    Amoebic liver abscess in the medical emergency of a North Indian hospital

    <p>Abstract</p> <p>Background</p> <p>Amoebic Liver abscess although fairly common in developing countries, yet, there is limited data on the clinical presentation to the emergency department. A retrospective analysis of 86 indoor cases of Amoebic Liver Abscess presenting to the emergency department over a 5-year period was carried out.</p> <p>Findings</p> <p>The mean age of patients was 40.5 ± 2.1 years (male-female ratio = 7:1). Fever, pain abdomen and diarrhea were seen in 94%, 90% and 10.5% respectively. Duration of symptoms less than 2 weeks was seen in 48% cases. Hepatomegaly was present in 16% cases only, a right sided pleural effusion in 14% cases and ascites in 5.7%. On ultrasound, a right lobe abscess was seen in 65%, a left lobe abscess in 13% and multiple abscesses in both the lobes in 22% cases. Seventy one cases underwent per-cutaneous pigtail catheter drainage for a mean period of 13.4 ± 0.8 days. The mortality rate was 5.8%. On multivariate regression and correlation analysis, a higher number of inserted pigtail catheters correlated to mortality.</p> <p>Conclusions</p> <p>Amoebic liver abscess presents commonly to the emergency department and should be suspected in persons with prolonged fever and pain abdomen. Conservative management for uncomplicated amoebic liver abscess and insertion of single per-cutaneous pigtail catheter drainage for complicated amoebic liver abscess are efficacious as treatment modalities.</p

    Physico-chemical attributes and organoleptic assessment of guava (Psidium guajava L.) cultivars grown in eastern Uttar Pradesh

    A study was conducted to evaluate the different guava cultivars for their physico-chemical composition and organoleptic assessment during the year 2012-2013. Results of study indicated that Gorakh Bilas Pasand cultivar proved to be superior on the basis of physical characters (Length-7.64 cm, Breadth-7.79 cm , Weight-240.60 g, Number of seeds per fruit-251 etc.) followed by Lucknow-49. However, Lucknow-49 was found noteworthy in respect of chemical composition (TSS-13.00 oBrix, Acidity-0.50%, pH-5.86, Vitamin C-300.36 mg/100g etc). In organoleptic assessment, it was found that ‘liked very much’ rating was provided by consumer to Lucknow-49. On the basis of overall findings, it was concluded that ‘Lucknow-49’ was superior in most of characters studied and might be one of the promising cultivars for quality fruits under eastern Uttar Pradesh conditions

    Molecular Mechanism of Cancer Susceptibility Associated with Fok1 Single Nucleotide Polymorphism of VDR in Relation to Breast Cancer

    Breast cancer is the leading cause of death among women worldwide. It is a multi-factorial disease caused by genetic and environmental factors. Vitamin D has been hypothesized to lower the risk of breast cancer via the nuclear vitamin D receptor (VDR). Genetic variants of these vitamin D metabolizing genes may alter the bioavailability of vitamin D, and hence modulate the risk of breast cancer. Materials and Methods: The distribution of Fok1 VDR gene (rs2228570) polymorphism and its association with breast cancer was analysed in a case–control study based on 125 breast cancer patients and 125 healthy females from North Indian population, using PCR-RFLP. An In silico exploration of the probable mechanism of increased risk of breast cancer was performed to investigate the role of single nucleotide polymorphisms (SNPs) in cancer susceptibility. Results: The Fok1 ff genotype was significantly associated with an increased risk of breast cancer (p=0.001; χ2=13.09; OR=16.909; %95 CI=2.20 - 130.11). In silico analysis indicated that SNPs may lead to a loss in affinity of VDR to calcitriol, and may also cause the impairment of normal interaction of liganded VDR with its heterodimeric partner, the retinoid X receptor (RXR), at protein level, thereby affecting target gene transcription. Conclusion: Breast cancer risk and pathogenesis in females can be influenced by SNPs. SNPs in VDR may cause alterations in the major molecular actions of VDR, namely ligand binding, heterodimerization and transactivation. VDRE binding and co-activator recruitment by VDR appear to be functionally inseparable events that affect vitamin D-elicited gene transcription. This indicates that breast cancer risk and pathogenesis in females may be influenced by SNPs

    Taking a leaf from Jantri for traditional medicament- an ancient manuscript in Tankri

    The present paper highlights the importance vis-à-vis plight of a traditional script Tankri of Chamba, Himachal Pradesh. Through surveys in 70 villages that involved interviews with 550 residents, only one respondent was found to be versed in it. He uses an ancient manuscript named Jantri, scripted in Tankri, for prescribing medicines. Interactions and literature review revealed that Tankri was once a princely script of Chamba that now has no takers. Tankri originated from the Brahmic family of scripts and the Jantri holds information on uses of natural resources for curing human and animal related ailments. A total of 32 resources that include 24 of plant origin, 3 of animal origin, and 5 minerals were used for making formulations to cure a total of 13 diseases. Writing amulets in Tankri on tree bark using traditional ink was found to be a characteristic of the region. Unfortunately, the script is fast disappearing and so is the knowledge associated with it. Its disappearance would be a great loss to mankind. It is high time that conservation and decoding of such scripts is prioritized. Developing a database of knowledge holders and digitizing the information recorded in Tankri may be the starting point of this

    Characterization of Gold Thin Films Deposited by Centuries-old Fire-gilding Method

    117-125We have explored the centuries-old fire-gilding method which is being practiced by craftsmen in Nepal. In this technique, gold films are deposited on metal by heating the paste of gold amalgam layer coated on the metal surface. Samples of gold thin films deposited on copper substrate by this traditional method were characterized by x-ray diffraction (XRD) for structural properties, and atomic force microscopy (AFM) and scanning electron microscopy (SEM) for surface morphology. The XRD pattern of the samples showed the formation of polycrystalline gold film on the copper substrate. Microstructure characterization of the surface morphologies from the AFM and SEM images showed the gold films have porous feature with submicron size pores. We also performed optical reflectance measurements of the gold films deposited by this method to investigate the effects of double coating of gold amalgam layers, burnishing process and iron oxide treatment on the gold film. We found that the optical reflectance is increased as the surface smoothness is increased by double coating and burnishing. We also found that the vivid golden luster is enhanced by iron oxide treatment due to increased reflectance in the red spectral region

    “Different Strokes”: A management dilemma

    Stroke is a common medical emergency resulting from numerous pathophysiological mechanisms and with varied clinical manifestations; as such, the diagnosis of stroke requires diligent clinical assessment. When different stroke syndromes occur in the same patient, it may cause a dilemma in terms of diagnosis and management. This continuing medical education article describes an interesting patient with recurrent neurological events, highlighting the complex pathophysiological processes associated with cerebrovascular syndromes. It offers readers the opportunity to apply their own basic neuroscience knowledge and clinical skills to solve the challenges encountered during the course of diagnosing and treating this patient. Specifically, the article aims to familiarise readers with an approach to diagnosing brainstem strokes and the diverse manifestations of a common stroke syndrome.     In silico CD4+, CD8+ T-cell and B-cell immunity associated immunogenic epitope prediction and HLA distribution analysis of Zika virus

    Zika virus (ZIKV) is a mosquito-borne flavivirus distributed all over Africa, South America and Asia. The infection with the virus may cause acute febrile sickness that clinically resembles dengue fever, yet there is no vaccine, no satisfactory treatment, and no means of evaluating the risk of the disease or prognosis in the infected people.In the present study, the efficacy of the host’s immune response in reducing the risk of infectious diseases was taken into account to carry out immuno-informatics driven epitope screening strategy of vaccine candidates against ZIKV. In this study, HLA distribution analysis was done to ensure the coverage of the vast majority of the population. Systematic screening of effective dominant immunogens was done with the help of Immune Epitope & ABCPred databases. The outcomes suggested that the predicted epitopes may be protective immunogens with highly conserved sequences and bear potential to induce both protective neutralizing antibodies, T & B cell responses. A total of 25 CD4+ and 16 CD8+ peptides were screened for T-cell mediated immunity. The predicted epitope "TGLDFSDLYYLTMNNKHWLV" was selected as a highly immunogenic epitope for humoral immunity. These peptides were further screened as non-toxic, immunogenic and non-mutated residues of envelop viral protein. The predicted epitope could work as suitable candidate(s) for peptide based vaccine development. Further, experimental validation of these epitopes is warranted to ensure the potential of B- and T-cells stimulation for their efficient use as vaccine candidates, and as diagnostic agents against ZIKV
