Assessment of GFR by four methods in adults in Ashanti, Ghana: the need for an eGFR equation for lean African populations
Background. Equations for estimating glomerular filtration rate (GFR) have not been validated in Sub-Saharan African populations, and data on GFR are few.
Methods. GFR by creatinine clearance (Ccr) using 24-hour urine collections and estimated GFR (eGFR) using the four-variable Modification of Diet in Renal Disease (MDRD-4)[creatinine calibrated to isotope dilution mass spectrometry (IDMS) standard], Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) and Cockcroft–Gault equations were obtained in Ghanaians aged 40–75. The population comprised 1013 inhabitants in 12 villages; 944 provided a serum creatinine and two 24-hour urines. The mean weight was 54.4 kg; mean body mass index was 21.1 kg/m2.
Results. Mean GFR by Ccr was 84.1 ml/min/1.73m2; 86.8% of participants had a GFR of 60 ml/min/1.73m2. Mean MDRD-4 eGFR was 102.3 ml/min/1.73m2 (difference vs. Ccr, 18.2: 95% CI: 16.8–19.5); when the factor for black race was omitted, the value (mean 84.6 ml/min/1.73m2) was close to Ccr. Mean CKD-EPI eGFR was 103.1 ml/min/1.73m2, and 89.4 ml/min/1.73m2 when the factor for race was omitted. The Cockcroft–Gault equation underestimated GFR compared with Ccr by 9.4 ml/min/1.73m2 (CI: 8.3–10.6); particularly in older age groups. GFR by Ccr, and eGFR by MDRD-4, CKD-EPI and Cockcroft–Gault showed falls with age: MDRD-4 5.5, Ccr 7.7, CKD-EPI 8.8 and Cockcroft–Gault 11.0 ml/min/1.73m2/10 years. The percentage of individuals identified with CKD stages 3–5 depended on the method used: MDRD-4 1.6% (7.2 % without factor for black race; CKD-EPI 1.7% (4.7% without factor for black race), Ccr 13.2% and Cockcroft–Gault 21.0%.
Conclusions. Mean eGFR by both MDRD-4 and CKD-EPI was considerably higher than GFR by Ccr and Cockcroft–Gault, a difference that may be attributable to leanness. MDRD-4 appeared to underestimate the fall in GFR with age compared with the three other measurements; the fall with CKD-EPI without the adjustment for race was the closest to that of Ccr. An equation tailored specifically to the needs of the lean populations of Africa is urgently needed. For the present, the CKD-EPI equation without the adjustment for black race appears to be the most useful
Vehicle on Shoulder and Crash — Correlation or Causation?
This study sought to evaluate the relationships between vehicles on shoulder, congestion, and crashes. Three pertinent datasets on interstates were obtained and then integrated using a spatiotemporal approach. The analysis showed that about 36% of the crashes had vehicle on shoulder present in their vicinity, defined as 0.25 miles upstream and downstream of a crash site and 30 minutes before crash occurrence. The percentage increased considerably as spatial/temporal window expanded. In addition, congestion was found to be associated with about 25% of the crashes. The presence of both vehicle on shoulder and congestion was found for 11.7% of the crashes, signifying a high correlation between them and crashes. Based on crash narrative review, 1.8% of all crashes directly involved vehicles on shoulder and 23% of the carshes cited congestion as a contributor. However, there’s little indication in the crash narratives on how vehicles on shoulder contributed to crashes, beyond their direct involvement, or how they contributed to congestion which may led to crashes. Only 6 out of the 512 crashes flagged for review through the keyword search process specified a vehicle on shoulder as a contributor to congestion and subsequent crashes. While a small fraction of crashes were attributed to vehicles on shoulder, these crashes tended to be more severe than average interstate crashes
Filial factors of kwashiorkor survival in urban Ghana: Rediscovering the roles of the extended family
This paper discusses the findings of two field studies in urban Accra, Ghana that investigated the social and familial factors that were associated with survival of childhood kwashiorkor, a protein-caloric deficiency form of malnutrition that is endemic in that nation. Data was collected from qualitative interviews with family groups that included teenaged survivors of kwashiorkor, and the adults who were involved in the young person’s childhood rearing, including those who were responsible for compliance with the Ghana Ministry of Health malnutrition rehabilitation effort. Extensive interviews were documented in audio and video tape and field notes by a team that included the fields of social work, public health, nursing and sociology. All members of the participating families who were involved in the data collection were offered compensation for their time as well as full protection of privacy through the human subjects informed consent protocol and oversight of the University of Ghana, Eastern Michigan University and Wayne State University. The findings included reporting of a consistently critical role of the grandmothers and other senior women in the family units. The senior women either managed the economics and maintenance of the extended household, or took principal responsibility for sustaining the malnourished children’s participation in rehabilitation efforts. In some cases, the mothers were deceased and two or more senior women in the family carried out roles of parenting as well as familial economic support and coordination of care for the afflicted child. The findings suggest that full compliance with rehabilitation efforts for a single mother with multiple children and no extended familial support system would be very difficult and more likely to result in non-compliance and failure of the child to survive.  Suggestions are offered for family oriented, community health education regarding the irony of this form of malnutrition being endemic in communities that do not lack appropriate food. Implications for increased recognition and support for the elderly and senior family members to enhance child survival are discussed within the context of changing social and epidemiological profiles of urban centers in Ghana and elsewhere among developing nations of sub- Saharan Africa.Keywords: Kwashiorkor, Malnutrition, Rehabilitation compliance, Grandmothers, Endemic malnutritio

Germplasm collection and ethnobotany of taro (Colocasia esculenta L. Schott) from nineteen districts in the Ashanti, Eastern and Western regions of Ghana
Germplasm collection and ethnobotanical documentation are necessary for effective conservation and management of plant genetic resources. Taro (Colocasia esculenta L. Schott) is one of the staple root and tuber crops in Ghana. The study reports the germplasm and ethnobotanical information of taro collected from 19 districts in the Ashanti, Eastern and Western regions of Ghana. A germplasm collection expedition was undertaken in 58 towns in the districts. Fifty donors were interviewed on the ethnobotany of taro, using a questionnaire based on International Plant Genetic Resources Institute (IPGRI) descriptors for taro. Sixty taro accessions were collected from fields (34), home gardens (23), roadside stalls (2) and the wild (1). Respondents comprised of 27 males and 23 females. (62%). According to respondents of the survey, taro is used for food (100%), animal feed (44%) and folk medicine (4%). The corms (100%) and leaves (64%) are the parts of the plant used. The crop is grown mainly on a small scale for subsistent use by 70 percent of the respondents. Taro leaf blight (TLB) and lack of planting materials were the main constraints to large scale production. Respondents perceived the outbreak of TLB was due to the use of agrochemicals in farming practices in recent times (80%), irradiation (26%) and mythical reasons (10%). There is the need to educate taro growers on the causes and management of taro leaf blight
Suitability of sawdust from three tropical timbers for wood-cement composites
Construction material rising cost and global demand for economically-sustainable and environmentally-friendly building resources have necessitated the use of sawdust-cement composite. Wood constituents and cement incompatibility hinder its production and need careful selection of the timber. Sawdust suitability from Triplochiton scleroxylon, Entandrophragma cylindricum and Klainedoxa gabonensis for wood-cement composite was determined by identifying their chemical constituents and their composites’ physico-mechanical properties. T. scleroxylon recorded the minimum total extractive (6.12%), lignin (29.89%) and holocellulose (56.38%) and K. gabonensis the maximum (9.31, 31.59 and 57.5% respectively). Ash content was higher for T. scleroxylon (7.6%) but lower for K. gabonensis (1.53%). T. scleroxylon boards were stronger [Modulus of Elasticity (MOE) = 696.1 N/m 2 ] and more moisture-resistant [Moisture Absorption (MA) = 8.8%] than E. cylindricum (MOE = 625.9 N/m 2 ; MA = 9.5%). K. gabonensis boards crushed after manufacturing due to its incompatibility with cement. T. scleroxylon sawdust is suitable for wood-cement composites due to its more compatible chemical constituents (i.e., lower extractive, lignin, holocellulose contents and more ash) and its boards’ excellent physico-mechanical properties than those for the other timbers. Its sawdust-cement composites could be utilized for cladding and walling. The use of sawdust would increase green building resource base and reduce environmental pollution
Surveillance of bacterial pathogens of diarrhoea in two selected sub metros within the Accra metropolis
Background: In recent years, many localities within the Greater Accra Region (GAR) have witnessed several episodes of cholera outbreaks, with some deaths.Compared to previous epidemics, which usually followed heavy rains, recent outbreaks show no seasonality.Objectives: To investigate infective bacterial diseases in selected sub metros within the GAR.Methods: We used existing disease surveillance systems in Ghana, and investigated all reported cases of diarrhoea that met our case-definition. A three-daytraining workshop was done prior to the start of study, to sensitize prescribers at the Korle-Bu Polyclinic and Maamobi General hospital. A case-based investigationform was completed per patient, and two rectal swabs were taken for culture at the National Public Health and Reference Laboratory. Serotyping and antibiogramprofiles of identified bacteria were determined. Potential risk factors were also assessed using a questionnaire.Results: Between January and June 2012, a total of 361 diarrhoeal cases with 5 deaths were recorded. Out of a total of 218 rectal swabs cultured, 71 (32.6%) Vibrio cholerae O1 Ogawa serotypes, and 1 (0.5%) Salmonella (O group B) were laboratory confirmed. No Shigella was isolated. The Vibrio cholerae isolates were susceptible to ciprofloxacin and tetracycline. Greater than 80% of patients reported having drank sachet water 24 h prior to diarrhoea onset, and many (144/361) young adults (20-29 years) reported with diarrhoea.Conclusion: Enhanced surveillance of diarrhoeal diseases (enteric pathogens) within cholera endemic regions, will serve as an early warning signal, and reduce fatalities associated with infective diarrhoea.Keywords: Diarrhoeal disease surveillance, enteric pathogens, Vibrio cholerae, Salmonell
