "Scale of
motivation to have or not to have sex" (Leal & Maroco, 2010).
Results: The study revealed statistical differences
between gender and motivation of adolescents to have sex or
not (p = 0.000).The boys are the ones that appear more
motivated to have sex for reasons of health and hedonism, and
not have sex for fear, conservatism/disinterest and because it
is immoral. It should be noted that the motivation of girls is
related to the relational interdependence. Also the schooling
proved to be a statistically significant variable in relation to
adolescent sexual motivation (p=0.0001).
Conclusion: The results show that sex and
education influence the motivation of adolescents to have sex
or not. Knowing the factors that influence the motivation of
adolescents to have sex or not, allows parents and
professionals develop intervention strategies targeted to the
real needs of adolescents
Contributo dos Interlocutores nas Atitudes dos Alunos do 3º Ciclo do Ensino Básico face à Sexualidade
Resumo
Enquadramento: Tratar de educação sexual, para
além da atualidade do tema, prende-se, sobretudo, com a
necessidade de uma abordagem que leve à adoção de atitudes
favoráveis face à sexualidade e a comportamentos saudáveis
por parte dos adolescentes.
Objetivos: Identificar as preferências dos alunos na
escolha dos interlocutores em assuntos de sexualidade, bem
como analisar a relação com as atitudes face à sexualidade dos
alunos do 3º ciclo do ensino básico.
Métodos: O estudo realizado é de natureza
quantitativa, descritivo-correlacional e transversal. Amostra
não probabilística, por conveniência, de 545 alunos (262
rapazes e 283 raparigas), idade média de 13,95 anos (Dp= 1,25), maioritariamente residentes na aldeia (53,1%) e a
frequentar o 3º ciclo do ensino básico nas escolas do Concelho
de Tabuaço e Fundão. Foi aplicado um questionário visando a
caracterização sociodemográfica da amostra, o conhecimento
das suas vivências da sexualidade; foi, ainda, aplicada a Escala
de atitudes face à sexualidade em adolescentes (Nelas,
Fernandes, Ferreira, Duarte & Chaves, 2010).
Resultados: Da amostra global de alunos, os
principais interlocutores da sexualidade são os amigos
(59,8%), seguido da mãe (40,9%), pai (16,1%), namorado/a
(14,7%), irmãos (13,8%), professores (12,5%), e, por último,
do médico/enfermeiro (4,6%). A mãe influencia
significativamente e favoravelmente as atitudes face à
sexualidade (p=0,006) e, no reverso, os irmãos influenciam de
forma significativa e desfavoravelmente as atitudes face à
sexualidade (p=0,050).
Conclusão: Este estudo sugere que o papel da figura
materna e dos irmãos está diretamente relacionado com as
atitudes face à sexualidade dos alunos do 3ºciclo do ensino
básico e que aqueles têm uma importância essencial na
avaliação que estes fazem da sexualidade.Abstract
Background: Dealing with sex education, being a
matter of full actuality, is related mainly with the need for an
approach that leads to the adoption of positive attitudes
towards sexuality and healthy behavior by adolescents.
Objectives: Identify the preferences of students of
the 3rd cycle of basic education in the choice of the
interlocutors on matters of sexuality as well as analyse their
relationship with attitudes towards sexuality.
Methods: The fulfilled study is quantitative,
descriptive-correlational and tranversal. It is a
non-probabilistic sample by convenience of 545 students
(262 boys and 283 girls), on average age of 13.95 years old
(SD = 1.25), mostly living in the village (53.1%) and attend
the 3rd cycle of basic education in schools of Tabuaço and
Fundão. It was administered a questionnaire constitued by a
socio-demographic characterization and the experiences of
sexuality and by a scale of attitudes towards sexuality in adolescents (Nelas, Fernandes, Ferreira, Duarte & Chaves,
2010).
Results: Of the global sample of students, the main
interlocutors of the sexuality are friends (59.8%), followed by
the mother (40.9%), father (16.1%), boyfriend/girlfriend
(14.7%), siblings (13.8 %), teachers (12.5%) and, finally, the
doctor/nurse (4.6%). The mother significantly and favorably
influences the attitudes towards sexuality (p = 0.006) and, in
reverse, the siblings influence significantly and adversely the
attitudes towards sexuality (p = 0.050).
Conclusion: This study suggests that the role of the
mother and siblings is directly related to attitudes towards
sexuality of students of the 3rd cycle of basic education and
that those are vitally important in the evaluation of sexuality
that they do
CARDIOVASCULAR AND METABOLIC RISK IN PORTUGUESE WOMEN
Background: Overweight and increased waist circumference are early markers of cardiovascular and metabolic disease. The current obesity context recommends the assessment of individual risk factors as predictors of future comorbidities.
Objectives: Assess body mass index (BMI), blood pressure (BP) and waist circumference values and analyze its relationship with nutritional status in young Portuguese women.
Methods: Cross-sectional/observational study conducted in 2012 with 1424 Portuguese women, Mean age 34.47 years (SD = 5.29), mothers of preschool children also under study. Performed anthropometric and waist circumference measures, BMI classification based on WHO (2000) referential and Portugal norms, (DGS, 2004). The BP was classified according to the European Heart Journal (2007) recommendations.
Results: Overall, 31.9% of women were overweight (obesity 12.1%), significantly higher in women with low literacy, low-income, andfrom rural areas, while the high blood pressure (HBP) and metabolic risk associated with waist circumference were significantly higher only in those with low literacy. Metabolic risk appeared in 60.8% and women with overweight and obesity, signifcantly associate increased risk of metabolic disease (X2 = 212.37, p = 0.000), with a probability greater than 8 (OR = 8.158; CI = 6.024-11.048). Globally had normal-high and hypertension 22.8% of women, while 31.8% associated obesity and hypertension (X2 = 31.466, p = 0.000) and 27.6% increased waist circumference (X2 = 16.769, p = 0.000) with a risk about twice (OR = 2.447; CI = 1.781 - 3.362; OR= 1.990; CI = 1.427- 2.775 respectively).
Conclusions: Results showed the association between overweight, metabolic and cardiovascular risk, who reinforces the importance of promoting women’s health early
Childhood Obesity Determinants of Childhood Overweight
Introduction
The number of obese children has grown exponentially in the recent decades, which has been the subject of concern byseveral entities, health care and education. Thus we performed a framework, in theoretical terms, the concept of childhood obesity and its relationship with risk factors / protective, eating habits, physical activity and sedentary lifestyle, also recent studies with conclusions that are later confronted.
Method
Considering the issue under study, the participants in this study are children (n = 1424) born in the years 2006, 2007 and 2008, aged between 3 and 6 years enrolled in 2011–2012 school
year in kindergartens districts of Viseu, Leiria, Évora and Vila Real - Portugal
Obesidade infantil: práticas alimentares e percepção materna de competências
Doutoramento em Ciências e Tecnologias da SaúdeA obesidade infantil é um importante problema de saúde pública, não só pelos
efeitos adversos durante a infância mas porque tende a persistir na idade adulta,
constituindo um factor de risco para diversas doenças crónicas. Os alicerces de
uma vida saudável estruturam-se na vida pré-natal e sedimentam-se nos seis
primeiros anos de vida, sendo o crescimento da criança fortemente influenciado
pelo seu contexto ambiental familiar. Foi neste âmbito que emergiu como
objectivo geral deste estudo explorar as relações existentes entre os
determinantes infantis (antecedentes obstétricos e peri-natais) e parentais
(práticas alimentares, conhecimento dos pais sobre alimentação infantil,
percepção parental de competência e percepção do peso da criança) e o
desenvolvimento de excesso de peso em crianças pré-escolares.
Este estudo, de carácter observacional e transversal, foi realizado com 792
crianças pré-escolares, idade M= 4,39 anos (±0,91Dp) e seus pais, residentes
num concelho pertencente às NUTs III Dão-Lafões, sendo efectuada a avaliação
antropométrica e classificação nutricional das crianças com base no referencial
NCHS (CDC, 2000) e da OMS nos pais.
O protocolo de pesquisa incluiu instrumentos de medida que validamos para a
população portuguesa e a construção do Questionário de Conhecimentos sobre
Alimentação Infantil (QAI) cujas propriedades psicométricas certificam a sua
qualidade (Alfa de Cronbach = 0,942; Alfa de Cronbach teste re-teste = 0,977).
Nas crianças, 31,3% apresentavam excesso de peso (12,4% obesidade), assim
como 41,1% das mães (10,2% obesidade) e 64,4% dos pais (14,8% obesidade),
sendo mais evidente nas mães o risco metabólico associado ao Perímetro da
Cintura. As mães revelam mais conhecimentos sobre alimentação e sentimentos
de eficácia mais elevados com o papel parental, enquanto os pais manifestaram
mais sentimentos de motivação e satisfação.
Os resultados obtidos corroboram existir efeito significativo dos determinantes
infantis e parentais no excesso de peso da criança, designadamente: (i) do peso
ao nascer, com impacto dos nascidos grandes; (ii) da higiene do sono
especificamente dos que dormem menos de 11horas; (iii) dos que não brincam na
rua, (iv) das mães mais jovens, do IMC e risco metabólico dos Pais; (v) da
percepção parental da imagem corporal dos filhos, verificando-se que quanto
mais elevado o IMC das crianças, mais distorcida é esta percepção dos pais; (vi)
das crenças, atitudes e práticas alimentares e que permitem inferir que uma maior
preocupação com o peso da criança, maior controlo, restrição e menor pressão
para comer se associa a maior excesso de peso.
As inferências evidenciam que, na vigilância de saúde periódica se torna
imprescindível a valorização dos determinantes de risco biológicos e familiares do
excesso de peso infantil, considerando programas de intervenção centrados na
família, num processo que encontre sentido a partir daquilo que as famílias
experienciam, de forma a ajudá-las a criar recursos fortalecedores de
competência para uma parentalidade mais positiva.Childhood obesity is a major indicator of public health, not only because of its
adverse effects during childhood, but also because it tends to persist into
adulthood, constituting a risk factor for several chronic diseases. The
foundations of a healthy life are structured during prenatal life and gain
consistency during the first six years of life, being the child growth and
development highly influenced by their familiar context. It was in this context
that the major aim of this study emerged, as to investigate the correlations
between childhood (obstetric and prenatal background) and parental
determinants (feeding practices, parental knowledge of infant feeding, parental
perception of competence and children weight perception) and the overweight
development in preschool children.
This cross-sectional and observational study comprised by 792 preschool
children, average age=4,39 years old (±0,91 SD) and their parents, resident in
a NUTs III Dão-Lafões council. Children anthropometric evaluation and
nutritional classification was based on the NCHS reference (CDC, 2000) and
the WHO in parents.
The research protocol included measurement instruments validated for the
Portuguese population and the construction of a questionnaire “Questionário de
Conhecimentos sobre Alimentação Infantil” (QAI) which psychometric
properties certify its quality (Cronbach Alfa=0,942; Cronbach Alfa test retest=
0,977).
The children, 31,3% were overweight (12,4% obese), as 41,1% of the mothers
(10,2% obese) and 64,4% of the fathers (14,8% obese). Metabolic risk related
to Waist Circumference was more evident in mothers. Mothers revealed to
have better knowledge of feeding and higher feelings of efficiency with parental
role, whilst fathers showed higher feelings of motivation and satisfaction.
The results confirm the existence of significant effect of childhood and parental
determinants in children overweight, namely: (i) the newborn weight, with
impact on bigger newborns; (ii) the sleeping patterns, especially those who
sleep less than 11 hours; (iii) not playing outside; (iv) the youngest mothers, the
BMI and the parental metabolic risk; (v) the parental perception of their children
body image, showing that the highest the child BMI, the more distorted parents
perception; (vi) the beliefs, attitudes and feeding practices which showed that
more concern with child weight, more control and restriction and less pressure
to eat relates to more overweight.
In periodic monitoring surveillance visits, become of major importance the
valorisation of biologic and familiar risk factors related to overweight children,
considering family-centred intervention programs, in a process that makes
sense in the context of how each and every family experience, in order to help
them to create strengthening resources of their own competence for a more
positive parenting
Nutritional status in preschool children: current trends of mother’s body perception and concerns
Background: Studies in several countries indicate that parents show little concern and aren’t
much aware of the nutritional status of their children, and have the tendency to underestimate
the overweight which may present a difficulty when promoting healthier life styles. The aim of
this study was to identify maternal body image perceptions in preschool children and analyze its
relationship with nutritional status and family social demographic variables.
Methods: A transversal and descriptive study was developed in a sample of 1424 preschool children
and their mothers living in several parts of Portugal. The children were weighted and
measured by trained personnel, the BMI was calculated and the nutritional status was classified
according to the NCHS referential (CDC, 2000).1 Mother’s real and ideal body image perception
from their children was evaluated according to Collins’ body image silhouettes (1991).2
Results: The ideal and real perception was translated in a negative index on both children’s
genders, however 67.2% of the mothers revealed accuracy of perception, while 22.3% had a
negative and 12.8% a positive discrepancy, being this significant concerning gender and age of
the children. Children’s actual weight status (ie, 60.2% normal weight, 16.9% overweight, 17.4%
obese and 5.5% underweight) was significantly different from their parents’ perceptions (ie,
52.3% normal weight, 27.5% overweight 0.6% obesity and 19.6% underweight) explaining in
25.9% the variability of this perception. Globally 42.3% has no discrepancy but 44.3% thinks that
children were thin and 13.4% heavier. This difference is significant concerning the age and scholarship
of the mothers but has no relation with the income and residence.
Conclusion: Despite signs of a positive trend, mothers continue to show difficulty in recognizing
the children’s nutritional status, so effective public health strategies to increase parents’
awareness could be the first step in an effort to prevent childhood obesity
